Including osteoprotegerin and collagen IV in a score-based blood test for liver fibrosis increases diagnostic accuracy

BACKGROUND: Noninvasive methods for liver fibrosis evaluation in chronic liver diseases have been recently developed, i.e. transient elastography (Fibroscan™) and blood tests (Fibrometer®, Fibrotest®, and Hepascore®). In this study, we aimed to design a new score in chronic hepatitis C (CHC) by selecting blood markers in a large panel and we compared its diagnostic performance with those of other noninvasive methods. METHODS: Sixteen blood tests were performed in 306 untreated CHC patients included in a multicenter prospective study (ANRS HC EP 23 Fibrostar) using METAVIR histological fibrosis stage as reference. The new score was constructed by non linear regression using the most accurate biomarkers. RESULTS: Five markers (alpha-2-macroglobulin, apolipoprotein-A1, AST, collagen IV and osteoprotegerin) were included in the new function called Coopscore©. Using the Obuchowski Index, Coopscore© shows higher diagnostic performances than for Fibrometer®, Fibrotest®, Hepascore® and Fibroscan™ in CHC. Association between Fibroscan™ and Coopscore© might avoid 68% of liver biopsies for the diagnosis of significant fibrosis. CONCLUSION: Coopscore© provides higher accuracy than other noninvasive methods for the diagnosis of liver fibrosis in CHC. The association of Coopscore© with Fibroscan™ increases its predictive value

Elevated creatine kinase activity in primary hepatocellular carcinoma

BACKGROUND: Inconsistent findings have been reported on the occurrence and relevance of creatine kinase (CK) isoenzymes in mammalian liver cells. Part of this confusion might be due to induction of CK expression during metabolic and energetic stress. METHODS: The specific activities and isoenzyme patterns of CK and adenylate kinase (AdK) were analysed in pathological liver tissue of patients undergoing orthotopic liver transplantation. RESULTS: The brain-type, cytosolic BB-CK isoenzyme was detected in all liver specimens analysed. Conversely, CK activity was strongly increased and a mitochondrial CK (Mi-CK) isoenzyme was detected only in tissue samples of two primary hepatocellular carcinomas (HCCs). CONCLUSION: The findings do not support significant expression of CK in normal liver and most liver pathologies. Instead, many of the previous misconceptions in this field can be explained by interference from AdK isoenzymes. Moreover, the data suggest a possible interplay between p53 mutations, HCC, CK expression, and the growth-inhibitory effects of cyclocreatine in HCC. These results, if confirmed, could provide important hints at improved therapies and cures for HCC

Evaluation de l'impact de la mise en place de la prescription connectée et de la formation à la juste prescription des examens biologiques dans un service des maladies infectieuses et tropicales (intérêt en épidémiologie interventionnelle)

La prescription d examens biologiques joue un rôle déterminant dans la prise en charge médicale des patients, et a considérablement augmenté au cours des dix dernières années. Si une part de cette inflation traduit une amélioration des pratiques, une fraction non négligeable semble pourtant correspondre à une prescription inappropriée, responsable de nombreux effets délétères. Différentes stratégies ont été expérimentées afin de maîtriser cette dérive, et tendre vers une juste prescription des examens biologiques. Dans ce contexte, l hôpital Saint-Antoine s est engagé dans une démarche d amélioration de la qualité de ses prescriptions. L introduction d un système de prescription connectée des examens biologiques a été décidée, complétée dans un second temps par l éducation des médecins prescripteurs et la mise en place d alertes informatiques. Cette intervention concernait le service des maladies infectieuses et tropicales, pilote pour l étude. Les critères de jugement étaient le volume total d examens biologiques prescrits, le volume d examens biologiques répétés et leurs délais de répétition, avant et après les deux volets de l intervention, et ce pour un panel d examens usuels sélectionnés. L analyse des résultats ne retrouvait aucun effet de la mise en place de la prescription connectée sur l ensemble des critères choisis. En revanche, la période correspondant à l éducation des prescripteurs et à l installation des alertes informatiques enregistrait une diminution sensible du volume total d examens biologiques, du volume d examens répétés, et des répétitions précoces de prescription de protéine C-réactive. L intervention initiée a donc permis de manière simple, de contrôler efficacement la prescription d examens biologiques, et de diminuer les répétitions inutiles. Dans notre étude, la mise en place de la prescription connectée, à elle seule, n a pas eu d effet sur les prescriptions. Néanmoins, l installation de cet outil est l occasion d initier une ou plusieurs stratégies de maîtrise de la prescription d examens biologiques. Parmi celles-ci, l éducation des prescripteurs et l utilisation d alertes répétition semblent pertinentes.Laboratory tests prescription is crucial to take care of patients, and has considerably increased during the last ten years. If a part of this enhancement results from an improvement of medical consciousness, inappropriate prescription seems to be the principal source of this trend, causing several deleterious effects. Several strategies have been experimented to control this problem. Considering this situation, Saint-Antoine hospital (Paris XII) engaged an initiative for better laboratory tests prescription. Computer physician order entry (CPOE) was installed, completed in second place by physicians education and repetition informatics alerts. This intervention was conducted in the department of infectious and tropical diseases of the hospital, chosen as testing department. Outcomes were the total volume of laboratory tests, the volume of repeated tests, and time before repeated prescriptions, before and after the intervention, regarding a sample of selected tests. CPOE did not have any effect on the outcomes. On the contrary, physician s education and alerts allowed an important reduction of laboratory tests prescription, and an extension of the time preceding a repeated prescription.Thus, our intervention was an effective and simple way to contain laboratory tests prescription. L'écureuil roux européen (sciurus vulgaris) (synthèse bibliographique et application a la forêt de la Baule-Escoublac (44))

L'écureuil roux européen est, malgré son mode de vie diurne, un rongeur que l'on connaît assez peu car il reste en général assez sauvage. Pourtant dans certaines communes comme la Baule-Escoublac, il côtoie de manière quasi-intime les hommes. L'étude a pour but de faire le point sur les connaissances actuelles : ainsi les trois premières parties sont consacrées à la description des caractères morphologiques de ce petit mammifère, à l'étude des fonctions de nutrition puis des fonctions de reproduction. Pour ce dernier point l'auteur insiste particulièrement sur la dynamique de population de cette espèce et sur les dangers encourus face à l'évolution des pratiques sylvicoles et face à l'introduction d'un dangereux concurrent comme l'écureuil gris nord-américain.
