Invertible Dirac operators and handle attachments on manifolds with boundary
For spin manifolds with boundary we consider Riemannian metrics which are
product near the boundary and are such that the corresponding Dirac operator is
invertible when half-infinite cylinders are attached at the boundary. The main
result of this paper is that these properties of a metric can be preserved when
the metric is extended over a handle of codimension at least two attached at
the boundary. Applications of this result include the construction of
non-isotopic metrics with invertible Dirac operator, and a concordance

Multiple myeloma presenting with high-output heart failure and improving with anti-angiogenesis therapy: two case reports and a review of the literature
<p>Abstract</p> <p>Introduction</p> <p>Common manifestations of multiple myeloma include osteolytic lesions, cytopenias, hypercalcemia, and renal insufficiency. Patients may also exhibit heart failure which is often associated with either past therapy or cardiac amyloidosis. A less recognized mechanism is high-output heart failure. Diuretic therapy in this setting has little efficacy in treating the congested state. Furthermore, effective pharmacotherapy has not been established. We report two patients with multiple myeloma and high-output heart failure who failed diuretic therapy. The patients were given dexamethasone in conjunction with lenalidomide and thalidomide, respectively. Shortly thereafter, each patient demonstrated a significant improvement in symptoms. This is the first report of successful treatment of multiple myeloma-induced high-output failure via the utilization of these agents.</p> <p>Case presentation</p> <p>Two patients with multiple myeloma were evaluated for volume overload. The first was a 50-year-old man with refractory disease. Magnetic resonance imaging demonstrated diffuse marrow replacement throughout the pelvis. Cardiac catheterization conveyed elevated filling pressures and a cardiac output of 15 liters/minute. He quickly decompensated and required mechanical ventilation. The second patient was a 61-year-old man recently diagnosed with multiple myeloma and volume overload. Skeletal survey demonstrated numerous lytic lesions throughout the pelvis. His cardiac catheterization also conveyed elevated filling pressures and a cardiac output of 10 liters/minute. Neither patient responded to diuretic therapy and they were subsequently started on dexamethasone plus lenalidomide and thalidomide, respectively. The first patient's brisk diuresis allowed for extubation within 48 hours after the first dose. He had a net negative fluid balance of 15 liters over 10 days. The second patient also quickly diuresed and on repeat cardiac catheterization, his cardiac output had normalized to 4.7 liters/minute.</p> <p>Conclusion</p> <p>Multiple myeloma can cause high-output failure. The mechanism is likely extensive bony involvement causing innumerable intramedullary arteriovenous fistulas. Diuretic therapy is not effective in treating this condition. Lenalidomide and thalidomide, both of which inhibit angiogenesis, seem to be viable treatment options. Based on the rapid and effective results seen in these two patients, a potential novel mechanism of 'pharmacologic fistula ligation' with these agents may be the most effective way to treat this presentation.</p

Readmission After Lobectomy for Lung Cancer
ObjectiveThe aim of this study was to identify independent predictors of hospital readmission for patients undergoing lobectomy for lung cancer.Summary background dataHospital readmission after lobectomy is associated with increased mortality. Greater than 80% of the variability associated with readmission after surgery is at the patient level. This underscores the importance of using a data source that includes detailed clinical information.MethodsUsing the Society of Thoracic Surgeons (STS) General Thoracic Surgery Database (GTSD), we conducted a retrospective cohort study of patients undergoing elective lobectomy for lung cancer. Three separate multivariable logistic regression models were generated: the first included preoperative variables, the second added intraoperative variables, and the third added postoperative variables. The c statistic was calculated for each model.ResultsThere were 39,734 patients from 277 centers. The 30-day readmission rate was 8.2% (n = 3237). In the final model, postoperative complications had the greatest effect on readmission. Pulmonary embolus {odds ratio [OR] 12.34 [95% confidence interval (CI),7.94-19.18]} and empyema, [OR 11.66 (95% CI, 7.31-18.63)] were associated with the greatest odds of readmission, followed by pleural effusion [OR 7.52 (95% CI, 6.01-9.41)], pneumothorax [OR 5.08 (95% CI, 4.16-6.20)], central neurologic event [OR 3.67 (95% CI, 2.23-6.04)], pneumonia [OR 3.13 (95% CI, 2.43-4.05)], and myocardial infarction [OR 3.16 (95% CI, 1.71-5.82)]. The c statistic for the final model was 0.736.ConclusionsComplications are the main driver of readmission after lobectomy for lung cancer. The highest risk was related to postoperative events requiring a procedure or medical therapy necessitating inpatient care
Ponzano-Regge model revisited III: Feynman diagrams and Effective field theory
We study the no gravity limit G_{N}-> 0 of the Ponzano-Regge amplitudes with
massive particles and show that we recover in this limit Feynman graph
amplitudes (with Hadamard propagator) expressed as an abelian spin foam model.
We show how the G_{N} expansion of the Ponzano-Regge amplitudes can be
resummed. This leads to the conclusion that the dynamics of quantum particles
coupled to quantum 3d gravity can be expressed in terms of an effective new non
commutative field theory which respects the principles of doubly special

Computational personality recognition in social media
A variety of approaches have been recently proposed to automatically infer users' personality from their user generated content in social media. Approaches differ in terms of the machine learning algorithms and the feature sets used, type of utilized footprint, and the social media environment used to collect the data. In this paper, we perform a comparative analysis of state-of-the-art computational personality recognition methods on a varied set of social media ground truth data from Facebook, Twitter and YouTube. We answer three questions: (1) Should personality prediction be treated as a multi-label prediction task (i.e., all personality traits of a given user are predicted at once), or should each trait be identified separately? (2) Which predictive features work well across different on-line environments? (3) What is the decay in accuracy when porting models trained in one social media environment to another
Optimal topological simplification of discrete functions on surfaces
We solve the problem of minimizing the number of critical points among all
functions on a surface within a prescribed distance {\delta} from a given input
function. The result is achieved by establishing a connection between discrete
Morse theory and persistent homology. Our method completely removes homological
noise with persistence less than 2{\delta}, constructively proving the
tightness of a lower bound on the number of critical points given by the
stability theorem of persistent homology in dimension two for any input
function. We also show that an optimal solution can be computed in linear time
