Concepts and strategies
This review focuses on the most recent advances in the diagnostic and prognostic work-up of adult acute lymphoblastic leukemia (ALL), and on their implications in the clinical management of the disease. Over the years, information obtained through extensive immunophenotyping, karyotyping, molecular genetics, multidrug resistance and, more recently, genomic profiling is progressively contributing to a better understanding of the biology of this complex disease, to the identification of subgroups of patients with different clinical outcomes, to a more precise monitoring of minimal residual disease, to the use of different therapeutic protocols based on prognostic indicators and, finally, to the design of innovative and specific treatment strategies. The next few years will tell us if this biologically-guided approach, which is progressively individualizing the management of adult ALL patients, will ultimately impact on the prognosis of a disease that has stagnated over many decade.Esta revisĂŁo focaliza os mais recentes avanços no diagnĂłstico e prognĂłstico da leucemia linfoblástica aguda do adulto e suas implicações no manuseio clĂnico desta doença. Com o passar dos anos, informações obtidas atravĂ©s de extensa pesquisa em imunofenotipagem, citogenĂ©tica, genĂ©tica molecular, resistĂŞncia a mĂşltiplas drogas e, mais recentemente, perfil genĂ´mico tĂŞm contribuĂdo progressivamente para o melhor entendimento da biologia desta doença complexa, na identificação de sub grupos de pacientes com evolução clĂnica distintas, no mais preciso monitoramento da doença residual mĂnima, no uso de diferentes protocolos baseados em indicadores prognĂłsticos e, mais recentemente, tambĂ©m no desenho de tratamentos inovativos e especĂficos. High pseudotumor cerebri incidence in tretinoin and arsenic treated acute promyelocytic leukemia and the role of topiramate after acetazolamide failure
Dual differentiation therapy with arsenic trioxide and tretinoin (all-trans-retinoic acid; ATRA) for the management of low and intermediate risk acute promyelocytic leukemia has recently been recommended by the National Comprehensive Cancer Network. Some less common toxicities of the combination may have yet to be fully realized. Of ten patients we have treated thus far, five (50%) have developed pseudotumor cerebri. In one patient, temporary discontinuation of ATRA and initiation of acetazolamide controlled symptoms. In four patients, topiramate was substituted for acetazolamide to relieve symptoms and allow ATRA dose re-escalation. We conclude that providers should monitor for pseudotumor cerebri and consider topiramate if acetazolamide fails
