Discrepancies between patients' and partners' perceptions of unsupportive behavior in chronic obstructive pulmonary disease

The literature on chronic diseases indicates that partner support, as perceived by patients, contributes to well-being of patients in either a positive or a negative way. Previous studies indicated that patients' and partners' perceptions of unsupportive partner behavior are only moderately related. Our aim was (1) to investigate whether discrepancies between patients' and partners' perceptions of two types of unsupportive partner behavior-overprotection and protective buffering-were associated with the level of distress reported by patients with chronic obstructive pulmonary disease (COPD) and (2) to evaluate whether the direction of the differences between patients' and partners' perceptions was associated with distress (i.e., whether patient distress was associated with greater patient or greater partner reports of unsupportive partner behavior). A cross-sectional study was performed using the data of a sample of 68 COPD patients and their spouses. Distress was assessed using the Hopkins Symptom Checklist-25. Patients' and partners' perceptions of unsupportive partner behavior were assessed with a questionnaire measuring overprotection and protective buffering. Distress was independently associated with patients' perceptions of protective buffering and discrepancies in spouses' perceptions of overprotection. Regarding the direction of the discrepancy, we found that greater partner reports of overprotection as compared with patient reports were related to more distress in COPD patients. Cockpit displays of traffic information: Airline pilots opinions about content, symbology, and format

A number of candidate computer-generated cockpit displays of traffic information (CDTI) displays and display options were simulated statically and were shown to 23 airline pilots who were asked to respond to more than 250 questions about them. The pilots indicated that the amount and complexity of navigation information displayed should increase with altitude and map scale. Terrain information should appear automatically if a pilot's own aircraft descends below the minimum safe altitude and should include only those obstruction within 2,000 ft or less. Few pilots that weather information should be displayed on a CDTI, but if it was, it should be at pilot request only. A chevron-shaped symbol, located so that the majority of the map area was ahead was preferred. The position, altitude, ground speed, ground track, weight class, and flightpath history of other aircraft should be presented graphically by coding the shape of the symbol for other aircraft or presented digitally in data tags displayed at pilot request. All pilots thought that color coding was necessary to recognize different categories of information quickly and accurately. The majority of pilots felt that a CDTI would provide useful information even though its presence might increase their workload somewhat particularly during its introductory stages

De Open Onderneming: Een bedrijfsethisch vraagstuk In deze rede wordt de oproep vanuit de samenleving tot een grotere maatschappelijke openheid van bedrijven geplaatst in een bedrijfsethisch perspectief. Enerzijds zijn er bedrijfsethische redenen voor openheid. Anderzijds zijn er ook morele begrenzingen. Aan de hand van het maatschappelijk jaarverslag van ondernemingen wordt de noodzakelijke bijdrage van de bedrijfsethiek ge??llustreerd. Zeven morele vuistregels worden gepresenteerd voor het door bedrijven selecteren van de onderwerpen en indicatoren voor hun maatschappelijk jaarverslag. Op basis van deze vuistregels wordt een visie besproken op de opzet en invoering van het maatschappelijk jaarverslag en worden enkele aanbevelingen gedaan voor een verbetering van de kwaliteit van maatschappelijk jaarverslagen. Maatschappelijk verantwoord ondernemen is het antwoord van bedrijven op die vraag om verantwoording. Openheid staat hierin centraal. Openheid wordt veelal opgevat als 'transparantie'. Openheid houdt echter meer in. Het gaat ook om open staan voor maatschappelijke ontwikkelingen en maatschappelijke noden. Open staan voor maatschappelijke noden, ofwel maatschappelijk verantwoord ondernemen, is in de kern een ethisch vraagstuk. De gangbare ethiek schiet echter tekort om de problemen die ondernemingen ervaren bij het invullen van de maatschappelijke ruimte te conceptualiseren. De pluraliteit van waarden is een gegeven waar ondernemingen rekening mee moeten houden bij het invullen van hun maatschappelijke rol. De gangbare ethiek neigt er toe deze pluraliteit van waarden te reduceren tot ????n doorslaggevende waarde. Deze wijze van kijken naar maatschappelijke vraagstukken leidt tot een technocratische aanpak van het probleem. Maatschappelijk verantwoord ondernemen vergt een dialoog met en tussen de stakeholders. Development of an exercise testing protocol for patients with a lower limb amputation:results of a pilot study

Due to a decrease in physical activity, lower limb amputees experience a decline in physical fitness. This causes problems in walking with a prosthesis because energy expenditure in walking with a prosthesis is much higher than in walking with two sound legs. Exercise training may therefore increase the functional walking ability of these patients. To generate a safe and effective aerobic training program, exercise testing of amputees is recommended. The objectives of this study were to develop a maximal exercise testing protocol for lower limb amputees and to compare two different testing methods: combined arm-leg ergometry and arm ergometry. The protocols were tested in five amputee patients. Combined ergometry elicited a higher oxygen uptake and heart rate than arm ergometry. Electrocardiography during combined ergometry was easier to read. Combined ergometry was judged most comfortable by the amputees. The exercise testing protocol was useful in lower limb amputees to determine their maximal aerobic capacity and their main exercise limitation. Future exercise training programs may be based on this testing protocol. Combined arm-leg ergometry is appropriate for unilateral amputees without significant claudication of the remaining leg. Continuous arm ergometry is suitable for unilateral amputees with significant claudication of the remaining limb or bilateral amputees

Identifying Patients with COPD in Need for Psychosocial Care Through Screening with the HSCL-25 and the CCQ Mental State

High levels of psychological distress are documented in patients with COPD. This study investigates the extent to which patients with a high score on the Hopkins Symptoms Checklist-25 (HSCL-25) or with a high score on the Mental State scale of the Clinical COPD Questionnaire (CCQ) endorse a need for psychosocial care, and investigates several characteristics of patients with a need. Outpatients with COPD of the Department of Pulmonary Diseases of a University Medical Center were assessed with the HSCL-25, CCQ and a question on need for psychosocial care. For patients indicating a need, the percentage of patients with HSCL-25 ≥39 was compared with the percentage of patients with CCQ Mental State >2 and tested with a Chi-square. In total 323 patients participated; 57% of them were distressed according to the HSCL-25 (≥39) and 20% according to the CCQ Mental State (>2); 28% reported a need for psychosocial care. For patients reporting a need for psychosocial care a higher percentage was identified by the HSCL-25 than by the CCQ Mental State (χ2 = 9.41, p <. 002) and they were younger than patients without a need (t = 4.48, p <. 001). No differences existed for sex, FEV1, FEV1% predicted or medical comorbidities. The HSCL-25 identified more patients in need than the CCQ Mental State scale. However, not all patients with a need were identified. No relationship was found between need for psychosocial care and illness variables or comorbidities. Distress screening is questioned as the most effective way to identity patients with COPD in need for psychosocial care
