    Comparative study of neuroimaging features and clinical symptoms in patients with eclampsia

    Background: Eclampsia is a life-threatening emergency that remains a major cause for feto-maternal morbidity and mortality. The purpose of our study was to access various computed tomographic scan (CT) findings in eclampsia patients and compare neurological symptoms with radiological findings. Methods: A prospective analytical study was undertaken in department of obstetrics and gynecology, J. K. Lon hospital, Kota during the period of 2019-2020. Women who presented as eclampsia and admitted in indoor wards were included in the study. Data analyzed included various maternal and fetal parameters, CT scan findings and outcome of pregnancy. Results: The incidence of eclampsia was 1.1% of total deliveries. High risk factors associated with eclampsia were primigravida (70%), maternal age (70% in 21-25 years age group), illiteracy (64%), inadequate antenatal care (96%), early gestation (68%). On CT scan findings 52% patients had abnormal CT scan findings of which most common was cerebral edema (57.7%). Parieto-occipital lobe was most common region to be affected. Altered sensorium was found to be significantly associated with abnormal CT scan findings.Conclusions: Eclampsia is a major cause of fetal and maternal morbidity and mortality.  CT scan in eclampsia have significant role in early diagnosis of patients with cerebral pathologies and these CT scan findings were associated with the level of consciousness and number of convulsive episodes.  Thus, CT scan helps in further management of these patients by multidisciplinary approach

    Evaluation of oxidative stress in severe acute malnourished children at malnutrition treatment centre of Sardar Patel Medical College, Bikaner, Rajasthan, India

    Background:Malnutrition represents one of the most severe health problems in India. Free radicals play an important role in immunological response, which induces the oxidative surplus in severe acute malnutrition. Severe dietary deficiency of nutrients leads to increased oxidative stress in cellular compartments. The goal of this study was to evaluation of oxidative stress in severe acute malnourished children at malnutrition treatment centre of S. P. Medical College, Bikaner, Rajasthan, India.Methods: The cross sectional study was conducted after the approval of ethical committee, in the Department of Biochemistry and Paediatrics at Sardar Patel medical College of Bikaner. The present study included 100 children between the age of 6 months to 5 years with the help of Paediatrician. Serum Lipid peroxide was measured by precipitating lipoproteins with trichloroacetic acid and boiled with thiobarbituric acid which reacts with Malondialdehyde to give pink colour as per Kei satoh's method.Results: Significantly increased levels of serum malondialdehyde (p <0.001) were found in the patients as compared to those in controls.Conclusion: Deficiency of various nutrients in severe acute malnutrition leads to generation of heavy oxidative stress

    Efficacy of combination of tablet mifepristone and misoprostol for first trimester pregnancy termination at different gestational age

    Background: Medical method of abortion is a safe, effective and affordable method for first trimester termination. However, nausea, vomiting, diarhoea, excess bleeding and incomplete abortion are known side effects. The aim of study was to compare the success rate of abortions in both groups, to compare the induction-abortion interval in both groups and to study the side effects of the drugs.Methods: This study was conducted in department of obstetrics and gynecology, NIMS Medical College, Jaipur, India during June 2017-August 2017. Total hundred cases were taken and divided into two groups. Group A was gestational age upto 49 days, Group B was gestational age 50-63 days. Both groups were given tab. mifepristone 200mg. followed by 48 hrs. by 4 tablets of tab misoprostol.Results: The results of the given regimen were highly successful and complete abortion was achieved in 96% and 94% in group A and group B respectively. The difference between two groups was statistically not significant (p value =0.64). Failure rates were seen in only 4% of group A and 6% of group B. Pain abdomen was seen in 16% of group A and 38% of group B patients. The difference between both groups was statistically significant (p value=0.02). Nausea was seen in 10% of group A and 30% of group B patients. The difference between both groups is statistically significant(p value=0.018).Conclusions: First trimester pregnancy can be successfully terminated with combination of mifepristone and misoprostol upto 63 days (9 weeks) of pregnancy

In the modern world, we have a long list of measures that includes both medical and surgical, which can help us to reduce obesity out of which the therapeutic vaccines show promise. Recent studies have shown that therapeutic vaccines may be new targets for development of anti-obesity medications. These vaccines will aim at multiple targets, which include a spectrum of problems ranging from metabolic derangements to infectious agents including suppressing the appetite-stimulating hormone and blocking absorption of nutrients

    Due to the increasing demand and restriction in building of the new lines complex modern power system presents serious challenges regarding stability of power system. Transient stability is also known as main source of the system insecurity. Because of that in the event of faults and disturbances it has to be maintained for power systems stable operation. In earlier power system to improve the transient stability supplementary controllers are used. These supplementary controllers are the shunt FACT devices like STATCOM (Static Synchronous Compensator) and SVC (Static VAR Compensator). Thesis suggests an application regarding NN (Neural Network). NN determines controller signal of STATCOM for grid active power and reactive power hence this improves stability of power system. Suggested controller is tested on 3-bus 2-machine system. The parameters like terminal voltage of buses, active power transmission line and reactive power of the system. The results which obtained are compared with the FLC system, Neural Network (NN). This proved that the Neural Network STATCOM gives better power result compare to that of STATCOM with FLC system. The effectiveness is verified by the Mat Lab/Simulink Software

    Evaluation of Integrated Childhood Development Services (ICDS) program implementation in an urban slum of Delhi, India

    Background: The Integrated Childhood Development Services (ICDS) scheme is India’s foremost program imparting comprehensive and cost-effective services to meet the multi-dimensional needs of children. Following a populist approach, the program has now increased its umbrella coverage to reproductive age, pregnant and lactating women. The impact of such development and existing program performance remain debatable due to the paucity of evaluation research.Methods: A mix-methods descriptive case study was done using adapted ICDS monitoring tool in a pre-identified slum. The slum was purposely chosen for its intensive habitation of the poor and marginalized population. Primary data were collected through personalized interviews with program staff, health functionaries, and community representatives. Secondary data were collected from records available at Anganwadi centre (AWC). The data were triangulated and analyzed with results being expressed in narrative, simple proportions and percentages.Results: The mean coverage of ICDS services was 58.3%. Maximum coverage recorded for Supplementary Nutrition (SN) and minimal for Nutrition and Health Education (NHE). SN, immunization, Pre-school education (PSE) and growth monitoring (0-3 years) were regularly held. Maternal and child health services were unsatisfactory. Poor community perception reported for AWC and ICDS services with exception of SN.Conclusions: Present case study unveils poor infrastructure, coverage and community participation for ICDS services. Immunisation and referral networking is often dysfunctional reflecting the need for enhanced intersectoral cooperation. Adapted tools could serve as an effective strategy for evaluating and facilitating need-based improvements

It means good conduct. It signifies the physical and mental conducts of a man and it has been designated as an Rasayana – a vitalizer agent for entire span of life. This is a measure having no involvement of drugs and if it is sincerely taken into practice, it may replenish and maintain total life process free from ailments. A careful analysis of qualities of Achara Rasayana reveal that most of them are related with Mana, Dhi, Dhriti along with good conduct and these results in maximum benefit. The principles of Achar Rasayana are more relevant today than ever. It goes into the heart of the issue we are trying to overcome in order to be well and healthy. Environmental toxicity and the food chain we are accustomed to have threatened our lives. The integral connection between nature and man is an understatement

    HEPATIC DYSFUNCTION IN DENGUE: A TERITARY CARE HOSPITAL EXPERINCE

    Objective: The objective of the study was to evaluate liver dysfunction in patients with dengue infections and corelation between liver function test and platelet count. Methods: Hospital-based case−control study conducted on 141 hospitalized with Dengue infection (NS 1 and IgM positive). Dengue seropositive patients are selected and subjected to complete blood count and liver function tests were analyzed. Results: The participants were found to have elevated levels of SGOT, SGPT levels, and lower levels of serum albumin and platelet count as compare to control on evaluation. The significant negative correlation was noted between SGOT/SGPT levels and baseline platelet counts levels. The Pearson correlation between platelet count and SGOT showed r=−0.185 and p&lt;0.01 which proves that when platelet count decreases, the SGOT levels increases. Similarly, for the correlation between platelet count and SGPT showed r=−0.166 and &lt;0.01 which proves that when platelet count decreases, the SGPT levels increase. Conclusion: Statistically significant corelation was observed between liver enzymes with platelet count. Furthermore, the severity of dengue infection predicted the severity of liver derangements. It is recommended that patients with dengue infections be screened for hepatic dysfunction. As hepatic dysfunction in dengue is transient and reversible, early identification of the same would help to reduce life threatening complications

    Evaluation of functional outcome by modified mayo wrist score in intra- articular distal end radius fracture managed by plate osteosynthesis

    Background: Fractures of distal end of radius continue to pose a therapeutic challenge. Intra articular and extra articular malalignment can lead to various complications like post traumatic osteoarthrosis, decreased grip strength and endurance, as well as limited motion and carpal instability.Methods: A prospective study of forty patients with intra-articular distal end radial fractures treated under the department of orthopaedics, PIMS, Udaipur, were included in this study. All patients managed by plate osteosynthesis were followed up to 9 months.Results: When outcome according to modified Mayo wrist scoring system was assessed, only one patient (2.50%) scored with poor grade. 11 (27.50%) were graded as excellent, 19 (22.50%) as grade good, 9 (22.50%) as grade fair. 32 (80%) patients had union within 2-3 months and 06 (15%) patients had union in 3-4 months. There were 2 (5%) case of delayed union.Conclusions: Assessment of functional outcome by modified mayo wrist score in intra-articular distal end radius fracture managed by plate osteosynthesis gives an excellent scoring system to assess outcome of communited intrarticular distal radius fractures with reference to function of the hand and wrist of the individual. Also, it will help to predict the likely functional outcome in relation to fracture pattern of distal radius.
