    A qualidade de vida em idosos usuários de próteses auditivas com adaptação aberta e retroauriculares

Centro de Ciências da Saúde.Fonoaudiologia.Introdução: A audição exerce grande influência na qualidade de vida do idoso, e visando melhorar a sua capacidade comunicativa, faz-se necessária a reabilitação desta capacidade, por meio do uso da prótese auditiva. Visando atender, minimizar e suprir as necessidades de indivíduos com perda auditiva de configuração descendente, no caso da presbiacusia, foi desenvolvido um novo modelo para a adaptação da prótese auditiva conhecido como adaptação aberta. Este modelo de prótese não necessita da utilização de molde auricular e o som é transmitido ao meato acústico externo através de um micro-tubo acoplado a uma pequena oliva. A literatura recomenda a utilização de medidas subjetivas, como questionários para entender o processo de adaptação com as próteses auditivas. O questionário WHOQOL-bref tem sido recomendado para avaliar a qualidade de vida e tem sido utilizado mundialmente, pois foi criado dentro de uma escala com perspectiva transcultural. Objetivo: Comparar a qualidade de vida dos idosos usuários de próteses auditivas retroauriculares com os usuários de próteses auditivas com adaptação aberta. Metodologia: Participaram deste estudo 12 idosos, com faixa etária de 60 a 82 anos, sendo seis do gênero masculino e seis do gênero feminino. Em relação ao tipo de adaptação sete foram adaptados com próteses retroauriculares s e cinco com próteses mini-retroauriculares com tubo fino e oliva aberta. A pesquisa foi realizada em dois Serviços de Atenção à Saúde Auditiva e duas empresas particulares localizadas na cidade de Florianópolis. Foram entrevistados idosos entre seis meses e um ano após a adaptação das próteses auditivas, sendo aplicado o questionário Abreviado de Avaliação da Qualidade de Vida (WHOQOL-bref) e questionário de uso e manutenção da prótese auditiva realizado pela própria autora. Resultados: Verificou-se que os usuários de próteses auditivas retroauriculares e com adaptação aberta apresentaram, os respectivos resultados, por domínio avaliado do WHOQOL-bref: físico 62,76 e 74,29; psicológico 65,1 e 76,39, relações sociais 63,1 e 75,00, meio ambiente 64,95 e 70,63. Em relação ao gênero, as mulheres apresentam os seguintes resultados: físico 74,4, psicológico 76,39, relações sociais 72,22 e meio ambiente 72,4. Já os homens apresentaram os respectivos resultados: físico 63,54, psicológico 65,28, relações sociais 65,28 e meio ambiente 65,28. Conclusão: Foi possível concluir que os idosos usuários de prótese auditiva com adaptação aberta apresentaram uma melhor qualidade de vida em todos os domínios avaliados, que os idosos usuários de prótese auditiva retroauricular.Introduction: Hearing has great influence on the quality of life of the elderly, and to improve their ability to communicate, it is necessary rehabilitation of this capability through the use of hearing aids. Seeking to assist, minimize and fulfill the needs of individuals with downward-sloping hearing loss, in the case of presbycusis a new model of hearing aid fitting known as open fit has been developed. This hearing aid model does not require the use of ear mold and the sound is transmitted to the ear canal through a micro-tube attached to a small olive. The literature recommends the use of subjective measures such as questionnaires to understand the adaptation process of hearing aids. The WHOQOL-BREF questionnaire has been recommended to assess the quality of life and has been used worldwide, since it was created in a scale with cross-cultural perspective. Objective: To compare the quality of life of elderly who are BTE hearing aid users with open fit hearing aid users. Methodology: The study included 12 elderly, aged between 60-82 years, six males and six females. Regarding the type of adaptation, seven BTE hearing aids and five mini-BTE hearing aids with slim tube and open olive were fitted. The research was conducted in two Hearing Health Care Services and two private companies located in the city of Florianópolis. Respondents were interviewed between six months and one year after the hearing aid fitting, trough the WHO Quality of Life questionnaire (WHOQOL-bref) and the “Use and Maintenance of Hearing Aids” questionnaire developed by the author. Results: It was found that BTE hearing aids users and open fit BTE users presented the following results for each domain of WHOQOL-BREF: physical health 62,76 - 74,29; psychological 65,1 - 76,39, social relationship 63,1 - 75,00, environment 64,95 - 70,63. Regarding gender, women showed as results: physical 74,4, psychological 76,39, social relationships 72,22 and environment 72,4. In contrast, men presented their results: physical health 63,54, psychological 65,28, social relationships 65,28 and environment 65,28. Conclusion: It was concluded that the elderly users of open fit hearing aids users presented better quality of life in all domains assessed when compared to regular BTE hearing aids

    Non-surgical treatment of massive traumatic corpus callosum hematoma after blunt head injury: A case report

    Massive hematoma of the corpus callosum caused by blunt head trauma is an extremely rare lesion. Most frequent traumatic lesions involve the corpus callosum are diffuse axonal injuries. They might be associated with small hemorrhagic foci in the hemispheric and brain stem white matter, intraventricular hemorrhages, subarachnoid hemorrhages, traumatic lesions of the septum pellucidum and fornix. Many cases of corpus callosum injury present with permanent disconnection syndrome. We present a case of a 32-year-old female suffered blunt head trauma resulted in massive corpus callosum hematoma which was managed non-surgically. The patient initially had a reduced conscious level and symptoms of disconnection syndrome, and significant recovery was observed at 6 months follow up

    Exclusion of the anterior communicating artery with endovascular flow diverters – A possible treatment method of a wide-necked aneurysm

    Introduction: We describe a case of a patient with severe subarachnoid hemorrhage from the rupture of difficult-to-treat morphology, a suspected partially thrombosed anterior communicating artery aneurysm. Case presentation: The patient was admitted with World Federation of Neurosurgeons (WFNS) score of 4 and a Fisher grade IV hemorrhage. Angiography demonstrated a wide neck anterior communicating artery aneurysm not suitable for the standard coiling and surgical treatment. On the 10th day after the bleeding, endovascular treatment was performed to exclude the anterior communicating artery from the circulation by implanting flow diverters to A2 to A1 on both sides. On Day 18, a CT scan showed communicating hydrocephalus, and thus the patient was treated with a ventriculoperitoneal shunt. At the four-month follow-up, angiography showed O'Kelly-Marotta grade D aneurysm occlusion, and the patient's modified Rankin score was 0. The patient made a complete recovery. Discussion: Even though this was a rare application of the flow diverter, other treatment approaches, including detachable coil, stent implantation, and surgical clipping were considered less safe and less effective treatment in this case of anterior communicating artery aneurysm. Conclusion: Endovascular exclusion of the anterior communicating artery from the circulation may be a safe and effective treatment approach in cases without significant perforant artery branches where the conventional endovascular treatment is not considered to be applicable. © 2023 The Author
