    Amaç: Bu araştırmada; sağlık yönetimi alanında çalışan yönetici ve idari personelin örgütsel sessizlik düzeylerinin problem çözme becerileri üzerine etkisinin incelenmesi amaçlanmıştır.Gereç ve Yöntem: Kesitsel tipte, tanımlayıcı olan bu araştırmanın verileri, İstanbul ilinde faaliyet gösteren bir kamu hastanesinde çalışan sağlık çalışanlarından elde edilmiştir. Araştırma kapsamına dâhil edilen ilgili hastanenin, yönetim ve idari birimlerinde çalışan personel sayısı 325 olup, bu sayının tamamına ulaşılması hedeflenmiş, 273 personelden geri dönüş sağlanmıştır. Veriler anket formu ile toplanmıştır.Bulgular: Katılımcıların %63,4’ü kadın, %62,6’sı evli, %55,3’ü çocuk sahibi, %36,3’ü lisans eğitim düzeyine sahip, yaş ortalaması 36,45±8,0’dir. Örgütsel sessizlik düzeyi puan ortalaması 38,91±7,65 ve problem çözme becerisi düzeyi ortalaması 87,47±22,71 olarak bulunmuştur. Örgütsel sessizlik düzeyinin problem çözme becerisi üzerine etkisi incelendiğinde; örgütsel sessizlik kabullenici alt boyutundaki artışın problem çözme becerisini artırdığı (P:0,000, B:10,779), savunmacı sessizlik alt boyutundaki artışın problem çözme becerisini artırdığı (P:0,000, B:7,649), korumacı sessizlik alt boyutundaki artışın problem çözme becerisini azalttığı (P:0,000, B:-6,277) bulunmuştur. Sonuçlar: Araştırmada çalışanların örgütsel sessizlik düzeylerinin problem çözme becerileri üzerine etkisi incelendiğinde, örgütsel sessizlik düzeyindeki artışın problem çözme becerisinde düşüşe sebep olduğu sonucuna ulaşılmıştır. Cinsiyet açısından erkeklerin kadınlardan daha fazla kabullenici sessizlik tutumu göstermekte olduğu ve kadınların problem çözme becerilerinin erkeklerden yüksek olduğu bulunmuştur.--------------------Aim: In this study; The aim of this study is to investigate the effect of organizational silence levels on problem solving skills of managers and administrative staff working in the field of health management.Materials and Method: The data of this cross-sectional, descriptive study was obtained from health workers working in a public hospital in Istanbul. The number of personnel working in the management and administrative units of the relevant hospital included in the scope of the research is 325, and the total number of these personnel is targeted to be reached and 273 employees have been returned. Data were collected by questionnaire form.Results: 63,4% of the participants were women, 62,6% were married, 55,3% had children, 36,3% had undergraduate education, and the average age was 36,45 ± 8.0. The average score of organizational silence was 38,91 ± 7,65 and the average of problem solving skills was 87,47 ± 22,71. When the effect of organizational silence level on problem solving skills is examined; the increase in the organizational silence acceptor sub-dimension increased the problem solving ability (P:0,000, B:10,779, the increase in the defensive silence sub-dimension increased the problem-solving ability (P:0,000, B:7,649), the increase in the protective silence sub-dimension decreased the problem-solving ability (P:0,000, B:-6,277).Conclusion: When the effects of organizational silence levels on problem solving skills were examined, it was concluded that the increase in organizational silence level caused a decrease in problem solving skills. In terms of gender, it was found that males had more accepting silence attitudes than females and women's problem solving skills were higher than males

    Effect of Bariatric Sleeve Gastrectomy Technique on Women’s Lower Urinary Tract Symptoms and Quality of Life: A Prospective Study

    Objective Obesity triggers lower urinary tract symptoms (LUTS) secondary to accumulation of excess fat which may lead to increase in intra-abdominal/intravesical pressures and subsequent impairment in pelvic floor muscles. However, it is considered that weight loss resolve these symptoms. In this study, we aimed to investigate the effects of bariatric surgery and especially Sleeve gastrectomy (SG) on women’s LUTS, and quality of life. Materials and Methods A total of 53 patients who have undergone laparoscopic SG in our clinics between April 2014 and March 2015 were included in this prospective study. Age, body weight and body mass index (BMI) of the participants were preoperatively and postoperatively recorded. The patients have pre/post-operatively filled the Beck depression inventory (BDI), International prostate symptom score (IPSS), International Consultation on Incontinence Questionnaire-Short Form (ICIQ-SF) and the 36-Item Short Form health survey (SF-36) scores were recorded. Additionally, post-operative decrease in body weight and BMI of the patients was recorded. Results There was a statistically significant decrease in mean BDI, IPSS and ICIQ-SF scores and SF-36 (prominent increment in physical and mental component summary (PCS and MCS) scores), when compared with pre-operative values. A positive correlation was found between BMI and parameters including age, BDI and IPSS. However, no significant correlation was present between BMI and the parameters including ICIQ-SF, PCS and MCS. Conclusion Negative effect of obesity on LUTS and quality of life cannot be ignored. We assume that bariatric surgery can induce dramatic weight loss, amelioration in symptoms of urinary dysfunction and increase in quality of life of women
