    Some Theorems for Feed Forward Neural Networks

    In this paper we introduce a new method which employs the concept of "Orientation Vectors" to train a feed forward neural network and suitable for problems where large dimensions are involved and the clusters are characteristically sparse. The new method is not NP hard as the problem size increases. We `derive' the method by starting from Kolmogrov's method and then relax some of the stringent conditions. We show for most classification problems three layers are sufficient and the network size depends on the number of clusters. We prove as the number of clusters increase from N to N+dN the number of processing elements in the first layer only increases by d(logN), and are proportional to the number of classes, and the method is not NP hard. Many examples are solved to demonstrate that the method of Orientation Vectors requires much less computational effort than Radial Basis Function methods and other techniques wherein distance computations are required, in fact the present method increases logarithmically with problem size compared to the Radial Basis Function method and the other methods which depend on distance computations e.g statistical methods where probabilistic distances are calculated. A practical method of applying the concept of Occum's razor to choose between two architectures which solve the same classification problem has been described. The ramifications of the above findings on the field of Deep Learning have also been briefly investigated and we have found that it directly leads to the existence of certain types of NN architectures which can be used as a "mapping engine", which has the property of "invertibility", thus improving the prospect of their deployment for solving problems involving Deep Learning and hierarchical classification. The latter possibility has a lot of future scope in the areas of machine learning and cloud computing.Comment: 15 pages 13 figure

    Detection of Cognitive States from fMRI data using Machine Learning Techniques

    Over the past decade functional Magnetic Resonance Imaging (fMRI) has emerged as a powerful technique to locate activity of human brain while engaged in a particular task or cognitive state. We consider the inverse problem of detecting the cognitive state of a human subject based on the fMRI data. We have explored classification techniques such as Gaussian Naive Bayes, k-Nearest Neighbour and Support Vector Machines. In order to reduce the very high dimensional fMRI data, we have used three feature selection strategies. Discriminating features and activity based features were used to select features for the problem of identifying the instantaneous cognitive state given a single fMRI scan and correlation based features were used when fMRI data from a single time interval was given. A case study of visuo-motor sequence learning is presented. The set of cognitive states we are interested in detecting are whether the subject has learnt a sequence, and if the subject is paying attention only towards the position or towards both the color and position of the visual stimuli. We have successfully used correlation based features to detect position-color related cognitive states with 80% accuracy and the cognitive states related to learning with 62.5% accuracy

    KAP Study on Immunization of Children in a City of North India – A 30 Cluster Survey

    Background: To determine the knowledge, attitude and practices about immunization among respondents of children aged 12-23 months.\ud Methods: A total of 510 respondents were interviewed in the urban slums of Lucknow district of India, using 30 cluster sampling technique from January 2005 to April 2005. A pre-tested structured questionnaire was used to elicit the information about the knowledge, attitude and practices of the respondents regarding immunization. \ud Results: Knowledge regarding the disease prevented, number of doses and correct age of administration of BCG was highest among all the categories of respondents. The paramedical worker was the main source of information to the respondents of completely (52.0%) and partially immunized (48.5%) children while community leaders for unimmunized children. Those availing private facilities were more completely immunized, as compared to the government facilities. 55.8% of those who took 20 minutes to reach the immunization site were completely immunized as compared to 64.1% of those who took more than 20 minutes.\ud Conclusion: Considering the incomplete knowledge, and inappropriate practices of the people, the policy makers and medical professionals require Herculean efforts to raise the knowledge and to break the old beliefs of the peopl

    The post-lunch effect of regular and decaffeinated coffee on psychomotor performance: a randomized double-blind cross-over study

    Background: This study aimed to compare the effect of regular coffee and decaffeinated coffee on psychomotor performance in healthy volunteers during post-lunch period.Methods: In this randomized double-blind cross-over study, adult healthy volunteers were given hot coffee (3 g each of regular or decaffeinated coffee) during post-lunch period. Psychomotor functions (critical flicker-fusion frequency (CFF), choice reaction time (CRT) and error count in hand-steadiness test (HST)), blood pressure and heart rate were measured pre-lunch, pre-coffee (1-hour post-lunch) and 1-hour post-coffee consumption. Subjective ratings of sleepiness and mood were also assessed during post-lunch sessions.Results: The mean age of the participants (n=16) was 27.4±2.7 years with a male: female ratio of 7:9. There was no significant deterioration in psychomotor performance post-lunch when compared to pre-lunch on both the days. The mean CFF, CRT, errors committed in HST and cardiovascular parameters did not differ significantly between regular coffee and decaffeinated groups during post-lunch sessions. There was no significant difference in values of cardiovascular parameters as well as subjective ratings of sleep and mood between two groups.Conclusions: In healthy adult individuals’ consumption of both regular coffee and decaffeinated coffee during the post-lunch period did not affect psychomotor performance

    Lumbar Transforaminal Injection of Steroids versus Platelet-Rich Plasma for Prolapse Lumbar Intervertebral Disc with Radiculopathy: A Randomized Double-Blind Controlled Pilot Study

    Study Design Double-blind randomized controlled pilot study. Purpose The purpose of this study was to compare outcomes of steroids with autologous platelet-rich plasma (PRP) administered by lumbar transforaminal injection (LTI) in patients with lumbar radiculopathy. Overview of Literature Degenerative disc disease of the lumbar spine is one of the most common conditions managed by spine surgeons in routine practice. Once conservative management fails, LTI is diagnostic and often therapeutic. Steroids are the gold standard drug used for LTI but have limitations and side effects. Methods In this single-center double-blind randomized controlled pilot study, 46 patients were recruited and randomized by the lottery method. The Visual Analog Scale (VAS) for leg pain, modified Oswestry Disability Index (mODI), and Short-Form 12 (SF-12) were assessed at 1 week, 3 weeks, 6 weeks, 6 months, and 1 year. Results Both groups were comparable in terms of demographics, preprocedure VAS scores, mODI, and SF-12 scores (p=0.52). At the 1-week follow-up, the steroid group had significantly better improvement than the PRP group (p=0.0001). At the 3-week follow-up, both groups showed comparable outcomes; however, the PRP group had better symptom improvement. At 6 weeks and 6 months, the PRP group had better outcomes (VAS, p<0.0001; ODI, p=0.02; SF-12, p=0.002). Moreover, 17 and 16 patients in the steroid and PRP groups underwent repeat LTI with steroids or surgery because of pain recurrence during follow-up. At 1 year, no difference in outcomes was observed. Conclusions PRP may be a useful alternative to steroids for LTI in lumbar radiculopathy. Although improvement was delayed and 1-year outcomes were comparable, the 6-week and 6-month outcomes were better with PRP than with LTI. Multiple PRP injections may be beneficial because of its autologous nature. However, further studies with a larger number of participants, longer follow-up, and repeat LTIs are warranted to draw definite conclusions

    Influence of hospital policy on partograph use in tertiary care facilities in India: a cross sectional survey

    Background: Partograph is an effective, but underutilized tool for monitoring labour. This study examined the influence of hospital policy on the knowledge, attitude, practice and skills of doctors towards partograph plotting.Methods: A cross-sectional survey among doctors posted in labour rooms of tertiary care public teaching hospitals of India was carried out, using self-administered questionnaire to assess knowledge, attitude and practice; and labour case scenarios were provided to assess partograph plotting skills. Chi-square test or Fisher’s exact test were employed to test the significance of association between knowledge, attitude, practice and skills of 150 doctors working in hospitals with (n=3) and without (n=2) policy of routine partograph plotting.Results: The majority of doctors in both groups had correct knowledge about different partograph components. However, doctors working in hospitals with a policy of partograph use, demonstrated significantly better skills using case scenarios for plotting partographs (p<0.01), and a positive attitude towards plotting partograph and its use as a decision support tool (p=0.000) as compared to doctors working in hospitals without a routine partograph plotting policy.Conclusions: Hospital policy of routine partograph plotting may positively influence utilization of partograph in tertiary care public hospitals in India

    Withania somnifera Improves Semen Quality in Stress-Related Male Fertility

    Stress has been reported to be a causative factor for male infertility. Withania somnifera has been documented in Ayurveda and Unani medicine system for its stress-combating properties. However, limited scientific literature is available on this aspect of W. somnifera. We undertook the present study to understand the role of stress in male infertility, and to test the ability of W. somnifera to combat stress and treat male infertility. We selected normozoospermic but infertile individuals (N = 60), further categorized in three groups: normozoospermic heavy smokers (N = 20), normozoospermics under psychological stress (N = 20) and normozoospermics with infertility of unknown etiology (N = 20). Normozoospermic fertile men (N = 60) were recruited as controls. The subjects were given root powder of W. somnifera at a rate of 5 g/day for 3 months. Measuring various biochemical and stress parameters before and after treatment, suggested a definite role of stress in male infertility and the ability of W. somnifera to treat stress-related infertility. Treatment resulted in a decrease in stress, improved the level of anti-oxidants and improved overall semen quality in a significant number of individuals. The treatment resulted in pregnancy in the partners of 14% of the patients

    Gender Differences in Perception and Care-seeking for Illness of Newborns in Rural Uttar Pradesh, India

    Although gender-based health disparities are prevalent in India, very little data are available on care-seeking patterns for newborns. In total, 255 mothers were prospectively interviewed about their perceptions and action surrounding the health of their newborns in rural Uttar Pradesh, India. Perception of illness was significantly lower in incidence (adjusted odds ratio=0.56, 95% confidence interval 0.33-0.94) among households with female versus male newborns. While the overall use of healthcare providers was similar across gender, the average expenditure for healthcare during the neonatal period was nearly four-fold higher in households with males (Rs 243.3±537.2) compared to females (Rs 65.7±100.7) (p=0.07). Households with female newborns used cheaper public care providers whereas those with males preferred to use private unqualified providers perceived to deliver more satisfactory care. These results suggest that, during the neonatal period, care-seeking for girls is neglected compared to boys, laying a foundation for programmes and further research to address gender differences in neonatal health in India
