    Physicians commonly ignore hypothermia, an often-underappreciated event associated with mortality in trauma patients, in general due to its prevalence and belief that it is secondary to the injury itself (secondary hypothermia). Over the past several decades, hypothermia in trauma has been studied concerning its effects on mortality; however, very little has been done to identify the major risk factors associated with it.     Documentation of GEMASS entry to touchdown simulation

    The entry-to-touchdown space shuttle orbiter simulation capability incorporated into the GEMASS subprogram 33 (3-DOF) is documented. A digital autopilot interfaces between GEMASS and the guidance. Vehicle attitude is determined by use of the ability of GEMASS to integrate differential equations in addition to the equations of motion. Vehicle aerodynamic characteristics are obtained from an aerodynamic data tape and control surface deflections required to trim the vehicle, and trimmed aerodynamic coefficients are determined internally. Several indicators to allow evaluation of subsystem performance are included; as is the ability of the user to activate any of several error dispersion sources. The performance of the simulation compares well with more sophisticated simulations

    An Unusual Presentation of a Posterior Mediastinal Schwannoma Associated with Traumatic Hemothorax

    Schwannomas of the thoracic cavity are typically an asymptomatic, benign neurogenic neoplasm of the posterior mediastinum. In this case, we present a traumatic hemothorax as the initial presentation for a previously undiscovered mediastinal mass. The patient presented with shortness of breath and right-sided chest pain after being struck in the chest with a soccer ball. An operative exploration was pursued due to persistent hemothorax with hemodynamic instability despite resuscitation and adequate thoracostomy tube placement. The intraoperative etiology of bleeding was discovered to be traumatic fracture of a large hypervascular posterior mediastinal schwannoma. Surgical resection is the treatment of choice for these tumors. Specific serological markers do not exist for this tumor, and radiographic findings can be variable, so tissue diagnosis is of importance in differentiating benign from malignant schwannomas, as well as other posterior mediastinal tumors. However, most patients have excellent survival following complete resection

    Impact assessment of agricultural research in West Africa: An application of the propensity score matching methodology

    Ex-post evaluation of agricultural research is aimed to empirically provide evidence of past investments’ effectiveness. This paper is intended to measure the immediate impact of livestock research activities on cattle farmers’ knowledge about trypanosomosis and its curative and preventive control strategies. According to the quasi-experimental design of the intervention, it is shown that its impact will be adequately estimated by propensity score matching (PSM). Based on data collected according to a knowledge, attitude and practice (KAP) questionnaire in the region of Kénédougou that is common to Mali and Burkina Faso, results indicate a significant gain in farmers’ know-how due to participation in livestock research activities

    Economic analysis of animal disease control inputs at farm level: the case of trypanocide use in villages under risk of drug resistance in West Africa

    This paper presents an economic analysis of the use of drugs (isometamidium and diminazene) in controlling African Animal Trypanosomosis (AAT), a serious disease of cattle and small ruminants in villages that exhibit resistance to isometamidium in Burkina Faso and Mali in West Africa. The study applies a production function framework integrating a damage control function to assess the short term productivity effect of trypanocide use under different epidemiological conditions. We found that the marginal value products of isometamidium in all epidemiological conditions, and the marginal value product of diminazene in high-prevalence-high-resistance conditions are positive and greater than one revealing an underuse of trypanocidal drugs in those conditions. The economical optimum level of isometamidium is far larger than the current use level. In a strict economic interpretation, this implies that in the short term cattle farmers could increase the profitability if they increase trypanocide input beyond current levels. On the other hand, if the use of trypanocide increases, cattle farmers will also be more likely to experience future losses from trypanocide resistance. In this paper we demonstrated the feasibility of applying the damage control framework for measuring the productivity of veterinary therapeutic drugs at farm level in poor African countries

    Discussion sur l’etude des politiques relatives aux stratégies de gestion de la chimiorésistance dans le cadre de la lutte contre la trypanosomose en Afrique de l’Ouest, Mali. Le projet a pour objectif d’assurer l’efficacité des trypanocides comme une composante effective des strategies intégrées et améliorées de contrôle de Ia trypanosomose animale dans Ia region ouest-africaine. Pour atteindre cet objectif, des organisations nationales de recherche et de developpement, des institutions internationales et régionales de recherche, et des universités allemandes travaillent en partenariat afin de développer aux niveaux local et regional des strategies de reduction de risque de Ia chimiorésistance. L’accent est mis principalement sur l‘information et des supports techniques aux paysans, aux prestataires de service en sante animale, aux vétérinaires professionnels et aux décideurs politiques. Les informations et supports techniques ont pour but de promouvoir Ia lutte intégrée et l’utilisation rationnelle des trypanocides afin de réduire les risques a long terme de Ia chimiorésistance sans compromettre Ia capacité des éleveurs a pouvoir protéger leurs animaux contre les effets néfastes de la trypanosomose animale. Le projet est réalisé au Burkina Faso, au Mali et en Guinée par l’lnstitut International de Recherche sur l’Elevage (lLRl) en collaboration avec d’autres organisations. Des approaches de solutions sont alors proposees de maniere a apporter une solution durable au phenomene de la chimioresistance

    Muang Rae Geothermal System: Drilling and Borehole Geophysics, 1000-m Core Hole into Granitic Rock, Amphoe Pai, Mae Hong Son Province, Northern Thailand

    In 2015, a 1-km core hole was drilled into granitic rock at the Muang Rae geothermal area. Previous drilling in 1993 to 200 m was unsuccessful in obtaining flows, but one of the “dry” wells began flowing in August, 2013 at 9.75 L/s of 96.9°C water from a fracture zone 80 m deep. In the sandy bed of the Pai River, 150 m south of that well is a 600-m long line of hot seeps, oriented NE, with highest temperature measured at 94.5°C. Geology is foliated Triassic (?) granitic rock cut by a NE-SW-trending normal fault zone (dip 65° NW) with extensive fluorite mineralization. Mineral equilibrium geothermometer analysis predicts a reservoir temperature of 132°C. The core hole was sited NW of the seep zone to drill into the fault zone at 600 m. MT indicated a low-resistivity anomaly 150-500 m deep at the site. The well did not flow, but was pumped at 6.4 L/s of 86°C water with 2.5 m of drawdown. Temperature logs indicate the well intersects a fracture with flowing hot water of 91.5°C at 600 m. The log shows a temperature inversion related to the transient effect of conductive heating of the surrounding rock by hot-water flow in the fracture. The bottom 150 m of the well shows a 23°C/km temperature gradient, which reflects the regional crustal conductive gradient

    Ulcer occurrence on adjacent toes and hallux valgus deformity after amputation of the second toe in diabetic patients

    BACKGROUND Amputation of the second toe is associated with destabilization of the first toe. Possible consequences are hallux valgus deformity and subsequent pressure ulcers on the lateral side of the first or on the medial side of the third toe. The aim of this study was to investigate the incidence and possible influencing factors of interdigital ulcer development and hallux valgus deformity after second toe amputation. METHODS Twenty-four cases of amputation of the second toe between 2004 and 2020 (mean age 68 ± 12 years; 79% males) were included with a mean follow-up of 36 ± 15 months. Ulcer development on the first, third, or fourth toe after amputation, the body mass index (BMI) and the amputation level (toe exarticulation versus transmetatarsal amputation) were recorded. Pre- and postoperative foot radiographs were evaluated for the shape of the first metatarsal head (round, flat, chevron-type), the hallux valgus angle, the first-second intermetatarsal angle, the distal metatarsal articular angle and the hallux valgus interphalangeal angle by two orthopedic surgeons for interobserver reliability. RESULTS After amputation of the second toe, the interdigital ulcer rate on the adjacent toes was 50% and the postoperative hallux valgus rate was 71%. Neither the presence of hallux valgus deformity itself (r = .19, p = .37), nor the BMI (r = .09, p = .68), the shape of the first metatarsal head (r = - .09, p = .67), or the amputation level (r = .09, p = .69) was significantly correlated with ulcer development. The interobserver reliability of radiographic measurements was high, oscillating between 0.978 (p = .01) and 0.999 (p = .01). CONCLUSIONS The interdigital ulcer rate on the first or third toe after second toe amputation was 50% and hallux valgus development was high. To date, evidence on influencing factors is lacking and this study could not identify parameters such as the BMI, the shape of the first metatarsal head or the amputation level as risk factors for the development of either hallux valgus deformity or ulcer occurrence after second toe amputation.     Calibrating and testing tissue equivalent proportional counters with 37Ar

    A method for testing and calibrating tissue equivalent proportional counters with37Ar is described.37Ar is produced by exposure of argon in its normal isotope composition to thermal neutrons. It is shown that - up to volume ratios of 0.01 of argon to the tissue equivalent gas - there is no appreciable effect of the argon admixture on the function of the proportional counter. Conventional calibration methods with characteristic x-rays or with -particles require modifications of the detectors, and they test only small sub-volumes in the counters. In contrast, argon permits calibrations and tests of the resolution that are representative for the entire counter volume and that do not require changes in detector construction. The method is equally applicable to multi-element proportional counters; it is here exemplified by its application to a long cylindrical counter of simplified design that is part of such a multi-element configuration
