    Correlation Between Histopathological Grading and Appearance of Obstruction With Level of Carsino-Embryonic Antigen (CEA) in Colorectal Carcinoma

    CEA (Carcinoembryonic Antigen) is a blood colorectal tumor marker test that widely used in a clinical setting. The aim of this research is to assess the correlation between carcinoembryonic antigen (CEA) and the degree of tumor differentiation, and between obstruction and age of patients and colorectal adenocarcinoma. This research was a retrospective analytic observational study with a cross-sectional approach. This research used medical record of colorectal carcinoma patients without other malignancy obtained from Wahidin Sudirohusodo Hospital having complete medical records from 2015 to 2017. Initial anamnesis, the result of radiology, laboratory, and the result of pathology were obtained and analyzed using univariate and bivariate analyses with the value of CEA cutoff 5ng/ml. The result of 107 samples, most of the subjects examined is female (60.7%), between the ages of 41-60 years (48.6%) with the levels of CEA >= 5 (55.1%). Histopathology examination results showed most of the differentiate were moderate (68.2%). While the obstruction was found in 34 subjects (31.8%). We found a significant relationship between the presence of an obstruction with levels of CEA (p < 0.001), and degree of differentiating with the CEA levels also showed a significant relationship (p < 0.001). The result of the analysis indicates that there is a significant correlation between CEA level and the degree of differentiation and obstruction. Obstruction has a more significant correlation with high CEA level. There is no significant correlation between age group and CEA level

    Fifty five years old male with gastrointestinal stromal tumor: a case report

    Esophageal Gastrointestinal Stromal Tumors (GISTs) were extremely rare, with an incidence of 0,2% of GIST cases. Majority of esophageal GISTs were classified as high-risk category (70.83%), thus, it was required more aggressive approaches, e.g. radical surgery, chemotherapy and radiotherapy. This was a case of a 55-year-old male patient who was referred to surgery department complained of atypical chest pain and swallowing difficulty. Based on thorax X-Ray, it was suspected mediastinal mass. In addition, based on chest CT scan, it was suspected a mass at distal esophagus with multiple nodular lesions in liver dd/fatty liver. X-ray Oesophagus Maag Duodenum (OMD): Esophageal mass at 1/3 middle-distal part. Upper endoscopy diagnosis showed a fragile circularly spreading tumor mass which it was easily bleeds. Biopsy also showed a mild esophageal dysplasia with non-specific inflammation and necrotic tissue which was difficult to assess. This patient underwent McKeown esophagectomy. Moreover, pathology report showed diagnosis of a malignant tumor which fitted with criteria of malignant GIST, with both ends tumor-free. Finally, the patient was administered for imatinib as adjuvant therapy 1x400mg daily. This report illustrated complexity in diagnosing and treating esophageal GIST. The tumor size and mitotic rate of tumor were associated with poor survival

    Faktor Risiko Terjadinya Batu Empedu di RSUP Dr. Wahidin Sudirohusodo Makassar

    Tujuan: untuk mengetahui apakah terdapat hubungan beberapa faktor seperti jenis kelamin, usia, diabetes, obesitas, dan hiperlipidemia, sebagai penyebab munculnya batu empedu di RSUP Dr. Wahidin Sudirohusodo. Metode: penelitian ini dilakukan dengan pengambilan sampel secara retrospektif pada pasien yang dirawat diruang bedah di Rumah Sakit Wahidin Sudirohusodo Makassar bulan Januari sampai dengan Desember 2013, sampel diambil secara konsekutif berurutan sebesar jumlah sampel yang dibutuhkan. Hasil: sampel yang masuk kriteria inklusi dari penelitian ini sebanyak 196 orang.  Dari 196 orang 114 (58,5%) orang dengan batu empedu, 101 perempuan (51,5%) dan 95 laki-laki (48,5%). Terdapat 86 (75,4%) penderita dengan batu empedu yang berusia di bawah 40 tahun dan 28 (24,6%) penderita berusia lebih dari 40 tahun. Berdasarkan jenis kelamin 26 (22,8%) penderita laki-laki dengan batu empedu dan 88 (77,2%) perempuan dengan batu empedu. Penderita DM dengan batu empedu sebanyak 103 (90,4%), pasien dengan obesitasyang menderita batu empedu sejumlah 97 (85,1%), 88 (77,2%) penderita dengan kadar trigliserida yang meningkat dengan batu empedu, dan 95 (83,3 %) pasien dengan kadar kolesterol yang meningkat mengalami batu empedu. Simpulan: jenis kelamin perempuan, usia di bawah 40 tahun, penderita diabetes, obesitas dan hiperlipidemia merupakan faktor risiko menimbulkan batu empedu

    The Relationship of Early Feeding and the Length of Stay in Post-gastrointestinal Surgery Patients

    Background: The length of stay in postoperative patients are varied. Prolonged duration of treatment and rest is due to several factors, extrinsic factors such as inadequate nutrition fulfillment, surgical techniques, medications, and wound management. Meanwhile intrinsic factors consist of age, circulation disorder, pain, and comorbidities. This study aims to investigate the influence of early feeding on the length of stay in post-gastrointestinal surgery patients in Dr. Wahidin Sudirohusodo Hospital from January to December 2016. This research is a cross-sectional analytic approach by using secondary data from medical records. The highest number of patients with a length of stay less than nine days is at <24 hours early feeding with percentage 60% and the highest number of patients with a length of stay more than nine days is at > 48 hours nutrition intake with rate 94.1%. There was a significant relationship between initial feeding time and the length of stay in the postoperative gastrointestinal patient. The results suggest to clinicians to provide early nutrition to the patient after having gastrointestinal surgery

    One Stage Transanal Swenson-Like Pull-Through Operation: Our Early Experience

    Operative management of Hisrchsprung Disease (HD) has evolved from staged operation to one stage minimal invasive operation.  One stage transanal Swenson-like pullthrough operation is the latest concept of one stage procedure in managing HD.  We evaluate early outcome of one stage transanal Swenson-like pullthrough operation for managing PH located at rectosigmoid in our institution during 2017.  Our follow up evaluate the incidence of post operative stricture or stenosis, anastomotic leakage, enterocolitis, urinary dan defecating function.  Six patients underwent transanal resection with age ranging from 11 months to 5 years.  Average length of resected intestinal is 10 ± 5 cm.  Mostly, patient came back to office after 3 months.  No post operative incidence of urethral injury, anastomotic leakage or retraction at anastomotic site were reported.  There were  reports of 1 patient with intraoperative bleeding, 1 patient anal excoriation due to enterocolitis and 1 patient with stenosis.  There were no patient reported with urinating and defecating disorders.  There were no mortality incidence in this study. Despite the need of long term observation for evaluating urinating and defecating function, modification of transanal Swenson-like method might be used in managing HD

    Background: Length of stay (LOS) is to show how many days a patient is admitted to the hospital during one treatment period. The best length of daycare is 6-9 days. Some factors affect the length of stay of the patient. This study aims to determine the factors associated with the duration of treatment on digestive surgical patients in Dr. Wahidin Sudirohusodo Hospital, Makassar, Indonesia.Methods: This research was analytical, descriptive research method using cross-sectional study design, and sampling used is total sampling technique with 419 patients. Data analysis was obtained to see the relationship between independent variables (sex, age, secondary diagnosis, surgery setting, surgery risk, and outcome) and the dependent variable (length of stay).Results: The study found that there were four factors related to the duration of treatment of digestive surgical patients that are secondary diagnosis (p = 0.000), surgery setting (p = 0.000), surgery risk (p = 0.000), and out patient condition (p = 0.000). While no significant relation the age with p value = 0.140, and sex p value = 0.917 with length of stay.Conclusions: There were four factors have a significant relationship with the length of stay (LOS) on a digestive surgical patient in dr. Wahidin Sudirohusodo Hospital such as secondary diagnosis, surgery setting, the risk of surgery and outpatient condition.

    Background: Impairment of apoptosis is a hallmark of cancer. Tumor resistance to apoptosis usually caused by deregulation of the expression of BCL-2 family protein or mutation of the tumor suppressor gene p53. Over expression of Bcl-2 is commonly found in various types of cancer including breast cancer. Studies mentioned that analysis of Bcl-2 might predict response to selected endocrine and chemotherapies.
