    Reliability of the Lymph Node Ratio in the Prediction of Gastric Cancer Survival

    Background. Lymph node metastasis is the major determinant factor in the prognosis of gastric cancer. There is still no definite consensus on the lymph node number that should be harvested during gastric cancer surgery. Lymph Node Ratio (LNR) is defined as the ratio of metastatic nodes to the total number of pathologically examined lymph nodes. LNR has been proposed to be a sensitive prognostic factor in patients with gastric cancer. In this study the reliability of the LNR is tested for being a prognostic factor in gastric cancer survival. Methods. Medical records of 244 patients, with neither distant metastases nor neoadjuvant treatment underwent curative gastrectomy, were analyzed retrospectively in terms of survival according to the lymph node ratio (LNR). Patients were divided in two groups by using LNR cut-off value. Results. LNR of 0.4 was proved to be the best cut-off value to predict the prognosis of patients with gastric cancer. Univariate and multivariate analysis revealed that age over 65 (p < 0.001), and LNR ≥ 0.4 (p = 0.02) were independent factors in gastric cancer survival. Patients with LNR ≥ 0.4 presented with worse outcomes regarding other prognostic parameters (tumor differentiation, tumor diameter, lymphovascular invasion or perineural invasion), despite similar numbers of lymph nodes being harvested in both groups during surgery. Conclusion. Lymph node ratio is a reliable parameter to predict the survival in gastric cancer

    Gastric bezoars are rare but well known intraluminal formations in the gastrointestinal system. Reports for gastric bezoar treatment include dissolution, endoscopic retrieval and surgery (open and laparoscopic). We are presenting a case of 32 years old male patient presented with giant gastric bezoar complicated with large ulcer. The patient was operated laparoscopically with success. Laparoscopic surgery is safe alternative to open surgery for giant gastric bezoars treatment

    Analysis of Survival in Complete Pathological Response after Long-Course Chemoradiotherapy in Patients with Advanced Rectal Cancer

    Background: Neoadjuvant chemoradiotherapy prior to surgery is the standard treatment for locally advanced rectal cancer. This consists in the patient’s complete pathological response being achieved with no residual tumor presence in the resected specimen, which results in survival improvement. Methods: This retrospective study aimed to examine the rate of complete pathological response in patients with advanced rectal cancer treated with neoadjuvant long-course chemoradiotherapy and to examine the survival differences between the different tumor regression grade (TRG) scores. Results: A total of 154 patients were operated prior to long-course chemoradiotherapy with a total of 50 Gy plus FOLFOX protocol. Complete pathologic response was achieved in 29 (18.8%) patients. There was no statistical difference for the different pathologic responses according to gender, type of surgery, and number of harvested lymph nodes. Mean survival for all the groups was 37.2 months. Survival within a different TRG score exhibited statistical significance (p = 0.006). Overall, the survival rate during the follow-up period was of 81.8%. Conclusions: The complete pathological response rate in this study was of 18.8%. High tumor regression grade scores (TRG0 and TRG1) had a survival rate of over 90% during follow-up. Multivariate analysis identified perineural invasion and tumor regression grade as independent factors that affect survival

    Eğitim ve öğretim işinin yapılandırılmasında paydaşlar arasındaki iş birliği oldukça önemlidir. Bu iş birliği sayesinde eğitim ve öğretimle ilgili yapılacak bütün faaliyetlerde eğitim kurumu, ilçe, il ve ülke genelinde bir bütünlük oluşturmak, ortak kararlar alabilmek ve bu kararların sonuçlarını değerlendirmek daha kolay hale gelebilmektedir. Bu çalışmada, sosyal bilgiler dersi için düzenlenen okul alan zümre toplantıları, ilçe alan zümre toplantıları ve il alan zümre toplantılarında alınan kararların Sosyal Bilgiler Dersi Öğretim Programı’nda yer alan temel becerileri ele alma durumları incelenmiştir. Nitel araştırma desenlerinden durum (vaka) çalışması yöntemi kullanılmıştır. Araştırmanın çalışma grubu, amaçlı örnekleme yöntemlerinden maksimum çeşitlilik örnekleme yöntemi ile belirlenen 2018-2019 eğitim-öğretim yılı Konya il merkezinde yer alan resmi ortaokul ve imam hatip ortaokulları ile il ve merkez ilçelerinden alınan ikişer tutanaktan oluşmaktadır. Veri toplama aracı olarak doküman incelemesi yapılmış, verilerin analizinde betimsel analiz yöntemi kullanılmıştır. Çalışma sonucunda sosyal bilgiler dersi zümre toplantılarında, temel becerilerden iş birliği, dijital okuryazarlık, harita okuryazarlığı, problem çözme, sosyal katılım, tablo, grafik ve diyagram çözme ve yorumlama becerilerine yeterli ölçüde yer verildiği, finansal okuryazarlık, hukuk okuryazarlığı, kalıp yargı ve ön yargıyı fark etme, konum analizi, politik okuryazarlık gibi becerilere ise yer verilmediği tespit edilmiştir. Ayrıca tutanaklarda birbirinin mükerreri ifadelerin çokça yer aldığı belirlenmiştir. The study group of the research consists of per two minutes taken from the public secondary schools and imam hatip (vocational religious) secondary schools of province and central districts, determined by the maximum variations sampling of the purposeful sampling during the 2018-2019 academic year in the city center of Konya. Document analysis has been performed as a data collection tool and descriptive analysis method has been used to analyze the data. At the end of the study, it has been pointed out that collaboration, digital literacy, map literacy, problem-solving, social participation, table, graphics and diagram solving and interpretation skills are sufficiently included but financial literacy, legal literacy, self-awareness of stereotype and prejudice, position analysis, political literacy are not included as basic skills in the board and committee meetings of social studies. In addition, it has been determined that repetitive statements are frequently included in the minutes. It is proposed to focus on basic skills not included in the board and committee meetings and to prepare the minutes with authentic ideas

MethodsIn total, 327 patients with diagnoses of persistent AR or mild-to-severe persistent asthma were randomized into 2 intervention groups and 2 control groups upon their admission at outpatient clinics. The intervention groups (POPET-AR and POPET-Asthma) received a mobile phone application (physician on call patient engagement trial [POPET]), enabling them to communicate with their physician, and record their health status and medication compliance. The AR groups completed the Rhinitis Quality of Life Questionnaire (RQLQ) at initiation and at the first month of the study. The asthma groups completed the Asthma Control Test (ACT) at initiation and at the third month of the study. ResultsThe POPET-AR group showed better clinical improvement than the control group in terms of the overall RQLQ score as well in measures of general problems, activity, symptoms other than nose/eye, and emotion domains (p 19) compared with the control group (27%); this was statistically significant (p < 0.05). ConclusionUse of a mobile engagement platform, such as POPET, can have a significant impact on health outcomes and quality of life in both AR and asthma, potentially decreasing the number of hospital admissions, repeat doctor visits, and losses in productivity. Improvements were seen in domains related to activity, productivity, perception of disease, and emotion.     Evaluation of Patient Satisfaction with Different Hearing Aids: A Study of 107 Patients

    WOS: 000471779900002PubMed: 28122108We retrospectively investigated patient satisfaction with different types of hearing aids in 107 patients60 males and 47 females, aged 8 to 84 years (mean: 53.8)with unilateral or bilateral hearing loss, each of whom used two different hearing devices for at least 3 years per device. The International Outcome Inventory for Hearing Aids, Turkish edition (IOI-HA-TR) was used to evaluate satisfaction levels; we also calculated our own total individual subjective satisfaction (TISS) scores. We divided 16 different hearing devices into two types: device 1 and device 2; on average, device 2 had more channels, a lower minimum frequency, and a higher maximum frequency. We found that the IOI-HA-TR scores and TISS scores were higher and usage time was greater during device 2 use, and that there was a positive correlation between IOI-HA-TR and TISS scores. A total of 69 patients (64.5%) used device 2 for more than 8 hours per day, while 38 patients (35.5%) used it for 4 to 8 hours per day during the final 2 weeks of the trial. In contrast, 40 patients (37.4%) used device 1 for more than 8 hours, 50 (46.7%) used it for 4 to 8 hours, and the remaining 17 (15.9%) used it for less than 4 hours; the difference in the duration of use of the two devices was statistically significant (p < 0.001). Younger patients and patients with more education were more satisfied with their devices than were older patients and those who were not as well educated. We conclude that devices with good technologic features such as more channels, a lower minimum frequency, and a higher maximum frequency result in better hearing. Also, based on the age difference that we observed, we recommend that psychological support be provided to older patients with aided hearing to enhance their mental health and quality of life
