Cross-cultural adaptation and psychometric properties of the Brazilian-Portuguese version of the VSP-A (Vécu et Santé Perçue de l'Adolescent), a health-related quality of life (HRQoL) instrument for adolescents, in a healthy Brazilian population
<p>Abstract</p> <p>Background</p> <p>Health-related quality of life (HRQoL) assessment, encompassing the adolescents' perceptions of their mental, physical, and social health and well-being is increasingly considered an important outcome to be used to identify population health needs and to provide targeted medical care. Although validated instruments are essential for accurately assessing HRQoL outcomes, there are few cross-culturally adapted tools for use in Brazil, and none designed exclusively for use among adolescents. The Vécu et Santé Perçue de l'Adolescent (VSP-A) is a generic, multidimensional self-reported instrument originally developed and validated in France that evaluates HRQoL of ill and healthy adolescents.</p> <p>Purpose</p> <p>To cross-culturally adapt and validate the Brazilian-Portuguese version of the VSP-A, a generic HRQoL measure for adolescents originally developed in France.</p> <p>Methods</p> <p>The VSP-A was translated following a well-validated forward-backward process leading to the Brazilian version. The psychometric evaluation was conducted in a sample of 446 adolescents (14-18 years) attending 2 public high schools of São Gonçalo City. The adolescents self-reported the Brazilian VSP-A, the validated Psychosomatic Symptom Checklist and socio-demographic information. A retest evaluation was carried out on a sub-sample (n = 195) at a two-week interval.</p> <p>The internal construct validity was assessed through confirmatory factor analysis (CFA), multi-trait scaling analyses, Rasch analysis evaluating unidimensionality of each scale and Cronbach's alpha coefficients. The reproducibility was evaluated by intra-class correlation coefficients (ICC). Zumbo's ordinal logistic regression analysis was used to detect differential item functioning (DIF) between the Brazilian and the French items. External construct validity was investigated testing expected differences between groups using one-way analysis of variance (ANOVA), Mann-Whitney tests and the univariate general regression linear model.</p> <p>Results</p> <p>CFA showed an acceptable fit (RMSEA=0.05; CFI=0.93); 94% of scaling success was found for item-internal consistency and 98% for item discriminant validity. The items showed good fit to the Rasch model except 3 items with an INFIT at the upper threshold. Cronbach's Alpha ranged from 0.60 to 0.85. Test-retest reliability was moderate to good (ICC=0.55-0.82). DIF was evidenced in 4 out of 36 items. Expected patterns of differences were confirmed with significantly lower physical, psychological well being and vitality reported by symptomatic adolescents.</p> <p>Conclusions</p> <p>Although DIF in few items and responsiveness must be further explored, the Brazilian version of VSP-A demonstrated an acceptable validity and reliability in adolescents attending school and might serve as a starting point for more specific clinical investigations.</p
Precarious work and methodological challenges to study hard-to-reach populations
OBJECTIVE: To describe the methodological challenges and strategies of a web survey on the working conditions and health among delivery workers. METHODS:The study population consisted of Brazilian delivery workers operating in the national territory. Procedures include building solid and ongoing collaboration with worker representatives and conducting a four-month data collection from February to May 2022, sharing the link to the online questionnaire on social media such as social networks (Facebook, Instagram) and messaging apps (WhatsApp, Telegram). RESULTS: The recruitment of 41 leaders or influencers of delivery workers increased the dissemination of the study, some of whom participated in the consensual validation of the questionnaire; the production of content for social media for the dissemination of the questionnaire link on social networks and applications, and the in-person dissemination of the study at the delivery workers’ meeting points during the workday played a fundamental role, totaling around 132 hours in 45 shifts. The strategies adopted for data collection with a hybrid approach to dissemination made it possible to carry out the web survey. After four months of the web survey, 564 delivery workers, 543 men and 18 women, responded to the online questionnaire.CONCLUSION: The web survey presented methodological strategies to overcome the challenge of reaching workers, including hybrid work, to increase the participation of workers, on whom epidemiological research is still scarce.</p
Social support and leisure-time physical activity: longitudinal evidence from the Brazilian Pró-Saúde cohort study
<p>Abstract</p> <p>Background</p> <p>Although social support has been observed to exert a beneficial influence on leisure-time physical activity (LTPA), multidimensional approaches examining social support and prospective evidence of its importance are scarce. The purpose of this study was to investigate how four dimensions of social support affect LTPA engagement, maintenance, type, and time spent by adults during a two-year follow-up.</p> <p>Methods</p> <p>This paper reports on a longitudinal study of 3,253 non-faculty public employees at a university in Rio de Janeiro (the Pró-Saúde study). LTPA was evaluated using a dichotomous question with a two-week reference period, and further questions concerning LTPA type (individual or group) and time spent on the activity. Social support was measured by the Medical Outcomes Study Social Support Scale (MOS-SSS). To assess the association between social support and LTPA, two different statistical models were used: binary and multinomial logistic regression models for dichotomous and polytomous outcomes, respectively. Models were adjusted separately for those who began LTPA in the middle of the follow up (engagement group) and for those who had maintained LTPA since the beginning of the follow up (maintenance group).</p> <p>Results</p> <p>After adjusting for confounders, statistically significant associations (p < 0.05) between dimensions of social support and group LTPA were found in the engagement group. Also, the emotional/information dimension was associated with time spent on LTPA (OR = 2.01; 95% CI 1.2-3.9). In the maintenance group, material support was associated with group LTPA (OR = 1.80; 95% CI; 1.1-3.1) and the positive social interaction dimension was associated with time spent on LTPA (OR = 1.65; 95% CI; 1.1-2.7).</p> <p>Conclusions</p> <p>All dimensions of social support influenced LTPA type or the time spent on the activity. However, our findings suggest that social support is more important in engagement than in maintenance. This finding is important, because it suggests that maintenance of LTPA must be associated with other factors beyond the individual's level of social support, such as a suitable environment and social/health policies directed towards the practice of LTPA.</p
Influencia de fatores psicossociais na cessacao do tabagismo: evidencias longitudinais no Estudo Pro-Saude
OBJECTIVE To evaluate the incidence of smoking cessation and its association with psychosocial factors. METHODS Data came from three consecutive waves of the Pro-Saude Study, a longitudinal study of non-faculty civil servants working at a university in Rio de Janeiro, Southeastern Brazil. Inclusion criteria were having participated in Phases 1 and 3 and being a smoker at baseline (Phase 1 – 1999). Those who had stopped smoking less than a year before the follow-up (Phase 3 – 2007) were excluded. The final study population consisted of 661 employees (78% of those eligible). Relative risks (RR) of smoking cessation were evaluated through Poisson regression with robust variance. RESULTS The cumulative incidence of smoking cessation in eight years of follow-up was 27.7%. Among the psychosocial factors evaluated in the multivariate analysis, only lack of experience of physical violence was associated with higher smoking cessation (RR = 1.67, 95%CI 1.09;2.55). CONCLUSIONS The incidence of smoking cessation was high, and the fact that associations were not found with most factors evaluated suggests that much of the effect found is due to the impact of public policies implemented in Brazil over the past decades. The association between no exposure to violence and higher incidence of smoking cessation draws attention to the importance of this factor in tobacco control policies.OBJETIVO Analizar la incidencia de la suspensión del tabaquismo y su asociación con factores psicosociales. MÉTODOS En estudio de cohorte, bajo el punto de vista longitudinal, se analizaron datos de tres fases consecutivas del Estudio Pro-Salud entre funcionarios técnicos-administrativos de una universidad en Rio de Janeiro, Brasil. Los criterios de inclusión fueron haber participado en las Fases 1 y 3 y ser fumador en la línea de base (Fase 1 – 1999). Se excluyeron individuos que, en el seguimiento (Fase 3 – 2007), habían dejado de fumar hace menos de un año. La población final de estudio estuvo compuesta por 661 funcionarios (78% de los elegibles). Los riesgos relativos (RR) de la suspensión del tabaquismo fueron evaluados por el modelo de regresión de Poisson con varianza robusta. RESULTADOS La incidencia acumulada por la suspensión del hábito de fumar en ocho años de seguimiento fue de 27,7%. Entre los factores psicosociales evaluados, ausencia de experiencia de violencia física se presentó asociada a la mayor suspensión de tabaquismo en el análisis multivariado (RR= 1,67; IC95% 1,09;2,55). CONCLUSIONES La incidencia de suspensión de tabaquismo fue alta y el hecho de no haberse encontrado asociaciones con la mayoría de los factores evaluados sugiere que gran parte del efecto encontrado sea decurrente del impacto relativamente homogéneo de las políticas públicas implementadas en Brasil en las últimas décadas. La asociación encontrada entre ausencia de exposición a la violencia y mayor incidencia de interrupción de tabaquismo señala la importancia de éste factor en las políticas de control del tabaquismo.OBJETIVO Analisar a incidência de cessação de tabagismo e sua associação com fatores psicossociais. Estresse no trabalho e hipertensão arterial em mulheres no Estudo Pró-Saúde: Estudo Pró-Saúde (Pro-Health Study)
Con el objetivo de analizar la asociación entre estrés en el trabajo e hipertensión arterial en la población femenina, fue realizado estudio transversal con 1.819 mujeres participantes del Estudio Pro-Salud en Rio de Janeiro, Sureste de Brasil, entre 1999 y 2001. Fue utilizada la versión brasilera de la escala reducida de estrés en el trabajo (modelo demanda-control). La prevalencia global de hipertensión arterial conferida (>;140/90 mmHg y/o uso de medicamento anti-hipertensivo) fue de 24%. Comparadas con participantes con trabajo clasificado como de baja exigencia, las tasas ajustadas de alta exigencia, pasivos y activos, fueron, respectivamente, de 0,93 (IC 95%: 0,72;1,20), 1,06 (IC 95%: 0,86;1,32) y 1,14 (IC 95%: 0,88;1,47). A cross-sectional study was performed with 1,819 women who participated in the Estudo Pró-Saúde (Pro-Health Study), in the city of Rio de Janeiro, Southeastern Brazil, between 1999 and 2001. The Brazilian version of the short version of the Job Stress Scale (demand-control model) was used. Overall prevalence of measured hypertension (>;140/90 mmHg and/or antihypertensive drug use) was 24%. Compared to participants with jobs classified as low strain, adjusted prevalence ratios for hypertension in women who performed passive and active high-strain jobs were, respectively, 0.93 (95% CI: 0.72;1.20), 1.06 (95% CI: 0.86;1.32) and 1.14 (95% CI: 0.88;1.47). Longitudinal analyses should be performed to clarify the role of these work environment psychosocial characteristics as a determinant of hypertension

Effectiveness of Insecticide Spraying and Culling of Dogs on the Incidence of Leishmania infantum Infection in Humans: A Cluster Randomized Trial in Teresina, Brazil
Background: To evaluate the effect of insecticide spraying for vector control and elimination of infected dogs on the incidence of human infection with L. infantum, a randomized community intervention trial was carried out in the city of Teresina, Brazil. Methods/Principal Findings Within each of ten localities in the city, four blocks were selected and randomized to 4 interventions: 1) spraying houses and animal pens with insecticide; 2) eliminating infected dogs; 3) combination of spraying and eliminating dogs, and 4) nothing. The main outcome is the incidence of infection assessed by the conversion of the Montenegro skin test (MST) after 18 months of follow-up in residents aged ≥1 year with no previous history of visceral leishmaniasis (VL). Reactions were measured at 48–72 h, induration of ≥5 mm considered positive. Interventions were executed after the baseline interview and repeated 6 and 12 months later. The effects of each type of intervention scheme on the incidence of infection were assessed by calculating relative risks and 95% confidence intervals using Poisson population-averaged regression models with robust variance. Among the 1105 participants, 408 (37%) were MST positive at baseline. Of the 697 negatives, only 423 (61%) were reexamined at the end of the follow-up; 151 (36%) of them converted to a positive MST. Only dog culling had some statistically significant effect on reducing the incidence of infection, with estimates of effectiveness varying between 27% and 52%, depending on the type of analysis performed. Conclusions/Significance: In light of the continuous spread of VL in Brazil despite the large scale deployment of insecticide spraying and dog culling, the relatively low to moderate effectiveness of dog culling and the non-significant effect of insecticide spraying on the incidence of human infection, we conclude that there is an urgent need for revision of the Brazilian VL control program
Classification and regression tree (CART) model to predict pulmonary tuberculosis in hospitalized patients
Background: Tuberculosis (TB) remains a public health issue worldwide. The lack of specific clinical symptoms to diagnose TB makes the correct decision to admit patients to respiratory isolation a difficult task for the clinician. Isolation of patients without the disease is common and increases health costs. Decision models for the diagnosis of TB in patients attending hospitals can increase the quality of care and decrease costs, without the risk of hospital transmission. We present a predictive model for predicting pulmonary TB in hospitalized patients in a high prevalence area in order to contribute to a more rational use of isolation rooms without increasing the risk of transmission. Methods: Cross sectional study of patients admitted to CFFH from March 2003 to December 2004. A classification and regression tree (CART) model was generated and validated. The area under the ROC curve (AUC), sensitivity, specificity, positive and negative predictive values were used to evaluate the performance of model. Validation of the model was performed with a different sample of patients admitted to the same hospital from January to December 2005. Results: We studied 290 patients admitted with clinical suspicion of TB. Diagnosis was confirmed in 26.5% of them. Pulmonary TB was present in 83.7% of the patients with TB (62.3% with positive sputum smear) and HIV/AIDS was present in 56.9% of patients. The validated CART model showed sensitivity, specificity, positive predictive value and negative predictive value of 60.00%, 76.16%, 33.33%, and 90.55%, respectively. The AUC was 79.70%. Conclusions: The CART model developed for these hospitalized patients with clinical suspicion of TB had fair to good predictive performance for pulmonary TB. The most important variable for prediction of TB diagnosis was chest radiograph results. Vaccine effectiveness of heterologous CoronaVac plus BNT162b2 in Brazil
There is considerable interest in the waning of effectiveness of coronavirus disease 2019 (COVID-19) vaccines and vaccine effectiveness (VE) of booster doses. Using linked national Brazilian databases, we undertook a test-negative design study involving almost 14 million people (~16 million tests) to estimate VE of CoronaVac over time and VE of BNT162b2 booster vaccination against RT-PCR-confirmed severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection and severe COVID-19 outcomes (hospitalization or death). Compared with unvaccinated individuals, CoronaVac VE at 14-30 d after the second dose was 55.0% (95% confidence interval (CI): 54.3-55.7) against confirmed infection and 82.1% (95% CI: 81.4-82.8) against severe outcomes. VE decreased to 34.7% (95% CI: 33.1-36.2) against infection and 72.5% (95% CI: 70.9-74.0) against severe outcomes over 180 d after the second dose. A BNT162b2 booster, 6 months after the second dose of CoronaVac, improved VE against infection to 92.7% (95% CI: 91.0-94.0) and VE against severe outcomes to 97.3% (95% CI: 96.1-98.1) 14-30 d after the booster. Compared with younger age groups, individuals 80 years of age or older had lower protection after the second dose but similar protection after the booster. Our findings support a BNT162b2 booster vaccine dose after two doses of CoronaVac, particularly for the elderly
