    Purtscher-like retinopathy following valsalva maneuver effect: case report

    <p>Abstract</p> <p>Introduction</p> <p>Purtscher's retinopathy is a rare condition that is noted in cases related to various types of trauma. The characteristic finding in the fundus is the presence of multiple Purtscher flecken. Purtscher-like retinopathy has a similar presentation in the fundus, but without an association with trauma.</p> <p>Case presentation</p> <p>A 43-year old Malay man presented with a sudden onset of central foggy vision in the left eye after holding his breath for two minutes while catching a falling object. It was not associated with floaters, flashes of light, or head trauma. His vision in the right eye was 6/6, and in his left eye it was finger counting. He had bilateral temporal sub-conjunctival hemorrhages. An examination of his left fundus revealed multiple white cotton wool spots and dot-blot retinal hemorrhages with diffuse retinal edema at the posterior pole. His right fundus was noted to have only mild temporal peri-papillary edema associated with a few dot-blot hemorrhages. Fundus fluorescein angiography showed good arterial perfusion and no evidence of leaking or neo-vascularization. A diagnosis of Purtscher-like retinopathy was made, and the patient was treated with indomethacin tablets for six weeks. At his six-week follow-up examination, his left eye visual acuity had improved to 6/12. His bilateral sub-conjunctival hemorrhage had resolved. His left fundus showed residual multiple cotton wool spots and reduced retinal edema.</p> <p>Conclusions</p> <p>Treatment with non-steroidal anti-inflammatory drugs seems to be effective in reducing edema in patients with Purtscher-like retinopathy.</p

    Performance Evaluation Effect of Nb2O5 Particulate on the Microstructural, Wear and Anti-corrosion Resistance of Zn–Nb2O5 Coatings on Mild Steel for Marine Application

    In this study, we developed Zn–Nb2O5 composite coatings from sulfate bath for wear and corrosion performance on mild steel by electrodeposition technique. The effect of Nb2O5 particulate on the Zn–Nb2O5 properties was investigated. The particle volume fraction was varied within between 10 and 20 wt%. The structural properties of the composite coatings were characterized using scanning electron microscope equipped with energy-dispersive spectrometer. The hardness and wear of the composite coating were measured with diamond base microhardness indenter tester and sliding CETR reciprocating wear testers, respectively. The corrosion properties were examined in 3.65% NaCl using AUTOLAB 101 Metrohm potentiostat–galvanostat with linear polarization technique. The results showed that average hardness value of 192.6 and 200.6 HV and passivation potential of 4.39E?08 and 5.30E?08(X) were obtained for the 10–20 wt% Nb2O5 particulate on the Zn–Nb2O5 coatings. The wear performance improves by 63.4% as against the control sample. In all, this study established that up to 20 wt% of Nb2O5 in Zn–Nb2O5 composite coating significant corrosion, wear and microhardness propagation resistance of mild steel was attained

    The Added Value of Radiographs in Diagnosing Knee Osteoarthritis Is Similar for General Practitioners and Secondary Care Physicians; Data from the CHECK Early Osteoarthritis Cohort.

    OBJECTIVE: The purpose of this study was to evaluate the added value of radiographs for diagnosing knee osteoarthritis (KOA) by general practitioners (GPs) and secondary care physicians (SPs). METHODS: Seventeen GPs and nineteen SPs were recruited to evaluate 1185 knees from the CHECK cohort (presenters with knee pain in primary care) for the presence of clinically relevant osteoarthritis (OA) during follow-up. Experts were required to make diagnoses independently, first based on clinical data only and then on clinical plus radiographic data, and to provide certainty scores (ranging from 1 to 100, where 1 was "certainly no OA" and 100 was "certainly OA"). Next, experts held consensus meetings to agree on the final diagnosis. With the final diagnosis as gold standard, diagnostic indicators were calculated (sensitivity, specificity, positive/negative predictive value, accuracy and positive/negative likelihood ratio) for all knees, as well as for clinically "certain" and "uncertain" knees, respectively. Student paired t-tests compared certainty scores. RESULTS: Most diagnoses of GPs (86%) and SPs (82%) were "consistent" after assessment of radiographic data. Diagnostic indicators improved similarly for GPs and SPs after evaluating the radiographic data, but only improved relevantly in clinically "uncertain" knees. Radiographs added some certainty to "consistent" OA knees (GP 69 vs. 72, p < 0.001; SP 70 vs. 77, p < 0.001), but not to the consistent no OA knees (GP 21 vs. 22, p = 0.16; SP 20 vs. 21, p = 0.04). CONCLUSIONS: The added value of radiographs is similar for GP and SP, in terms of diagnostic accuracy and certainty. Radiographs appear to be redundant when clinicians are certain of their clinical diagnosis

    Analysis of the functional repertoire of a mutant form of survivin, K129E, which has been linked to lung cancer

    Background Survivin is a protein that is normally present only in G2 and M-phases in somatic cells, however, in cancer cells, it is expressed throughout the cell cycle. A prosurvival factor, survivin is both an inhibitor of apoptosis and an essential mitotic protein, thus it has attracted much attention as a target for new oncotherapies. Despite its prevalence in cancer, reports of survivin mutations have mostly been restricted to loci within its promoter, which increase the abundance of the protein. To date the only published mutation within the coding sequence is an adenine > guanine substitution in exon 4. This polymorphism, which was found in a cohort of Korean lung cancer patients, causes a lysine > glutamic acid mutation (K129E) in the protein. However, whether it plays a causative role in cancer has not been addressed. Methods Using site directed mutagenesis we recapitulate K129E expression in cultured human cells and assess its anti-apoptotic and mitotic activities. Results K129E retains its anti-apoptotic activity, but causes errors in mitosis and cytokinesis, which may be linked to its reduced affinity for borealin. Conclusion K129E expression can induce genomic instability by introducing mitotic aberrations, thus it may play a causative role in cancer

    Surface enhancement of engineering materials is necessary for preventing service failure and corrosion attacks industrially. The surface modification, strengthening effect and electrochemical comparative study of Zn-Al2O3-CeO3 and Zn-TiO2-CeO3 coating on mild steel was investigated. Deposition was performed to obtain a better surface adherent coating using the electrodeposition technique. Co-deposition of mild steel resulted into surface modification attributes to the complex alloys that were developed. Films of mild steel were electrodeposited on zinc electrodes using the chloride bath solutions. The effect of deposition potentials was systematically studied using a focus ion beam scanning electron microscope (FIB-SEM) and an atomic force microscope (AFM) to observe the surface morphology, topography and the surface adherent properties of the coatings. The elemental composition and the phases evolved in composite coatings were measured by means of the energy dispersed spectrometer (EDS). The microhardness measurements and corrosion behaviours of the deposits were investigated. Weight loss measurement was conducted on the plated samples to observe the rate of corrosion and it was observed that there was severe corrosion on the controlled sample in comparison to the plated samples and that Zn-TiO2-CeO3 resisted more corrosion attacks

    Spatio-Temporal Pattern in the Changes in Availability and Sustainability of Water Resources in Afghanistan

    Water is gradually becoming scarce in Afghanistan like in many other regions of the globe. The objective of this study was to evaluate the spatial changes in the availability and sustainability of water resources in Afghanistan. The Terrestrial Water Storage (TWS) data of the Gravity Recovery and Climate Experiment (GRACE) satellite obtained from three different institutes, having 1° × 1° spatial resolution for the period 2002–2016 was used for this purpose. Sen’s slope method was used to assess the rate of change, and the Modified Mann–Kendall test was used for the evaluation of the significance of trends in TWS. After, the concept of reliability–resiliency–vulnerability (RRV) was used for assessing the spatial distribution of sustainability in water resources. The results revealed a significant decrease in water availability in the country over the last 15 years. The decrease was found to be highest in the central region where most of the population of the country resides. The reliability in water resources was found high in the northeast Himalayan region and low in the southwest desert; resilience was found low in the central region, while the vulnerability was found high in the south and the southeast. Overall, the water resources of the country were found most sustainable in the northeast and southwest and least in the south and the central parts. The maps of water resource sustainability and the changes in water availability produced in the present study can be used for long-term planning of water resources for adaptation to global changes. Besides, those can be used for the management of water resources in a sustainable and judicious manner

    Effect of WO3 Nanoparticle Loading on the Microstructural, Mechanical and Corrosion Resistance of Zn Matrix/TiO2-WO3 Nanocomposite Coatings for Marine Application

    In this study, for marine application purposes, we evaluated the effect of process parameter and particle loading on the microstructure, mechanical reinforcement and corrosion resistance properties of a Zn-TiO2-WO3 nanocomposite produced via electrodeposition. We characterized the morphological properties of the composite coatings with a Scanning Electron Microscope (SEM) equipped with an Energy Dispersive Spectrometer (EDS). We carried out mechanical examination using a Dura Scan hardness tester and a CERT UMT-2 multi-functional tribological tester. We evaluated the corrosion properties by linear polarization in 3.5% NaCl. The results show that the coatings exhibited good stability and the quantitative particle loading greatly enhanced the structural and morphological properties, hardness behavior and corrosion resistance of the coatings. We observed the precipitation of this alloy on steel is greatly influenced by the composite characteristics

    Uncovering novel mutational signatures by de novo extraction with SigProfilerExtractor

    Mutational signature analysis is commonly performed in cancer genomic studies. Here, we present SigProfilerExtractor, an automated tool for de novo extraction of mutational signatures, and benchmark it against another 13 bioinformatics tools by using 34 scenarios encompassing 2,500 simulated signatures found in 60,000 synthetic genomes and 20,000 synthetic exomes. For simulations with 5% noise, reflecting high-quality datasets, SigProfilerExtractor outperforms other approaches by elucidating between 20% and 50% more true-positive signatures while yielding 5-fold less false-positive signatures. Applying SigProfilerExtractor to 4,643 whole-genome- and 19,184 whole-exome-sequenced cancers reveals four novel signatures. Two of the signatures are confirmed in independent cohorts, and one of these signatures is associated with tobacco smoking. In summary, this report provides a reference tool for analysis of mutational signatures, a comprehensive benchmarking of bioinformatics tools for extracting signatures, and several novel mutational signatures, including one putatively attributed to direct tobacco smoking mutagenesis in bladder tissues

    Game Plan: What AI can do for Football, and What Football can do for AI

    The rapid progress in artificial intelligence (AI) and machine learning has opened unprecedented analytics possibilities in various team and individual sports, including baseball, basketball, and tennis. More recently, AI techniques have been applied to football, due to a huge increase in data collection by professional teams, increased computational power, and advances in machine learning, with the goal of better addressing new scientific challenges involved in the analysis of both individual players’ and coordinated teams’ behaviors. The research challenges associated with predictive and prescriptive football analytics require new developments and progress at the intersection of statistical learning, game theory, and computer vision. In this paper, we provide an overarching perspective highlighting how the combination of these fields, in particular, forms a unique microcosm for AI research, while offering mutual benefits for professional teams, spectators, and broadcasters in the years to come. We illustrate that this duality makes football analytics a game changer of tremendous value, in terms of not only changing the game of football itself, but also in terms of what this domain can mean for the field of AI. We review the state-of-theart and exemplify the types of analysis enabled by combining the aforementioned fields, including illustrative examples of counterfactual analysis using predictive models, and the combination of game-theoretic analysis of penalty kicks with statistical learning of player attributes. We conclude by highlighting envisioned downstream impacts, including possibilities for extensions to other sports (real and virtual)
