IntroducciónEn las últimas décadas, la población mayor de 80 años que requiere una cirugía por estenosis aórtica degenerativa ha aumentado sustancialmente.Material y métodosSe compararon los resultados a corto y medio plazo de la sustitución valvular aórtica convencional (SVAo) en estenosis aórtica degenerativa en pacientes mayores y menores de 80 años intervenidos en nuestro centro entre abril de 2004 y diciembre de 2008. Los predictores de mortalidad y eventos adversos mayores cardio y cerebrovasculares (MACCE) en el postoperatorio y el seguimiento se determinaron por análisis multivariante.ResultadosCuatrocientos cincuenta y un pacientes fueron incluidos; 94 (20,8%) eran mayores de 80 años. La cirugía cardíaca previa (odds ratio [OR]: 4,08; p=0,047), la insuficiencia renal (OR: 6,75; p < 0,001), la cirugía coronaria concomitante (OR: 2,57; p=0,034), el sexo femenino (OR: 2,49; p=0,047) y la hipertensión pulmonar grave (OR: 3,68; p=0,024) fueron predictores independientes de mortalidad hospitalaria. En el seguimiento, la edad (hazard ratio [HR]: 2,24; p=0,02), la vasculopatía periférica (HR: 5,1; p < 0,001) e hipertensión arterial (HTA) (HR: 5,2; p=0,025) fueron predictores independientes de mortalidad tardía. Sólo la vasculopatía periférica (HR: 3,55; p=0,014) e HTA (HR: 8,24; p=0,04) fueron predictores independientes de mortalidad cardíaca tardía. La insuficiencia renal (OR: 2,57; p=0,005), hipertensión pulmonar grave (OR: 3,49; p=0,005) y cirugía coronaria asociada (OR: 2,49; p=0,002) fueron predictores independientes de MACCE postoperatorios. Diabetes mellitus (HR: 2,03; p=0,033) y vasculopatía periférica (HR: 2,3; p=0,041) predijeron una mayor incidencia de MACCE en el seguimiento.ConclusionesLa edad mayor de 80 años no empeora el pronóstico a corto y medio plazo tras la SVAo por estenosis aórtica grave degenerativa
Identificação de parvovírus suíno em javalis a partir do sequenciamento parcial do gene VP-2
Este estudo descreve a detecção e a identificação de DNA de parvovírus suíno (PVS) em amostras de órgãos de dois javalis, por PCR e sequenciamento direcionado ao gene VP-2. Pools de órgãos (baço, rins, fígado, linfonodos e tonsila) de três javalis adultos e assintomáticos de Paraguaçu Paulista, SP, criados com propósitos comerciais, foram submetidos à detecção de PVS, resultando em duas amostras positivas após reações de nested-PCR direcionadas aos genes NS-1 e VP-2. Os fragmentos parciais de VP-2 foram sequenciados e comparados a sequências homólogas de cepas NADL-2 e Kresse, demonstrando identidade nucleotídica de 100%. Com relação a 29 cepas de PVS previamente isoladas no Brasil, o grau de identidade nucleotídica variou de 99 a 100% (uma a três substituições de nucleotídeos). Estes resultados demonstram, pela primeira vez, a detecção direta por PCR de parvovírus suíno em javalis, confirmada por análise de sequenciamento genétic

HIV risk behaviours differ by workplace stability among Mexican female sex workers with truck driver clientele
Background. In a study of female sex workers (FSWs) servicing truck driver clients in Mexican border cities, we evaluated differences in HIV/STI risk behaviours determined by workplace. Design and Methods. Our study was cross-sectional and its population comprised 100 FSWs from Nuevo Laredo (US border) and 100 FSWs from Ciudad Hidalgo (Guatemalan border). The main outcome was that the primary place of sex work was unstable in a public place (street, vehicle, gas station, etc.) intead of stable (bar, brothel, and hotel). Logistic regression was used to identify correlates associated with trading sex at unstable workplaces in the last month. Results. Of the FSWs surveyed, 18% reported an unstable workplace. The majority of FSWs surveyed were young (<30 years), single, had <9th grade education, and had worked in the sex trade for a median of 4.9 years. After controlling for study site, FSWs with unstable vs stable workplaces were more likely to have a majority/all truck driver clientele, but were less likely to have visited a gynaecologist in the last year (OR 0.1, 95% CI 0.03-0.4) or ever had an HIV test (OR 0.1, 95% CI 0.06-0.3), and there was a trend towards lower condom use self-efficacy scores (OR 0.8 per unit increase, 95% CI 0.7-1.0). On multivariate regression, unstable workplace was associated with having majority/all truck driver clientele, being surveyed in Nuevo Laredo, and decreased odds of ever having an HIV test. Conclusions. Among Mexican FSWs with truck driver clients, providing safe indoor spaces for sex work may help facilitate public health interventions that improve HIV/STI prevention and reproductive health outcomes

The merger that led to the formation of the Milky Way's inner stellar halo and thick disk
The assembly process of our Galaxy can be retrieved using the motions and
chemistry of individual stars. Chemo-dynamical studies of the nearby halo have
long hinted at the presence of multiple components such as streams, clumps,
duality and correlations between the stars' chemical abundances and orbital
parameters. More recently, the analysis of two large stellar surveys have
revealed the presence of a well-populated chemical elemental abundance
sequence, of two distinct sequences in the colour-magnitude diagram, and of a
prominent slightly retrograde kinematic structure all in the nearby halo, which
may trace an important accretion event experienced by the Galaxy. Here report
an analysis of the kinematics, chemistry, age and spatial distribution of stars
in a relatively large volume around the Sun that are mainly linked to two major
Galactic components, the thick disk and the stellar halo. We demonstrate that
the inner halo is dominated by debris from an object which at infall was
slightly more massive than the Small Magellanic Cloud, and which we refer to as
Gaia-Enceladus. The stars originating in Gaia-Enceladus cover nearly the full
sky, their motions reveal the presence of streams and slightly retrograde and
elongated trajectories. Hundreds of RR Lyrae stars and thirteen globular
clusters following a consistent age-metallicity relation can be associated to
Gaia-Enceladus on the basis of their orbits. With an estimated 4:1 mass-ratio,
the merger with Gaia-Enceladus must have led to the dynamical heating of the
precursor of the Galactic thick disk and therefore contributed to the formation
of this component approximately 10 Gyr ago. These findings are in line with
simulations of galaxy formation, which predict that the inner stellar halo
