    Research and Development of the Pupil Identification and Warning System using AI-IoT

    Currently, pupils being left in the classroom, in the house or in the car is happening a lot, causing unintended incidents. The reason is that parents or caregivers of pupils go through busy and tiring working hours, so they accidentally leave pupils in the car, indoors, or forget to pick up students at school. In this paper, we developed an algorithm to recognize students who use neural networks and warn managers, testing on a model integrated Raspberry Pi 4 kit programmed on Python in combination with cameras, sim modules, and actuators to detect and alert abandoned pupils to the manager to take timely remedial measures and avoid unfortunate circumstances. With the ability to manage students, the system collects and processes images and data on student information for artificial intelligence (AI) systems to recognize when operating. The system of executive structures serves to warn when students are left in the car, in the classroom, or in the house to avoid unintended incidents or safety risks

    Based on the sustainability assessment indicators conducted previously, the authors studied and built the sustainable indicators for the factors involved in the assessment with 6 themes and 38 indicators as well as sustainable levels (5 levels) in order to assess the sustainable conditions in coastal zone at the  pilot site in Phu Cat district. The authors also proposed to apply the method of analysis hierarchy process (AHP) to establish a set of weights for each subject and indicator. After calculating the assessment results for each indicator through the method of fuzzy comprehensive evaluation (FCE) by the membership functions, the overall evaluation results for the entire Phu Cat district were done. Through the implementation of these methods the evaluation results showed that the sustainability of Phu Cat district is at average level.Dựa vào các bộ chỉ thị đánh giá tính bền vững đã công bố, các tác giả nghiên cứu, thiết lập bộ chỉ thị với 6 nhóm chủ đề và 38 chỉ thị và các bậc bền vững (5 bậc) cho các yếu tố phục vụ công tác đánh giá tính bền vững đới bờ, thí điểm tại huyện Phù Cát, tỉnh Bình Định.     Assessment of the results of clinical pharmacists’ interventions on the use of antibiotics at a medical center in Vietnam between 2021 and 2022

    Context: The rational use of antibiotics is a priority when antibiotic resistance has become severe. Clinical pharmacists’ interventions can help increase the rate of rational antibiotic use. Aims: To determine the effect of clinical pharmacists’ interventions on the use of antibiotics and factors related to inappropriate antibiotic use in inpatients at the Department of Surgery, Gia Rai Town Medical Center, Vietnam. Methods: An interventional and cross-sectional descriptive study was conducted from January 1, 2021, to June 30, 2022, at the Department of Surgery, Gia Rai Town Medical Center, Vietnam. Results: There were 710 patients (355 patients in pre-intervention and 355 patients in post-intervention) included in this study. The group of antibiotics used the most was beta-lactam (pre- and post-intervention rates were 60.2% and 61.0%, respectively). The parenteral route was the most commonly used (63.9% in pre-intervention and 60.7% in post-intervention). The rationality of the indication increased after the intervention (from 85.7% to 96.2%); the rationality of the dose increased after the intervention (from 90.5% to 95.2%); the rationality of the number of times of use increased after the intervention (from 90.4% to 98.4%); the rationality of the time increased after the intervention (from 94.8% to 95.1%); the general rationality increased after the intervention (from 66.5% to 85.6%). Infections were associated with inappropriate antibiotic use (p<0.05). Conclusions: Effective clinical pharmacists’ interventions helped raise the rate of rational use of antibiotics. Besides, inappropriate antibiotic use was found to be associated with infections, emphasizing the need for targeted interventions in this field

    Depression and its associated factors among pregnant women in central Vietnam.

    To date, little attention has been given to prenatal depression, especially in low and middle-income countries. The aim of this research was to assess the prevalence of depression and its associated factors amongst pregnant women in a central Vietnamese city. This cross-sectional study included 150 pregnant women from 29 to 40 weeks of gestation, from eight wards of Hue city, via quota sampling from February to May 2019. We employed the Patient Health Questionnaire (PHQ-9) to assess depression.     Results of Epizootiological Survey on Plague and Other Zoonotic Infections in the Northern Provinces of the Socialist Republic of Vietnam During Spring Months of 2019

    Objective of the study is to detect circulation of plague agent and agents of other zoonotic infections (tularemia, pseudotuberculosis, leptospirosis, West Nile fever, Lassa fever, Dengue fever, Chikungunya fever, CrimeanCongo hemorrhagic fever, Q fever, Hantaviruses, tick-borne encephalitis, human monocytic ehrlichiosis, granulocytic anaplasmosis, and borreliosis) among small mammals and their ectoparasites in the territory of seven northern provinces of the Socialist Republic of Vietnam. Materials and methods. We have carried out epizootiological survey of seven northern provinces of the Socialist Republic of Vietnam (Dien Bien, Lai Chau, Lào Cai, Hà Giang, Lạng Sơn, Cao Bằng, and Quảng Ninh). Over the period of the study, 3400 trap/nights were accumulated, 179 specimens of small mammals caught, belonging to 10 species, 213 fleas of seven different species – collected, and 143 specimens of gamaside ticks falling under two species. The material gathered was investigated using enzyme immunoassay and polymerase chain reaction at the premises of mobile laboratory for monitoring and diagnostics. Results and discussion. Two-fold testing of 136 blood samples from small mammals revealed antibodies to F1 of Y. pestis in 14 (10.3 %) of them. Investigation of 158 samples of lung and kidney suspensions of small mammals showed that 22 (13.9 %) samples contained 16S rRNA of pathogenic leptospira, Leptospira spp. Analysis of 60 brain samples for the presence of Leptospira spp. revealed three (5.0 %) positive ones. 25 samples of gamaside ticks were tested for the presence of the DNA of Q fever, plague, tularemia and granulocytic anaplasmosis agents, and for the RNA of tick-borne encephalitis, human monocytic ehrlichiosis, and borreliosis agents.     Physicians, Primary Caregivers and Topical Repellent: All Under-Utilised Resources in Stopping Dengue Virus Transmission in Affected Households

    BACKGROUND: Primary health care facilities frequently manage dengue cases on an ambulatory basis for the duration of the patient’s illness. There is a great opportunity for specific messaging, aimed to reduce dengue virus (DENV) transmission in and around the home, to be directly targeted toward this high-risk ambulatory patient group, as part of an integrated approach to dengue management. The extent however, to which physicians understand, and can themselves effectively communicate strategies to stop focal DENV transmission around an ambulatory dengue case is unknown; the matter of patient comprehension and recollection then ensues. In addition, the effectiveness of N,N-diethyl-3-methylbenzamide (DEET)-based insect repellent in protecting dengue patients from Aedes aegypti mosquitoes’ bites has not been investigated. METHODOLOGY: A knowledge, attitude and practice (KAP) survey, focusing on the mechanisms of DENV transmission and prevention, was performed using semi-structured questionnaires. This survey was targeted towards the patients and family members providing supportive care, and physicians routinely involved in dengue patient management in Southern Vietnam. An additional clinical observational study was conducted to measure the efficacy of a widely-used 13% DEET-based insect repellent to repel Ae. aegypti mosquitoes from the forearms of dengue cases and matched healthy controls. PRINCIPAL FINDINGS: Among both the physician (n = 50) and patient (n = 49) groups there were several respondents lacking a coherent understanding of DENV transmission, leading to some inappropriate attitudes and inadequate acute preventive practices in the household. The application of insect repellent to protect patients and their relatives from mosquito bites was frequently recommended by majority of physicians (78%) participating in the survey. Nevertheless, our tested topical application of 13% DEET conferred only ~1hr median protection time from Ae. aegypti landing. This is notably shorter than that advertised on the manufacturer’s label. No differences in landing time between febrile dengue cases or matched healthy controls (n = 19 experiments) were observed. CONCLUSIONS/SIGNIFICANCE: Our study identifies missed opportunities for primary care physicians to improve public health through communication of strategies that could prevent focal dengue transmission in and around a case household. We advocate better access to more efficient communication methods for physicians and auxilliary health workers, supporting to educate those at high risk of DENV transmission. Our empirical testing of a widely-available 13% DEET-based repellent was limited in its protective efficacy against Ae. aegypti mosquito bites, and therefore DENV transmission, suggesting more frequent application is necessary to be beneficial

    Lovastatin for the Treatment of Adult Patients With Dengue: A Randomized, Double-Blind, Placebo-Controlled Trial.

    BACKGROUND: Dengue endangers billions of people in the tropical world, yet no therapeutic is currently available. In part, the severe manifestations of dengue reflect inflammatory processes affecting the vascular endothelium. In addition to lipid lowering, statins have pleiotropic effects that improve endothelial function, and epidemiological studies suggest that outcomes from a range of acute inflammatory syndromes are improved in patients already on statin therapy. METHODS: Following satisfactory review of a short pilot phase (40 mg lovastatin vs placebo in 30 cases), we performed a randomized, double-blind, placebo-controlled trial of 5 days of 80 mg lovastatin vs placebo in 300 Vietnamese adults with a positive dengue NS1 rapid test presenting within 72 hours of fever onset. The primary outcome was safety. Secondary outcomes included comparisons of disease progression rates, fever clearance times, and measures of plasma viremia and quality of life between the treatment arms. RESULTS: Adverse events occurred with similar frequency in both groups (97/151 [64%] placebo vs 82/149 [55%] lovastatin; P = .13), and were in keeping with the characteristic clinical and laboratory features of acute dengue. We also observed no difference in serious adverse events or any of the secondary outcome measures. CONCLUSIONS: We found lovastatin to be safe and well tolerated in adults with dengue. However, although the study was not powered to address efficacy, we found no evidence of a beneficial effect on any of the clinical manifestations or on dengue viremia.     Perceived stress during the prenatal period: assessing measurement invariance of the Perceived Stress Scale (PSS-10) across cultures and birth parity

    Maternal prenatal stress places a substantial burden on mother’s mental health. Expectant mothers in low- and middle-income countries (LMICs) have thus far received less attention than mothers in high-income settings. This is particularly problematic, as a range of triggers, such as exposure to traumatic events (e.g. natural disasters, previous pregnancy losses) and adverse life circumstances (e.g. poverty, community violence), put mothers at increased risk of experiencing prenatal stress. The ten-item Perceived Stress Scale (PSS-10) is a widely recognised index of subjective experience of stress that is increasingly used in LMICs. However, evidence for its measurement equivalence across settings is lacking. This study aims to assess measurement invariance of the PSS-10 across eight LMICs and across birth parity. This research was carried out as part of the Evidence for Better Lives Study (EBLS, vrc.crim.cam.ac.uk/vrcresearch/EBLS). The PSS-10 was administered to N = 1,208 expectant mothers from Ghana, Jamaica, Pakistan, the Philippines, Romania, South Africa, Sri Lanka and Vietnam during the third trimester of pregnancy. Confirmatory factor analysis suggested a good model fit of a two-factor model across all sites, with items on experiences of stress loading onto a negative factor and items on perceived coping onto a positive factor. Configural and metric, but not full or partial scalar invariance, were established across all sites. Configural, metric and full scalar invariance could be established across birth parity. On average, first-time mothers reported less stress than mothers who already had children. Our findings indicate that the PSS-10 holds utility in assessing stress across a broad range of culturally diverse settings; however, caution should be taken when comparing mean stress levels across sites

    A Multi-Center Randomised Controlled Trial of Gatifloxacin versus Azithromycin for the Treatment of Uncomplicated Typhoid Fever in Children and Adults in Vietnam

    BACKGROUND: Drug resistant typhoid fever is a major clinical problem globally. Many of the first line antibiotics, including the older generation fluoroquinolones, ciprofloxacin and ofloxacin, are failing. OBJECTIVES: We performed a randomised controlled trial to compare the efficacy and safety of gatifloxacin (10 mg/kg/day) versus azithromycin (20 mg/kg/day) as a once daily oral dose for 7 days for the treatment of uncomplicated typhoid fever in children and adults in Vietnam. METHODS: An open-label multi-centre randomised trial with pre-specified per protocol analysis and intention to treat analysis was conducted. The primary outcome was fever clearance time, the secondary outcome was overall treatment failure (clinical or microbiological failure, development of typhoid fever-related complications, relapse or faecal carriage of S. typhi). PRINCIPAL FINDINGS: We enrolled 358 children and adults with suspected typhoid fever. There was no death in the study. 287 patients had blood culture confirmed typhoid fever, 145 patients received gatifloxacin and 142 patients received azithromycin. The median FCT was 106 hours in both treatment arms (95% Confidence Interval [CI]; 94-118 hours for gatifloxacin versus 88-112 hours for azithromycin), (logrank test p = 0.984, HR [95% CI] = 1.0 [0.80-1.26]). Overall treatment failure occurred in 13/145 (9%) patients in the gatifloxacin group and 13/140 (9.3%) patients in the azithromycin group, (logrank test p = 0.854, HR [95% CI] = 0.93 [0.43-2.0]). 96% (254/263) of the Salmonella enterica serovar Typhi isolates were resistant to nalidixic acid and 58% (153/263) were multidrug resistant. CONCLUSIONS: Both antibiotics showed an excellent efficacy and safety profile. Both gatifloxacin and azithromycin can be recommended for the treatment of typhoid fever particularly in regions with high rates of multidrug and nalidixic acid resistance. The cost of a 7-day treatment course of gatifloxacin is approximately one third of the cost of azithromycin in Vietnam. TRIAL REGISTRATION: Controlled-Trials.com ISRCTN67946944
