A New High-Resolution Map of World Mountains and an Online Tool for Visualizing and Comparing

Answers to the seemingly straightforward questions “what is a mountain?” and “where are the mountains of the world?” are in fact quite complex, and there have been few attempts to map the mountains of the earth in a consistent and rigorous fashion. However, knowing exactly where mountain ecosystems are distributed on the planet is a precursor to conserving them, as called for in Sustainable Development Goals 6 and 15 of the United Nations 2030 Agenda for Sustainable Development. In this article we first compare 3 characterizations of global mountain distributions, including a new, high-resolution (250 m) map of global mountains derived from terrain characteristics. We show how differences in conceptual definition, methodology, and spatial resolution of source data can result in differences in the extent and location of lands classed as mountains. For example, the new 250-m resource documents a larger global mountain extent than previous characterizations, although it excludes plateaus, hilly forelands, and other landforms that are often considered part of mountain areas. We then introduce the Global Mountain Explorer, a new web-based application specifically developed for exploration, visualization, and comparison of these maps. This new open-access tool is an intuitive and versatile resource suitable for a broad range of users and applications

Multifocal multi-organ ischaemia and infarction in a preterm baby due to maternal intravenous cocaine use: a case report

<p>Abstract</p> <p>Introduction</p> <p>Although the adverse effects of cocaine use in pregnancy are well recognised, we believe this case highlights the importance of considering the route of administration, and suggests the possibility of multifocal damage relating to intravenous use.</p> <p>Case presentation</p> <p>A Caucasian female baby of 29-weeks' gestation was spontaneously delivered and subsequently developed multi-organ failure considered unrelated to simple prematurity. Intensive care was re-orientated following the development of massive intraventricular haemorrhage.</p> <p>Conclusion</p> <p>This case illustrates the need for regular cranial ultrasound in babies of pregnancies at risk due to intravenous cocaine use and also the necessity of counselling women who misuse cocaine in the antenatal period. As such, this article will be of most interest to paediatric and obstetric staff.</p

Functional Substitution by TAT-Utrophin in Dystrophin-Deficient Mice

Thiopurine monotherapy is effective in ulcerative colitis but significantly less so in Crohn’s disease: long-term outcomes for 11 928 patients in the UK inflammatory bowel disease bioresource

Objective: Thiopurines are widely used as maintenance therapy in inflammatory bowel disease (IBD) but the evidence base for their use is sparse and their role increasingly questioned. Using the largest series reported to date, we assessed the long-term effectiveness of thiopurines in ulcerative colitis (UC) and Crohn’s disease (CD), including their impact on need for surgery. Design: Outcomes were assessed in 11 928 patients (4968 UC, 6960 CD) in the UK IBD BioResource initiated on thiopurine monotherapy with the intention of maintaining medically induced remission. Effectiveness was assessed retrospectively using patient-level data and a definition that required avoidance of escalation to biological therapy or surgery while on thiopurines. Analyses included overall effectiveness, time-to-event analysis for treatment escalation and comparison of surgery rates in patients tolerant or intolerant of thiopurines. Results: Using 68 132 patient-years of exposure, thiopurine monotherapy appeared effective for the duration of treatment in 2617/4968 (52.7%) patients with UC compared with 2378/6960 (34.2%) patients with CD (p<0.0001). This difference was corroborated in a multivariable analysis: after adjusting for variables including treatment era, thiopurine monotherapy was less effective in CD than UC (OR 0.47, 95% CI 0.43 to 0.51, p<0.0001). Thiopurine intolerance was associated with increased risk of surgery in UC (HR 2.44, p<0.0001); with a more modest impact on need for surgery in CD (HR=1.23, p=0.0015). Conclusion: Thiopurine monotherapy is an effective long-term treatment for UC but significantly less effective in CD
