Referential, vague or impersonal? Pronominal reference and identity. Evidence from the academic writing of Italian students of English
The issue of authorial stance has been the topic of much research. A rather obvious way to claim authorial presence is to use first person reference and the corresponding determiners. However, not only is the I emerging from the text plural, but different genres are associated with differing degrees of I-ism, and these conventions vary from culture to culture. In this paper I explore the issue of authorial stance in connection with identity by focusing on the use of the first person pronouns and their respective determiners in the academic writing of Italian students of English. The study shows that referential, vague and impersonal uses of personal pronouns are intertwined in a complex way and create shades of impersonality in a heteroglossic space in which a reconstruction of the self takes place
Treibholz in Flussläufen â Gefahren und Dynamik
Large Woody Debris (LWD) are an integral component of the fluvial environment. They represent an environmental resource, but even a risk factor for the amplification they could give to the destructive power of a flood event. At present wood dynamics in rivers is not systematically considered within the procedures for the elaboration of hazard maps resulting in underestimation of hazard impacts. The assessment inconsistency comes from the complexity of the question: several aspects in wood processes are not yet well known and the superposition of different physical phenomena results in difficulty to predict critical scenarios.
The research activity is aimed at improve management skills for the assessment of the hydrologic risk associated to the presence of LWD in rivers, improving knowledge about LWD dynamic processes and proposing effective tools for monitoring and mapping river catchments vulnerability.
Using critical review of the published works, field surveys and experimental investigations LWD damaging potential has been analysed to support the identification of the exposed sites and the redaction of hazard maps. Particularly, a survey sheets form for direct measurements has been implemented and tested in field to provide an investigation instruments for wood and river reach monitoring.
Based on a critical analysis of the current state of the art an improved theoretical mechanistic model of LWD entrainment has been proposed, and tested with flume experiments, considering this feature a crucial aspect in wood dynamics. The proposed approach is able to provide a threshold parameter and to discriminate between entrainment modes (sliding, rolling, floating). For each particular configuration of the debris, the provided correlation establishes the threshold value for the proposed entrainment criterion that allows determining the probability of motion and the relative entrainment mode, for a given discharge with a certain recurrence interval.Treibholz ist eine wichtige Ressource unserer Umwelt und ein bedeutendes Element natĂźrlicher Flussläufe. Neben seiner Bedeutung als wichtiger Umweltfaktor stellt es bei Abdrift durch Hochwasser infolge seiner zerstĂśrenden Kraft aber auch ein groĂes Gefährdungspotential dar. Gegenwärtig werden die Auswirkungen von verdriftetem Treibholz bei der Erstellung von Gefahrenkarten nicht berĂźcksichtigt. Dadurch werden die von ihm ausgehenden Gefahren unterschätzt. Die unzuverlässige Einschätzung, der durch Treibholz verursachten ZerstĂśrungen, resultiert aus der Komplexität der sich beim Verdriften abspielenden Prozesse. Schwierigkeiten bei der Einschätzung kritischer Szenarien bereiten neben den artspezifischen Eigenschaften des Holzes vor allem die sich gegenseitig beeinflussenden, unterschiedlichen physikalischen Prozesse.
Das Ziel der vorliegenden Arbeit besteht deshalb in der Verbesserung von Planungsgrundlagen zur sicheren Risikoeinschätzung von Flussläufen mit Treibholz. Ein weiterer Schwerpunkt besteht in der Verbesserung der Grundlagen ßber die dynamischen Prozesse bei der Destabilisierung von Treibgut sowie der Erarbeitung von Monitoringprogrammen und Karten zur Identifizierung von Gefährdungspotentialen entlang des Flusslaufes.
Auf der Grundlage einer detaillierten Literaturrecherche, Kartierungen im Feld und experimentellen Untersuchungen wurde das vorhandene Gefährdungspotential, resultierend aus der Einwirkung von Treibgut, einer kritischen Analyse unterzogen. Die eingeschlagene Vorgehensweise diente der Identifizierung gefährdeter Gebiete und der Erstellung von Gefahrenkarten. Insbesondere wurden ErhebungsbĂśgen fĂźr die direkte MaĂnahmenerfassung entwickelt und im Feld getestet. Sie eignen sich als Monitoringinstrument zur Erfassung des Zustandes der Flussläufe und des Holzes.
Basierend auf einer umfangreichen Dokumentation des Standes des Wissens wurde ein analytischer Ansatz fßr den Bewegungsbeginn von Treibholz entwickelt und mit Daten aus Laborexperimenten untermauert. Er stellt einen wichtigen Aspekt bei der Erfassung und Beschreibung der Dynamik von Treibholz dar. Der empfohlene Ansatz beinhaltet einen Schwellenwert mit dem die unterschiedlichen Bewegungszustände wie Gleiten, Rollen und Abheben erfasst werden kÜnnen. Fßr jede Konfiguration von Holzteilen wird ein Schwellenwert fßr den Bewegungsbeginn angegeben, so dass fßr die drei unterschiedlichen Bewegungszustände das Abdriften von Treibgut fßr vorgegebene Abflßsse abgeschätzt werden kann
NGF e modulazione adrenergica nello scompenso cardiaco
NGF E MODULAZIONE ADRENERGICA NELLO SCOMPENSO CARDIACO

Premessa. L'attivazione del sistema nervoso simpatico (SNS) rappresenta un elemento fisiopatologico essenziale nella progressione e nella prognosi dello scompenso cardiaco, soprattutto alla luce del modello interpretativo neuro-ormonale. Essa si caratterizza per una serie di alterazioni bio-molecolari, talvolta contraddittorie. Oltre ad una ridotta densitĂ di fibre simpatiche, nel cuore scompensato si osserva infatti un ridotto reuptake e, al contempo, un aumentato spillover della noradrenalina (NA), come se il cuore fosse anche funzionalmente denervato. Inoltre, il sistema recettoriale β-adrenergico è intaccato sia quantitativamente, per la ridotta espressione dei recettori β1 e β2 sulla superficie del cardiomiocita, sia qualitativamente, per fenomeni di desensibilizzazione. Nonostante allo stato attuale siano disponibili presidi farmacologici (β-bloccanti, ACE-inibitori, sartanici) e non farmacologici (training aerobico) in grado di agire â direttamente o indirettamente â sul SNS, la prognosi dello scompenso cardiaco rimane ancora infausta. NGF (Nerve Growth Factor) è un fattore di crescita con attivitĂ neurotrofica prodotto da diversi tipi cellulari, anche al di fuori del sistema nervoso. Recentemente è stato osservato che la sua produzione cardiaca è ridotta nei soggetti affetti da scompenso rispetto a controlli sani. Inoltre, NGF è in grado - in vitro ed in modelli animali di scompenso â di indurre sul cardiomiocita modificazioni opposte a quelle appena descritte, ovvero di aumentare densitĂ di innervazione simpatica, lâuptake della NA ed lâespressione β-recettoriale.
Scopo. Obiettivo del nostro lavoro è valutare, nei pazienti affetti da scompenso cardiaco, la presenza di correlati clinici con il livello di produzione cardiaca di NGF, e stabilire se esso influenza la risposta cardiovascolare ad una serie di stimoli adrenergici.
Metodi. Quattordici pazienti, con diagnosi di scompenso sistolico, sono stati arruolati prospettivamente presso lâUnitĂ Operativa di Medicina Cardiovascolare dellâIstituto di Fisiologia Clinica - Fondazione Toscana G. Monasterio. Tutti i pazienti (7 maschi; etĂ 59 Âą 11 anni; frazione di eiezione 26.8 Âą 8.3; classe NYHA I-II 86%, III-IV 14%) erano in trattamento medico ottimale, ma in wash-out di terapia β-bloccante. Ciascuno ha ricevuto una caratterizzazione clinica, strumentale e bio-umorale completa, ed è stato sottoposto ad una batteria di prove neurovegetative (deep-breathing, manovra di Valsalva, handgrip, tilt test, test dellâatropina e dellâesmololo). Inoltre è stata valutata la competenza cronotropa al test da sforzo cardiopolmonare e la sensibilitĂ barocettiva, ed infine è stata condotta unâanalisi spettrale nel dominio del tempo e della frequenza sulla registrazione Holter ECG di 48 ore. Le concentrazioni plasmatiche di NGF e delle catecolamine (A e NA), sono state calcolate - con metodo ELISA e HPLC rispettivamente - su campioni ematici prelevati dal seno coronarico, da arteria e da sangue venoso misto, nel corso di un cateterismo cardiaco.
Risultati. La concentrazione di NGF rilevata nel sangue venoso misto è risultata inferiore (mediana 11,05 pg/ml; range 8,93-14,37 pg/ml, 5°-95° percentile) ai valori relativi a soggetti sani riportati in letteratura (20-100 pg/ml). Contrariamente a quanto osservato in precedenza, il gradiente trans-cardiaco di NGF non è risultato invariabilmente positivo, infatti, nella metĂ dei soggetti, la concentrazione di NGF in seno coronarico era inferiore a quella arteriosa. Nessuna differenza statisticamente significativa è stata osservata, nei parametri clinici, strumentali e bio-umorali, tra queste due sottopopolazioni. Nella popolazione totale, la concentrazione di NGF in seno coronarico correlava positivamente con la sensibilitĂ barocettiva (p=0,032) e con un indice di variabilitĂ della frequenza cardiaca, SDANN (p=0,044); quella in arteria con lâentitĂ della risposta allâatropina (p<0,001), con la deviazione standard dellâintervallo RR, SDrr, p=0.036 e con SDANN, p=0,01. Nel gruppo con gradiente positivo, lâentitĂ del gradiente correlava negativamente con la risposta al test dellâesmololo (p<0,001).
Discussione. Nel presente lavoro abbiamo osservato una diminuita concentrazione plasmatica di NGF nei pazienti con scompenso cardiaco, attribuibile tanto ad una perdita numerica quanto ad una riduzione della capacitĂ secretiva dei cardiomiociti. Inoltre la correlazione positiva tra livelli di NGF in seno coronarico, sensibilitĂ barocettiva e indici nel dominio nel tempo di variabilitĂ della frequenza cardiaca â entrambi importanti fattori prognostici nello scompenso cardiaco â suggerisce che il rilascio di NGF da parte del cardiomiocita possa rappresentare un fattore capace di influenzare positivamente, in funzione compensatoria, la modulazione autonomica della risposta cronotropa del nodo del seno
Biomarkers for the diagnosis and management of heart failure
Heart failure (HF) is a significant cause of morbidity and mortality worldwide. Circulating biomarkers reflecting pathophysiological pathways involved in HF development and progression may assist clinicians in early diagnosis and management of HF patients. Natriuretic peptides (NPs) are cardioprotective hormones released by cardiomyocytes in response to pressure or volume overload. The roles of B-type NP (BNP) and N-terminal pro-B-type NP (NT-proBNP) for diagnosis and risk stratification in HF have been extensively demonstrated, and these biomarkers are emerging tools for population screening and as guides to the start of treatment in subclinical HF. On the contrary, conflicting evidence exists on the role of NPs as a guide to HF therapy. Among the other biomarkers, high-sensitivity troponins and soluble suppression of tumorigenesis-2 are the most promising biomarkers for risk stratification, with independent value to NPs. Other biomarkers evaluated as predictors of adverse outcome are galectin-3, growth differentiation factor 15, mid-regional pro-adrenomedullin, and makers of renal dysfunction. Multi-marker scores and genomic, transcriptomic, proteomic, and metabolomic analyses could further refine HF management
Cytocompatibility and Uptake of Halloysite Clay Nanotubes
Halloysite is aluminosilicate clay with hollow tubular structure of 50 nm external diameter and 15 nm diameter lumen. Halloysite biocompatibility study is important for its potential applications in polymer composites, bone implants, controlled drug delivery, and for protective coating (e.g., anticorrosion or antimolding). Halloysite nanotubes were added to different cell cultures for toxicity tests. Its fluorescence functionalization by aminopropyltriethosilane (APTES) and with fluorescently labeled polyelectrolyte layers allowed following halloysite uptake by the cells with confocal laser scanning microscopy (CLSM). Quantitative Trypan blue and MTT measurements performed with two neoplastic cell lines model systems as a function of the nanotubes concentration and incubation time indicate that halloysite exhibits a high level of biocompatibility and very low cytotoxicity, rendering it a good candidate for household materials and medicine. A combination of transmission electron microscopy (TEM), scanning electron microscopy (SEM), and scanning force microscopy (SFM) imaging techniques have been employed to elucidate the structure of halloysite nanotubes
The place of vericiguat in the landscape of treatment for heart failure with reduced ejection fraction
The significant morbidity and mortality associated with heart failure with reduced (HFrEF) or preserved ejection fraction (HFpEF) justify the search for novel therapeutic agents. The nitric oxide (NO)âsoluble guanylate cyclase (sGC)-cyclic guanosine monophosphate (cGMP) pathway plays an important role in the regulation of cardiovascular function. This pathway is disrupted in HF resulting in decreased protection against myocardial injury. The sGC activator cinaciguat increases cGMP levels by direct, NO-independent activation of sGC, and may be particularly effective in conditions of increased oxidative stress and endothelial dysfunction, and then reduced NO levels, but this comes at the expense of a greater risk of hypotension. Conversely, sGC stimulators (riociguat and vericiguat) enhance sGC sensitivity to endogenous NO, and then exert a more physiological action. The phase 3 VICTORIA trial found that vericiguat is safe and effective in patients with HFrEF and recent HF decompensation. Therefore, adding vericiguat may be considered in individual patients with HFrEF, particularly those at higher risk of HF hospitalization; the efficacy of the sacubitril/valsartan-vericiguat combination in HFrEF is currently unknown
