    Cell-free microRNAs as early predictors of graft viability during ex vivo normothermic machine perfusion of human donor livers

    Background Cell-free microRNAs (miRs) have emerged as early and sensitive biomarkers for tissue injury and function. This study aimed to investigate whether the release of hepatocyte-derived microRNAs (HDmiRs) and cholangiocyte-derived miRs (CDmiRs) correlates with hepato-cholangiocellular injury and function during oxygenated, normothermic machine perfusion (NMP) of human liver grafts. Methods Donor livers (n = 12), declined for transplantation, were subjected to oxygenated NMP (6 hours) after a period of static cold storage (median 544 minutes (IQR 421-674)). Perfusate and bile samples were analyzed by qRT-PCR for HDmiR-122 and CDmiR-222. Spearman correlations were performed between miR levels and currently available indicators and classic markers. Results Both HDmiR-122 and CDmiR-222 levels in perfusate at 30 minutes of NMP strongly correlated with hepatocyte injury (peak perfusate AST) and cholangiocyte injury (peak biliary LDH). In bile, only CDmiR-222 correlated with these injury markers. For hepato-cholangiocellular function, both miRs in perfusate correlated with total bilirubin, while HDmiR-122 (in perfusate) and CDmiR-222 (in bile) correlated with bicarbonate secretion. Both the relative ratio of HDmiR-122/CDmiR-222 and AST in perfusate at 30 minutes significantly correlated with cumulative bile production, but only the relative ratio was predictive of histopathological injury after 6 hours NMP. Conclusion Early levels of HDmiR-122 and CDmiR-222, in perfusate and/or bile, are predictive of excretory functions and hepato-cholangiocellular injury after 6 hours NMP. These miRs may represent new biomarkers for graft viability and function during machine perfusion

    De Staat van zoönosen geeft jaarlijks een overzicht van infectieziekten die overgaan van dier op mens, de zogenoemde zoönosen. Het gaat om de mate waarin meldingsplichtige zoönosen voorkomen en de ontwikkelingen daarvan op de lange termijn. Hierbij betreft het zowel het aantal ziektegevallen bij mensen als het voorkomen van deze ziekteverwekkers bij dieren. Ook worden elk jaar opmerkelijke voorvallen uitgelicht en wordt een thema behandeld. Voor de meeste zoönosen zijn in 2014 geen uitgesproken veranderingen waargenomen. Wel is het aantal mensen met leptospirose (waarvan de bekendste vorm de ziekte van Weil is) aanmerkelijk hoger dan het vorige jaar, van gemiddeld 30 gevallen in de afgelopen jaren naar 97 in 2014. Ook steeg het aantal Hantavirusinfecties (van gemiddeld 13 in de voorgaande jaren naar 36 in 2014). UItgelicht Een opmerkelijke gebeurtenis in 2014 is dat twee patiënten in een ziekenhuis zijn opgenomen met een ernstige longontsteking na een infectie met Chlamydia caviae. Beide patiënten bleken thuis cavia's te houden die een luchtweginfectie hadden doorgemaakt. Verder was er sinds 2003 weer een uitbraak van vogelgriep bij pluimveebedrijven veroorzaakt door een hoogpathogeen virus. Hierbij zijn vier van de vijf besmette bedrijven onafhankelijk van elkaar besmet geraakt. Het virus was vermoedelijk afkomstig van trekkende watervogels. Het is onbekend of dit virustype overdraagbaar is op de mens; wereldwijd zijn daar geen gevallen van bekend. Vogels Het thema van dit jaar is 'Onze gevleugelde vrienden' en gaat over zoönosen die via vogels kunnen worden overgebracht, zoals de papegaaienziekte. Hierbij wordt onder andere beschreven op welke vliegroutes van trekvogels gezamenlijke broed- en voederplaatsen liggen waar ze elkaar kunnen treffen en zoönoseverwekkers aan elkaar zouden kunnen overdragen.The Zoonotic Diseases Report provides an annual overview of infectious diseases transmitted from animals to humans, so-called zoonotic diseases or zoonoses. It focuses on the degree to which notifiable zoonoses occur and how they develop over the long term. Specifically, this concerns both the number of human cases and the occurrence of these pathogens in animals. Noteworthy incidents of zoonoses are also highlighted each year and a particular theme is discussed. For most zoonoses, no pronounced changes were observed in 2014. Nonetheless, the number of people with leptospirosis (of which the most well-known form is Weil's disease) was considerably higher than in the previous year, rising from an average of 30 cases in recent years to 97 cases in 2014. The number of Hantavirus infections also rose (from an average of 13 cases in the previous years to 36 cases in 2014). A closer look A notable event in 2014 is the admission of two patients to hospital with serious lung infections after being infected with Chlamydia caviae. Both patients kept guinea pigs at home that suffered from respiratory infections. There was also the first outbreak of highly pathogenic avian influenza since 2003 affecting five poultry farms. Four of the five farms became contaminated independently of each other. It is thought that the virus was transmitted from waterfowls. It is not known whether the particular virus strain can be transmitted to humans; worldwide no cases of such transmission have been reported. Birds This year's theme is 'Our winged friends'. It focuses on zoonoses that can be transmitted by birds, such as psittacosis (parrot fever). The migratory routes that coincide with shared breeding and feeding locations where migratory birds can come into contact with one another and thereby possibly transmit zoonotic pathogens to each other are also described.NVW

    Waiting list mortality and the potential of donation after circulatory death heart transplantations in the Netherlands

    BACKGROUND: With more patients qualifying for heart transplantation (HT) and fewer hearts being transplanted, it is vital to look for other options. To date, only organs from brain-dead donors have been used for HT in the Netherlands. We investigated waiting list mortality in all Dutch HT centres and the potential of donation after circulatory death (DCD) HT in the Netherlands. METHODS: Two different cohorts were evaluated. One cohort was defined as patients who were newly listed or were already on the waiting list for HT between January 2013 and December 2017. Follow-up continued until September 2018 and waiting list mortality was calculated. A second cohort of all DCD donors in the Netherlands (lung, liver, kidney and pancreas) between January 2013 and December 2017 was used to calculate the potential of DCD HT. RESULTS: Out of 395 patients on the waiting list for HT, 196 (50%) received transplants after a median waiting time of 2.6 years. In total, 15% died while on the waiting list before a suitable donor heart became available. We identified 1006 DCD donors. After applying exclusion criteria and an age limit of 50 years, 122 potential heart donors remained. This number increased to 220 when the age limit was extended to 57 years. CONCLUSION: Waiting list mortality in the Netherlands is high. HT using organs from DCD donors has great potential in the Netherlands and could lead to a reduction in waiting list mortality. Cardiac screening will eventually determine the true potential

    Identification and Validation Model for Informative Liquid Biopsy-Based microRNA Biomarkers:Insights from Germ Cell Tumor In Vitro, In Vivo and Patient-Derived Data

    Liquid biopsy-based biomarkers, such as microRNAs, represent valuable tools for patient management, but often do not make it to integration in the clinic. We aim to explore issues impeding this transition, in the setting of germ cell tumors, for which novel biomarkers are needed. We describe a model for identifying and validating clinically relevant microRNAs for germ cell tumor patients, using both in vitro, in vivo (mouse model) and patient-derived data. Initial wide screening of candidate microRNAs is performed, followed by targeted profiling of potentially relevant biomarkers. We demonstrate the relevance of appropriate (negative) controls, experimental conditions (proliferation), and issues related to sample origin (serum, plasma, cerebral spinal fluid) and pre-analytical variables (hemolysis, contaminants, temperature), all of which could interfere with liquid biopsy-based studies and their conclusions. Finally, we show the value of our identification model in a specific scenario, contradicting the presumed role of miR-375 as marker of teratoma histology in liquid biopsy setting. Our findings indicate other putative microRNAs (miR-885-5p, miR-448 and miR-197-3p) fulfilling this clinical need. The identification model is informative to identify the best candidate microRNAs to pursue in a clinical setting

    A Proof of Concept Study on Real-Pime LiMAx CYP1A2 Liver Function Assessment of Donor Grafts During Normothermic Machine Perfusion

    No single reliable parameter exists to assess liver graft function of extended criteria donors during ex-vivo normothermic machine perfusion (NMP). The liver maximum capacity (LiMAx) test is a clinically validated cytochromal breath test, measuring liver function based on 13CO2 production. As an innovative concept, we aimed to integrate the LiMAx breath test with NMP to assess organ function. Eleven human livers were perfused using NMP. After one hour of stabilization, LiMAx testing was performed. Injury markers (ALT, AST, miR-122, FMN, and Suzuki-score) and lactate clearance were measured and related to LiMAx values. LiMAx values ranged between 111 and 1838 µg/kg/h, and performing consecutive LiMAx tests during longer NMP was feasible. No correlation was found between LiMAx value and miR-122 and FMN levels in the perfusate. However, a significant inverse correlation was found between LiMAx value and histological injury (Suzuki-score, R = − 0.874, P < 0.001), AST (R = − 0.812, P = 0.004) and ALT (R = − 0.687, P = 0.028). Furthermore, a significant correlation was found with lactate clearance (R = 0.683, P = 0.043). We demonstrate, as proof of principle, that liver function during NMP can be quantified using the LiMAx test, illustrating a positive correlation with traditional injury markers. This new breath-test application separates livers with adequate cytochromal liver function from inadequate ones and may support decision-making in the safe utilization of extended criteria donor grafts

    Human Bile Contains Cholangiocyte Organoid-Initiating Cells Which Expand as Functional Cholangiocytes in Non-canonical Wnt Stimulating Conditions

    Diseases of the bile duct (cholangiopathies) remain a common indication for liver transplantation, while little progress has been made over the last decade in understanding the underlying pathophysiology. This is largely due to lack of proper in vitro model systems to study cholangiopathies. Recently, a culture method has been developed that allows for expansion of human bile duct epithelial cells grown as extrahepatic cholangiocyte organoids (ncECOs) in non-canonical Wnt-stimulating conditions. These ncECOs closely resemble cholangiocytes in culture and have shown to efficiently repopulate collagen scaffolds that could act as functional biliary tissue in mice. Thus far, initiation of ncECOs required tissue samples, thereby limiting broad patient-specific applications. Here, we report that bile fluid, which can be less invasively obtained and with low risk for the patients, is an alternative source for culturing ncECOs. Further characterization showed that bile-derived cholangiocyte organoids (ncBCOs) are highly similar to ncECOs obtained from bile duct tissue biopsies. Compared to the previously reported bile-cholangiocyte organoids cultured in canonical Wnt-stimulation conditions, ncBCOs have superior function of cholangiocyte ion channels and are able to respond to secretin and somatostatin. In conclusion, bile is a new, less invasive, source for patient-derived cholangiocyte organoids and makes their regenerative medicine applications more safe and feasible

    Predicting unplanned hospital visits in older home care recipients: a cross-country external validation study.

Several risk scores have been developed to identify older adults at risk of unplanned hospital visits. It is unclear whether risk scores developed in one country, perform as well in another. This study validates seven risk scores to predict unplanned hospital admissions and emergency department (ED) visits in older home care recipients from six countries. Methods: We used the IBenC sample (n = 2446), a cohort of older home care recipients from six countries (Belgium, Finland, Germany, Iceland, Italy and The Netherlands) to validate four specific risk scores (DIVERT, CARS, EARLI and previous acute admissions) and three frailty indicators (CHESS, Fried Frailty Criteria and Frailty Index). Outcome measures were unplanned hospital admissions, ED visits or any unplanned hospital visits after 6 months. Missing data were handled by multiple imputation. Performance was determined by assessing calibration and discrimination (area under receiver operating characteristic curve (AUC)). Results: Risk score performance varied across countries. In Iceland, for any unplanned hospital visits DIVERT and CARS reached a fair predictive value (AUC 0.74 [0.68-0.80] and AUC 0.74 [0.67-0.80]), respectively). In Finland, DIVERT had fair performance predicting ED visits (AUC 0.72 [0.67-0.77]) and any unplanned hospital visits (AUC 0.73 [0.67-0.77]). In other countries, AUCs did not exceed 0.70. Conclusions: Geographical validation of risk scores predicting unplanned hospital visits in home care recipients showed substantial variations of poor to fair performance across countries. Unplanned hospital visits seem considerably dependent on healthcare context. Therefore, risk scores should be validated regionally before applied to practice. Future studies should focus on identification of more discriminative predictors in order to develop more accurate risk scores.
