Biological mechanisms of antipsychotics adverse reactions in the field of sexual dysfunctions in schizophrenia
A number of patients with schizophrenia suffer from drug-related sexual and endocrine problems. Questionnaire
studies suggest that 40-60% of the patients report sexual dysfunctions. Sexual dysfunction in patients with schizophrenia
seems to be associated with the disease itself, psychosocial factors, medical health, as well as to the use of
antypsychotics. Many of the conventional or typical antipsychotics have been associated with sexual dysfunction,
such as decreased libido, erectile dysfunction, anorgasmia, delayed or retrograde ejaculation, oligomenorrhoea, and
amenorrhoea. The contemporary literature hardly gives any information on systematic studies on the frequency and
quality of sexual functioning in patients with schizophrenia. The limited information available suggests that some
antipsychotics often induce sexual side effects and comparative studies are almost completely absent. The underlying
mechanisms of sexual side effects induced by antipsychotics are only partly known. Several mechanisms contribute to
the effects of antipsychotics on sexual functioning. Dopamine seems to be involved in almost all aspects of sexual
behaviour and antipsychotics induced dopamine blockade playing a role in the impairment of all stages of sexual
performance. The prolactin elevation associated with dopamine blockade interferes with erection as well as with
other aspects of sexual performance in a dose dependent manner. Also some antipsychotics induce noradrenergic
blockade, which is associated with disturbances in ejaculation and possibly lubrication. In addition serotonergic,
histaminergic and hormonal effects contribute to the actual sexual performance. The aim of this paper is to depict the
mechanisms underlying the sexual dysfunction associated with antipsychotic treatment.Wielu pacjentów chorujących na schizofrenię cierpi z powodu zaburzeń seksualnych i endokrynologicznych związanych
z przyjmowanymi lekami. Według badań kwestionariuszowych 40-60% pacjentów zgłasza zaburzenia
seksualne. Zaburzenia seksualne u pacjentów ze schizofrenia mogą być związane z samą chorobą, zaburzeniami
somatycznymi, czynnikami psychospołecznymi, jak również ze stosowaniem leków przeciwpsychotycznych. Zaburzenia
seksualne takie jak: spadek libido, zaburzenia erekcji, anorgazmia, opóźniona lub wsteczna ejakulacja,
zaburzenia miesiączkowania (oligo-, amenorrhea) związane są ze stosowaniem zarówno klasycznych jak i atypowych
neuroleptyków. We współczesnym piśmiennictwie nie ma zbyt wielu informacji dotyczących badań nad
intensywnością i jakością życia seksualnego chorych na schizofrenię. Ograniczona ilość danych wskazuje, że niektóre
leki przeciwpsychotyczne często powodują działania niepożądane z zakresu funkcji seksualnych, ale brak
jest badań porównawczych. Mechanizmy powodujące działania niepożądane w postaci dysfunkcji seksualnych
zostały częściowo poznane. Do wystąpienia zaburzeń seksualnych może się przyczynić kilka mechanizmów. Wydaje
się, że dopamina odgrywa rolę w prawie wszystkich aspektach zachowań seksualnych a leki przeciwpsychotyczne
blokujące jej działanie mają udział w upośledzaniu prawie wszystkich etapów aktu seksualnego. Podwyższone
stężenie prolaktyny w połączeniu z blokowaniem działania dopaminy zaburza mechanizm erekcji, podobnie
jak innych aspektów aktywności seksualnej, w sposób zależny od dawki. Niektóre neuroleptyki blokują działanie
adrenergiczne, co związane jest z zaburzeniami ejakulacji i prawdopodobnie także lubrykacji. Działanie serotoninergiczne,
histaminergiczne i efekty hormonalne, również mają wpływ na funkcje seksualne. Celem tej pracy jest
przedstawienie mechanizmów leżących u podstaw zaburzeń funkcji seksualnych związanych z leczeniem przeciwpsychotycznym
Factors contributing to the incidence of adverse drug reactions in a course of therapy with zolpidem
Zolpidem jest krótkodziałającym lekiem nasennym, należącym do rodziny pochodnych imidazopirydyny. Lek wywiera
działanie nasenne przez wiązanie z kompleksem receptora benzodiazepinowego GABA-A, łącząc się preferencyjnie
z tymi receptorami, które zawierają podjednostkę alfa 1. Mechanizm ten, w odróżnieniu od benzodiazepin,
prawdopodobnie wiąże się z niższym ryzykiem wystąpienia uzależnienia. W literaturze jednak opisano wiele
przypadków szkodliwego używania, uzależnienia oraz działań niepożądanych z napadami drgawkowymi włącznie,
które są związane z farmakoterapią przy użyciu zolpidemu.
W wyższym zakresie dawek działanie kliniczne zolpidemu wydaje się porównywalne z działaniem benzodiazepin.
Dlatego używanie szkodliwe, uzależnienie i napady drgawkowe w przebiegu zespołu abstynencyjnego należą do
spektrum działań niepożądanych leku.
Rozwinięcie działań niepożądanych w dużej mierze jest zdeterminowane czynnikami farmakodynamicznymi oraz
farmakokinetycznymi. Dodatkowo czynnikiem takim wydaje się płeć pacjenta. Praca stanowi zwięzły przegląd
czynników przyczyniających się do zwiększonego ryzyka rozwinięcia działań niepożądanych w trakcie leczenia
zolpidemem w kontekście bezpieczeństwa oraz skuteczności prowadzonej farmakoterapii.Zolpidem is a short acting hypnotic drug belonging to imidazopyridine family. It produces its hypnotic effects via
the GABA-A benzodiazepine receptor complex and binds preferentially to those receptors containing the alpha-1
subunit. In comparison with benzodiazepines this mechanism is thought to reduce liability to induce dependence.
However, several case reports of zolpidem abuse and dependence have been published along with a small number
cases demonstrating seizures after sudden zolpidem withdrawal.
At higher doses the clinical effects of zolpidem seem to be comparable to those of benzodiazepines and abuse,
dependence and withdrawal seizures belong to the spectrum of its adverse drug reactions (ADRs).
The susceptibility factors for adverse drug reactions include pharmacokinetic and pharmacodynamic issues. Moreover,
an important issue of patients’ sex as the susceptibility factor for ADRs is addressed. The paper briefly
discusses that issue and emphasizes role of susceptibility factors for zolpidem ADRs with regard to efficacy and
safety in scope of patient’s zolpidem pharmacotherapy
Trazodone - efficacy and safety in clinical practice
Trazodon należy do grupy antagonistów receptora 5-HT2 i inhibitorów wychwytu zwrotnego serotoniny (SARI).
Jest lekiem przeciwdepresyjnym, który stosuje się w szerokim spektrum zaburzeń, począwszy od zaburzeń afektywnych
i zaburzeń snu, kończąc na farmakoterapii uzależnień. Lek ten jest obecnie jedynym przedstawicielem tej
grupy zarejestrowanym w Polsce. Ze względu na charakterystyczny profil działania trazodon stosuje się zarówno w
monoterapii, jak i w połączeniu z innymi preparatami. Celem pracy jest przegląd właściwości farmakologicznych
trazodonu ze szczególnym odniesieniem do praktyki klinicznej. Omówiono również działania niepożądane związane
ze stosowaniem trazodonu.Trazodone is a SARI (serotonin(2A) antagonists/reuptake inhibitor) antydepressive drug which is commonly used in
a wide variety of mental disorders from mood and sleep disorders to the pharmacotherapy of the drugs abuse. At
present, trazodone is the only SARI representative registered in Poland. The characteristic profile of trazodone
action makes it a suitable pharmaceutical agent in both monotherapy and in combined therapy with other drugs.
The aim of this paper is to review the pharmacological properties of trazodone in scope of the contemporary
clinical practice with its therapeutical applications, drug tolerance, safety and side-effects
DIFFUSION-WEIGHTED IMAGING OF THE BRAIN IN BIPOLAR DISORDER: A CASE REPORT
Background: Some investigations strongly support a role of glial abnormalities in the pathophysiology of bipolar disorder. The
degree of white matter axonal and myelination disruption is measured through the rate of water molecule diffusion. High ADC
measures correspond to relatively unimpeded water diffusion, while low ADC measures reflect preserved myelinated axons.
Case report: Parietal and occipital areas may be involved in the pathophysiology of bipolar disorder, particularly in cognition
and perception, along with the prefrontal and temporal cortices for the disruption of emotional processing. In the literature the
widespread alterations of the cortical white matter microstructure is documented.
Conclusions: This case reports demonstrates the features of the increased mean ADC values in the left occipital lobe. Future
DWI studies are expected to investigate the correlation of white matter changes with the functional impairment, which often persists
during euthymia in bipolar disorder

Trazodon należy do grupy antagonistów receptora 5-HT2 i inhibitorów wychwytu zwrotnego serotoniny.
Jest lekiem przeciwdepresyjnym, który stosuje się w szerokim spektrum zaburzeń, począwszy od zaburzeń afektywnych
i zaburzeń snu, kończąc na farmakoterapii uzależnień. Celem pracy był przegląd zastosowań terapeutycznych
trazodonu, ze szczególnym odniesieniem do praktyki klinicznej farmakoterapii depresji, gdzie w związku
z charakterystycznym profilem farmakodynamicznym lek ten stosuje się zarówno w monoterapii, jak
i w połączeniu z innymi preparatami. Omówiono również zastosowanie leku w zaburzeniach lękowych oraz
w zaburzeniach snu, które należą do klasycznych zastosowań terapeutycznych trazodonu, a także wskazano na
możliwe zastosowanie leku w wybranych postaciach zaburzeń zachowania w przebiegu otępienia oraz w leczeniu
używania szkodliwego/uzależnienia od substancji psychoaktywnych.

along with its use in behavioural symptoms of dementia and substance use/abuse.

MR FINDINGS IN NEUROSYPHILIS – A LITERATURE REVIEW WITH A FOCUS ON A PRACTICAL APPROACH TO NEUROIMAGING
Background: Syphilis is a sexually transmitted disease caused by Treponema pallium. The invasion of the central nervous system
is observed in 5-10% of untreated patients and may occur at any stage of the disease. The diagnosis of the early stage of syphilitic
infection is complex as many patients present either nonspecific symptoms or are asymptomatic. The identification of the most
common radiologic characteristics of neurosyphilis is important in the proper diagnosis of the infection.
Method: The paper reviews the MR findings in neurosyphilis presented in the literature with the emphasis on common traits,
patterns and factors in MR neuroimaging in neurosyphilis.
Results: Twelve papers were selected as suitable for analysis. Psychiatric symptomatology was the most common clinical
manifestations of neurosyphilis. As MRI findings in neurosyphilis were not highly specific it proved being the most sensitive
technique in detecting the changes in cerebral vasculitis commonly presenting with focal areas of high signal intensity in any
distribution with areas of infarction and hemorrhage observed at times. In neurosyphilis medium and small vessels are usually
involved. Parenchymal and meningeal enhancement was usually demonstrated. The most frequently involved arteries are middle
cerebral artery and branches of the basilar artery. However, normal findings were also common.
Conclusions: The most common MR diagnostic features are medium contrast enhancement, atrophy, white matter lesions,
cerebral infarction, oedema. Radiologists, neurologists, and psychiatrists should take neurosyphilis into consideration when facing
the differential diagnosis in central vascuclar disorders
CORTISOL AS AN INDICATOR OF HYPOTHALMIC-PITITUARY-ADRENAL AXIS DYSREGULATION IN PATIENTS WITH PANIC DISORDER: A LITERATURE REVIEW
Dysregulation of hypothalamic–pituitary-adrenal axis (HPA) is seen in numerous mental disorders. Data of HPA axis
disturbance in panic disorder are inconsistent. In panic disorder HPA axis hyperactivity has been observed with elevated cortisol
levels. However, hypocortisolism has also been noted. Salivary cortisol as a biomarker of HPA-axis activity has received special
attention. The aim of this paper is to review the findings on cortisol levels in panic disorder

IMPULSIVITY IN ANXIETY DISORDERS. A CRITICAL REVIEW
Background: Anxiety symptoms and disorders are common. High comorbidity between anxiety and other psychiatric disorders
has been observed in community. Still, the relationship between impulsivity and anxiety disorders is controversial and not well
explored.
Material and methods: The aim of this paper is to review measures of trait impulsivity in anxiety disorders. A literature review
of the theoretical bases of the relationship between anxiety disorders and impulsivity is presented.
Results: Impulsivity is a key feature of numerous psychiatric disorders. Traditional conceptualizations suggest that impulsivity
might display a negative relationship with anxiety. However, an association of impulsivity in patients with anxiety disorders is
present. Some studies support proposition that anxiety may influence impulsivity in individuals with predisposition toward behavioural
disinhibition.
Conclusion: There is a link between anxiety and impulsivity in psychiatric patients characterized by problems with impulse
control (e.g. pathological gambling, self-harming behaviour, eating disorders), mood disorders and anxiety disorders. Behavioural
and pharmacological interventions for decreasing impulsivity may effectively be used in the treatment
