Baja endemicidad de parásitos intestinalesen tres distritos de Puriscal

individuals) in three Costa Rican rural districts was conducted by the Institute
for Health Research (INISA). A very low prevalence of all parasites investigated
was demonstrated in comparison with two surveys conducted in 1966
and 1982 in similar rural and urban areas. Giardia was the most frequently
encountered parasite (12,17o), followed by Entamoeba histolytica (5%),
Ancylostoma-Necator (4%), Trichuris (1.5%), Ascaris (1%) and Hymenolepis
nana (0.5%). Strongyloides, Taenia, Dientamoeba, Balantidium and Cryptosporidium
were not found. The low prevalence of intestinal parasites seems to correlate with the availability of water supply (in 76% of homes), the disposal
of feces in latrines (90,4%) or water toilets (9.6%), the common wearing of
shoes and the periodic deworming of the population. The improvement of
other socioeconomic indicators such as income and education, and the marked
dispersion of dwelligns explain the drastic reduction in parasites in recent
yearsDe noviembre de 1983 a junio de 1984, se realizó un estudio transversal en el INISA para investigar la prevalencia de parásitos intestinales en 44 familias (198 individuos) de los distritos de Barbacoas, Grifo Alto y Candelarita del cantón de Puriscal. Se encontraron parásitos patógenos en el 24,7% de los individuos. Giardia fue el parásito más frecuente (12,1%), seguido por Entamoeba histolytica (5%), Ancylostoma-Necator (4%), Trichuris (1,5%), Ascaris (1%) e Hymenolepis nana (0,5%). No se encontré, Strongyloides, Taenia, Dientamoeba, Balantidium ni Cryptosporidium. La investigación mostró que el 76% de los hogares se abastecían de agua de cañería propia, comunal o del Instituto Costarricense de Acueductos y Alcantarillados, y que el 100% de las familias disponen las excretas adecuadamente (90% en letrina y 10% en otro tipo de servicio sanitario). Esta investigación mostró una muy baja prevalencia de parásitos intestinales patógenos lo cual indica que las tres comunidades presentan: alto nivel educativo y de tecnología familiar, cobertura por lo programas de medicina preventiva y curativa, y un nivel socioeconómico adecuado.Universidad de Costa Rica. Criptosporidiosis en niños de Costa Rica: estudio transveral y longitudinal

Instituto de investigaciones en Salud, 1984Se estudió una población de niños lactantes y preescolares, con y sin diarrea, de las áreas urbano y rural de los altiplanos de Costa Rica, observados respectivamente en un servicio de emergencia de un hospital metropolitano y en una estación de campo rural. Los niños del hospital formaron parte de una investigación vertical, mientras que los rurales pertenecían a falanges observadas longitudinalmente en su propio ecosistema. Se encontró Cryptosporidium asociado a diarrea aguda en el 4,3% , generalmente con cuadros severos en el área urbana y diarreas leves en la rural. La infección en el área urbana generalmente se presentó en niños menores de un año de edad, mientras que en la rural, los niños fueron de más edad. Esta situación probablemente está relacionada con los hábitos de alimentación y destete, por cuanto no se observaron infecciones en niños que recibían el seno materno, ya que los niños del área rural recibieron lactancia natural durante los primeros meses de vida, mientras que los del área urbana no fueron amamantados o fueron destetados prematuramente. Cryptosporidium apareció en los meses cálidos y húmedos en ambas poblaciones, lo que sugiere una influencia climatológica sobre la infección. Todos los casos urbanos presentaron deshidratación, que evolucion6 satisfactoriamente con rehidratación oral. Dos casos rurales mostraron tendencia a la cronicidad.The present report is a systematic study of children, with and without diarrhea from Costa Rican metropolitan areas and southern rural higlands. Children were observed, respectively, at emergencies, Hospital Nacional de Niños, and in a field station in Puriscal; urban children were studied vertically, rural children were observed prospectively (cohort study). Cryptosporielhon sp. was found in 4.3% of the cases of diarrhea; diahrrea was generally severe in urban children, but mild in the rural, Infection was detected in urban children less than one year of age: contrasting, no rural infants were found infected, which might be related to breast-feeding, since Puriscal infants were intensively breast-fed for several months, while many urban infants were not breast-fed or were weaned earlier. Cryptosporidium sp. appeared during the warm, rainy and humid months of May through August, when the coccidium was associated with 14.8% of the urban and 15.4% of the rural diarrheas. All urban cases presented dehydration which was corrected with oral rehydration salt therapy, and occasionally with intravenous fluids; dehydration was not common in the rural cases.Universidad de Costa Rica. Estado nutricional del niño de Puriscal durante la crisis, 1979-1984

since 1979 in Puriscal, a rural region in the mountainous South of Costa
Rica. Beginning with gestation and following through infancy and preschool
age, the study includes complete cohorts of children. Teams of researches
and assistants in Puriscal (Field Station), Hospital San Juan de Dios (Milk
Bank) and the University of Costa Rica (INISA), are coordinated for the study.
The analysis of biomedical data collected from 1979 through 1984 did not
show significant variation in means and variance of birth weight by cohort,
nor in incidence of prematurity, low birth weight and intrauterine growth retardation
(IUGR) during the economic crisis (1982-1983) with regard to the
pre-crisis (1979-1980). However, there was an increase in the number of
very small and very large babies during the crisis. The Weight for Age and
Weight for Height indices in infants and preschool children were as adequate
during the crisis as during the pre-crisis. Also, there was not deterioration of
the nutritional state, since cohorts grew older through lactation and preschool
age. However, a marked increase in infant mortality, primarily of the neonatal
period, was observed in 1982 and 1983. Such an increase was not related
to increments in incidence of prematurity or IUGR but was associated with
poor maternal health, as reflected in an excess in unwanted pregnancy,
morbidity and stress among mothers who delivered small or ill babies or who
lost their child in infancy, as compared to mothers who did not.
The National Health System must place greater emphasis in correcting
deficiencies which have logically developed as a consequence of the crisis,
and must fulfill the primary health care (PHC) needs in regions that at present
have a poorly developed PHC infrastructureDesde setiembre de 1979 se viene realizando un estudio sobre el crecimiento y desarrollo del niño de Puriscal, a partir de Ia gestación y a través de Ia lactancia y edad preescolar. Para el estudio se coordina equipos de investigadores y sus asistentes en Puriscal (Estación de Campo), Hospital San Juan de Dios (Banco de Leche) y sede central de Ia Universidad de Costa Rica. El análisis de datos médicos, dietéticos, antropométricos y de mortalidad de las madres y sus niños, no revelo variación significativa en el promedio y desviacién estándar del peso al nacer, ni en Ia incidencia de prematuridad y retardo del crecimiento intrauterino (RCIU) durante Ia crisis (1982-1983) con respecto a la pre-crisis (1979-1980). Se note, sin embargo, un incremento en la incidencia de recién nacidos muy pequeños o muy grandes durante la crisis con respecto a Ia pre-crisis. El análisis de las relaciones Peso/Edad y Peso/Talla en lactantes y preescolares mostró que eI estado nutricional fue tan bueno durante Ia crisis como previo a la misma. Además, no se observe) deterioro en el estado nutricional con el envejecimiento de las falanges, esto es, de Ia transición de la lactancia hacia Ia edad preescolar. Sin embargo, se observar un aumento marcado en la mortalidad infantil, fundamentalmente en el periodo neonatal, durante 1982 y 1983. Tal aumento no se asocio a incrementos en Ia incidencia de prematuramente o RCIU, sino mas bien a deterioro de la salud materna, evidenciable en el aumento del embarazo no deseado y en el de riesgos como morbilidad, durante la gestación y deterioro en la atención prenatal y del parto. No se logró demostrar que el aumento en Ia mortalidad infantil, estuviese relacionado con una disminución del consumo de alimentos durante el embarazo. El Sistema Nacional de Salud debe poner más énfasis en la corrección de deficiencias en Ia atención primaria que lógicamente se generaron con la crisis económica, y debe impulsar con mayor vehemencia la extensión de la cobertura, en aquellas regiones en donde Ia carente o deficiente infrastructura actual, indudablemente contribuye a que se mantengan focos de alta mortalidad infantil y achicamiento.Universidad de Costa Rica. Diarrea por Campylobacter fetus jejuni y otros agentes infecciosos en niños del área rural de Puriscal, Costa Rica
artículo -- Universidad de Costa Rica. Instituto de Investigaciones en Salud, 1984Between September 1979 and September 1981 a field study was conducted on the etiology of
diarrhea' disease in the area of Puriscal, Costa Rica. The presence of enteric pathogens was investigated in
the stools of 267 diarrheic children and 190 healthy controls. Both soups belong to yearly cohorts recruited
at birth as part of a longitudinal multidisciplinary study of mothers and children. Campylobacter fetus lefuni
was identified as the only pathogen in the stools of 24 diarrheic children (9%) and in four healthy controls
(2% ), a significant difference (p< 0,05). The clinical features of the episodes were: irritability (77%), blood
in stools (35%), anorexia (38,5%), and fever and vomiting (36%). Dehydration was not important among
infected children (only one with 5% dehydration). All children received oral salt solutions and only two
were treated with antibiotics. Rotaviruses were the main etiologic agents (17%) and Campylobacter ranked
second (10,5% ). The frecuency of enterotoxigenic Enterobacteriaceae was similar in sick children and in
controls (10%and 12%respectively).Entre setiembre de 1979 y setiembre de 1981, estudiamos 267 niños con diarrea aguda y 190 testigos, con el objeto de determinar la etiología y evaluar la importancia de Campylobacter fetus jejuni como agente etiológico y su influencia sobre el estado nutritional. Todos los niños tenían entre 0 y 24 meses de edad y pertenecen a las falanges anuales de un estudio longitudinal que realiza el INISA en el cantón de Puriscal, Costa Rica. Campylobacter fue el único agente en 24 niños con diarrea (9% ) y en cuatro niños sanos (2% ), encontrándose una diferencia significativa entre ambos grupos (x 2 = 9,05; p < 0,05). Además, se le encontró asociado a rotavirus o enterobacteriáceas toxigénicas en cuatro niños enfermos (1,5% ) y en dos testigos (1% ). El cuadro clínico se caracterizó por la evacuación de cinco a diez deposiciones líquidas al día, con sangre visible macroscópicamente en 39% de los casos. Los niños mostraron irritabilidad (77% ), anorexia (38,5% ), fiebre (36%) y vómitos (36%). Solo un niño presentó deshidratación del 5% y únicamente dos recibieron antibioticoterapia. El resto de los niños se recuperó satisfactoriamente mediante la administración de SUERORAL. El principal agente etiologico fue el rotavirus (17% ), seguido de Campylobacter (10,5%). Estudio de Puriscal IX. Estado nutricional y supervivencia del niño lactante
artículo -- Universidad de Costa Rica. Instituto Investigaciones en Salud, 1982. Parte de estudios consecutivos realizados en la localidad de Puriscal.There is no evidence of malnutrition in the Canton of Puriscal as revealed by
careful prospective study of all children born from September 19 of 1979
onwards. Several factors may account for this remarkable situation. First, many
years of systematic and sustanied work by the Ministry of Health to extend
coverage of health services, has resulted in a high technological sanitary level of
the population and art almost universal coverage of vaccinations. Secondly, there
has been a strenghtening of health practices as a result of programs by the Ministries
of Education and Helath, the mass communication media, the Costa Rican
Association of Demography and other national entities, with the added benefit of many international organizations such as the Pan American Health Organization
and UNICEF. Thirdly, there has been a major protection of the neonate
against infections in the last five years, by promoting human milk consumption
with rooming—in and other hospital practices; INISA stimulated these actions and
extended them into the postnatal period by a closefollow—up of mothers and
their children. Fourthly, an important socioeconomic development has been
recorded in Puriscal in recent years which translated into a higher level of information,
employment, and income. Finally, effective community organization has
enabled INISA to coordinate efforts to identify high—risk families for problem—
solving using government and community resources. Accepting that these factors
are responsible for the good nutrition and health of the infant population of
Puriscal, one should ask what specific circumstances and interventions made
possible its dramatic decline in infant mortality in recent years (to about 9 per
1000 live births), which is more pronounced than that of the country as a whole.
The decline may not be totally dependent on the orthodox rural and community
health programs, and the interrelation of several factors superimposed on the
above mentioned improvements may explain it These are: better prenatal care
and maternal nutrition; spacing of pregnancies; provision of human colostrum and
milk to all neonates; and improved medical practice to diagnose and treat the
high •risk neonate. The current crisis could result in a deterioration of the
excellent health situation of Puriscal, but this does not seem to be occurring.
Monthly evaluations of the population by weight/heght, weight/age and heghtiage
indices with regard to the NCHS growth curves, reveal that Puriscal anthropometry
scores better than the rest of the rural area of Costa Rica. A recent
evaluation (September, 1982) did not show stunting or wastage, and according
to the severe criterion of weight/age, only one malnourished child was discovered
and this had a serious underlying problem. The signficance of these findings
for national development, and the possible contribution of INISA to their
occurrence is discussed in the present paper. It is recommended that the Puriscal
Study be continued for addtionai years and that it be reproduced in other areas
of the country, using the infrastructure of the existing rural health programs.Como se indicara en otros trabajos de esta serie, no existe evidencia de que
Ia desnutrición sea un problema prioritario en el Cantón de Puriscal, según revela
el seguimiento cuidadoso de todos los niños nacidos de mujeres de Puriscal a partir
de septiembre de 1979. En la búsqueda de una posible explicación de tan deseable
resultado, varios factures parecen tener importancia. En primer termino, los
resultados observados han sido influenciados por muchos años de trabajo sistemático
y sostenido por el Ministerio de Salud para ampliar Ia cobertura de servicios
de salud, lo que se refleja en el alto nivel tecnológico sanitario de la población y en
la excelente cobertura de inmunizaciones, pare mencionar kilo dos aspectos de los
programas del Gobierno. En segundo termino, ha habido un robustecimiento
de Ia practica de salud en la población, labor en la que ha participado el Ministerio de Educación. Ministerio de Salud, los medios de comunicación, Ia Asociacion
Demográfica Costarricense. y otras entidades , con amplia colaboración de la
Organizacion Panamericana de la Salud y el Unicef En tercer Iugar, ha habido
una mayor protección del neonato contra las infecciones, gracias al fomento de la
lactancia materna desde temprana edad en las sales de maternidad -estimulado
por la labor del INISA extendiéndose postnatalmente con el fomento de la lactancia
materna en forma casi universal durante el primer semestre de vida, labor
en la que ha participado el INISA en to que concierne a Puriscal. En cuarto lugar,
ha habido un buen desarrollo socioeconomico en Puriscal en los últimos arias,
que se ha traducido en un alto nivel de information, empleo, e ingreso. Finalmente,
la buena organizcion comunitaria ha permitido que el INISA coordine
esfuerzos pare identificar familial de alto riesgo y asi tratar de solucionar sus
problemas. Aceptando estos factores como responsabtes de Ia buena nutricion
y salad de la población infantil de Puriscal, cabe preguntarse que circunstancias
e intervenciones han hecho posible el dramático descenso en la tasa de mortalidad
infantil (a 9 por 1000 nacidos vivos) en fecha reciente, y que ha lido mils
rápido que en el resto del pais sin que parezca deberse exclusivamente a los programas
ortodoxos de salud rural y comunitaria. La Unica explicación viable
en el momento es Ia conjunción de cuatro factores mayor nutricion materna y
atencion prenatal; mayor espaciamiento entre los embarazos; provisión de Calostro
a prácticamente todos los neonato; una mayor atencion medica del neonato
de alto riesgo. La crisis económicos actual podría resultar en un deterioro de la
magnifica situación de Puriscal, pero ese no parece set el caso. Evaluaciones
mensuales de la población mediante los indices de peso/talla, peso/edad y talla/
edad empleando las curves del NCHS han revelado indices mils bajos de déficit
ponderal y estatural que en el resto del pais_ Una evaluación reciente
(septiembre bre, 1982) no revelo ni achicamiento ni desgaste. y segun el estricto criterio
de Gomez solo se observa un niño con déficit de mils del 400/o de peso/edad.
La significación de los hallazgos pare el pais, y la posible contribución del INISA
a esos logros mediante las intervenciones en el hospital y en el campo, se discuten
en el presente trabajo. Se recomienda continuar el Estudio de Puriscal y tratar
de reproducirlo en otras áreas del pais, así como emplear Ia infraestructura de
salud rural existente en el pais para implementar programas de seguimiento similares

Low endemicity and low pathogenicity of rotaviruses among rural children in Costa Rica
Artículo científico -- Universidad de Costa Rica. Instituto de Investigaciones en Salud. 1985Rotaviruses were prospectively studied in 51 rural Costa Rican children from birth to two
years. Samples of feces were collected weekly over a 33-month period. Rotavirus was detected
in 45 (1.04%) of 4,317 fecal specimens; 39 infections were documented (an incidence
of 0.5 infection per child-year), only five of which were associated with diarrhea
(a pathogenicity of 12.8%). Secretory antibody in fecal extracts, detected in six of 39 infections,
was short lived and did not protect against reinfection. Serum antibody was present
in 69.6% of two-year-old children, but was not detected in 18.8% with documented infections.
On the other hand, serum antibody was present in six of 14 children in whom rotavirus
was not detected, thus increasing the overall incidence to 0.6 infection per child-year. The
combination of prolonged breast-feeding, exposure to a lower infecting dose (compared
with urban children), and a higher standard of hygiene than expected may explain the

Extinction of Vibrio cholerae in acidic substrata: Contaminated fish marinated with lime juice (ceviche)
Millions of Vibrio cholerae O1 El Tor were rapidly elirninated when added to cornmercial ceviche prepared by marination of mahi-mahi fish in lime juice. Likewise, large masses of viable vibrios present in laboratory contaminated fish, were readily eliminated after immersion in lime juice, during the preparation of ceviche. The killing effect was evident within 5 min of exposure of vibrios to lime juice, with reductions of more than 99.9 % of the initial bacterial mass. After 2 h of marination of fish with lime juice (the minimum recornmended), no vibrios were detected in the lowest working dilutions (1: 10, 1: 1 00). The Vibrio mass eliminated by lime juice was 2 to 6 logarithms greater than the maximum infectious dose, and 4 to 8 logs greater than the minimum infectious dose to induce cholera El Tor. Also, the killing time was shorter than the elapsing time between preparing and serving food in homes or restaurants. The traditional marination of fish with lime juice or its addition to seafood and meals irnmediately before consumption, should be protected and promoted to prevent infection with cholera vibrios. Extinction of Vibrio choleare in acidic substrata: contaminated cabbage and lettuce treated with lime juice

Instituto de Investigaciones en Salud, 1994Lime juice killed Malian of Vibrio cholerae 01, El Tor, Inaba, present on cabbage and lettuce contaminated in the laboratory. The lethal effect was evident within 5 min of exposure to lime juice. No vibrios could be recovered at dilution 1:10 using alkaline peptone water (APW) and thiosulfate-citrate-bile salts-saccharose agar (TCBS). More than 99.9 % of the initial inoculum was effectively destroyed. The number of vibrios killed by lime juice was 2 to 6 logarithms greater than the maximum infecting dose, and 4 to 8 logs greater than the minimum infecting dose for cholera El Tor. The time interval needed for killing was smaller than the usual waiting time for serving food in homes and restaurants. The addition of lime juice to non-acidic foods, beverages and water, is strongly recommended to prevent infection with cholera vibrios and other acid-sensitive microorganisms. This measure is particularly important for rural and slum populations in the tropics and subtropics.El jugo de limón agrio elimina millones de Vibrio cholerae 01, El Tor, presentes en repollo y lechuga contaminados en el laboratorio. El efecto letal fue evidente al cabo de 5 min de exposición al jugo de limón, lográndose una reducción de más del 99.9 % del inoculo inicial. No se recuperaron vibrios a la dilucien 1:10, cultivando en agua peptonada alcalina (APA) y en agar tiosulfato-citrato-sales biliares-sacarosa (TCBS). El número de vibrios destruido por el jugo de limón fue de 2 a 6 logaritmos mayor que la dosis infecciosa máxima, y de 4 a 8 logaritmos mayor que la dosis infecciosa mínima del cólera El Tor. El tiempo requerido para matar los vibriones fue menor que el intervalo usual de espera en hogares o restaurantes. El vinagre tiene un efecto letal sobre el V. cholerae, similar al del jugo de limón. Se recomienda fuertemente la adición de jugo de limón agrio a los alimentos no ácidos, bebidas y agua de beber, para prevenir la infección con vibrios del cólera y otros microorganismos sensibles al ácido. Esta medida es particularmente importante para poblaciones rurales y urbanas marginadas de los trópicos y subtropicos.Universidad de Costa Rica.
