    The multi-thermal and multi-stranded nature of coronal rain

    In this work, we analyse coordinated observations spanning chromospheric, TR and coronal temperatures at very high resolution which reveal essential characteristics of thermally unstable plasmas. Coronal rain is found to be a highly multi-thermal phenomenon with a high degree of co-spatiality in the multi-wavelength emission. EUV darkening and quasi-periodic intensity variations are found to be strongly correlated to coronal rain showers. Progressive cooling of coronal rain is observed, leading to a height dependence of the emission. A fast-slow two-step catastrophic cooling progression is found, which may reflect the transition to optically thick plasma states. The intermittent and clumpy appearance of coronal rain at coronal heights becomes more continuous and persistent at chromospheric heights just before impact, mainly due to a funnel effect from the observed expansion of the magnetic field. Strong density inhomogeneities on spatial scales of 0.2"-0.5" are found, in which TR to chromospheric temperature transition occurs at the lowest detectable scales. The shape of the distribution of coronal rain widths is found to be independent of temperature with peaks close to the resolution limit of each telescope, ranging from 0.2" to 0.8". However we find a sharp increase of clump numbers at the coolest wavelengths and especially at higher resolution, suggesting that the bulk of the rain distribution remains undetected. Rain clumps appear organised in strands in both chromospheric and TR temperatures, suggesting an important role of thermal instability in the shaping of fundamental loop substructure. We further find structure reminiscent of the MHD thermal mode. Rain core densities are estimated to vary between 2x10^{10} cm^{-3} and 2.5x10^{11} cm^{-3} leading to significant downward mass fluxes per loop of 1-5x10^{9} g s^{-1}, suggesting a major role in the chromosphere-corona mass cycle.Comment: Abstract is only short version. Residual confounding or reverse causation cannot be excluded. Therefore, we estimated the causal association of milk consumption with stroke and CHD risk through instrumental variable (IV) and gene-outcome analyses. IV analysis included 29 328 participants (4611 stroke; 9828 CHD) of the European Prospective Investigation into Cancer and Nutrition (EPIC)-CVD (eight European countries) and European Prospective Investigation into Cancer and Nutrition-Netherlands (EPIC-NL) case-cohort studies. rs4988235, a lactase persistence (LP) SNP which enables digestion of lactose in adulthood was used as genetic instrument. Intake of milk was first regressed on rs4988235 in a linear regression model. Next, associations of genetically predicted milk consumption with stroke and CHD were estimated using Prentice-weighted Cox regression. Gene-outcome analysis included 777 024 participants (50 804 cases) from MEGASTROKE (including EPIC-CVD), UK Biobank and EPIC-NL for stroke, and 483 966 participants (61 612 cases) from CARDIoGRAM, UK Biobank, EPIC-CVD and EPIC-NL for CHD. In IV analyses, each additional LP allele was associated with a higher intake of milk in EPIC-CVD (beta = 13 center dot 7 g/d; 95 % CI 8 center dot 4, 19 center dot 1) and EPIC-NL (36 center dot 8 g/d; 95 % CI 20 center dot 0, 53 center dot 5). Genetically predicted milk intake was not associated with stroke (HR per 25 g/d 1 center dot 05; 95 % CI 0 center dot 94, 1 center dot 16) or CHD (1 center dot 02; 95 % CI 0 center dot 96, 1 center dot 08). In gene-outcome analyses, there was no association of rs4988235 with risk of stroke (OR 1 center dot 02; 95 % CI 0 center dot 99, 1 center dot 05) or CHD (OR 0 center dot 99; 95 % CI 0 center dot 95, 1 center dot 03).     Impact of Cognitive Impairment on Functional Outcome in Stroke

    The aim of this study was to investigate the effect of the cognitive impairment on functional status in patients with subacute stroke. Fifty-two patients with subacute stroke were included in the study. Mini mental state examination (MMSE) test was used for the evaluation of cognitive status. Patients were separated into two groups according to their cognitive functions. Functional follow-up parameters were activities of daily living (ADL), global recovery and ambulation status. All patients were evaluated on admission to rehabilitation unit, at discharge and 6 months after discharge. Forty-four patients were completed the study. Mean age was 66 and 57 years; disease duration on admission was 4,8 and 3,5 months in the cognitively impaired and normal groups, respectively. Significant improvement was found in terms of functional follow-up parameters in both groups at discharge (P < .05). Functional follow-up parameters did not show statistically significant difference between the groups. But community ambulation rate was higher in cognitively normal group at the sixth month visit. As a result of this study, inpatient rehabilitation was effective both cognitively normal and impaired subacute stroke patients

EUV darkening and quasi-periodic intensity variations are found to be strongly correlated with coronal rain showers. Progressive cooling of coronal rain is observed, leading to a height dependence of the emission. A fast-slow two-step catastrophic cooling progression is found, which may reflect the transition to optically thick plasma states. The intermittent and clumpy appearance of coronal rain at coronal heights becomes more continuous and persistent at chromospheric heights just before impact, mainly due to a funnel effect from the observed expansion of the magnetic field. Strong density inhomogeneities of 0.″2-0.″5 are found, in which a transition from temperatures of 105 to 104 K occurs. The 0.″2-0.″8 width of the distribution of coronal rain is found to be independent of temperature. The sharp increase in the number of clumps at the coolest temperatures, especially at higher resolution, suggests that the bulk distribution of the rain remains undetected. Rain clumps appear organized in strands in both chromospheric and TR temperatures. We further find structure reminiscent of the magnetohydrodynamic (MHD) thermal mode (also known as entropy mode), thereby suggesting an important role of thermal instability in shaping the basic loop substructure. Rain core densities are estimated to vary between 2 × 1010 and 2.5 × 1011cm−3, leading to significant downward mass fluxes per loop of 1–5 × 109 g s−1, thus suggesting a major role in the chromosphere-corona mass cycle.Publisher PDFPeer reviewe

    Observing the formation of flare-driven coronal rain

GV are funded by the European Research Council under the European Union Seventh Framework Programme (FP7/2007-2013) / ERC grant agreement nr. 291058Flare-driven coronal rain can manifest from rapidly cooled plasma condensations near coronal loop-tops in thermally unstable post-flare arcades. We detect 5 phases that characterise the post-flare decay:heating, evaporation, conductive cooling dominance for ~120 s, radiative/ enthalpy cooling dominance for ~4700 s and finally catastrophic cooling occurring within 35-124 s leading to rain strands with s periodicity of 55-70 s. We find an excellent agreement between the observations and model predictions of the dominant cooling timescales and the onset of catastrophic cooling. At the rain formation site we detect co-moving, multi-thermal rain clumps that undergo catastrophic cooling from ~1 MK to ~22000 K. During catastrophic cooling the plasma cools at a maximum rate of 22700 K s-1 in multiple loop-top sources. We calculated the density of the EUV plasma from the DEM of the multi-thermal source employing regularised inversion. Assuming a pressure balance, we estimate the density of the chromospheric component of rain to be 9.21x1011 ±1.76x1011 cm-3 which is comparable with quiescent coronal rain densities. With up to 8 parallel strands in the EUV loop cross section, we calculate the mass loss rate from the post-flare arcade to be as much as 1.98x1012 ± 4.95x1011 g s-1. Finally, we reveal a close proximity between the model predictions of 105.8 K and the observed properties between 105.9 K and 106.2 K, that defines the temperature onset of catastrophic cooling. The close correspondence between the observations and numerical models suggests that indeed acoustic waves (with a sound travel time of 68 s) could play an important role in redistributing energy and sustaining the enthalpy-based radiative cooling.Publisher PDFPeer reviewe

    Modelling home care organisations from an operations management perspective

    Home Care (HC) service consists of providing care to patients in their homes. During the last decade, the HC service industry experienced significant growth in many European countries. This growth stems from several factors, such as governmental pressure to reduce healthcare costs, demographic changes related to population ageing, social changes, an increase in the number of patients that suffer from chronic illnesses, and the development of new home-based services and technologies. This study proposes a framework that will enable HC service providers to better understand HC operations and their management. The study identifies the main processes and decisions that relate to the field of HC operations management. Hence, an IDEF0 (Integrated Definition for Function Modelling) activity-based model describes the most relevant clinical, logistical and organisational processes associated with HC operations. A hierarchical framework for operations management decisions is also proposed. This analysis is derived from data that was collected by nine HC service providers, which are located in France and Italy, and focuses on the manner in which operations are run, as well as associated constraints, inputs and outputs. The most challenging research areas in the field of HC operations management are also discussed

    Complex chromosome 17p rearrangements associated with low-copy repeats in two patients with congenital anomalies

    Recent molecular cytogenetic data have shown that the constitution of complex chromosome rearrangements (CCRs) may be more complicated than previously thought. The complicated nature of these rearrangements challenges the accurate delineation of the chromosomal breakpoints and mechanisms involved. Here, we report a molecular cytogenetic analysis of two patients with congenital anomalies and unbalanced de novo CCRs involving chromosome 17p using high-resolution array-based comparative genomic hybridization (array CGH) and fluorescent in situ hybridization (FISH). In the first patient, a 4-month-old boy with developmental delay, hypotonia, growth retardation, coronal synostosis, mild hypertelorism, and bilateral club feet, we found a duplication of the Charcot-Marie–Tooth disease type 1A and Smith-Magenis syndrome (SMS) chromosome regions, inverted insertion of the Miller-Dieker lissencephaly syndrome region into the SMS region, and two microdeletions including a terminal deletion of 17p. The latter, together with a duplication of 21q22.3-qter detected by array CGH, are likely the unbalanced product of a translocation t(17;21)(p13.3;q22.3). In the second patient, an 8-year-old girl with mental retardation, short stature, microcephaly and mild dysmorphic features, we identified four submicroscopic interspersed 17p duplications. All 17 breakpoints were examined in detail by FISH analysis. We found that four of the breakpoints mapped within known low-copy repeats (LCRs), including LCR17pA, middle SMS-REP/LCR17pB block, and LCR17pC. Our findings suggest that the LCR burden in proximal 17p may have stimulated the formation of these CCRs and, thus, that genome architectural features such as LCRs may have been instrumental in the generation of these CCRs
