Use of evidence to support healthy public policy: a policy effectiveness-feasibility loop
Public policy plays a key role in improving population health and in the control of diseases, including non-communicable diseases.
However, an evidence-based approach to formulating healthy public policy has been difficult to implement, partly on account of barriers
that hinder integrated work between researchers and policy-makers. This paper describes a “policy effectiveness–feasibility loop” (PEFL) that
brings together epidemiological modelling, local situation analysis and option appraisal to foster collaboration between researchers and
policy-makers. Epidemiological modelling explores the determinants of trends in disease and the potential health benefits of modifying
them. Situation analysis investigates the current conceptualization of policy, the level of policy awareness and commitment among key
stakeholders, and what actually happens in practice, thereby helping to identify policy gaps. Option appraisal integrates epidemiological
modelling and situation analysis to investigate the feasibility, costs and likely health benefits of various policy options. The authors illustrate
how PEFL was used in a project to inform public policy for the prevention of cardiovascular diseases and diabetes in four parts of the eastern
Mediterranean. They conclude that PEFL may offer a useful framework for researchers and policy-makers to successfully work together to
generate evidence-based policy, and they encourage further evaluation of this approach
A Hydrophobic Gate in an Ion Channel: The Closed State of the Nicotinic Acetylcholine Receptor
The nicotinic acetylcholine receptor (nAChR) is the prototypic member of the
`Cys-loop' superfamily of ligand-gated ion channels which mediate synaptic
neurotransmission, and whose other members include receptors for glycine,
gamma-aminobutyric acid, and serotonin. Cryo-electron microscopy has yielded a
three dimensional structure of the nAChR in its closed state. However, the
exact nature and location of the channel gate remains uncertain. Although the
transmembrane pore is constricted close to its center, it is not completely
occluded. Rather, the pore has a central hydrophobic zone of radius about 3 A.
Model calculations suggest that such a constriction may form a hydrophobic
gate, preventing movement of ions through a channel. We present a detailed and
quantitative simulation study of the hydrophobic gating model of the nicotinic
receptor, in order to fully evaluate this hypothesis. We demonstrate that the
hydrophobic constriction of the nAChR pore indeed forms a closed gate.
Potential of mean force (PMF) calculations reveal that the constriction
presents a barrier of height ca. 10 kT to the permeation of sodium ions,
placing an upper bound on the closed channel conductance of 0.3 pS. Thus, a 3 A
radius hydrophobic pore can form a functional barrier to the permeation of a 1
A radius Na+ ion. Using a united atom force field for the protein instead of an
all atom one retains the qualitative features but results in differing
conductances, showing that the PMF is sensitive to the detailed molecular

On the Nature of Andromeda IV
Lying at a projected distance of 40' or 9 kpc from the centre of M31,
Andromeda IV is an enigmatic object first discovered during van den Bergh's
search for dwarf spheroidal companions to M31. Being bluer, more compact and
higher surface brightness than other known dwarf spheroidals, it has been
suggested that And IV is either a relatively old `star cloud' in the outer disk
of M31 or a background dwarf galaxy. We present deep HST WFPC2 observations of
And IV and the surrounding field which, along with ground-based long-slit
spectroscopy and Halpha imagery, are used to decipher the true nature of this
puzzling object. We find compelling evidence that And IV is a background galaxy
seen through the disk of M31. The moderate surface brightness (SB(V)~24), very
blue colour (V-I<~0.6), low current star formation rate (~0.001 solar mass/yr)
and low metallicity (~10% solar) reported here are consistent with And IV being
a small dwarf irregular galaxy, perhaps similar to Local Group dwarfs such as
IC 1613 and Sextans A. Although the distance to And IV is not tightly
constrained with the current dataset, various arguments suggest it lies in the
range 5<~D<~8 Mpc, placing it well outside the confines of the Local Group. It
may be associated with a loose group of galaxies, containing major members UGC
64, IC 1727 and NGC 784. We report an updated position and radial velocity for

Free-Living Physical Activity Energy Expenditure Is Strongly Related to Glucose Intolerance in Cameroonian Adults Independently of Obesity
OBJECTIVE—We examined the cross-sectional association between objectively measured free-living physical activity energy expenditure (PAEE) and glucose tolerance in adult Cameroonians without known diabetes
VSOP and Ground-based VLBI Imaging of the TeV Blazar Markarian 421 at Multiple Epochs
We present thirty VLBI images of the TeV blazar Markarian 421 (1101+384) at
fifteen epochs spanning the time range from 1994 to 1997, and at six different
frequencies from 2.3 to 43 GHz. The imaged observations include a
high-resolution 5 GHz VLBI Space Observatory Programme (VSOP) observation with
the HALCA satellite on 1997 November 14; full-track VLBA observations from 1994
April, 1996 November, and 1997 May at frequencies between 5 and 43 GHz; six
epochs of VLBA snapshot observations at frequencies between 2 and 15 GHz from
Radio Reference Frame studies; and five geodetic VLBI observations at 2 and 8
GHz from the archive of the Washington VLBI Correlator Facility located at the
U.S. Naval Observatory. The dense time coverage of the images allows us to
unambiguously track components in the parsec-scale jet over the observed time
range. We measure the speeds of three inner jet components located between 0.5
and 5 mas from the core (0.3 to 3 pc projected linear distance) to be 0.19 +/-
0.27, 0.30 +/- 0.07, and -0.07 +/- 0.07 c (H_{0}=65 km s^{-1} Mpc^{-1}). If the
sole 43 GHz image is excluded, all measured speeds are consistent with no
motion. These speeds differ from tentative superluminal speeds measured by
Zhang & B\aa\aa th from three epochs of data from the early 1980's. Possible
interpretations of these subluminal speeds in terms of the high Doppler factor

Quantitative plane-resolved crystal growth and dissolution kinetics by coupling in situ optical microscopy and diffusion models : the case of salicylic acid in aqueous solution
The growth and dissolution kinetics of salicylic acid crystals are investigated in situ by focusing on individual microscale crystals. From a combination of optical microscopy and finite element method (FEM) modeling, it was possible to obtain a detailed quantitative picture of dissolution and growth dynamics for individual crystal faces. The approach uses real-time in situ growth and dissolution data (crystal size and shape as a function of time) to parametrize a FEM model incorporating surface kinetics and bulk to surface diffusion, from which concentration distributions and fluxes are obtained directly. It was found that the (001) face showed strong mass transport (diffusion) controlled behavior with an average surface concentration close to the solubility value during growth and dissolution over a wide range of bulk saturation levels. The (1Ě…10) and (110) faces exhibited mixed mass transport/surface controlled behavior, but with a strong diffusive component. As crystals became relatively large, they tended to exhibit peculiar hollow structures in the end (001) face, observed by interferometry and optical microscopy. Such features have been reported in a number of crystals, but there has not been a satisfactory explanation for their origin. The mass transport simulations indicate that there is a large difference in flux across the crystal surface, with high values at the edge of the (001) face compared to the center, and this flux has to be redistributed across the (001) surface. As the crystal grows, the redistribution process evidently can not be maintained so that the edges grow at the expense of the center, ultimately creating high index internal structures. At later times, we postulate that these high energy faces, starved of material from solution, dissolve and the extra flux of salicylic acid causes the voids to close
Markarian 421's Unusual Satellite Galaxy
We present Hubble Space Telescope (HST) imagery and photometry of the active
galaxy Markarian 421 and its companion galaxy 14 arcsec to the ENE. The HST
images indicate that the companion is a morphological spiral rather than
elliptical as previous ground--based imaging has concluded. The companion has a
bright, compact nucleus, appearing unresolved in the HST images. This is
suggestive of Seyfert activity, or possibly a highly luminous compact star
cluster. We also report the results of high dynamic range long-slit
spectroscopy with the slit placed to extend across both galaxies and nuclei. We
detect no emission lines in the companion nucleus, though there is evidence for
recent star formation. Velocities derived from a number of absorption lines
visible in both galaxies indicate that the two systems are probably tidally
bound and thus in close physical proximity. Using the measured relative
velocities, we derive a lower limit on the MKN 421 mass within the companion
orbit (R \sim 10 kpc) of 5.9 \times 10^{11} solar masses, and a mass-to-light
ratio of >= 17. Our spectroscopy also shows for the first time the presence of
H\alpha and [NII] emission lines from the nucleus of MKN 421, providing another
example of the appearance of new emission features in the previously
featureless spectrum of a classical BL Lac object. We see both broad and narrow
line emission, with a velocity dispersion of several thousand km s^{-1} evident
