The clinical application of PET/CT: a contemporary review
The combination of positron emission tomography (PET) scanners and x-ray computed tomography (CT) scanners into a single PET/CT scanner has resulted in vast improvements in the diagnosis of disease, particularly in the field of oncology. A decade on from the publication of the details of the first PET/CT scanner, we review the technology and applications of the modality. We examine the design aspects of combining two different imaging types into a single scanner, and the artefacts produced such as attenuation correction, motion and CT truncation artefacts. The article also provides a discussion and literature review of the applications of PET/CT to date, covering detection of tumours, radiotherapy treatment planning, patient management, and applications external to the field of oncology
A stepped wedge cluster randomized control trial of dried blood spot testing to improve the uptake of hepatitis C antibody testing within UK prisons
Background: The prevalence of hepatitis C (HCV) is elevated within prison populations, yet diagnosis in prisons remains low. Dried blood spot testing (DBST) is a simple procedure for the detection of HCV antibodies; its impact on testing in the prison context is unknown. Methods: We carried out a stepped-wedge cluster-randomized control trial of DBST for HCV among prisoners within five male prisons and one female prison. Each prison was a separate cluster. The order in which the intervention (training in use of DBST for HCV testing and logistic support) was introduced was randomized across clusters. The outcome measure was the HCV testing rate by prison. Imputation analysis was carried out to account for missing data. Planned and actual intervention times differed in some prisons; data were thus analysed by intention to treat (ITT) and by observed step times. Results: There was insufficient evidence of an effect of the intervention on testing rate using either the ITT intervention time (OR: 0.84; 95% CI: 0.68�1.03; P = 0.088) or using the actual intervention time (OR: 0.86; 95% CI: 0.71�1.06; P = 0.153). This was confirmed by the pooled results of five imputed data sets. Conclusions: DBST as a stand-alone intervention was insufficient to increase HCV diagnosis within the UK prison setting. Factors such as staff training and allocation of staff time for regular clinics are key to improving service delivery. We demonstrate that prisons can conduct rigorous studies of new interventions, but data collection can be problematic. SafeCare®: Historical Perspective and Dynamic Development of an Evidence-Based Scaled-Up Model for the Prevention of Child Maltreatment
AbstractSafeCare is an evidence-based parent-training program that reduces child maltreatment, particularly neglect. The risk of child maltreatment, a public health issue affecting millions of U.S. children each year, can be markedly reduced by interventions such as SafeCare that deliver in-home services. Drawing from applied behavioral analysis roots, SafeCare focuses on providing parents with concrete skills in three areas: health, home safety, and parent-child/-infant interaction. This paper will include an overview of the SafeCare model, an historical perspective of its history and dynamic development, description of the theoretical underpinnings of the model, a description of the program targets and content by describing its modules and delivery, an overview of program outcomes, and data discussion of dissemination and implementation
Distinct fos-expressing neuronal ensembles in the ventromedial prefrontal cortex mediate food reward and extinction memories
In operant learning, initial reward-associated memories are thought to be distinct from subsequent extinction-associated memories.
Memories formed during operant learning are thought to be stored in “neuronal ensembles.” Thus, we hypothesize that different
neuronal ensembles encode reward- and extinction-associated memories. Here, we examined prefrontal cortex neuronal ensembles
involved in the recall of reward and extinction memories of food self-administration.Wefirst trained rats to lever press for palatable food
pellets for 7 d (1 h/d) and then exposed them to 0, 2, or 7 daily extinction sessions in which lever presses were not reinforced. Twenty-four
hours after the last training or extinction session, we exposed the rats to either a short 15 min extinction test session or left them in their
homecage (a control condition). We found maximal Fos (a neuronal activity marker) immunoreactivity in the ventral medial prefrontal
cortex of rats that previously received 2 extinction sessions, suggesting that neuronal ensembles in this area encode extinction memories.
We then used the Daun02 inactivation procedure to selectively disrupt ventral medial prefrontal cortex neuronal ensembles that were
activated during the 15 min extinction session following 0 (no extinction) or 2 prior extinction sessions to determine the effects of
inactivating the putative food reward and extinction ensembles, respectively, on subsequent nonreinforced food seeking 2 d later.
Inactivation of the food reward ensembles decreased food seeking, whereas inactivation of the extinction ensembles increased food
seeking. Our results indicate that distinct neuronal ensembles encoding operant reward and extinction memories intermingle within the
same cortical area
Application of Focal Conflict Theory to Psychoeducational Groups: Implications for Process, Content, and Leadership
Group psychoeducation is a common group type used for a range of purposes. The literature presents balancing content and process as a challenge for psychoeducational group leaders. While the significance of group psychoeducation is supported, practitioners are given little direction for addressing process in these groups. Focal Conflict Theory (FCT) is a model for conceptualizing and intervening in group process that has been applied to therapy and work groups. This article presents the challenges of psychoeducational groups, describes FCT, and discusses its application to psychoeducational groups using case examples. Implications for leaders of psychoeducation groups are discussed
The Impact of Massive Stars on the Formation of Young Stellar Clusters
Massive OB stars play an important role in the evolution of molecular clouds
and star forming regions. The OB stars both photo--ionize molecular gas as well
as sweep up and compress interstellar gas through winds, ionization fronts, and
supernovae. In this contribution, we examine the effect massive stars have on
the formation of young stellar clusters. We first discuss the processes by
which OB stars destroy cluster--forming molecular cores, and hence terminate
star formation. We overview observational evidence that OB stars forming in
young stellar clusters destroy their parental cores on a timescale of 0.1 Myr,
and we discuss some of the implications of this result. We then summarize
extensive observations of the NGC 281 molecular cloud complex, and present
evidence that two types of triggered star formation are occurring in this
complex. Our goal is to underscore the impact massive stars have on cluster
