    Uso de risedronato na consolidação e formação do calo na fratura de Colles em mulheres na pós‐menopausa – Estudo Solid

    ResumoObjetivoEste estudo multicêntrico, randomizado, aberto, grupo paralelo avaliou a eficácia de Actonel® 35 mg mais cálcio/vitamina D versus cálcio/vitamina D isoladamente na preservação da densidade mineral óssea (DMO) em mulheres pós‐menopausadas com fratura de Colles.MétodosPacientes com fratura de Colles em sete dias foram aleatoriamente designadas para receber Actonel® 35 mg semanalmente mais cálcio/vitamina D (Grupo AO [GAO]) ou cálcio/vitamina D (grupo O [GO]) isoladamente. As pacientes foram avaliadas após 90 e 180 dias de tratamento.ResultadosCompletaram as avaliações 59 pacientes no GAO e 56 no OG. No fim do estudo, a DMO do rádio no local da fratura mostrou variação negativa no GO (32,8%) que foi discretamente menor no GAO (20,8%), assim como uma perda menor na DMO no GAO comparado com o OG. Houve diferença na proporção de paciente com perda da DMO no fim do estudo nos dois grupos de tratamento em favor do GAO, apesar de não estatisticamente significante. Não houve diferença significativa na identificação radiológica da formação do calo entre os grupos de tratamento. Na maioria das pacientes a identificação radiológica do calo ocorreu depois de 90 dias.ConclusãoMulheres pós‐menopausadas com fratura de Colles que receberam risedronato sódico, além do cálcio/vitamina D, comparado com cálcio/vitamina D não mostraram diferença significativa na perda da DMO na fratura do antebraço, com tendência de efeito protetor do risedronato na perda da DMO devido à imobilização. O tempo até a consolidação da fratura não foi afetado.AbstractObjectiveThis open, randomized and blinded parallel‐group multicenter study evaluated the efficacy of Actonel® (35mg) plus calcium/vitamin D versus calcium/vitamin D alone for preserving bone mineral density (BMD) in postmenopausal women with Colles fractures.MethodsPatients with a Colles fracture for seven days were randomized to receive either Actonel® (35mg) once a week plus calcium/vitamin D (ACD group) or calcium/vitamin D alone (CD group). The patients were evaluated after 90 and 180 days of treatment.ResultsCompleted all the evaluations 59 ACD patients and 56 CD patients. At the end of the study, the BMD of the radius at the fracture location showed a negative change in the CD group (32.8%). The loss of BMD in the ACD group (20.8%) was slightly less than in the CD group. There was a difference in the proportions of patients with BMD losses at the end of the study period in the two treatment groups, in favor of the ACD group, although this was not statistically significant. There was no significant difference in radiological identification of callus formation between the treatment groups. In the majority of the patients, the callus could be radiologically identified after 90 days.ConclusionPostmenopausal women with Colles fractures who received risedronate sodium plus calcium/vitamin D did not show any significant difference in BMD loss in forearm fractures, in comparison with those who received calcium/vitamin D alone. Risedronate presented a tendency towards a protective effect regarding BMD loss due to immobilization. The time taken for fracture consolidation to be achieved was unaffected

    A modelagem digital do terreno (MDT) é um modelo matemático, o qual possibilita a representação tridimensional do comportamento de uma determinada variável. Uma das vantagens da MDT é a facilidade de estudo de um fenômeno sem que haja a necessidade de estar diretamente no local escolhido para a pesquisa. Esse trabalho tem como objetivo a análise da agricultura, do PIB, do IDHe da população da microrregião Litoral Sul do estado da Paraíba, bem como as consequências ao ecossistema natural por conta do desmatamento causado pelo avanço das lavouras de cana-de-açúcar (Saccharum officinarum L.). O trabalho mostra como o incentivo à produção da batata-doce (Ipomoeabatatas) pode ser útil na diversificação do setor primário.The digital terrain modeling (DTM) is a mathematical model, which enables the three-dimensional representation of the behavior of a particular variable. One of the advantages of the MDT is the facilityin studying a phenomenon without the need of being directly in the location chosen for the research.     Qualidade de vida de pacientes submetidos à artroplastia total primária de joelho: Life quality of patients submitted to knee’s primary total arthroplasty

    O objetivo dessa pesquisa foi estudar a qualidade de vida dos pacientes submetidos à artroplastia total de joelho. Foram avaliados 41 pacientes submetidos a artroplastia total de joelho, operados em um hospital universitário, no período de janeiro de 2012 a junho de 2017, em relação a qualidade de vida, por meio de entrevista presencial usando a versão brasileira validada do Questionário de Qualidade de Vida SF-36 e a Pergunta de Satisfação, aplicados 3 anos e 5 anos após procedimento (seguimento até fevereiro de 2017 a julho de 2022). Dos pacientes avaliados, 35 eram do sexo feminino e 06 do sexo masculino, a idade mínima foi de 41 anos e máxima de 76 anos, com média de idade de 68,6 ±4,7 anos, tempo de seguimento de 5 anos. Na avaliação quanto à satisfação 95,1% dos pacientes ficaram satisfeitos com a intervenção. Observou-se que não houve diferença estatisticamente significante entre os satisfeitos e insatisfeitos para os componentes capacidade funcional, aspecto físico, dor, saúde geral e vitalidade, do questionário SF-36. Por outro lado, para os componentes aspecto social, aspecto emocional e saúde mental, o grupo satisfeito apresentou média estatisticamente mais elevada, ou seja, menos comprometido, que o grupo não satisfeito. O impacto da cirurgia na qualidade de vida foi positivo em todos os pacientes, inclusive nos não satisfeitos, onde os componentes relacionados aos aspectos físicos foram positivos, porém tinham componente emocional com escore baixo, que influenciou negativamente na qualidade de vida e na satisfação

    Nanostructured polymeric system based of cashew gum for oral admnistration of insulin

    The subcutaneous administration of insulin has been the treatment of millions of diabetics in the world.However, for such via insulin is invasive and not mimics the physiological action causing side effects. Theoral route would be the most physiological and comfortable option, but the oral bioavailability of insulin islow by proteolytic activity and reduced permeability of the gastrointestinal tract. The aim of the study was todevelop a nanostructured system integrating biomaterials for oral insulin delivery. Cashew gum (CG) is apolysaccharide extracted from the exudate of the plant Anacardium occidentale. It is a biopolymer composedof simple sugars and glucuronic acid and it can be used in nanostructured systems for the incorporation ofmolecules. The exudate was isolated, dissolved in water, filtered, precipitated in ethanol and purified. TheCG was characterized by infrared spectroscopy and molecular weight by size exclusion chromatography.Nanoparticles were prepared through ionotropic gelation integrating cashew gum, dextran sulfate and poloxamercontaining insulin stabilized with chitosan, poly(ethyleneglycol) and coated with albumin. The particleswere analyzed for particle size, zeta potential and insulin entrapment efficiency. The FTIR spectrum for CGshowed a band at 3395 cm-1 due to the stretching vibration of O-H, a band at 2926 cm-1 of C-H vibrations;absorption at 1639 cm-1 of O-H type from bound water molecules and bands at 1143, 1073 and 1024 cm-1 duevibrations of the C-O-C from glycosidic bonds and O-H of alcohols. The peak molar mass of GC was 2.35 ×104 g/mol. The particles had a size of 156 nm and after coating, size of 5387 nm with 92% insulin entrapmentefficiency and zeta potential of -51 mV indicating electrostatic stabilization. The results suggest an innovativecashew gum base system for oral insulin administration.Keywords: Nanostructures, biomaterials, cashew gum, insulin, oral delivery

    Virus Infections on Prion Diseased Mice Exacerbate Inflammatory Microglial Response

    We investigated possible interaction between an arbovirus infection and the ME7 induced mice prion disease. C57BL/6, females, 6-week-old, were submitted to a bilateral intrahippocampal injection of ME7 prion strain (ME7) or normal brain homogenate (NBH). After injections, animals were organized into two groups: NBH (n=26) and ME7 (n=29). At 15th week after injections (wpi), animals were challenged intranasally with a suspension of Piry arbovirus 0.001% or with NBH. Behavioral changes in ME7 animals appeared in burrowing activity at 14 wpi. Hyperactivity on open field test, errors on rod bridge, and time reduction in inverted screen were detected at 15th, 19th, and 20th wpi respectively. Burrowing was more sensitive to earlier hippocampus dysfunction. However, Piry-infection did not significantly affect the already ongoing burrowing decline in the ME7-treated mice. After behavioral tests, brains were processed for IBA1, protease-resistant form of PrP, and Piry virus antigens. Although virus infection in isolation did not change the number of microglia in CA1, virus infection in prion diseased mice (at 17th wpi) induced changes in number and morphology of microglia in a laminar-dependent way. We suggest that virus infection exacerbates microglial inflammatory response to a greater degree in prion-infected mice, and this is not necessarily correlated with hippocampal-dependent behavioral deficits

    Early and Late Pathogenic Events of Newborn Mice Encephalitis Experimentally Induced by Itacaiunas and Curionópolis Bracorhabdoviruses Infection

    In previous reports we proposed a new genus for Rhabdoviridae and described neurotropic preference and gross neuropathology in newborn albino Swiss mice after Curionopolis and Itacaiunas infections. In the present report a time-course study of experimental encephalitis induced by Itacaiunas and Curionopolis virus was conducted both in vivo and in vitro to investigate cellular targets and the sequence of neuroinvasion. We also investigate, after intranasal inoculation, clinical signs, histopathology and apoptosis in correlation with viral immunolabeling at different time points. Curionopolis and Itacaiunas viral antigens were first detected in the parenchyma of olfactory pathways at 2 and 3 days post-inoculation (dpi) and the first clinical signs were observed at 4 and 8 dpi, respectively. After Curionopolis infection, the mortality rate was 100% between 5 and 6 dpi, and 35% between 8 and 15 dpi after Itacaiunas infection. We identified CNS mice cell types both in vivo and in vitro and the temporal sequence of neuroanatomical olfactory areas infected by Itacaiunas and Curionopolis virus. Distinct virulences were reflected in the neuropathological changes including TUNEL immunolabeling and cytopathic effects, more intense and precocious after intracerebral or in vitro inoculations of Curionopolis than after Itacaiunas virus. In vitro studies revealed neuronal but not astrocyte or microglial cytopathic effects at 2 dpi, with monolayer destruction occurring at 5 and 7 dpi with Curionopolis and Itacaiunas virus, respectively. Ultrastructural changes included virus budding associated with interstitial and perivascular edema, endothelial hypertrophy, a reduced and/or collapsed small vessel luminal area, thickening of the capillary basement membrane, and presence of phagocytosed apoptotic bodies. Glial cells with viral budding similar to oligodendrocytes were infected with Itacaiunas virus but not with Curionopolis virus. Thus, Curionopolis and Itacaiunas viruses share many pathological and clinical features present in other rhabdoviruses but distinct virulence and glial targets in newborn albino Swiss mice brain

    Do Diabetes à Doença Renal: Uma Revisão Sistemática dos Benefícios Nefroprotetores dos Inibidores de SGLT2

    Esta revisão sistemática aborda o papel nefroprotetor dos inibidores do cotransportador de sódio-glicose tipo 2 (SGLT2) em pacientes com diabetes. A análise compreensiva de estudos clínicos e diretrizes atuais destaca não apenas a eficácia desses agentes no controle glicêmico, mas também seus efeitos diretos na prevenção da progressão da doença renal crônica (DRC). Os inibidores de SGLT2 demonstraram consistentemente redução significativa da proteinúria, efeitos hemodinâmicos benéficos, e uma associação clara com a redução do risco cardiovascular. As diretrizes da KDIGO reconhecem o papel integral desses medicamentos na gestão da nefropatia diabética. Esses achados sugerem que os inibidores de SGLT2 representam uma abordagem terapêutica promissora para pacientes com diabetes e risco de complicações renais
