    Towards a global partnership model in interprofessional education for cross-sector problem-solving

    Objectives A partnership model in interprofessional education (IPE) is important in promoting a sense of global citizenship while preparing students for cross-sector problem-solving. However, the literature remains scant in providing useful guidance for the development of an IPE programme co-implemented by external partners. In this pioneering study, we describe the processes of forging global partnerships in co-implementing IPE and evaluate the programme in light of the preliminary data available. Methods This study is generally quantitative. We collected data from a total of 747 health and social care students from four higher education institutions. We utilized a descriptive narrative format and a quantitative design to present our experiences of running IPE with external partners and performed independent t-tests and analysis of variance to examine pretest and posttest mean differences in students’ data. Results We identified factors in establishing a cross-institutional IPE programme. These factors include complementarity of expertise, mutual benefits, internet connectivity, interactivity of design, and time difference. We found significant pretest–posttest differences in students’ readiness for interprofessional learning (teamwork and collaboration, positive professional identity, roles, and responsibilities). We also found a significant decrease in students’ social interaction anxiety after the IPE simulation. Conclusions The narrative of our experiences described in this manuscript could be considered by higher education institutions seeking to forge meaningful external partnerships in their effort to establish interprofessional global health education

    Impact of Cerebral Microbleeds in Stroke Patients with Atrial Fibrillation

    OBJECTIVES: Cerebral microbleeds are associated with the risks of ischemic stroke and intracranial hemorrhage, causing clinical dilemmas for antithrombotic treatment decisions. We aimed to evaluate the risks of intracranial hemorrhage and ischemic stroke associated with microbleeds in patients with atrial fibrillation treated with Vitamin K antagonists, direct oral anticoagulants, antiplatelets, and combination therapy (i.e. concurrent oral anticoagulant and antiplatelet) METHODS: We included patients with documented atrial fibrillation from the pooled individual patient data analysis by the Microbleeds International Collaborative Network. Risks of subsequent intracranial hemorrhage and ischemic stroke were compared between patients with and without microbleeds, stratified by antithrombotic use. RESULTS: A total of 7,839 patients were included. The presence of microbleeds was associated with an increased relative risk of intracranial hemorrhage (aHR 2.74, 95% confidence interval 1.76 - 4.26) and ischemic stroke (aHR 1.29, 95% confidence interval 1.04 - 1.59). For the entire cohort, the absolute incidence of ischemic stroke was higher than intracranial hemorrhage regardless of microbleeds burden. However, for the subgroup of patients taking combination of anticoagulant and antiplatelet therapy, the absolute risk of intracranial hemorrhage exceeded that of ischemic stroke in those with 2-4 microbleeds (25 vs 12 per 1,000 patient-years) and ≄11 microbleeds (94 vs 48 per 1,000 patient-years). INTERPRETATION: Patients with atrial fibrillation and high burden of microbleeds receiving combination therapy have a tendency of higher rate of intracranial hemorrhage than ischemic stroke, with potential for net harm.     Multi-disciplinary optimization engine for parallel robots

    The present research focuses on the formulation of a recursive and cohesive design methodology for parallel robotic mechanisms. By using a multi-body dynamic simulation engine together with multi-disciplinary optimization methods, coupled mechanical, control can be considered in one integrated design framework. Optimization algorithms can then be employed to optimize design parameters simultaneously to meet performance objectives. To demonstrate the feasibility and effectiveness of the proposed methodology, design cases will be optimized and compared with the use of traditional sequential design method. The motivation of the work is to simplify the integrated design optimization framework for complex mechatronics system. Finite element modelling has been identified as the method to provide more accurate dynamic modelling of complex geometrical structures. However, the mere size of the multi-dimensional problems often hinders its incorporation in other optimization algorithm unless severe model reduction is performed. The goal of the current work is to demonstrate the fidelity provided by the proposed simulator and describe its place in the overall design framework for parallel manipulators. The simulator shows good agreement with an independent finite-element model and experimental results. Based on evaluations of simulation and experimental results, proposed design methodology is shown to be suitable for complex design optimization problems involving multiple disciplines of design variables, design criteria and local optima. The thesis explains the integrated design of parallel machines with configuration-dependent dynamics. To effectively evaluate and simulate parallel machines in time-domain, SimMechanics simulation engine is proposed to be used along side with implementation of FEM models of key mechanical structures. The following document describes past research on the subject, the major studies carried out in multi-disciplinary areas to complete the current research, the problem formulation of integrated design optimization and the concluded results and recommendations.     Prognostic Model of COVID-19 Severity and Survival among Hospitalized Patients Using Machine Learning Techniques

    We conducted a statistical study and developed a machine learning model to triage COVID-19 patients affected during the height of the COVID-19 pandemic in Hong Kong based on their medical records and test results (features) collected during their hospitalization. The correlation between the values of these features is studied against discharge status and disease severity as a preliminary step to identify those features with a more pronounced effect on the patient outcome. Once identified, they constitute the inputs of four machine learning models, Decision Tree, Random Forest, Gradient and RUSBoosting, which predict both the Mortality and Severity associated with the disease. We test the accuracy of the models when the number of input features is varied, demonstrating their stability; i.e., the models are already highly predictive when run over a core set of (6) features. We show that Random Forest and Gradient Boosting classifiers are highly accurate in predicting patients’ Mortality (average accuracy ∼99%) as well as categorize patients (average accuracy ∼91%) into four distinct risk classes (Severity of COVID-19 infection). Our methodical and broad approach combines statistical insights with various machine learning models, which paves the way forward in the AI-assisted triage and prognosis of COVID-19 cases, which is potentially generalizable to other seasonal flus
