Leadership in destination management: its implications in a systemic approach.
This paper aims at understanding the existing links between leadership and governance in destinations. The research emphasizes the process through which a subject becomes a pivotal actor in a systemic approach. Once the pivotal actor is legitimated, it is important to verify the relationship between successful leadership and governance choices and configurations. The research uses a comparative case study analysis on destinations in which one specificactor—that can be entirely public, public-private or totally private—assumes the leadership in order to define the strategic orientation of the local actors and to coordinate them in order to promote destination development. This research clarifies the main steps that can legitimate a pivotal subject in a destination and the main leadership's governing choices that allow to manage a destination in a systemic approach. The paper tries to single out and define the links between leadership and governance choices, in order to get to the main aspects of destination management leadership at both a systemic and an individual level of analysis

This paper aims at exploring the link between internationalization strategies and local identity. More precisely, this study analyzes how firms manage this relationship, understanding if a firm can express the local identity and to what extent this latter can be extended through without jeopardizing the essential soul of its products/services.The analysis is developed according to a double level that includes the recognition of both distinctive resources and competences and the specific relational capabilities necessary in the internationalization process. Furthermore, the strength of this paper is represented by the examination of different types of firms (single location full-service restaurants, chain restaurants and fast food restaurants) operating in the food service sector.</p

The aim of this article is to analyze cultural firms' evolution, in terms of strategic management, in a global era. As regards the methods, we propose a methodological approach based on the combination of Resource Based Theory and Service Dominant Logic, according to which "customers do not buy goods or services: they buy offerings which render services which create value". According to this perspective, cultural firms need to manage the strategic interaction between internal/external resources (territorial factors and firm specific resources) and internal/external networks (including customers and trade actors). In this process, the customer becomes a strategic resource: he/she is involved in cultural activities (i.e. edu-tainment) and becomes value co-producer and co-creator. In this optic, cultural activities have to be connected to the territory, in order to be real attractive factors, connected with local identity. This can be the new horizon for cultural firms to become competitive on the global market
Stroke subtypes and their possible implication in stroke prevention drug strategies
Thrombotic strokes can affect large or small arteries in the brain. Drugs to prevent atherosclerosis complication such as thrombotic strokes, should be drugs able to prevent the accumulation of intravascular fat, reduce vascular proliferation, decrease blood pressure levels with the resulting shear stress, reduce platelet aggregation, and possibly partially or totally reverse carotid plaques. Any of the commonly used antihypertensive drugs lower the incidence of stroke, with larger reductions in BP resulting in larger reductions in risk. Experimental and clinical data suggest that reducing the activity of the renin-angiotensin aldosterone system (RAAS) may have beneficial effects beyond the lowering of blood pressure to reduce stroke incidence. In clinical trials, statins consistently reduced the risk of ischemic stroke in patients with or without CHD whereas the data on the effects of other lipid modifying drugs on stroke risk are limited. Approximately 25% of strokes are recurrent. Antiplatelet therapy is indicated for the prevention of recurrent stroke in patients with a history of noncardioembolic minor stroke or transient ischemic attack (TIA). Although clinicians may choose acetylsalicylic acid (ASA) as first-line therapy for secondary prevention, clinical guidelines and evidence from trials suggest that ASA may not be the most effective strategy. A recent review discussed results from clinical trials that have compared the efficacy of ASA monotherapy versus ASA + extended release dipyridamole in secondary stroke prevention. Therefore it is difficult to extrapolate the real benefit of pharmacological prevention strategies against atherothrombotic subtype for excellence in the TOAST classification subtype that is represented by the LAAS and also with regard to lacunar subtype as an expression of lipohyalinosis process which is a further aspect of atherosclerosis
Sexual function in women diagnosed with hereditary breast and ovarian cancer syndrome
Simple Summary The sexual well-being of women with hereditary breast and ovarian cancer (HBOC) syndrome is often overlooked by clinicians. The aim of the study is to analyze the correlation between HBOC syndrome itself and the potential risk of sexual dysfunction. Understanding this link could help improve clinical practices and the management and counseling of women, emphasizing the importance of openly addressing sexuality-related issues, providing a more targeted support to women affected by this syndrome.Abstract Background: Hereditary breast and ovarian cancer syndrome (HBOC) predisposes women to an increased risk mainly of breast and tubo-ovarian cancer. The aim of the study is to investigate whether being diagnosed with HBOC syndrome is itself a risk factor for sexual dysfunction. Methods: An ad hoc questionnaire, including baseline demographic and clinical data, and the Sexual Function Questionnaire 28 (SFQ28) were administered to HBOC female carriers (study group) and to a control group. Results: After propensity score matching (1:1), we enrolled 202 women, 101 in the study group and 101 in the control group. In a multivariate analysis, we finally found that menopausal status was the only risk factor for a significant low score in the domains Desire (HR 0.66; CI95% 0.47-0.93; p = 0.017), Arousal (Lubrication) (HR 0.52; CI95% 0.34-0.80; p = 0.003), Arousal (Cognitive) (HR 0.64; CI95% 0.44-0.95; p = 0.027), and Orgasm (HR 0.33; CI95% (0.16-0.70; p = 0.004), independent of risk-reducing surgery for gynecological malignancy. Psycho-oncology support is a protective factor for the Enjoyment domain (HR 1.38; CI95% 1.05-1.81; p = 0.022). Conclusions: HBOC syndrome itself does not affect SFQ28 domains, while menopausal status significantly influences sexual health, with potential mitigating effects of psycho-oncological support
The Role of Optical Coherence Tomography in an Atypical Case of Oculocutaneous Albinism: A Case Report
Background: Oculocutaneous albinism is a group of autosomal recessive disorders featuring hypopigmentation of the hair, skin and eyes. Ocular signs associated with the disease are nystagmus, decreased visual acuity, hypopigmentation of the retina, foveal hypoplasia, translucency of the iris, macular transparency, photophobia and abnormal decussation of nerve fibers at the chiasm. Case Report: An 8-year-old Caucasian girl presented to our clinic ‘Referral Center for Hereditary Retinopathies’ of the Second University of Naples with a diagnosis of Stargardt disease and a progressive reduction in visual acuity in both eyes. She underwent a complete ophthalmic examination including standard electroretinography and optical coherence tomography (OCT). A molecular analysis was also performed. Best-corrected visual acuity was 20/30 in the right eye and 20/40 in the left eye. Biomicroscopy of the anterior segment revealed a transparent cornea, in situ and transparent lens and normally pigmented iris. A mild diffuse depigmentation and macular dystrophy were observed at fundus examination. Standard electroretinography showed normal scotopic and photopic responses. OCT revealed high reflectivity across the fovea without depression. The typical OCT pattern led us to direct the molecular analysis towards the genes involved in oculocutaneous albinism. The molecular analysis identified mutations in the TYR gene. Conclusion: In this case, the role of OCT was crucial in guiding the molecular analysis for the diagnosis of albinism. OCT is therefore instrumental in similar cases that do not present typical characteristics of a disease. The case also proves the relevance of molecular analysis to confirm clinical diagnoses in hereditary retinal diseases

The results identify the development status and the leading trends in terms of impact, main journals, papers, topics, authors, and countries. The analysis and the graphical presentations are crucial, as they can help both researchers and practitioners to better understand the state of the art of sustainable tourism in the experiential and digital era
