    A new strategic framework for water-related health research

    The aim of this study was to compile a new strategic framework to guide the funding and management of research in waterrelated human health in South Africa. This framework had to identify the research areas of highest need in the country and provide an effective, yet simple, tool for the management of research projects. A review of current water-related human health research was first undertaken. Using this review as background document, input was solicited from key people in Government, water boards, metro councils, science councils and universities. As part of the study, the country’s research support infrastructure for water-related health research was investigated. The framework was finalised at a stakeholder workshop. A structure of Thrust Areas, with Programmes under each Thrust Area, was suggested for research management and funding. A matrix system of research thrusts versus impacts/risks, interventions and governance was further constructed to assist in the identification of research gaps, rendering the framework a very useful tool in the funding and management of water-related human health research.Keywords: water, public health, research needs, research managemen

    Kappia lobulata (Apocynaceae, Periplocoideae), a new genus from South Africa

    Kappia, a new genus from the Fish River Valley in the Eastern Cape Province, South Africa is presented. At first described as Raphionacme lobulata Venter and R.L.Verh. [Venter, H.J.T., Verhoeven, R.L. 1988. Raphionacme lobulata (Periplocaceae), a new species from the eastern Cape Province, South Africa. South African Journal of Botany 54, 603–606.] based on a single specimen collected in 1936, recently discovered plants of this species proved it to be a new genus. In habit Kappia resembles Baseonema Schltr. and Rendle, Batesanthus N.E.Br., Mondia Skeels and Stomatostemma N.E.Br. However, as far as floral structure is concerned, Kappia reveals more affinity with Raphionacme Harv. DNA sequence data show Kappia to be distinct from Batesanthus, Mondia and Raphionacme Harv. and weakly supported as a sister to Stomatostemma

    Post‑fledging movement and spatial ecology of the endangered Cape Vulture (\u3ci\u3eGyps coprotheres\u3c/i\u3e)

    The post-fledging dependence period (PFDP) is one of the most critical stages in the life history of some avian species. Birds are particularly sensitive to mortality during this stage as they must learn essential skills, such as efficient locomotion, proficient food location and predator avoidance. Knowledge of the PFDP would provide valuable information for conservation management of endangered species, many of which experience high juvenile fatality rates. Post-fledging movements of five endangered Cape Vultures were recorded using Global Positioning System/Global System for Mobile communication telemetry in South Africa. Home range sizes, distances travelled from the nest and habitat use were determined over 11 months during the PFDP. Fledglings increased their home range progressively for the first 2 months, then exhibited a rapid increase in home range size associated with dispersal from their natal colony. Maximum net daily distance also rapidly increased following the dispersal period. A preference for protected areas and woody vegetation (representing cliff faces used for roosting) in terms of habitat use for foraging was evident. The knowledge of the movement and habitat use of juvenile Cape Vultures can aid in the effective conservation planning for the species. Conservation programs in identified areas can be focused on power line and wind-farm mitigation, areas of supplementary feeding and anti-poisoning events

    Ontology‐driven perspective of CFRaaS

    A Cloud Forensic Readiness as a Service (CFRaaS) model allows an environment to preemptively accumulate relevant potential digital evidence (PDE) which may be needed during a post‐event response process. The benefit of applying a CFRaaS model in a cloud environment, is that, it is designed to prevent the modification/tampering of the cloud architectures or the infrastructure during the reactive process, which if it could, may end up having far‐reaching implications. The authors of this article present the reactive process as a very costly exercise when the infrastructure must be reprogrammed every time the process is conducted. This may hamper successful investigation from the forensic experts and law enforcement agencies perspectives. The CFRaaS model, in its current state, has not been presented in a way that can help to classify or visualize the different types of potential evidence in all the cloud deployable models, and this may limit the expectations of what or how the required PDE may be collected. To address this problem, the article presents the CFRaaS from a holistic ontology‐driven perspective, which allows the forensic experts to be able to apply the CFRaaS based on its simplicity of the concepts, relationship or semantics between different form of potential evidence, as well as how the security of a digital environment being investigated could be upheld. The CFRaaS in this context follows a fundamental ontology engineering approach that is based on the classical Resource Description Framework. The proposed ontology‐driven approach to CFRaaS is, therefore, a knowledge‐base that uses layer‐dependencies, which could be an essential toolkit for digital forensic examiners and other stakeholders in cloud‐security. The implementation of this approach could further provide a platform to develop other knowledge base components for cloud forensics and security

    H3 histamine receptor-mediated activation of protein kinase calpha inhibits the growth of cholangiocarcinoma in vitro and in vivo

    Histamine regulates functions via four receptors (HRH1, HRH2, HRH3, and HRH4). The D-myo-inositol 1,4,5-trisphosphate (IP(3))/Ca(2+)/protein kinase C (PKC)/mitogen-activated protein kinase pathway regulates cholangiocarcinoma growth. We evaluated the role of HRH3 in the regulation of cholangiocarcinoma growth. Expression of HRH3 in intrahepatic and extrahepatic cell lines, normal cholangiocytes, and human tissue arrays was measured. In Mz-ChA-1 cells stimulated with (R)-(alpha)-(-)-methylhistamine dihydrobromide (RAMH), we measured (a) cell growth, (b) IP(3) and cyclic AMP levels, and (c) phosphorylation of PKC and mitogen-activated protein kinase isoforms. Localization of PKC alpha was visualized by immunofluorescence in cell smears and immunoblotting for PKC alpha in cytosol and membrane fractions. Following knockdown of PKC alpha, Mz-ChA-1 cells were stimulated with RAMH before evaluating cell growth and extracellular signal-regulated kinase (ERK)-1/2 phosphorylation. In vivo experiments were done in BALB/c nude mice. Mice were treated with saline or RAMH for 44 days and tumor volume was measured. Tumors were excised and evaluated for proliferation, apoptosis, and expression of PKC alpha, vascular endothelial growth factor (VEGF)-A, VEGF-C, VEGF receptor 2, and VEGF receptor 3. HRH3 expression was found in all cells. RAMH inhibited the growth of cholangiocarcinoma cells. RAMH increased IP(3) levels and PKC alpha phosphorylation and decreased ERK1/2 phosphorylation. RAMH induced a shift in the localization of PKC alpha expression from the cytosolic domain into the membrane region of Mz-ChA-1 cells. Silencing of PKC alpha prevented RAMH inhibition of Mz-ChA-1 cell growth and ablated RAMH effects on ERK1/2 phosphorylation. In vivo, RAMH decreased tumor growth and expression of VEGF and its receptors; PKC alpha expression was increased. RAMH inhibits cholangiocarcinoma growth by PKC alpha-dependent ERK1/2 dephosphorylation. Modulation of PKC alpha by histamine receptors may be important in regulating cholangiocarcinoma growth.     Epilepsy in rural South African children prevalence, associated disability and management

    Objective. To determine the prevalence of epilepsy and its associated disabilities in rural South African children aged 2 - 9 years.Setting. Eight villages in the district of Bushbuckridge, Northern Province, South Africa_Design. A two-phase design was used_ The first phase involved screening children on a house-to-house baSis by interviewing mothers or caregivers using an internationally validated questionnaire for detecting childhood disability in developing countries. The second phase consisted of a paediatric/ neurodevelopmental assessment of the children who screened positive.Results. A total of 6692 children were screened; 722 (10.8%) had a paediatric evaluation and 49 (0.73%) had epilepsy. The· lifetime and active prevalences of epilepsy in these children were 7.3/1 000 and 6.7/1 000 respectively. Associated developmental disability was recorded in 35 affected children (71.4%), including 8 (16.3%) in whom this was moderate to severe. More than a half of the children with epilepsy (57.1%) did not receive anticonvulsant medication.Conclusion. The prevalence of epilepsy in the rural childhood population investigated is higher than that recorded in most similar studies from sub-Saharan Africa, and the poor utilisation of 'appropriate anticonvulsant treatment is cause for concern. This study highlights the paucity of relevant information on the epidemiology of epilepsy in South Africa and that the system available for its management, especially in rural areas, appears to have functional deficiencies. Appropriate research is needed to identify the problems in service delivery and to enable the planning and implementation of an appropriate primary health care-based system for the diagnosis and management of epilepsy in children

    Prevalence of free-living acanthamoeba and its associated bacteria in energy-efficient hot water systems in South Africa

    As part of the Eskom rebate programme, energy-efficient hot water systems such as solar water heaters (low pressure), heat pumps and energy-efficient showerheads were rolled out to the public as a measure to conserve and save energy. There has been a concern that these systems may not reach the required high temperatures, especially during winter, and, as a result of this, Acanthamoeba and its associated bacteria such as Legionella pneumophila, Pseudomonas aeruginosa and nontuberculous mycobacteria could flourish within these systems causing a potential health risk to consumers. This study examined the relationship between Acanthamoeba and its associated bacteria at different temperature ranges. A total of 156 water (69) and biofilm samples (87) were collected from a solar water heater, heat pump, geyser and showerheads and examined for these organisms using amoebal enrichment and molecular techniques. Amoeba could be cultivated from 45 (65.2%) of the water samples and 56 (64.4%) of the biofilm samples. The study confirmed the presence of Legionella pneumophila, Pseudomonas aeruginosa and nontuberculous mycobacteria in the hot water systems at both of the simulated winter (20°C to 30°C) and higher summer (40°C to 55°C) temperatures as well as the control system. There was a significant positive correlation between the presence of Acanthamoeba and the presence of Pseudomonas. Based on this association it is suggested that Pseudomonas aeruginosa could be investigated as an indicator organism for the presence of Acanthamoeba and opportunistic pathogens

    Theory of commensurable magnetic structures in holmium

    The tendency for the period of the helically ordered moments in holmium to lock into values which are commensurable with the lattice is studied theoretically as a function of temperature and magnetic field. The commensurable effects are derived in the mean-field approximation from numerical calculations of the free energy of various commensurable structures, and the results are compared with the extensive experimental evidence collected during the last ten years on the magnetic structures in holmium. In general the stability of the different commensurable structures is found to be in accord with the experiments, except for the tau=5/18 structure observed a few degrees below T_N in a b-axis field. The trigonal coupling recently detected in holmium is found to be the interaction required to explain the increased stability of the tau=1/5 structure around 42 K, and of the tau=1/4 structure around 96 K, when a field is applied along the c-axis.Comment: REVTEX, 31 pages, 7 postscript figure

    Aortic arch baroreceptor stimulation in an experimental goat model: a novel method to lower blood pressure

    The effect of aortic baroreceptor stimulation on blood pressure manipulation was assessed using the goat species Capra aegagrus hircus. The aim of this study was to manipulate blood pressure with future intention to treat high blood pressure in humans. The ages of the animals ranged from 6 months to 2 years. A standard anesthesia protocol was used. A lateral thoracotomy was performed to gain access to the aortic arch. Data was collected with the Vigileo system. Pre stimulation blood pressure was compared with maximum post stimulation blood pressure values. Results were analyzed with the Wilcoxon signed rank test. In the study 38 animals were enrolled. Baroreceptor stimulation was performed for each animal using 3 different electrodes each of which emits an electrical impulse. In the pilot phase of the study, the median baseline blood pressure prior to stimulation of the baroreceptors was 110.8 mmHg. After stimulation the median blood pressure decreased to 88 mmHg. The average decrease in blood pressure was 22.8 mmHg. This decrease of blood pressure after stimulation of the baroreceptors is statistically significant (p < 0.0001) and the proof of concept was shown. During the extended phase all three probes had a significant effect on blood pressure lowering (p < 0.0001). The study confirmed that aortic baroreceptor stimulation has an effect on blood pressure lowering. This is a novel field of blood pressure manipulation. The hemodynamic effects of long-term aortic baroreceptor stimulation are unknown. Further investigations need to be done to determine whether a similar effect can be induced in different species such as primates and humans

    Overview of international naturopathic practice and patient characteristics: results from a cross-sectional study in 14 countries.

    BACKGROUND:Naturopathy is a distinct system of traditional and complementary medicine recognized by the World Health Organization and defined by its philosophic approach to patient care, rather than the treatments used by practitioners. Worldwide, over 98 countries have practicing naturopaths, representing 36% of all countries and every world region. The contributions of naturopaths to healthcare delivery services internationally has not been previously examined. Thus, the primary intention of this research was to conduct an international survey of naturopathic practice and patient characteristics in order to gain insight to the breadth of their practices and the type of clinical conditions routinely encountered. METHODS:The cross-sectional study was conducted in naturopathic clinics in 14 countries within 4 world regions including the European (Portugal, United Kingdom, Switzerland, Spain), Americas (Canada, United States, Chile, Brazil), Western Pacific (Hong Kong, Australia, New Zealand) and African (South Africa). Naturopathic practitioners in each country were invited to prospectively complete an online survey for 20 consecutive cases. The survey was administered in four languages. RESULTS:A total of 56 naturopaths from 14 countries participated in the study, providing a mean of 15.1 cases each (SD 7.6) and 851 cases in total. Most patients were female (72.6%) and all age categories were represented with a similar proportion for 36-45 years (20.2%), 46-55 years (19.5%), and 56-65 years (19.3%). A substantial majority (75%) of patients were considered by the participant to be presenting with chronic health conditions. The most prevalent category of health conditions were musculoskeletal (18.5%), gastrointestinal (12.2%), and mental illness (11.0%). The most common treatment categories prescribed or recommended to patients by the participants were dietary changes (60.5%), lifestyle and behaviour changes (56.9%), herbal medicines (54.2%) and nutritional supplements (52.1%). Many patients were known by participants to be receiving care from a general practitioner (43.2%) or a specialist medical practitioner (27.8%). CONCLUSIONS:Naturopathic practitioners provide health care for diverse health conditions in patients in different age groups. The global population would benefit from researchers and policy makers paying closer attention to the potential risks, benefits, challenges and opportunities of the provision of naturopathic care within the community
