Risk Factors and Interpretation of Inconclusive Endoscopic Ultrasound-Guided Fine Needle Aspiration Cytology in the Diagnosis of Solid Pancreatic Lesions
Background: The inconclusive cytological findings of endoscopic ultrasound-guided fine needle aspiration (EUS-FNA) remain a major clinical challenge and often lead to treatment delays. Methods: Patients who had undergone EUS-FNA sampling for solid pancreas lesions between 2014 and 2021 were retrospectively enrolled. The “atypical” and “non-diagnostic” categories of the Papanicolaou Society of Cytopathology System were considered inconclusive and the “negative for malignancy” category of malignancy was suspected clinically. We determined the frequency and predictors of inconclusive cytological finding. Results: A total of 473 first EUS-FNA samples were included, of which 108 cases (22.83%) were inconclusive. Significant increases in the odds of inconclusive cytological findings were observed for lesions with a benign final diagnosis (OR 11.20; 95% CI 6.56–19.54, p 4 cm (OR 0.16; 95% CI 0.08–0.31, p < 0.001) compared to lesions ≤2 cm. Conclusions: The more than two punctures per EUS-FNA sampling with larger-diameter needle (19 G or 22 G) using the slow-pull and standard suction techniques in combination may decrease the probability of inconclusive cytological findings
Formulation and investigation of turmeric extract and sodium benzoate loaded capsules
Turmeric has been studied and used as a plant derivate active ingredient for centuries. Several effects of turmeric have been described, however the poor solubility of its active ingredients during the formulation development may limit oral applicability. The aim of research was to involve the development of a dietary supplement of a hard capsule containing turmeric (Curcuma longa) (ground powder from the root standardized to curcuminoid content) and sodium benzoate. The manufacturing technology and the analytic method of the formulation have been developed. Formulation studies according to the Ph. Hg. VIII. standards have been performed. The product may have insulin sensitizing and memory enhancing effects, related animal studies are ongoing
A familiáris hiperkoleszterinémia korszerű gyógyszeres kezelési lehetőségei
A koleszterin és egyéb lipidek anyagcseréje központi szerepet játszik az emberi szervezet működésében, a lipidanyagcsere-folyamatok összetett és harmonikus működésében létrejövő rendellenességek azonban komoly egészségkárosodást okozhatnak. A familiáris hiperkoleszterinémia az LDL-anyagcsere veleszületett rendellenessége, ami korai érelmeszesedést okoz, és ennek szövődményeként többek között korai kardiovaszkuláris halálozáshoz vezethet. A familiáris hiperkoleszterinémia heterozigóta formája hozzávetőlegesen 500 emberből egyet érint, a súlyosabb homozigóta forma egymillió emberből egynél fordul elő.
Familiáris hiperkoleszterinémiában szenvedő heterozigóta betegeknél nehéz fenntartani az optimális lipidparamétereket, a nagyon ritkán előforduló homozigóta formában pedig napjaink terápiás eszközeivel ez sokszor kivitelezhetetlen. A betegség időben történő adekvát kezelésével megelőzhető lenne a szívkoszorúér-megbetegedés és a többi súlyos szövődmény, ezért kiemelten fontos olyan gyógyszerek kifejlesztése, amelyek kedvező mellékhatásprofillal képesek az LDL-koleszterin szintjének hatékony csökkentésére.
A familiáris hiperkoleszterinémia patomechanizmusainak alaposabb megismerése újfajta farmakológiai célpontokat világított meg az elmúlt években. A közelmúltban ígéretes kísérletek történtek PCSK9 fehérje elleni antitestekkel, a koleszterinészter-transzferprotein (CETP) gátlásával, az apoB-100 antiszensz oligonukleotiddal történő inhibíciójával, valamint a mikroszomális triglicerid-transzferprotein (MTTP) gátlásával. Az utóbbi két elvre épülő gyógyszereket (mipomersen, illetve lomitapid) már engedélyezték az Egyesült Államokban 2013-ban, illetve 2012-ben. A PCSK9 inhibitorai közül az evolocumab esetleg már 2015-ben megjelenhet a gyógyszerpiacon.

A nyelv ritka, benignus, ectomesenchymalis chondromyxoid tumora
Absztrakt:
Az ectomesenchymalis chondromyxoid tumor a szájüreg igen ritka benignus
lágyrész-daganata, napjainkig mindösszesen 93 esetet közöltek a nemzetközi
szakirodalomban. Klinikailag csaknem kizárólag a nyelvhát elülső harmadában
alakul ki, lassan növekvő, változatos nagyságú, fájdalmatlan, tömött, submucosus
csomó formájában jelentkezik. A tumort szövetileg myxoid/chondromyxoid stromában
hálózatos szerkezetbe rendeződött, kerek, poligonális vagy orsó alakú sejtek
építik fel. Habár a megjelenés nagyban hasonlít a bőr és a lágyrészek
myoepitheliomájára, a hisztogenezis máig nem tisztázott, azonban egy újabban
megjelent, nagy esetszámot tartalmazó közlemény RREB1-MKL2
génfúziót igazolt az esetek 90%-ában, ami a daganat önálló entitását
bizonyíthatja. A pontos szövettani diagnózis felállítását az immunhisztokémiai
vizsgálatok biztosítják. Közleményünkben egy 62 éves férfi betegünk esetét
mutatjuk be, aki a nyelvcsúcsán észlelt egy 3 cm nagyságú, fájdalmatlan, lassan
növekvő csomót. A tervezett műtéti beavatkozáson nem jelent meg, csak 4 évvel
később jelentkezett ismét, ekkorra a sebészi kezelésbe már beleegyezett. A
daganat eltávolítását követően 24 hónappal panaszmentes, lokális recidíva nem

Abstract:
Ectomesenchymal chondromyxoid tumor is a rare benign intraoral soft tissue
neoplasm. Till date, 93 cases have been reported in literature. Clinically, it
presents as a slowly growing, painless, firm, submucosal swelling mainly
occurring on the anterior two thirds of the dorsum of the tongue.
Histopathologically, the tumor is characterized by a well circumscribed, lobular
proliferation of round, polygonal, or fusiform cells in a net-like pattern in a
myxoid to chondromyxoid background. Although it shows a morphological
resemblance to myoepitheliomas of soft tissue and skin, the exact histogenesis
remains unclear; a recent publication of a large series has demonstrated
recurrent RREB1-MKL2 gene fusions in 90% of the cases,
suggesting the possibility of a new entity. The diagnosis is largely supported
by immunhistochemical investigations. Here, we present a new case of ECT
occurring in a 62-year-old male who presented with a leading complaint of slowly
growing painless mass arising from the anterior part of the tongue measuring
about 3 cm. This lesion was diagnosed 4 years before recent admittance, but the
patient did not undergo the scheduled surgical intervention and was lost to
follow-up. As the tumor was still persisting and moderately enlarging, he came
back to our clinic. Finally, the mass was completely resected; after 24 month of
follow-up, he is doing well, with no signs of recurrence. Orv Hetil. 2019;

An unusual cause of infant's stridor - congenital laryngocele
Congenital laryngocele is an uncommon cause of neonatal stridor. There are only a few cases reported in the literature. The authors present a successfully treated case of an infant, whose life could only be saved by urgent tracheostomy. On the 5th postoperative day endoscopic excision and marsupialization provided patent airway. The patient could be decannulated. During follow-up no recurrence was observed
Graph theory in action: evaluating planned highway tracks based on connectivity measures
Maintaining connectivity among local populations in a fragmented landscape is crucial for the survival of many species. For isolated habitat patches, stochastic fluctuations and reduced gene flow can lead to high risk of extinction. The connectivity of the landscape is especially crucial for the carabid species living in the fragmented forests of the Bereg plain (NE Hungary and W Ukraine) because a highway will be constructed through the plain. Our purpose is to (1) evaluate the impacts of three possible highway tracks, (2) suggest a solution that is realistic with less impact on connectivity than other plans and (3) discuss how to decrease the disadvantageous effects of each track. Our results, based on a network analysis of landscape graph of patches and ecological corridors, indicate that the intended highway could have deleterious consequences on forest-living carabids. Relatively simple actions, like the establishment of stepping stones, could compensate for the loss of habitat connectivity and promote the survival of carabids, or minor modi?cations in one possible track could diminish its adverse effects. While many other studies would be needed for a comprehensive assessment of the biotic impact of the highway, we provide an example on the usefulness of network analysis for land use management
