Heating and Cooling of Hot Accretion Flows by Non Local Radiation
We consider non-local effects which arise when radiation emitted at one
radius of an accretion disk either heats or cools gas at other radii through
Compton scattering. We discuss three situations:
1. Radiation from the inner regions of an advection-dominated flow Compton
cooling gas at intermediate radii and Compton heating gas at large radii.
2. Soft radiation from an outer thin accretion disk Compton cooling a hot
one- or two-temperature flow on the inside.
3. Soft radiation from an inner thin accretion disk Compton cooling hot gas
in a surrounding one-temperature flow.
We describe how previous results are modified by these non-local
interactions. We find that Compton heating or cooling of the gas by the
radiation emitted in the inner regions of a hot flow is not important.
Likewise, Compton cooling by the soft photons from an outer thin disk is
negligible when the transition from a cold to a hot flow occurs at a radius
greater than some minimum . However, if the hot flow terminates at
, non-local cooling is so strong that the hot gas is cooled to
a thin disk configuration in a runaway process. In the case of a thin disk
surrounded by a hot one-temperature flow, we find that Compton cooling by soft
radiation dominates over local cooling in the hot gas for \dot{M} \gsim
10^{-3} \alpha \dot{M}_{Edd}, and R \lsim 10^4 R_{Schw}. As a result, the
maximum accretion rate for which an advection-dominated one-temperature
solution exists, decreases by a factor of , compared to the value

AIDS. 2017 January 02; 31(1): 147–157. doi:10.1097/QAD.0000000000001307OBJECTIVE: We assessed the relationship between phasing out stavudine in first-line antiretroviral therapy (ART) in accordance with WHO 2010 policy and single-drug substitutions (SDS) (substituting the nucleoside reverse transcriptase inhibitor in first-line ART) in sub-Saharan Africa.
DESIGN: Prospective cohort analysis (International epidemiological Databases to Evaluate AIDS-Multiregional) including ART-naive, HIV-infected patients aged at least 16 years, initiating ART between January 2005 and December 2012. Before April 2010 (July 2007 in Zambia) national guidelines called for patients to initiate stavudine-based or zidovudine-based regimen, whereas thereafter tenofovir or zidovudine replaced stavudine in first-line ART.
METHODS: We evaluated the frequency of stavudine use and SDS by calendar year 2004-2014. Competing risk regression was used to assess the association between nucleoside reverse transcriptase inhibitor use and SDS in the first 24 months on ART.
RESULTS: In all, 33 441 (8.9%; 95% confience interval 8.7-8.9%) SDS occurred among 377 656 patients in the first 24 months on ART, close to 40% of which were amongst patients on stavudine. The decrease in SDS corresponded with the phasing out of stavudine. Competing risks regression models showed that patients on tenofovir were 20-95% less likely to require a SDS than patients on stavudine, whereas patients on zidovudine had a 75-85% decrease in the hazards of SDS when compared to stavudine.
CONCLUSION: The decline in SDS in the first 24 months on treatment appears to be associated with phasing out stavudine for zidovudine or tenofovir in first-line ART in our study. Further efforts to decrease the cost of tenofovir and zidovudine for use in this setting is warranted to substitute all patients still receiving stavudine

Disruption of the Suprachiasmatic Nucleus in Fibroblast Growth Factor Signaling-Deficient Mice.
Fibroblast growth factor (Fgf) 8 is essential for the development of multiple brain regions. Previous studies from our laboratory showed that reduced Fgf8 signaling led to the developmental alterations of neuroendocrine nuclei that originated within the diencephalon, including the paraventricular (PVN) and supraoptic (SON) nuclei. To further understand the role of Fgf8 in the development of other hypothalamic nuclei, we examined if Fgf8 and its cognate receptor, Fgfr1, also impact the integrity of the suprachiasmatic nuclei (SCN). The SCN control an organism's circadian rhythm and contain vasoactive intestinal peptide (VIP)-producing neurons as the main input neurons. Mice hypomorphic for Fgf8, Fgfr1, or both were examined for their SCN volume and the number of VIP neurons on postnatal day (PN) 0; adult hypomorphic mice were further examined for SCN function by quantifying SCN neuronal activation using cFos as a marker. On PN0, mice homozygous for Fgf8 hypomorphy displayed the most severe reduction of the SCN volume and VIP neurons. Those heterozygous for Fgf8 hypomorphy alone or Fgf8 combined with Fgfr1 hypomorphy, called double heterozygotes (DH), showed normal SCN volume but significantly reduced VIP neurons, albeit less severely than the homozygotes. Adult wild type, heterozygous Fgf8 hypomorphs (F8 Het), and DH mice were also examined for SCN cFos activation at three time points: 1 h (morning), 6 h (afternoon), and 11 h (evening) after light onset. In F8 Het mice, a significant change in the pattern of cFos immunostaining that may reflect delayed morning SCN activation was observed. Overall, our studies provide evidence supporting that deficiencies in Fgf8 not only impact the structural integrity of the SCN but also the pattern of SCN activation in response to light
A Solution to the Hierarchy Problem with an Infinitely Large Extra Dimension and Moduli Stabilization
We construct a class of solutions to the Einstein's equations for dimensions
greater than or equal to six. These solutions are characterized by a
non-trivial warp factor and possess a non-compact extra dimension. We study in
detail a simple model in six dimensions containing two four branes. One of each
brane's four spatial directions is compactified. The hierarchy problem is
resolved by the enormous difference between the warp factors at the positions
of the two branes, with the standard model fields living on the brane with
small warp factor. Both branes can have positive tensions. Their positions, and
the size of the compact dimension are determined in terms of the fundamental
parameters of the theory by a combination of two independent and comparable
effects---an anisotropic contribution to the stress tensor of each brane from
quantum fields living on it and a contribution to the stress tensor from a bulk
scalar field. One overall fine tuning of the parameters of the theory is

Slightly Non-Minimal Dark Matter in PAMELA and ATIC
We present a simple model in which dark matter couples to the standard model
through a light scalar intermediary that is itself unstable. We find this model
has several notable features, and allows a natural explanation for a surplus of
positrons, but no surplus of anti-protons, as has been suggested by early data
from PAMELA and ATIC. Moreover, this model yields a very small nucleon
coupling, well below the direct detection limits. In this paper we explore the
