    Obesity and perceived work discrimination in Spain

    Obesity is increasingly becoming a source of discrimination in many domains of living, including at the workplace. In this study, we estimate obesity-related discrimination in work settings in Spain and explore its potential sources. We use data from the European Health Interview Survey conducted in 2009–2010. Our models control for a comprehensive set of demographic, socioeconomic, health, and work-related sickness characteristics. We run separate models for women and men, and stratify by type of occupation and by area obesity prevalence. Our results indicate that weight-related discrimination in work settings in Spain is concentrated among women with morbid obesity, particularly among those working in customer-facing jobs and living in areas with low-obesity prevalence. These findings emphasize the persistence of the gendered nature of obesity-related discrimination, and provide evidence of a form of induced statistical discrimination. Employers’ expectations of lower returns from obese workers in customer facing jobs might be driven by customers’ preferences caused by social stigma. Furthermore, the role of area-obesity prevalence highlights the impact of cultural social norms even within the same country

    An Upstream Hfq Binding Site in the fhlA mRNA Leader Region Facilitates the OxyS-fhlA Interaction

    To survive, bacteria must be able to adapt to environmental stresses. Small regulatory RNAs have been implicated as intermediates in a variety of stress-response pathways allowing dynamic gene regulation. The RNA binding protein Hfq facilitates this process in many cases, helping sRNAs base pair with their target mRNAs and initiate gene regulation. Although Hfq has been identified as a critical component in many RNPs, the manner by which Hfq controls these interactions is not known.To test the requirement of Hfq in these mRNA-sRNA complexes, the OxyS-fhlA system was used as a model. OxyS is induced in response to oxidative stress and down regulates the translation of fhlA, a gene encoding a transcriptional activator for formate metabolism. Biophysical characterization of this system previously used a minimal construct of the fhlA mRNA which inadvertently removed a critical element within the leader sequence of this mRNA that effected thermodynamics and kinetics for the interaction with Hfq.Herein, we report thermodynamic, kinetic and structural mapping studies during binary and ternary complex formation between Hfq, OxyS and fhlA mRNA. Hfq binds fhlA mRNA using both the proximal and distal surfaces and stimulates association kinetics between the sRNA and mRNA but remains bound to fhlA forming a ternary complex. The upstream Hfq binding element within fhlA is similar to (ARN)(x) elements recently identified in other mRNAs regulated by Hfq. This work leads to a kinetic model for the dynamics of these complexes and the regulation of gene expression by bacterial sRNAs

    A Bayesian semiparametric Markov regression model for juvenile dermatomyositis

    Juvenile dermatomyositis (JDM) is a rare autoimmune disease that may lead to serious complications, even to death. We develop a 2-state Markov regression model in a Bayesian framework to characterise disease progression in JDM over time and gain a better understanding of the factors influencing disease risk. The transition probabilities between disease and remission state (and vice versa) are a function of time-homogeneous and time-varying covariates. These latter types of covariates are introduced in the model through a latent health state function, which describes patient-specific health over time and accounts for variability among patients. We assume a nonparametric prior based on the Dirichlet process to model the health state function and the baseline transition intensities between disease and remission state and vice versa. The Dirichlet process induces a clustering of the patients in homogeneous risk groups. To highlight clinical variables that most affect the transition probabilities, we perform variable selection using spike and slab prior distributions. Posterior inference is performed through Markov chain Monte Carlo methods.     Mitochondrial metabolism: Yin and Yang for tumor progression

    Altered metabolism is a distinct feature of cancer cells. During transformation, the entire metabolic network is rewired to efficiently convert nutrients to biosynthetic precursors to sustain cancer cell growth and proliferation. Whilst the molecular underpinnings of this metabolic reprogramming have been described, its role in tumor progression is still under investigation. Importantly, the mitochondrion is a central actor in many of the metabolic processes that are altered in tumors. Yet, we have only begun to understand the dualities of mitochondrial function during cancer metastasis and therapy resistance. Paradoxically, mitochondrial metabolism can be both advantageous and detrimental to these processes, highlighting the need for a better understanding of the molecular and microenvironmental cues that define the role of this fascinating organelle.     Deciphering predictive factors for choice of thrombopoietin receptor agonist, treatment free responses, and thrombotic events in immune thrombocytopenia

We conducted an observational, retrospective, long-term follow-up multicenter study from November 2016 to January 2018 of 121 adult ITP patients initiating TPO-RA between January 2012 to December 2014. Data reflected that a platelet count ≤25 × 109/l at the time when the TPO-RA was initiated was associated with a 2.8 higher probability of receiving romiplostim vs. eltrombopag (P = 0.010). VE on TPO-RA was related to previous neoplasia in patients over 65 years (50% vs. 2.2%, P < 0.001), and to previous splenectomy in younger patients (100% vs. 33%, P = 0.001). Receiving romiplostim as first TPO-RA with no subsequent TPO-RA switching was associated with a 50% likelihood of TFR after 2.9 years of therapy (3.3 years in chronic ITP patients). These real-world data help deciphering some areas of uncertainty, and offer insight into some of the most relevant challenges of ITP which may help clinicians make appropriate treatment decisions in the management of adult ITP patients with TPO-RA

    Interaction of two modulational instabilities in a semiconductor resonator

    The interaction of two neighboring modulational instabilities in a coherently driven semiconductor cavity is investigated. First, an asymptotic reduction of the general equations is performed in the limit of a nearly vertical input-output characteristic. Next, a normal form is derived in the limit where the two instabilities are close to one other. An infinity of branches of periodic solutions are found to emerge from the unstable portion of the homogeneous branch. These branches have a nontrivial envelope in the bifurcation diagram that can either smoothly join the two instability points or form an isolated branch of solutions

    Phase-Locked Spatial Domains and Bloch Domain Walls in Type-II Optical Parametric Oscillators

    We study the role of transverse spatial degrees of freedom in the dynamics of signal-idler phase locked states in type-II Optical Parametric Oscillators. Phase locking stems from signal-idler polarization coupling which arises if the cavity birefringence and/or dichroism is not matched to the nonlinear crystal birefringence. Spontaneous Bloch domain wall formation is theoretically predicted and numerically studied. Bloch walls connect, by means of a polarization transformation, homogeneous regions of self-phase locked solutions. The parameter range for their existence is analytically found. The polarization properties and the dynamics of walls in one- and two transverse spatial dimensions is explained. Transition from Bloch to Ising walls is characterized, the control parameter being the linear coupling strength. Wall dynamics governs spatiotemporal dynamical states of the system, which include transient curvature driven domain growth, persistent dynamics dominated by spiraling defects for Bloch walls, and labyrinthine pattern formation for Ising walls.Comment: 27 pages, 16 figure

    Phase IV open-label study of the efficacy and safety of deferasirox after allogeneic stem cell transplantation

    This is the first prospective study of deferasirox in adult allogeneic hematopoietic stem cell transplant recipients with transfusional iron overload in hematologic malignancies. Patients at least six months post transplant were treated with deferasirox at a starting dose of 10 mg/kg/day for 52 weeks or until serum ferritin was less than 400 ng/mL on two consecutive occasions. Thirty patients were enrolled and 22 completed the study. A significant reduction from baseline in median serum ferritin and in liver iron concentration at 52 weeks was observed in the overall population: from 1440 to 755.5 ng/mL (P=0.002) and from 14.5 to 4.6 mg Fe/g dw (P=0.0007), respectively. Reduction in serum ferritin in patients who did not discontinue deferasirox therapy was significantly greater than that found in those who prematurely discontinued the treatment (from 1541 to 581 ng/mL vs. from 1416 to 1486 ng/mL; P=0.008). Drug-related adverse events, reported in 17 patients (56.7%), were mostly mild to moderate in severity. There were no drug-related serious adverse events. Twelve patients (40.0%) showed an increase of over 33% in serum creatinine compared to baseline and greater than the upper limit of normal on two consecutive visits. Two patients (6.7%) with active graft-versus-host disease showed an increase in alanine aminotransferase exceeding 10 times upper limit of normal; both resolved. In this prospective study, deferasirox provided a significant reduction in serum ferritin and liver iron concentration over one year of treatment in allogeneic hematopoietic stem cell transplant recipients with iron overload. In addition, the majority of adverse events related to deferasirox were mild or moderate in severity. (clinicaltrials.gov identifier:01335035)

    The number of transmission channels through a single-molecule junction

    We calculate transmission eigenvalue distributions for Pt-benzene-Pt and Pt-butadiene-Pt junctions using realistic state-of-the-art many-body techniques. An effective field theory of interacting π\pi-electrons is used to include screening and van der Waals interactions with the metal electrodes. We find that the number of dominant transmission channels in a molecular junction is equal to the degeneracy of the molecular orbital closest to the metal Fermi level.Comment: 9 pages, 8 figure
