    ABC training : a new theory-based form of cognitive-bias modification to foster automatization of alternative choices in the treatment of addiction and related disorders

    Recent years have seen a surge in the popularity of interventions that target common distortions in thinking (cognitive-bias modification, or CBM). Although there is evidence of their effectiveness as add-ons to regular treatment in alcohol addiction, the effects are typically small, and recent findings from lab studies have called into question the dominant theoretical underpinnings of CBM. We provide a novel theoretical approach in terms of automatic inferences that integrates previous findings and suggests ways to improve CBM into ABC training. In ABC training, patients are trained in the context of personally relevant antecedents (A) to make behavioral choices (B) that accord with patients' health goals in light of their consequences (C). We discuss preliminary evidence suggesting that ABC training might be a useful tool in the treatment of addictions and related disorders

    The potential role of temporal dynamics in approach biases: delay-dependence of a general approach bias in an alcohol approach-avoidance task

    Attractive cues have been shown to evoke automatic approach biases in tasks such as the Automatic Approach Task or Stimulus Response Compatibility task. An important but as yet not studied question is the role of temporal dynamics in such tasks: the impact of automatic processes may depend on the interval between cue and response. The current proof of principle study tested this hypothesized time-dependence of the approach bias. Secondary goals included the exploration of effects of alcohol cues and virtual hand stimuli. 22 participants performed an SRC task in which the delay between the presentation of the cue and the possibility to select the response was manipulated. Results revealed an approach bias that decayed over longer delays. Thus, the approach bias was indeed dependent on processes that are transiently evoked by cues. The results did not show significant effects of alcohol cues or a virtual hand. Temporal dynamics may be an essential feature of approach biases

    What's in a Trial? On the Importance of Distinguishing Between Experimental Lab Studies and Randomized Controlled Trials: The Case of Cognitive Bias Modification and Alcohol Use Disorders

    OBJECTIVE: Recently, the National Institutes of Health (NIH) redefined clinical trials to include any study involving behavioral or biomedical interventions. In line with a general framework from experimental medicine, we argue that it is crucial to distinguish between experimental laboratory studies aimed at revealing psychological mechanisms underlying behavior and randomized controlled trials (RCTs) in clinical samples aimed at testing the efficacy of an intervention. METHOD: As an illustration, we reviewed the current state of the evidence on the efficacy of cognitive bias modification (CBM) interventions in alcohol use disorders. RESULTS: A recent meta-analysis "cast serious doubts on the clinical utility of CBM interventions for addiction." That analysis combined experimental laboratory studies and RCTs. We demonstrated that, when studies are differentiated regarding study type (experimental laboratory study or RCT), mode of delivery (controlled experiment or Internet), and population (healthy volunteers or patients), the following effects are found: (a) short-lived effects of CBM on drinking behavior in experimental laboratory studies in students, but only when the bias is successfully manipulated; (b) small but robust effects of CBM on treatment outcome when administered as an adjunct to established treatments in clinical settings in RCTs with alcohol-dependent patients; and (c) nonspecific effects (reduced drinking irrespective of condition) in RCTs of CBM administered online to problem drinkers. CONCLUSIONS: We discuss how CBM might be improved when it is better integrated into regular treatment, especially cognitive behavioral therapy, and we conclude that disregarding the difference between experimental laboratory studies and RCTs can lead to invalid conclusions

    Impulsivity, Impulsive and Reflective Processes and the Development of Alcohol Use and Misuse in Adolescents and Young Adults

    This paper contrasts dual-process and personality approaches in the prediction of addictive behaviors and related risk behaviors. In dual-process models, behavior is described as the joint outcome of qualitatively different “impulsive” (or associative) and “reflective” processes. There are important individual differences regarding both types of processes, and the relative strength of both in a specific situation is influenced by prior behavior and state variables (e.g., fatigue, alcohol use). From this perspective, a specific behavior (e.g., alcohol misuse) can be predicted by the combined indices of the behavior-related impulsive processes (e.g., associations with alcohol), and reflective processes, including the ability to refrain from a motivationally salient action. Personality approaches have reported that general traits such as impulsivity predict addictive behaviors. Here we contrast these two approaches, with supplementary analyses on four datasets. We hypothesized that trait impulsivity can predict specific risky behaviors, but that its predictive power disappears once specific behavior-related associations, indicators of executive functioning, and their interaction are entered into the equation. In all four studies the observed interaction between specific associations and executive control (EC) was robust: trait impulsivity did not diminish the prediction of alcohol use by the interaction. Trait impulsivity was not always related to alcohol use, and when it was, the predictive power disappeared after entering the interaction between behavior-specific associations and EC in one study, but not in the other. These findings are interpreted in relation to the validity of the measurements used, which leads to a more refined hypothesis

    A Clinical Trial with Combined Transcranial Direct Current Stimulation and Attentional Bias Modification in Alcohol Dependent Patients

    Modifying attentional processes with attentional bias modification (ABM) might be a relevant add-on to treatment in addiction. This study investigated whether influencing cortical plasticity with transcranial direct current stimulation (tDCS) could increase training effects. tDCS could also help alcohol-dependent patients to overcome craving and reduce relapse, independent of training. These approaches were combined to investigate effects in the treatment of alcoholism. Ninety-eight patients (analytical sample = 83) were randomly assigned to 4 groups in a 2-by-2 factorial design. Patients received 4 sessions of ABM (control or real training) combined with 2 mA tDCS (active: 20 minutes or sham: 30 seconds) over the left dorsolateral prefrontal cortex. Alcohol bias and craving were assessed, and treatment outcome was measured as relapse after 1 year. Attentional bias scores indicated that during the training only the group with active tDCS and real ABM displayed an overall avoidance bias (p  0.2). However, effects on relapse after active tDCS were in the expected direction. There was no evidence of a beneficial effect of tDCS or ABM or the combination. Whether the absence of effect was due to issues with the outcome measurements (e.g., lack of craving, high dropout, and unreliable measurements) or aspects of the intervention should be further investigated.     Implicit Associations and Explicit Expectancies toward Cannabis in Heavy Cannabis Users and Controls

    Cognitive biases, including implicit memory associations are thought to play an important role in the development of addictive behaviors. The aim of the present study was to investigate implicit affective memory associations in heavy cannabis users. Implicit positive-arousal, sedation, and negative associations toward cannabis were measured with three Single Category Implicit Association Tests (SC-IAT’s) and compared between 59 heavy cannabis users and 89 controls. Moreover, we investigated the relationship between these implicit affective associations and explicit expectancies, subjective craving, cannabis use, and cannabis related problems. Results show that heavy cannabis users had stronger implicit positive-arousal associations but weaker implicit negative associations toward cannabis compared to controls. Moreover, heavy cannabis users had stronger sedation but weaker negative explicit expectancies toward cannabis compared to controls. Within heavy cannabis users, more cannabis use was associated with stronger implicit negative associations whereas more cannabis use related problems was associated with stronger explicit negative expectancies, decreasing the overall difference on negative associations between cannabis users and controls. No other associations were observed between implicit associations, explicit expectancies, measures of cannabis use, cannabis use related problems, or subjective craving. These findings indicate that, in contrast to other substances of abuse like alcohol and tobacco, the relationship between implicit associations and cannabis use appears to be weak in heavy cannabis users

    Training Working Memory in Adolescents Using Serious Game Elements: Pilot Randomized Controlled Trial

    Working memory capacity has been found to be impaired in adolescents with various psychological problems, such as addictive behaviors. Training of working memory capacity can lead to significant behavioral improvements, but it is usually long and tedious, taxing participants' motivation to train. This study aimed to evaluate whether adding game elements to the training could help improve adolescents' motivation to train while improving cognition. A total of 84 high school students were allocated to a working memory capacity training, a gamified working memory capacity training, or a placebo condition. Working memory capacity, motivation to train, and drinking habits were assessed before and after training. Self-reported evaluations did not show a self-reported preference for the game, but participants in the gamified working memory capacity training condition did train significantly longer. The game successfully increased motivation to train, but this effect faded over time. Working memory capacity increased equally in all conditions but did not lead to significantly lower drinking, which may be due to low drinking levels at baseline. We recommend that future studies attempt to prolong this motivational effect, as it appeared to fade over time.     Identifying psychosocial variables that predict safer sex intentions in adolescents and young adults

    Young people are especially vulnerable to sexually transmitted infections. The triad of deliberate and effective safer-sex behavior encompasses condom use, combined with additional information about a partner’s sexual health, and the kind of sex acts usually performed. To identify psychosocial predictors of young people’s intentions to have safer sex, as related to this triad we conducted an online study with 211 sexually active participants aged between 18 and 24 years. Predictors (i.e. perceived behavioural control, subjective norms and intention) taken from Fishbein and Ajzen’s Reasoned Action Approach (RAA), were combined with more distal variables (e.g. behavioral inhibition, sensation seeking, parental monitoring, and knowledge about sexually transmitted infections). Beyond the highly predictive power of RAA variables, additional variance was explained by the number of instances of unprotected sexual intercourse during the last twelve months and reasons for using barrier protection during first sexual intercourse. In particular, past condom nonuse behavior moderated perceived behavioral control related to intended condom use. Further, various distal variables showed significant univariate associations with intentions related to the three behaviors of interest. It may, therefore, be helpful to include measures of past behavior as well as certain additional distal variables in future safer-sex programs designed to promote health sustaining sexual behavior

    Problematic smartphone use and the quantity and quality of peer engagement among adolescents: A longitudinal study

    Problematic smartphone use (PSU) has recently attracted a lot of attention, especially among adolescents. The knowledge about the role peer engagement might play in the development of PSU is still limited. We aimed to investigate the bidirectional relationships between PSU, the quantity of online (i.e., passive and active social media messaging on smartphone) and offline peer engagement (i.e., intensity of face-to-face meeting with friends) and the quality of peer engagement (i.e., perceived competence in close friendships) among adolescents. Data from a three-wave longitudinal study among 2100 Dutch high school students (56.7% boys) was used. Cross-lagged models indicated that: (1) perceived competence in close friendships at T1 negatively predicted PSU at T2 and PSU at T2 negatively predicted perceived competence in close friendships at T3; (2) there were positive and reciprocal cross-lagged correlations between PSU and passive social media messaging on smartphone; (3) there were positive and reciprocal cross-lagged correlations between intensity of face-to-face meeting with friends and active social media messaging on smartphone. This implies that adolescents who perceive a low competence in close friendships and/or intensively check their smartphone for messages from their peers may be particularly vulnerable to developing problematic smartphone use over time

    A Test of Multisession Automatic Action Tendency Retraining to Reduce Alcohol Consumption Among Young Adults in the Context of a Human Laboratory Paradigm.

    BACKGROUND: Young adult heavy drinking is an important public health concern. Current interventions have efficacy but with only modest effects, and thus, novel interventions are needed. In prior studies, heavy drinkers, including young adults, have demonstrated stronger automatically triggered approach tendencies to alcohol-related stimuli than lighter drinkers. Automatic action tendency retraining has been developed to correct this tendency and consequently reduce alcohol consumption. This study is the first to test multiple iterations of automatic action tendency retraining, followed by laboratory alcohol self-administration. METHODS: A total of 72 nontreatment-seeking, heavy drinking young adults ages 21 to 25 were randomized to automatic action tendency retraining or a control condition (i.e., sham training ). Of these, 69 (54% male) completed 4 iterations of retraining or the control condition over 5 days with an alcohol drinking session on Day 5. Self-administration was conducted according to a human laboratory paradigm designed to model individual differences in impaired control (i.e., difficulty adhering to limits on alcohol consumption). RESULTS: Automatic action tendency retraining was not associated with greater reduction in alcohol approach tendency or less alcohol self-administration than the control condition. The laboratory paradigm was probably sufficiently sensitive to detect an effect of an experimental manipulation given the range of self-administration behavior observed, both in terms of number of alcoholic and nonalcoholic drinks and measures of drinking topography. CONCLUSIONS: Automatic action tendency retraining was ineffective among heavy drinking young adults without motivation to change their drinking. Details of the retraining procedure may have contributed to the lack of a significant effect. Despite null primary findings, the impaired control laboratory paradigm is a valid laboratory-based measure of young adult alcohol consumption that provides the opportunity to observe drinking topography and self-administration of nonalcoholic beverages (i.e., protective behavioral strategies directly related to alcohol use)
