    Long-Term Dose Optimization of Adalimumab via Dose Spacing in Patients with Psoriasis.

    Dose spacing (DS) can be useful for optimizing treatment with biologics in psoriasis patients. However, interval prolongation might increase the production of anti-drug antibodies (ADA) and, therefore, reduce the drug's effectiveness. The long-term effects of DS with adalimumab in psoriatic patients have not been reported. The goal of our study was to evaluate the long-term follow-up of psoriatic patients after adalimumab DS regarding the clinical course and determination of circulating adalimumab, TNFα levels, and anti-adalimumab antibodies. We retrospectively included seven patients treated with adalimumab for moderate-to-severe psoriasis and benefiting from DS from 2010 to 2021. The dose interval of adalimumab was extended to three weeks for all patients and then to four weeks for three of the seven patients. Adalimumab trough levels, TNFα levels, and ADA against adalimumab were measured. For six of the seven patients, absolute PASI values remained below 3 throughout the follow-up period (median = 8.0 years; range: 1.7-11.5) after DS. All the patients were satisfied with the effectiveness of their treatment regime. Within the follow-up period, an average of 63 doses of adalimumab per patient were spared. The median adalimumab trough levels were 4.7 µg/mL (range: 1.9-12.5). TNFα levels remained under 10 pg/mL (undetectable) in all except one patient. ADA against adalimumab remained negative (<10 µg/mL) during the follow-up in all patients. Our data indicate that therapeutic drug monitoring, including the measurement of trough concentrations and ADA, together with the clinical response and patient's preference, can be helpful for clinical decision making and treatment optimization in psoriasis

    Thyrotropin-releasing hormone (TRH) promotes wound re-epithelialisation in frog and human skin

    There remains a critical need for new therapeutics that promote wound healing in patients suffering from chronic skin wounds. This is, in part, due to a shortage of simple, physiologically and clinically relevant test systems for investigating candidate agents. The skin of amphibians possesses a remarkable regenerative capacity, which remains insufficiently explored for clinical purposes. Combining comparative biology with a translational medicine approach, we report the development and application of a simple ex vivo frog (Xenopus tropicalis) skin organ culture system that permits exploration of the effects of amphibian skin-derived agents on re-epithelialisation in both frog and human skin. Using this amphibian model, we identify thyrotropin-releasing hormone (TRH) as a novel stimulant of epidermal regeneration. Moving to a complementary human ex vivo wounded skin assay, we demonstrate that the effects of TRH are conserved across the amphibian-mammalian divide: TRH stimulates wound closure and formation of neo-epidermis in organ-cultured human skin, accompanied by increased keratinocyte proliferation and wound healing-associated differentiation (cytokeratin 6 expression). Thus, TRH represents a novel, clinically relevant neuroendocrine wound repair promoter that deserves further exploration. These complementary frog and human skin ex vivo assays encourage a comparative biology approach in future wound healing research so as to facilitate the rapid identification and preclinical testing of novel, evolutionarily conserved, and clinically relevant wound healing promoters

    Biologics, as IL17 inhibitors, are frequently prescribed for moderate to severe plaque psoriasis. Although mucocutaneous candida infections are a well-known side effect of IL-17 inhibitors, there is no data about dermatophyte infection under this therapy. Generalized tinea corporis can mimic the clinical picture of psoriasis, especially if concomitant treatment with topical corticosteroids is used. Therefore, physician should be aware of this differential diagnosis if they suspect a loss of efficacy of IL-17-inhibitors with a flare-up of psoriasis

    The upkeep of the building requires continuous monitoring to maintain the safety aspects as well as aesthetic elements of the building including facades, marble constructions and gardens. With the decrease of the amount of green areas, human race will face climatic catastrophes, which will affect the quality, type and life longitude of the Earth habitants. To tackle this issue, vertical gardens were designed to fit in the overwhelming world of concrete and to adapt in order to do photosynthesis. The growth of the plants is supported by spraying insecticide periodically. In this paper, we offer to use an unmanned aerial vehicle, to recognize the targeted green areas using HSV algorithm, track the generated trajectory and perform the spraying process all along the recognized area.     The method of synthesis of digital nonlinear control with an electro-hydraulic drive is proposed. It is based on the combination of the method of synthesis of the digital part of the system with a continuous prototype and the expert approach. At the first stage, the continuous regulator is synthesized based on known techniques. At the second stage, a transition to the digital regulator is performed with the help of the z -transformation and the difference equation is determined. The sampling period is selected at a frequency greater than the frequency that determines the bandwidth of the continuous electrohydraulic drive control system (its degree is determined by way of further modeling and evaluation of the results obtained). In the third stage, the modeling for digital control options with selected sampling frequencies and recalculated parameter values of the regulators is carried out.     High-temperature oxidation of acetylene by N2O at high Ar dilution conditions and in laminar premixed C2H2 + O2 + N2 flames

    High-temperature oxidation of acetylene (C2H2) is studied behind reflected shock waves and in laminar flames. Atomic resonance absorption spectroscopy (ARAS) is employed to record oxygen atom concentration profiles for the mixture of 10 ppm C2H2 + 10 ppm N2O + argon and temperatures from 1688 K to 3179 K, extending the range of such data available from the literature. Laminar burning velocity of C2H2 in a diluted oxidizer with 11–13% O2 in the O2 + N2 mixture is measured using the heat flux method and compared to the literature data for the 13% O2 mixture. An updated detailed kinetic mechanism is presented to model and analyze the results, and the selection of rate constants in the C2H2 sub-mechanism, whose importance was identified by the sensitivity analysis, is discussed. The performance of the new model is compared against several reaction schemes available from the literature, and kinetic differences between them are outlined. The new shock-wave data helped to improve the performance of the present model compared to its previous version. For the laminar flames, a particular importance of reactions involving C2H3 is identified, however, the reasons for the observed differences in model predictions are to a large extent located outside the C2H2 sub-mechanism, which were also identified
