    Thyroid papillary carcinoma presenting with femoral neck metastasis: a case report

    Thyroid carcinoma is relatively uncommon, accounting for 2% of all cancers. Although they usually present as a neck lump, occasionally they may be presented with a distant metastasis. In this study, a 65 year-old woman was referred to our clinic with a pain on her left hip at both rest and walking. A lytic area at the inferior femoral neck was found with plain radiography. To clarify the characteristics of the lesion, left hip magnetic resonance imaging (MRI) had been performed displaying 3×5 cm hyperintense lesion extending from medial part of the left femoral neck to the left femoral head. Laboratory findings were normal. An open biopsy had then been performed from the left femoral neck with a suspicion of a metastatic tumor. After Immunohistochemical assesment diagnosis was consistent with metastatic thyroid papillary cancer. Proximal femoral resection with clear margins were achieved by proximal femur tumor endoprosthesis.  In conclusion, papillary thyroid cancer have an excellent prognosis and doesn’t tend to metastese. But rarely, as seen in our case it can even present with syptoms of metastasic disease. Management strategy is the same as other solitary bone metastasis. Papillary thyroid cancer must be kept in mind as a differential diagnosis in solitary bone metatasis

    Adaptation and Validation of the Turkish Version of the Ford Insomnia Response to Stress Test in University Students

    OBJECTIVE: We adapted the Ford Insomnia Response to Stress Test to Turkish (FIRST-T) and validated it. METHODS: We randomly divided 774 Turkish university students into two equal groups for exploratory (EFA) and confirmatory factor analysis (CFA). McDonald\u27s omega and Cronbach\u27s alpha values were utilized for reliability analyses. Item response theory (IRT) approach also used for psychometric properties on the full sample. For discriminant validity, study sample were classified into high and low sleep reactivity groups, and their sociodemographic and sleep data were compared. RESULTS: EFA results suggested a one-factor structure of the FIRST-T, which was confirmed by CFA results. The FIRST-T had solid internal reliability. Item analysis results showed that all the items could distinguish between low and high scorers. This scale showed the same construct (clinical insomnia vs good sleepers) across the sexes in multi-group CFA and differential item functioning results. In the high FIRST-T score group, sleep quality, severity of insomnia, and anxiety scores were higher. In this group, more participants had clinical insomnia according to the Insomnia Severity Index (ISI) and were poor sleepers according to the Pittsburg Sleep Quality Index (PSQI) (p \u3c 0.01). CONCLUSION: The FIRST-T has robust psychometric properties that assesses sleep reactivity among university students

    A comparison of chevron and lindgren-turan osteotomy techniques in hallux valgus surgery: A prospective randomized controlled study

    Objective: The aim of this prospective randomized controlled single-blind study was to compare the results of Chevron and Lindgren-Turan osteotomy techniques for treatment of moderate hallux valgus. Methods: A total of 66 female patients (34 in Chevron group, 32 Lindgren-Turan group) were recruited in this study and followed up for an average of 26.08 months. Operative procedures were performed by 2 surgeons, and patients were evaluated by an another researcher who was blinded to the surgical technique. The groups were compared for their radiological and clinical results. Results: Both techniques was clinically and radiologically effective (p0.05). Compared to the Chevron group, the Lindgren-Turan group was found to have shorter surgical duration (p<0.05) and significantly more shortening at the first metatarsal (p<0.05). Conclusion: In moderate hallux valgus deformity, both the Chevron and Lindgren-Turan osteotomy techniques are clinically and radiologically safe, effective, and reliable alternatives. No superiority was detected in either technique. Although shortening at the first metatarsal in the Lindgren-Turan group was radiologically significant, the results were clinically tolerable

    Which Emerges First: Bone Marrow Edema or Nidus in Osteoid Osteoma?

    This case report describes a well-documented birth and evolution of an osteoid osteoma at the talus. Although initial radiologic images indicate mild bone marrow edema at first (without nidus), subsequent magnetic resonance imaging and computed tomographic images reveal pathognomonic nidus at the talus. During the evolution of the lesion, typical night pain was coincident with the occurrence of the nidus, as seen on magnetic resonance imaging. This may be interpreted that nidus formation may be related to the night pain. In this report, the first finding was bone marrow edema. Although our classic knowledge was that the edema follows the lesion, this report makes a difference. The relationship between bone marrow edema and osteoid osteoma has not been questioned in the literature before. We speculate that this report brings to mind, the question of which comes first? A bone marrow edema or nidus? Another question is: Does osteoid osteoma always start with such a dust cloud in the bone as we presented herein

Methods: This study was prospective, randomized, and controlled. In total, 128 patients were recruited and divided into two groups. The outcomes of those treated with the new suturing technique (group I) were compared with those of patients treated with the traditional suturing technique (group II), both of which were applied after the Winograd procedure had been completed. The clinical outcomes and recurrence rates of the two groups were compared. Results: Patients in group I required significantly more time to return to work or school than did those in group II (p = 0.015). We found no significant difference between youths (age < 18 years, n = 55) and adults (age = 19 years, n = 69) in this context (p = 0.161). The recurrence rate was significantly higher in group II than in group I (p = 0.011). The extent of satisfaction was significantly higher in group I (p = 0.042). Conclusions: Our new suturing technique is associated with lower recurrence and higher satisfaction rates. However, the times elapsing before shoes could be worn were similar in the two groups.     Effect of deep sub-zero treatment on mechanical properties of AA5XXX aluminum plates adjoined by MIG welding technique

    WOS: 000411168700006In this study, mechanical and microstructural characteristics of AA5XXX aluminum plates connected by MIG welding were investigated. Thus, after several preliminary experiments done to ensure quality requirements of the parameters, specimens in the form of long aluminum plates were welded, and then some of them were spared for the subsequent sub-zero treatment at about 146 degrees C for 12 and 24 h, respectively. To investigate the effect of this super cooling on the welded parts, several tensile, bending, and hardness tests in conjunction with micro-structure examinations were carried out and results were discussed in detail. It was concluded that during sub-zero treatment, microstructure gets finer and stabilized, positively affecting the overall strength of the material. There is no significant difference in the overall strength of the material right after the subzero treatment and that of the specimens rested for one year at room temperature after cryogenics treatments.     Clinical results of intraarticular distal radius fractures treated by us with external fixators

    Radius distal kırıklarının kapalı redüksiyon eksternal fiksasyon yöntemi ile tedavi sonuçlarının klinik ve radyolojik olarak değerlendirilmesini amaçladığımız bu çalışmada Şubat 2008- Ekim 2011 tarihleri arasında tedavi edilen 17 radius distal kırığı değerlendirildi. AO sınıflamasına göre 6 kırık tip C, 11 kırık ise tip B idi. Tüm kırıklar kapalı redüksiyon eksternal fiksatör ile tedavi edildi ve ortalama 25 ay (12-46 ay) takip edildi. Fonksiyonel değerlendirmede Gartland ve Werley skorlamasının Sarmiento modifikasyonu, radyolojik değerlendirmelerde ise Modifiye Sarmiento skorlaması kullanıldı. Güncel yaşam aktivite değerlendirilmesinde DASH skorlama sistemi kullanıldı. Radyolojik ölçümlerin sonucuna göre yapılan anatomik değerlendirmede 11 kırık (% 64) kusursuz, 4 kırık (% 24) iyi, 2 kırık (% 12) orta-kötü olarak tespit edildi. Radyolojik değerlendirmede orta-kötü sonuçları olan 2 olgunun biri C1 diğeri de C2 tipi kırıklar idi. Gartland ve Wesley skorlaması Sarmiento modifikasyonuna göre 9 olgu çok iyi, 5 olgu iyi, 2 olgu orta, 1 olgu kötü olarak değerlendirildi. Güncel yaşam aktivite değerlendirmelerinde DASH skorlama sistemine göre ortalama değer 8.57 idi. Toplam 4 (% 23) olguda Refleks Sempatik Distrofi ve 8 (% 47) olguda yüzeyel çivi dibi infeksiyonu gelişti. Radius distal uç kırıklarının tedavisinde eksternal fiksatör yöntemi, klinik ve radyolojik sonuçları açısından uygun olgularda iyi bir tedavi seçeneğidir.Evaluation of clinical and radiological results of distal radius fractures treated with closed reduction and external fixation was aimed in this study. Seventeen cases of distal radius fractures managed between February 2008 and October 2011 were evaluated . According to AO classification, six of these fractures were type C and eleven of them were type B fractures. All of the cases were treated with closed reduction and external fixation. The average length of follow up was 25 (12-46) months. Sarmiento modification of Gartland and Werley scoring system was used for functional evaluation, and DASH scoring system for the evaluation of daily life activities. In anatomical evaluation based on the results of radiological measurements, 11 cases (64 %) had perfect, 4 (24 %) had good, 2 (12 %) had moderate- poor results. These two bad results were obtained in the treatment of AO-C1 and C2 fractures. According to Sarmiento modification of Gartland and Werley scoring system, 9 cases had perfect, 5 had good, 2 had moderate and one had bad results. According to DASH scoring system , the mean score was 8.57 points. Four patients (23 %) suffered from reflex sympathetic dystrophy, and eight (47 %) cases had superficial pin tract infection. External fixator is a good treatment option in terms of clinical and radiological results in selected cases with distal radius fractures
