    Multiple Myeloma Index for Risk of Infection

    Based on our earlier research into the main characteristics and risk factors for infections in hospitalized patients with multiple myeloma, we created the numerical Multiple Myeloma Index for Risk of Infection (MMIRI) to predict infection in myeloma patients. The included factors that could influence the pathogenesis and incidence of infections were sex, perform ance status, Durie Salmon stage of disease, International Staging System, serum creatinine level, immune paresis, neutropenia, serum ferritin level, the presence of any catheters, disease duration, stable/progressive disease, and type of therapy. For each of these parameters, the strength of association with infection was statistically estimated and specific number of points was assigned to each of these parameters, proportional to the strength of the association. When designing the MMIRI, we included only those parameters that we determined were pathophysiologically associated with the infection. After further statistical analysis, we identified an optimal cutoff score of 6 or above as indicating a significant risk for infection, with a sensitivity of 93.2% and specificity of 80.2%. The scoring system in the retrospective receiver operating characteristic analysis showed an area under the curve of 0.918. The potential value of the MMIRI is the possibility of identifying those patients who would benefit from the prophylactic administration of antibiotics and other anti -infective measures while minimizing the contribution to antibiotic resistance related to the overuse of these drugs. As far as we know, this index represents the first attempt to create such an instrument for predicting the occurrence of infections in myeloma patients

    Kemijski profil sedimenata Plominskog zaljeva

    Granulometric, chemical, and leaching properties of sediments dredged in the Plomin Bay (Northern Adriatic Sea, Croatia) were investigated in order to asses the risk of remobilisation of heavy metals into the water column. In total 65 samples from 65 sampling sites were taken from different sediment depths within the bay. Analysis of variance confirmed the homogeneity of granulometric and elemental composition of the investigated sediment throughout its volume. Granulometric analysis showed that all samples corresponded to a pelitic fraction (<0.063 mm). Bulk elemental mass fractions in the sediments were similar to literature data on relatively unpolluted areas of the Adriatic Sea. High sedimentation rate caused by constant infl ow of material from the Boljunčica River drainage may be responsible for low levels of heavy metals and negligible infl uence of fl y and bottom ash from a nearby disposal site on the chemical composition of the sediments. In contact with sea water only 0.29 mg kg-1 of V, 0.04 mg kg-1 of Cr, 0.07 mg kg-1 of Ni, 0.33 mg kg-1 of Cu, 0.67 mg kg-1 of Zn and 0.06 mg kg-1 of Pb could be remobilised from sediment material into the water column. However, these values increased three to ten times in case of leaching with organic acids.Granulometrijska i kemijska svojstva te mogućnost otpuštanja teških metala ispitivani su u sedimentima Plominskog zaljeva (Sjeverni Jadran, Hrvatska) u svrhu utvrđivanja rizika od remobilizacije teških metala iz sedimenta u stupac vode. Uzeto je 65 uzoraka s različitih točaka i dubina unutar zaljeva. Analizom varijance potvrđena je granulometrijska i kemijska homogenost cijelog volumena sedimenta, što upućuje na jedan prevladavajući izvor tijekom cijeloga sedimentacijskog razdoblja. Granulometrijskom analizom je utvrđeno da u svim uzorcima prevladava sitnozrnata frakcija (<0,063 mm). Koncentracije elemenata u ukupnim uzorcima sedimenata slične su literaturnim vrijednostima objavljenim za relativno onečišćena područja Jadranskog mora. Velika brzina sedimentacije uzrokovana konstantnim donosom materijala iz slijevnog područja Boljunčice vjerojatan je uzrok niskih koncentracija teških metala i slabo vidljivog utjecaja odlagališta šljake i pepela na sastav sedimenata. U kontaktu s morskom vodom moguća je remobilizacija samo 0,29 mg kg-1 V, 0,04 mg kg-1 Cr, 0,07 mg kg-1 Ni, 0,33 mg kg-1 Cu, 0,67 mg kg-1 Zn i 0,06 mg kg-1 Pb iz sedimenta u stupac morske vode. Ipak ove vrijednosti su tri do deset puta povećane u slučaju izluživanja s pomoću organskih kiselina

    Advances in breast cancer treatment

    Breast cancer is the most common malignant tumour in women worldwide. Morbidity and mortality increases with age, significantly after the age of 45. Early detection of the disease is vital. The diagnosis and treatment of breast cancer today is significantly advanced. Magnetic resonance imaging has revolutionised diagnostics. If a biopsy shows invasive lobular carcinoma or even ductal carcinoma in situ, it is necessary to perform an magnetic resonance scan to rule out multicentricity or an extensive intraductal component, which may affect the surgical approach. Prognosis and treatment of breast cancer today depends on four molecular subtypes: Luminal A, luminal B, HER2-positive, and triple negative. Guidelines for surgical and oncological treatment today have also changed, patients with locally or systemically advanced breast cancer, with a tumour larger than 2.5 cm, confirmed as HER-2 positive or triple negative from biopsy results, should be subjected to neoadjuvant chemotherapy, and after the tumour is reduced, to surgery, at which point the operation can be considerably less involved than before. Furthermore, in the case of a positive sentinel lymph node, formerly mandatory axillary dissection can be replaced by axillary radiotherapy today with comparable prognosis and survival rate. Of new surgical techniques, skin- sparing mastectomy is worth mentioning, while intraoperative radiotherapy will be the future of treatment. The diagnosis and treatment of breast cancer has been significantly advanced. This review have revealed the current treatment of breast cancer on surgical, oncological fiels as well as in diagnosis

    Exploring and Monitoring of Methane Hydrate Deposits

    Relatively recently, in the last 20 years, it was discovered that methane hydrate (MH) deposits are globally distributed in the permafrost and oceans. Before 1965 when first deposits were discovered in nature, it was believed that MH can occur only in laboratory conditions or in vast parts of the Universe. Presently it is presumed that this solid crystalline compounds in which CH4 molecules occupies the water ice lattices (nominal chemical formula of MH is C4H62O23) can serve as an energy source favorably to the all of the world remaining conventional hydrocarbon sources. The worldwide estimates of MH deposits range from 2x1014 m3 to 3.053x1018 cubic meters. This uncertainty partly results from our limitations in geological understanding of the MH deposits, which is due to the relatively bad quality of data obtained by presently available seismic and electromagnetic techniques. Moreover, MH deposits can become vulnerable to climate changes, which were already occurring in geological past whit tremendous consequences for the global life on Earth. Thus, further development of advanced techniques is needed to enhance our abilities to better characterize, quantify and monitor the MH deposits. In the work presented 14 MeV neutrons and associated alpha particle imaging (API) where used to quantify the amount of MH in the sample. Samples were prepared from sea sediment, quartz sand and MH simulant. MH simulant with chemical formula C4H46O23 was made from sucrose (25 % by mass) and water. MH quantity was measured by measuring the carbon content in the sample [1-8]
