The Quest for Bandwidth Estimation Techniques for large-scale Distributed Systems
In recent years the research community has developed many techniques to estimate the end-to-end available bandwidth of an Internet path. This important metric has been proposed for use in several distributed systems and, more recently, has even been considered to improve the congestion control mechanism of TCP. Thus, it has been suggested that some existing estimation techniques could be used for this purpose. However, existing tools were not designed for large-scale deployments and were mostly validated in controlled settings, considering only one measurement running at a time. In this paper, we argue that current tools, while offering good estimates when used alone, might not work in large-scale systems where several estimations severely interfere with each other. We analyze the properties of the measurement paradigms employed today and discuss their functioning, study their overhead and analyze their interference. Our testbed results show that current techniques are insufficient as they are. Finally, we will discuss and propose some principles that should be taken into account for including available bandwidth measurements in large-scale distributed systems. 1
Face Recognition using Multi Region Prominent LBP Representation
Various face recognition methods are derived using local features among them the Local Binary Pattern (LBP) approach is very famous. The histogram techniques based on LBP is a complex task. Later Uniform Local Binary Pattern (ULBP) is derived on LBP, based on the bitwise transitions and ULBP’s are treated as the fundamental property of texture. The ULBP approach treated all Non-Uniform Local Binary Patterns’ (NULBP) into one miscellaneous label. Recently we have derived Prominent LBP (PLBP), Maximum PLBP (MPLBP) and Smallest PLBP (SPLBP). The PLBP consists of the majority of the ULBP’s and some of the NULBP’s. The basic disadvantage of these various variants of LBP’s is they are basically local approaches and completely failed in representing features derived from large regions or macrostructures, which are very much essential for faces. This paper derives PLBP’s on the large region. The rectangular region of this paper is assumed with a size of multiples of three and PLBPs are evaluated on dividing each region into multiple regions. The proposed Multi Region-PLBP (MR-PLBP) approach is tested on three facial databases namely Yale, Indian and AT&T ORL. The experimental results show the proposed approach significantly outperforms the other LBP based face recognition methods
Exploiting Cognitive Structure for Adaptive Learning
Adaptive learning, also known as adaptive teaching, relies on learning path
recommendation, which sequentially recommends personalized learning items
(e.g., lectures, exercises) to satisfy the unique needs of each learner.
Although it is well known that modeling the cognitive structure including
knowledge level of learners and knowledge structure (e.g., the prerequisite
relations) of learning items is important for learning path recommendation,
existing methods for adaptive learning often separately focus on either
knowledge levels of learners or knowledge structure of learning items. To fully
exploit the multifaceted cognitive structure for learning path recommendation,
we propose a Cognitive Structure Enhanced framework for Adaptive Learning,
named CSEAL. By viewing path recommendation as a Markov Decision Process and
applying an actor-critic algorithm, CSEAL can sequentially identify the right
learning items to different learners. Specifically, we first utilize a
recurrent neural network to trace the evolving knowledge levels of learners at
each learning step. Then, we design a navigation algorithm on the knowledge
structure to ensure the logicality of learning paths, which reduces the search
space in the decision process. Finally, the actor-critic algorithm is used to
determine what to learn next and whose parameters are dynamically updated along
the learning path. Extensive experiments on real-world data demonstrate the

A prospective, randomized, double blind, comparative study of intramuscular nalbuphine hydrochloride, butorphanol tartrate and pentazocine lactate for post-operative pain relief following abdominal hysterectomy
Background: This study aimed to compare the efficacy and safety of intramuscularly administered nalbuphine, butorphanol and pentazocine for post-operative pain relief after abdominal hysterectomy.Methods: Seventy-five adult female patients, aged between 20-50 years, belonging to American Society of Anaesthesiologists (ASA) class 1 and 2, posted for abdominal hysterectomy under spinal anesthesia were included in the study. The subjects were randomly divided into 3 groups (n=25 each) and given – Group A: pentazocine lactate (30 mg, 1mL), Group B: butorphanol tartarate (2 mg, 1 ml) and Group C: nalbuphine hydrochloride (10 mg, 1 mL) when post-operative pain intensity reached ≥4 mm on the Visual analogue scale (VAS). The onset, duration, time to peak effect and adverse events were recorded at regular intervals for 24 hours, postoperatively.Results: The mean time to onset of anesthesia was significantly faster (P<0.05 each) in the nalbuphine (10.2±2.2 minutes) and butorphanol (11.3±2 minutes) groups when compared to the pentazocine group (14±2.7 minutes). Duration of analgesic action was significantly longer (P<0.05 each) in the nalbuphine (236.4±75.1 minutes) and butorphanol (202±59.2 minutes) groups when compared to the pentazocine group (177.4±55.3 minutes). No significant differences in respiratory and cardiovascular parameters were noted between the groups. Nausea and vomiting was seen significantly higher in the pentazocine group (36%) when compared to butorphanol (20%) and nalbuphine (8%) groups (p<0.05 each).Conclusions: Intramuscular nalbuphine and butorphanol provided effective analgesia with rapid onset and longer duration of action, with lower incidence of nausea and vomiting when compared to pentazocine. In particular, nalbuphine can be a suitable agent to provide post-operative pain relief in gynecologic lower abdominal surgery
Clinical approach, diagnosis and medical management of acute pancreatitis among patients attending tertiary care hospital in Prakasam district, Andhra Pradesh, India
Background: There has been an increase in the incidence of acute pancreatitis reported globally and despite of improvements in access to care and interventional techniques, acute pancreatitis continues to be associated with significant morbidity and mortality. The present study was aimed to assess the clinical profile of acute pancreatitis and to assess the efficacy of various severity indices in view of outcome of patients.
Methods: A hospital based prospective cross-sectional study was conducted from October 2022-March 2023 in Gastro and Liver care center in Ongole, Prakasam District, Andhra Pradesh India. All consecutive 72 patients with a diagnosis of acute pancreatitis were included in this study.
Results: Out of total acute Pancreatitis cases 61 (84.7%) were males and 11 (15.3%) were females and acute abdominal pain (97.2%) and decreased appetite (95.8%) were the most common presenting complaints, 54.2% cases were due to Alcoholism, followed by hyperlipidemia with 20.8% and Gall stones 13.9%. All 72 (100%) received pancreatic supplements, 68 (94.4%) were given pain killers, and 65 (90.3%) were taken anti-ulcer agents. Twenty-three (31.9%) patients with 0 to 3 points as per CTSI Score and 4-6 range points were observed in 47 (65.3%) pancreatitis patients. Maximum (40.3%) were improved on 2nd day, 22 (30.6%) were on 3rd day. Positive correlation noticed between Amylase and in diagnosing acute Pancreatitis, it is significant at 0.05 level.
Conclusions: Early assessment of the clinical severity and identification of patients at risk is important for early intensive management and timely intervention and to improve quality of life. So, it is mandatory to assess the clinical severity using different scoring systems. and appropriate treatment based on guidelines

Volumetric laser endomicroscopy and its application to Barrett\u27s esophagus: results from a 1,000 patient registry.
Volumetric laser endomicroscopy (VLE) uses optical coherence tomography (OCT) for real-time, microscopic cross-sectional imaging. A US-based multi-center registry was constructed to prospectively collect data on patients undergoing upper endoscopy during which a VLE scan was performed. The objective of this registry was to determine usage patterns of VLE in clinical practice and to estimate quantitative and qualitative performance metrics as they are applied to Barrett\u27s esophagus (BE) management. All procedures utilized the NvisionVLE Imaging System (NinePoint Medical, Bedford, MA) which was used by investigators to identify the tissue types present, along with focal areas of concern. Following the VLE procedure, investigators were asked to answer six key questions regarding how VLE impacted each case. Statistical analyses including neoplasia diagnostic yield improvement using VLE was performed. One thousand patients were enrolled across 18 US trial sites from August 2014 through April 2016. In patients with previously diagnosed or suspected BE (894/1000), investigators used VLE and identified areas of concern not seen on white light endoscopy (WLE) in 59% of the procedures. VLE imaging also guided tissue acquisition and treatment in 71% and 54% of procedures, respectively. VLE as an adjunct modality improved the neoplasia diagnostic yield by 55% beyond the standard of care practice. In patients with no prior history of therapy, and without visual findings from other technologies, VLE-guided tissue acquisition increased neoplasia detection over random biopsies by 700%. Registry investigators reported that VLE improved the BE management process when used as an adjunct tissue acquisition and treatment guidance tool. The ability of VLE to image large segments of the esophagus with microscopic cross-sectional detail may provide additional benefits including higher yield biopsies and more efficient tissue acquisition.
