    Reducing Postoperative Opioids After Minimally Invasive Gynecologic Surgery with Multimodal Pain Control

    Introduction: We evaluated the efficacy of a multimodal pain regimen that approaches pain control by utilizing different mechanisms of action. This novel protocol utilizing liposomal bupivacaine, acetaminophen, tramadol and oxycodone as needed in reducing the overall opioid use by patients after undergoing robotic-assisted total laparoscopic hysterectomy in an obese population that is heavily afflicted by the opioid epidemic. Materials and Methods: We conducted a retrospective study wherein a sample of 100 (50 multimodal group and 50 controls) were taken from 433 eligible cases conducted over a 1 year period. Patient medical records were evaluated for demographics, surgical characteristics, opioid type and dose, pain scores, length of stay and complications. Opioids were converted to oral morphine dose equivalents. Results: Overall opioid use in the multimodal group decreased by 54% (75.1mg versus 35.5mg, p Discussion: A multimodal approach to pain control is an acceptable alternative to traditional methods of pain control, regardless of BMI, for those with benign or malignant disease and decreases opioid use by 44 to 62 percent with no concomitant increase in pain scores and may decrease pain by 9 to 24 percent

    In this review article, we revisit the connection between dressing of scattering states in quantum electrodynamics by clouds of soft photons, and their dressing by (generalised) Wilson line operators. In particular, we show that the leading and subleading soft conformal dressings considered in the context of celestial holography can be straightforwardly obtained from generalised Wilson lines, and that this only requires knowledge of the asymptotic behaviour of the photon field near null and timelike infinity.Comment: 14 pages + appendice

    Seismic Performance Testing of Partially and Fully Anchored Wood-Frame Shear Walls

    Earthquake performance of wood-frame shear walls was evaluated by comparing fully and partially anchored walls under monotonic, cyclic, and earthquake loads and comparing with code measures. Suitability of monotonic and cyclic testing to predict seismic performance was examined. Earthquake tests were conducted on 2440-mm-square walls with Douglas-fir studs. Two oriented strandboard panels were fastened to the frame with two gypsum wallboard panels on the opposite side. Partially anchored walls had two anchor bolts on the sill plate. Fully anchored walls had hold-downs at the ends. Four time histories were tested: three subduction zone ground motions and a strike-slip fault, all scaled to the Seattle design level. For fully anchored walls, subduction zone tests had capacities, energy dissipation, and failure modes most similar to cyclic tests. Wall displacement at maximum load was under-estimated by cyclic and overestimated by monotonic tests. For partially anchored walls, subduction zone and strike-slip earthquake tests had capacity, displacement at maximum load, initial stiffness, and ductility most similar to cyclic tests. Energy dissipation was most similar to monotonic tests, and failure modes were consistent with monotonic and cyclic tests. Partially anchored walls had lower capacity, displacement at maximum load, energy dissipation, and stiffness as compared with fully anchored walls

    Palytoxin Found in Palythoa sp. Zoanthids (Anthozoa, Hexacorallia) Sold in the Home Aquarium Trade

    Zoanthids (Anthozoa, Hexacorallia) are colonial anemones that contain one of the deadliest toxins ever discovered, palytoxin (LD50 in mice 300 ng/kg), but it is generally believed that highly toxic species are not sold in the home aquarium trade. We previously showed that an unintentionally introduced zoanthid in a home aquarium contained high concentrations of palytoxin and was likely responsible for a severe respiratory reaction when an individual attempted to eliminate the contaminant colonies using boiling water. To assess the availability and potential exposure of palytoxin to marine aquarium hobbyists, we analyzed zoanthid samples collected from local aquarium stores for palytoxin using liquid chromatography and high resolution mass spectrometry and attempted to identify the specimens through genetic analysis of 16S and cytochrome c oxidase 1 (COI) markers. We found four specimens of the same apparent species of zoanthid, that we described previously to be responsible for a severe respiratory reaction in a home aquarium, to be available in three aquarium stores in the Washington D.C. area. We found all of these specimens (n = 4) to be highly toxic with palytoxin or palytoxin-like compounds (range 0.5–3.5 mg crude toxin/g zoanthid). One of the most potent non-protein compounds ever discovered is present in dangerous quantities in a select species of zoanthid commonly sold in the home aquarium trade

    Improving Postoperative Pain Control After Cesarean Delivery with Enhanced Recovery in Patients on Buprenorphine Therapy

    Abstract: Introduction: Prescription drug abuse presents a significant challenge to the management of post operative pain.Pain control amongst the opioid addicted patient can be especially challenging. We aimed to improve pain control after caesarian delivery with enhanced recovery in patients who are on buprenorphine medication-assisted therapy for the treatment of opioid addiction. Materials and Methods: We conducted a pilot study by implementing a protocol using liposomal bupivacaine injected at the time of cesarean delivery. Patients were then given 500mg acetaminophen every 4 hours, 800mg ibuprofen every 8 hours and 0.3mg IV buprenorphine every 6 hours as needed. The patients’ maintenance dosing of buprenorphine was divided into doses throughout the day. In addition, patients were ambulated 4 hours after surgery and had their catheter removed from their bladder as soon as they could safely ambulate. Eleven patients were prospectively recruited and then compared to a retrospective sample of seventeen patients.     Reducing Postoperative Opioids After Minimally Invasive Hysterectomy with Enhanced Recovery

    I Introduction: We evaluated the efficacy of various strategies utilized for the control of postoperative pain after minimally invasive hysterectomy. The primary enhanced recovery after surgery (ERAS) protocol of interest utilized premedication (acetaminophen, celecoxib and pregabalin), then intraoperative subcutaneous liposomal bupivacaine followed by scheduled oral acetaminophen and ibuprofen postoperatively. Patients also had tramadol and oxycodone as needed for moderate or severe breakthrough pain, respectively. Materials and Methods: We conducted a retrospective cohort study that included all patients who underwent minimally invasive hysterectomy (total laparoscopic hysterectomy and laparoscopic-assisted vaginal hysterectomy) for both benign and oncologic indications over a 2-year period. We then compared six protocols, with 3 being ERAS protocols and 3 as traditional pain control methods. The control group was comprised of the traditional pain control group without intraoperative placement of local analgesia. Patient medical records were evaluated for demographics, surgical characteristics, opioid type and dose, pain scores, length of stay and complications. Opioids were converted to oral morphine dose equivalents. Results: 954 patients were included within the 6 protocols.     Yorkie Promotes Transcription by Recruiting a Histone Methyltransferase Complex

    SummaryHippo signaling limits organ growth by inhibiting the transcriptional coactivator Yorkie. Despite the key role of Yorkie in both normal and oncogenic growth, the mechanism by which it activates transcription has not been defined. We report that Yorkie binding to chromatin correlates with histone H3K4 methylation and is sufficient to locally increase it. We show that Yorkie can recruit a histone methyltransferase complex through binding between WW domains of Yorkie and PPxY sequence motifs of NcoA6, a subunit of the Trithorax-related (Trr) methyltransferase complex. Cell culture and in vivo assays establish that this recruitment of NcoA6 contributes to Yorkie’s ability to activate transcription. Mammalian NcoA6, a subunit of Trr-homologous methyltransferase complexes, can similarly interact with Yorkie’s mammalian homolog YAP. Our results implicate direct recruitment of a histone methyltransferase complex as central to transcriptional activation by Yorkie, linking the control of cell proliferation by Hippo signaling to chromatin modification
