Complications after different surgical methods of setting polypropylene suburethral meshes in women with stress urinary incontinence
The aim: to determine the complication's rate, based on a seven-year experience, after the stress urinary incontinence correction in women with different methods of setting suburethral polypropylene meshes. Material and Methods. The analysis of seven years of experience of surgical correction with different methods of setting suburethral meshes in 1260 women with stress urinary incontinence; obturator fixation the mesh inside-outside and outside-inside: supra-pubic fixation the mesh; non-trocar fixation the mesh type mini-sling was carried out. Results. The greatest frequency of complications were identified in the first two years of implementation the setting of suburethral meshes, after that the complication's rate decreased progressively and it was in the range 7-8% the last three years. The frequency of early operative complications at a different setting of suburethral meshes had no statistically significant differences, but there were more the statistically significant intraoperative and mesh-associated complications at suprapubic fixation. Conclusion. The study confirms that the installation of suburethral meshes by the method of suprapubic fixation of mesh is associated with a higher incidence of complications and should be performed only in the target group of patients who are not suitable for surgical correction in safer ways.</p
On the influence of noise on chaos in nearly Hamiltonian systems
The simultaneous influence of small damping and white noise on Hamiltonian
systems with chaotic motion is studied on the model of periodically kicked
rotor. In the region of parameters where damping alone turns the motion into
regular, the level of noise that can restore the chaos is studied. This
restoration is created by two mechanisms: by fluctuation induced transfer of
the phase trajectory to domains of local instability, that can be described by
the averaging of the local instability index, and by destabilization of motion
within the islands of stability by fluctuation induced parametric modulation of
the stability matrix, that can be described by the methods developed in the

Epidemiology and impact of colonization by multidrug-resistant Gram-negative bacteria on bloodstream infections in early phase of allogeneic hematopoietic stem cell transplantation
Objective.
To study epidemiology and impact of colonization by multidrug-resistant Gram-negative bacteria (MDRGNB) on bloodstream infections (BSI) during allogeneic hematopoietic stem cell transplantation (allo-HSCT).
Materials and Methods.
The retrospective study included 288 patients received the first allo-HSCT between 2018 and 2019. The median age was 32 (18–66) years, male – 53% (n = 152). The majority of patients had acute leukemia – 62% (n = 178) and received transplant from matched unrelated – 42% (n = 120) or haploidentical donor – 26% (n = 75). Relapse of underlying disease at the moment of all-HSCT was registered in 23% (n = 66) of patients.
Results.
Colonization of non-sterile sites before allo-HSCT by at least one MDRGNB was detected in 28% (n = 64). In most cases resistance is due to extended spectrum beta-lactamases (ESBL) – 86% (n = 55), while carbapenemases in combination with ESBL were detected in 14% (n = 9) of patients. After allo-HSCT the colonization was significantly higher than before transplantation (n = 161, 56%, p = 0.001), mainly due to carbapenemase- and ESBL-producing bacteria – 73% (n = 118) (p = 0.001). BSI in the early period after transplantation developed in 26% (n = 76), and in 56% (n = 43) was caused by MDRGNB. The etiology of BSI included K. pneumoniae – 51% in mostly cases. The etiology of BSI was the same bacteria that colonized non-sterile sites 2 weeks before the detection bacteria in bloodstream in 69% (n = 30) patients. Colonization by MDRGNB was associated with the development of BSI (p < 0.0001). The 100-day overall survival (OS) after all-HSCT was significantly lower in patients with colonization of non-sterile sites by MDRGNB compared with patients without colonization (60.6% vs 88.2%, p = 0.001).
Conclusions.
Colonization of MDRGNB after allo-HSCT reached 56%. K. pneumoniae was predominant etiology in both colonization and bloodstream infections. Colonization by MDRGNB was associated with the development of BSI and decreased OS after allo-HSCT
Innovative Approach to the Training System of High-Class Athletes in Youth Sport
В статье представлены результаты экспериментального проекта Министерства образования «Апробация модели обучения учащихся в лицее учреждения образования «Полесский государственный университет» в системе подготовки по гребным и водным видам спорта». Выполненный экспериментальный проект позволил организовать и оценить эффективность образовательного процесса, сочетающего обучение, воспитание и учебно-тренировочный процесс по гребным и водным видам спорта, на базе созданного в лицее университета специализированного спортивного класса. Разработанная в рамках реализации проекта технология научно-методического и медико-биологического обеспечения подготовки спортсменов высокого класса продемонстрировала свою эффективность как в организации тренировочного процесса – повышение спортивного мастерства учащихся, так и сохранении их состояния здоровья.The article presents the results of the pilot project of the Ministry of Education “Approbation of the model of teaching students in the lyceum of the educational institution” Polessky State University “in the system of training in rowing and water sports.” The completed pilot project made it possible to organize and evaluate the effectiveness of the educational process, combining training, education and training in rowing and water sports, on the basis of a specialized sports class created in the university lycee. Comprehensive evaluation of sports capabilities of strongest rowers and canoeists of Belarus
The article provides the comprehensive assessment of individual capacities and athletic potential of the strongest rowers and canoeists of the Republic of Belarus. The long-term monitoring of professional activity of athletes revealed the unique data on the state and processes in the cardiovascular system, the system of external respiration, central and peripheral nervous systems. A set of cardiorespiratory responses aimed at maintaining the most efficient level of operation of the gas transportation system was proved to be formed during physical exercises of highly qualified rowers and canoeists. Owing to analysis of morphofunctional data of the leading rowers, a certain model of a modern rower capable to show a world-class result was created. In addition to the study of morphological indices, psychological status and function of the cardiovascular system neurophysiological characteristics of elite athletes were studied and analyzed. The features of neurophysiological characteristics were detected, among which the values of vibration sensitivity (11,5 ± 1,2 s), maximum amplitude of oscillations for triceps (9000 ± 280 mV), single contraction time for triceps (375 ± 55 ms), biceps (805 ± 257 ms), speed of propagation of excitation in the median (63,5 ± 2,4 m/s) and ulnar nerves (62,2 ± 1,2 m/s). The use of inotropic, but not chronotropic, reserve of the heart, resulting in a marked increase in the index of myocardial contractility, is an effective compensatory reaction in athletes' hemodynamics in response to exercise. The study revealed a significant positive correlation of genotype 34SS gene encoding the muscle isoform of adenosine monophosphate deaminase (AMPD1) (rs17602729) with high levels of the index of myocardial contactility in elite athletes. The obtained results of the study suggest the indirect effect of the AMPD1 gene products on the cardiovascular system. The conducted comprehensive study helps assess the level of morpho-functional development of the strongest rowers and canoeists and give practical recommendations for coaches on individual correction of the training process.В статье дана комплексная оценка индивидуальных возможностей и спортивного потенциала сильнейших гребцов на байдарках и каноэ Республики Беларусь. В результате многолетнего мониторинга профессиональной деятельности спортсменов получены уникальные данные, характеризующие состояние и процессы, протекающие в сердечно-сосудистой системе, системе внешнего дыхания, центральной и периферической нервных системах. Установлено, что при физической нагрузке у гребцов на байдарках и каноэ высокой квалификации формируется комплекс кардиореспираторных реакций, направленных на поддержание наиболее эффективного уровня функционирования газотранспортной системы. Анализ морфофункциональных данных ведущих гребцов позволил создать определенную модель современного гребца, способного показать результат международного уровня. Наряду с исследованием морфологических показателей, психологического статуса и деятельности сердечно-сосудистой системы проведено изучение и дан анализ нейрофизиологических особенностей высококвалифицированных спортсменов. Выявлены особенности нейрофизиологических параметров, среди которых значения вибрационной чувствительности (11,5±1,2 с), максимальная амплитуда осцилляций с трицепса (9000±280 мкВ), длительность единичного сокращения трицепса (375±55 мс), бицепса (805±257 мс), скорость распространения возбуждения по срединному (63,5±2,4 м/с) и локтевому нервам (62,2±1,2 м/с). Эффективной компенсаторной реакцией гемодинамики у спортсменов в ответ на физическую нагрузку является использование не хронотропного, а инотропного резерва сердца, что выражается в более выраженном увеличении индекса сократительной способности миокарда. В исследовании выявлена достоверная корреляционная положительная связь носительства генотипа 34СС гена, кодирующего мышечную изоформу аденозинмонофосфатдезаминазы (AMPD1) (rs17602729) с высокими уровнями ИСМ у спортсменов высокой квалификации. Полученные результаты исследования позволяют говорить о косвенном влиянии продуктов гена AMPD1 на работу сердечно-сосудистой системы

The article presents the results of the pilot project of the Ministry of Education “Approbation of the model of teaching students in the lyceum of the educational institution” Polessky State University “in the system of training in rowing and water sports.” The completed pilot project made it possible to organize and evaluate the effectiveness of the educational process, combining training, education and training in rowing and water sports, on the basis of a specialized sports class created in the university lycee.
