Participating in a community of learners enhances resident perceptions of learning in an e-mentoring program: proof of concept
Background
Community learning and e-mentoring, learning methods used in higher education, are not used to any extent in residency education. Yet both have the potential to enhance resident learning and, in the case of community learning, introduce residents to basic lifelong learning skills. We set out to determine whether residents participating in an Internet based e-mentoring program would, with appropriate facilitation, form a community of learners (CoL) and hold regular community meetings. We also determined resident and faculty perceptions of CoL and Internet sessions as effective learning experiences. Methods
A six-month e-mentoring pilot was offered to 10 Radiology residents in the Aga Khan University Postgraduate Medical Education Program in Nairobi, Kenya (AKUHN) with a Professor of Radiology, located at University of Virginia, USA, acting as the e-mentor. Monthly Internet case-based teaching sessions were facilitated by the e-mentor. In addition, residents were coached by a community facilitator to form CoL and collectively work through clinical cases at weekly face-to-face CoL sessions.
Event logs described observed resident activity at CoL sessions; exit survey and interviews were used to elicit perceptions of CoL and Internet sessions as effective learning experiences. Results
Resident adoption of CoL behaviors was observed, including self-regulation, peer mentoring and collaborative problem solving. Analysis revealed high resident enthusiasm and value for CoL. Surveys and interviews indicated high levels of acceptance of Internet learning experiences, although there was room for improvement in audio-visual transmission. Faculty indicated there was a need for a larger multi-specialty study. Conclusions
The pilot demonstrated resident acceptance of community building and collaborative learning as valued learning experiences, addressing one barrier to its formal adoption in residency education curricula. It also highlighted the potential of e-mentoring as a means of expanding faculty and teaching materials in residency programs in developing countries
Using Microsatellite DNA to Understand Bobwhite Behavior and Population Structure (Post Abstract)
Northern bobwhite (Colinus virginianus) have a flexible mating system with varying degrees of parental investment in offspring. Questions of relatedness of mates and the dynamics of covey membership have not been answered. It is not known how different patterns of mating systems impact productivity of bobwhite populations. In addition to behaviors, the genetic structure of bobwhite populations likely varies across landscapes, and may depend on the distribution and abundance of habitat types. These issues have critical conservation and management implications, such as the impact of habitat fragmentation on gene flow. Recent advances in molecular techniques provide an opportunity to investigate these questions through examination of small, repetitive, highly variable regions of DNA known as microsatellites. Microsatellites provide the fine-scale resolution needed to objectively understand certain population structures and reproductive strategies. Microsatellite analysis techniques have been used successfully to research relatedness and extra-pair paternity of a number of species. Therefore, we have begun to investigate the genetic basis for many northern bobwhite behaviors related to reproduction. Our primary objectives are to determine: (1) relatedness of individuals within coveys and groups over time, (2) relatedness between reproductive pairs relative to random pairings, (3) relatedness of chicks in broods, (4) rates of extrapair fertilization, (5) rates of intra-specific nest parasitism, and (6) the relatedness of incubating birds to their eggs. To do so, we have developed a series of microsatellite markers for northern bobwhites. We radiomarked approximately 75% of bobwhites on our study area at Tall Timbers Research Station. We collected body feathers from adults and 1.5 mm patagial micro-biopsies from each chick (4 days old) found brooding with radiomarked adults for microsatellite analysis. We are determining relatedness and parentage, based on these microsatellite data, using programs RELATEDNESS and CERVUS. Additionally, we are testing our tissue collection techniques on a pen-reared population of bobwhites to determine the efficiency of DNA amplification via the polymerase chain reaction for 4 tissue collection techniques: patagial micro-biopsy, down and feather shaft, egg tooth, and egg membrane. Additionally, we are pairing pen-reared adult hens and males for different periods of time to determine the presence of sperm storage and test for evidence of sperm competition. Further, we are crossing individuals of known relatedness over several generations to test the accuracy of paternity inferences calculated by CERVUS with respect to bobwhite microsatellite data. Finally, by collecting feathers from hunterkilled bobwhites throughout the Red Hills and the southeast, we will compare the genetic structure of the bobwhite population(s) of the Red Hills, likely the last panmictic population in the southeast, to more isolated, declining populations. The Sexual Proclivities of Northern Bobwhites
Mating systems and mating behavior are historically viewed as static characteristics of populations. ‘Monogamy’, ‘polygamy’, ‘multiple clutch polygamy’, ‘polyandry’, etc. imply most individuals express similar social and genetic mating patterns and characterize the behavior of most or all individuals in a given population. Mating systems of different populations of northern bobwhites (Colinus virginianus) have been described as monogamous, polygamous, multi-clutch polygamous, ambisexually polygamous, and polyandrous in contrast to the expectation that behavior, at the species level, fits defined categories. Prior studies of bobwhite breeding behavior, to arrive at these classifications, were based on observations of social interactions and did not incorporate genetic outcomes. Thus, it is challenging to discern whether social behaviors predict genetic outcomes and whether bobwhite mating behavior varies among populations or if behavior is flexible within populations. We used 3 years of field observations and variation at 14 microsatellite loci of 601 adult and 841 neonatal bobwhites to estimate rates of extra-pair paternity in bobwhite broods, evaluate the utility of social behavior as an indicator of genetic outcomes, and evaluate the fit of bobwhite social and genetic behavior to classic mating system theories. Extra-pair paternity occurred in \u3e50% of bobwhite broods, whereas extra-pair matings resulted in few (x¯= 1.1) extra-pair offspring per nest. Social interactions between female and male bobwhites generally predicted the father of most offspring in a brood, but social interactions did not predict extra-pair matings better than chance. The mating behavior of individual females changed within and between breeding seasons, suggesting individual reproductive decisions of females were flexible. Teleradiology usage and user satisfaction with the telemedicine system operated by Médecins Sans Frontières
Médecins Sans Frontières (MSF) began a pilot trial of store-and-forward telemedicine in 2010, initially operating separate networks in English, French, and Spanish; these were merged into a single, multilingual platform in 2013. We reviewed the pattern of teleradiology usage on the MSF telemedicine platform in the 4-year period from April 2010. In total, 564 teleradiology cases were submitted from 22 different countries. A total of 1114 files were uploaded with the 564 cases, the majority being of type JPEG (n = 1081, 97%). The median file size was 938 kb (interquartile range, IQR 163-1659). A panel of 14 radiologists was available to report cases, but most (90%) were reported by only 4 radiologists. The median radiologist response time was 6.1 h (IQR 3.0-20). A user satisfaction survey was sent to 29 users in the last 6 months of the study. There was a 28% response rate. Most respondents found the radiologist's advice helpful and all of them stated that the advice assisted in clarification of a diagnosis. Although some MSF sites made substantial use of the system for teleradiology, there is considerable potential for expansion. More promotion of telemedicine may be needed at different levels of the organization to increase engagement of staff
Paediatric radiology seen from Africa. Part I: providing diagnostic imaging to a young population
Article approval pendingPaediatric radiology requires dedicated equipment, specific precautions related to ionising radiation, and specialist knowledge. Developing countries face difficulties in providing adequate imaging services for children. In many African countries, children represent an increasing proportion of the population, and additional challenges follow from extreme living conditions, poverty, lack of parental care, and exposure to tuberculosis, HIV, pneumonia, diarrhoea and violent trauma. Imaging plays a critical role in the treatment of these children, but is expensive and difficult to provide. The World Health Organisation initiatives, of which the World Health Imaging System for Radiography (WHIS-RAD) unit is one result, needs to expand into other areas such as the provision of maintenance servicing. New initiatives by groups such as Rotary and the World Health Imaging Alliance to install WHIS-RAD units in developing countries and provide digital solutions, need support. Paediatric radiologists are needed to offer their services for reporting, consultation and quality assurance for free by way of teleradiology. Societies for paediatric radiology are needed to focus on providing a volunteer teleradiology reporting group, information on child safety for basic imaging, guidelines for investigations specific to the disease spectrum, and solutions for optimising imaging in children
Brown Dwarf Candidates in the JADES and CEERS Extragalactic Surveys
By combining the JWST/NIRCam JADES and CEERS extragalactic datasets, we have
uncovered a sample of twenty-one T and Y brown dwarf candidates at best-fit
distances between 0.1 - 4.2 kpc. These sources were selected by targeting the
blue 1m - 2.5m colors and red 3m - 4.5m colors that arise
from molecular absorption in the atmospheres of T 1300K
brown dwarfs. We fit these sources using multiple models of low-mass stellar
atmospheres and present the resulting fluxes, sizes, effective temperatures and
other derived properties for the sample. If confirmed, these fits place the
majority of the sources in the Milky Way thick disk and halo. We observe proper
motion for seven of the candidate brown dwarfs with directions in agreement
with the plane of our galaxy, providing evidence that they are not
extragalactic in nature. We demonstrate how the colors of these sources differ
from selected high-redshift galaxies, and explore the selection of these
sources in planned large-area JWST NIRCam surveys. Deep imaging with
JWST/NIRCam presents an an excellent opportunity for finding and understanding

Efficacy of transoral fundoplication vs omeprazole for treatment of regurgitation in a randomized controlled trial.
Background
The aim of this randomized, crossover study was to determine if transoral fundoplication (TF) could further improve clinical outcomes in partial responders to high-dose (HD) proton-pump inhibitor (PPI) therapy and to evaluate durability of TF. Methods
In seven United States centers, patients with hiatal hernia ≤2 cm and abnormal esophageal acid exposure (EAE) were randomized to TF (n = 40) or HD PPIs (n = 23) group. At 6-month follow-up, PPI patients underwent crossover. We assessed clinical outcomes 6-month post TF in crossover patients (COP), as compared to 6-month of HD PPI therapy, and 12-month outcomes in patients initially randomized to TF. The primary outcome was symptom control evaluated by Reflux Disease Questionnaire and Reflux Symptom Index. Secondary outcomes included healing of esophagitis, normalization of EAE and PPI use after TF. We analyzed 21 COP and 39 TF patients. McNemar’s test or Fisher exact test was used to compare proportions. Results
Of 63 randomized patients, 3 were lost to follow-up, leaving 39 TF and 21 COP for analyses. In the COP, TF further improved control of regurgitation and of atypical symptoms achieved after six months of HD PPIs. Of 20 patients with GERD symptoms after six months of high-dose PPI therapy, 65% (13/20) reported global elimination of troublesome regurgitation and atypical symptoms post TF off PPIs; 67% (6/9) reported no troublesome regurgitation. Esophagitis further healed in 75% (6/8) of patients. Seventy-one percent of COP patients were off PPIs six months following TF. Normalization of EAE decreased from 52% after HD PPIs (on PPIs) to 33% after TF (off PPIs), p =0.388. In the original TF group, 12-month post TF, 77% of patients achieved complete symptom control, 82% ceased PPI therapy, 100% healed esophagitis and 45% normalized EAE. Conclusions
The results of this study indicate that in patients with incomplete symptom control on high-dose PPI therapy TF may provide further elimination of symptoms and esophagitis healing. In the original TF group, the clinical outcomes of TF remained stable between 6- and 12-month follow-up. Trial registration

JADES: Using NIRCam Photometry to Investigate the Dependence of Stellar Mass Inferences on the IMF in the Early Universe
The detection of numerous and relatively bright galaxies at redshifts z > 9
has prompted new investigations into the star-forming properties of
high-redshift galaxies. Using local forms of the initial mass function (IMF) to
estimate stellar masses of these galaxies from their light output leads to
galaxy masses that are at the limit allowed for the state of the LambdaCDM
Universe at their redshift. We explore how varying the IMF assumed in studies
of galaxies in the early universe changes the inferred values for the stellar
masses of these galaxies. We infer galaxy properties with the SED fitting code
Prospector using varying IMF parameterizations for a sample of 102 galaxies
from the JWST Advanced Deep Extragalactic Survey (JADES) spectroscopically
confirmed to be at z > 6.7, with additional photometry from the JWST
Extragalactic Medium Band Survey (JEMS) for twenty-one galaxies. We demonstrate
that models with stellar masses reduced by a factor of three or more do not

To high redshift and low mass: exploring the emergence of quenched galaxies and their environments at in the ultra-deep JADES MIRI F770W parallel
We present the robust selection of quiescent (QG) and post-starburst (PSB)
galaxies using ultra-deep NIRCam and MIRI imaging from the JWST Advanced Deep
Extragalactic Survey (JADES). Key to this is MIRI 7.7m imaging which
breaks the degeneracy between old stellar populations and dust attenuation at
by providing rest-frame -band. Using this, we identify 23 passively
evolving galaxies in UVJ color space in a mass-limited (log
) sample over 8.8 arcmin. Evaluation of this
selection with and without 7.7m shows that dense wavelength coverage
with NIRCam ( bands including medium-bands) can compensate for
lacking the band anchor, meaning that robust selection of high-redshift QGs
is possible with NIRCam alone. Our sample is characterized by rapid quenching
timescales ( Myr) with formation redshifts
and includes a potential record-holding massive QG at and two QGs with evidence for significant residual
dust content (). In addition, we present a large sample of 12
log PSBs, demonstrating that UVJ selection can be
extended to low mass. Analysis of the environment of our sample reveals that
the group known as the Cosmic Rose contains a massive QG and a dust-obscured
star-forming galaxy (a so-called Jekyll and Hyde pair) plus three additional
QGs within kpc. Moreover, the Cosmic Rose is part of a larger
overdensity at which contains 7/12 of our low-mass PSBs. Another 4
low-mass PSBs are members of an overdensity at ; this result strongly
indicates low-mass PSBs are preferentially associated with overdense
