    Renal dimensions measured by ultrasonography in children: variations as a function of the imaging plane and patient position

    The purpose of this study was to determine the effect of patient positioning on sonographic renal measurements and to test if the patient position alters the three-dimensional shape of the kidneys. The maximum longitudinal renal length and transverse renal width and depth were measured in the supine and prone position in 100 children (200 kidneys). Age ranged from 6 months to 16 years (mean age 5 years). The results were compared for statistically significant differences. The maximum measured longitudinal renal length was statistically significantly larger in the supine than in the prone position (supine position, left: 8.0cm; right: 7.7cm; prone position, left: 7.9cm, right: 7.6cm; P0.001) and renal volume (P>0.001) in the supine vs. prone positions. Our results show that position-induced reshaping of the kidneys is unlikely to be responsible for the discrepancy in maximum longitudinal renal measurements comparing supine with prone positions. Position-dependent changes in the degree of filling of the renal calyces and pelvis as well as errors in caliper distance measurements for the different scan depths (supine vs. prone) are more likely to be responsible for the encountered differences. Consequently, we recommend to add prone renal length measurements in addition to the supine measurements. In follow-up examinations, renal length measurements should only be compared that have been collected in the same patient positio

    Point of Learning: ein Projektbericht der Hohenheimer Lernorte

    Im Rahmen der mobilen Lehre Hohenheim wurde das Konzept der Lernorte entwickelt. Lehren und Lernen soll außerhalb des Hörsaals, an fĂŒr das jeweilige Lernziel relevanten Orten, ermöglicht werden. Zu diesem Zweck werden Smartphones als Lernassistenten eingesetzt. Im Projekt wurde die entsprechende Software entwickelt, die Bereitschaft zur Nutzung der neuen Technik sowie die didaktische Integrierbarkeit der Lernorte erkundet. Ein Ergebnis ist, dass sich durch den Einsatz Chancen ergeben, Lehrende und Lernende fĂŒr realitĂ€tsnahe Lehre zu sensibilisieren, klassische LernrĂ€ume zu verlassen sowie Lehre flexibler zu gestalten, um die Lernenden beim Wissenstransfer zu unterstĂŒtzen.     Microcirculation abnormalities in patients with fibromyalgia – measured by capillary microscopy and laser fluxmetry

    This unblinded preliminary case-control study was done to demonstrate functional and structural changes in the microcirculation of patients with primary fibromyalgia (FM). We studied 10 women (54.0 ± 3.7 years of age) with FM diagnosed in accordance with the classification criteria of the American College of Rheumatology, and controls in three groups (n = 10 in each group) – age-matched women who were healthy or who had rheumatoid arthritis or systemic scleroderma (SSc). All 40 subjects were tested within a 5-week period by the same investigators, using two noninvasive methods, laser fluxmetry and capillary microscopy. The FM patients were compared with the healthy controls (negative controls) and with rheumatoid arthritis patients and SSc patients (positive controls). FM patients had fewer capillaries in the nail fold (P < 0.001) and significantly more capillary dilatations (P < 0.05) and irregular formations (P < 0.01) than the healthy controls. Interestingly, the peripheral blood flow in FM patients was much less (P < 0.001) than in healthy controls but did not differ from that of SSc patients (P = 0.73). The data suggest that functional disturbances of microcirculation are present in FM patients and that morphological abnormalities may also influence their microcirculation

    Rotavirus Disease in Germany-A Prospective Survey of Very Severe Cases

    Objective: Rotavirus (RV) gastroenteritis is a notifiable disease in Germany. The reports to the authorities contain few data concerning the severity of disease. The aims of this study were to determine incidence and outcome of very severe cases of RV disease. Methods: Cases of very severe RV disease were collected by the German Paediatric Surveillance Unit for rare diseases (Erhebungseinheit fur seltene padiatrische Erkrankungen in Deutschland) using anonymous questionnaires based on hospitalized patients between April 2009 and March 2011. Inclusion criteria were detection of RV antigen in feces, patient aged 0-16 years and 1 or more of the following criteria: intensive care treatment, hypernatremia or hyponatremia (> 155 mmol/ L or < 125 mmol/ L), clinical signs of encephalopathy (somnolence, seizures, apnea) and RV-associated death. Results: During 2 years, 130 cases of very severe RV disease were reported, 101 of 130 were verified. Seventeen patients had nosocomial infection, of whom 14 were neonates in intensive care. Among those, 12 infants had verified or suspected necrotizing enterocolitis. Eighty-four community-acquired cases were reported, median age was 10.5 months (0-108 months). The median hospital stay was 6 days, and 48 patients needed intensive care treatment. Among children less than 5 years of age, the yearly incidence of community-acquired very severe RV disease was 1.2 of 100,000 (95% confidence interval: 0.9-1.4/100,000). A total of 26 of 84 and 10 of 84 patients had severe hypernatremia or hyponatremia, respectively, and 58 of 84 patients had signs of encephalopathy. Three deaths were reported (1 nosocomial and 2 community acquired). Conclusions: RV infection in Germany can have a life-threatening course. A substantial number are nosocomial infections

    Can we use verbal estimation to dissect the internal clock? Differentiating the effects of pacemaker rate, switch latencies, and judgment processes

    Behavioural timing is frequently assumed to be based on the accumulation of pulses from a pacemaker. In humans, verbal estimation is often used to determine whether the effect of factors which influence subjective time become more pronounced at longer durations - that is, if they affect the slope of the judgment function, consistent with a change in the rate of the pacemaker. Here, participants judged blank intervals marked by two squares which either did or did not differ in size. In Experiment 1, a small change in marker size produced shorter temporal judgments than a large change. This effect was independent of objective duration and indicates that the slope changes seen in previous work are not an inevitable artefact of the verbal estimation procedure. However, Experiments 2 and 3 included conditions where the markers did not change size and established (a) that the effect of marker size depends on the other stimuli presented during the experiment, and (b) that slope effects occur even when they cannot possibly be due to a change in the rate of the pacemaker. Taken together, these results urge some caution in the use of verbal estimation as a methodology for deconstructing the putative internal clock

    A PC parallel port button box provides millisecond response time accuracy under Linux

    For psychologists, it is sometimes necessary to measure people's reaction times to the nearest millisecond. This article describes how to use the PC parallel port to receive signals from a button box to achieve millisecond response time accuracy. The workings of the parallel port, the corresponding port addresses, and a simple Linux program for controlling the port are described. A test of the speed and reliability of button box signal detection is reported. If the reader is moderately familiar with Linux, this article should provide sufficient instruction for him or her to build and test his or her own parallel port button box. This article also describes how the parallel port could be used to control an external apparatus

    1WGA J2223.7-0206: a Narrow-Line Quasi-Stellar Object in the XMM-Newton field of view of 3C445

    We report the discovery of a Narrow Line QSO located at about 1.3' from the Broad Line Radio Galaxy 3C445. The source,1WGA J2223.7-0206, although already revealed by ROSAT has never been optically identified previously. An XMM-Newton observation of 3C445 has allowed, for the first time, an accurate X-ray spectral study of 1WGA J2223.7-0206, revealing an ultra-soft spectrum and fast flux variations typical of Narrow Line AGN. The 0.2-10 keV spectrum is well represented by a power law (Gamma=2.5) plus a black body component (kT = 117 eV) absorbed by Galactic NH. About 80% of the X-ray flux is emitted below 2 keV. The 0.2-2 keV flux is observed to decrease by about a factor 1.6 in about 5000 s. The optical observations, triggered by the X-ray study, confirm the Narrow Line AGN nature of this source. The continuum is blue with typical AGN emission lines, pointing to a redshift z=0.46. The full width half maximum of H_beta is 2000 km/secand the flux rat io [OIII}]/H_beta=0.21. The optical luminosity (M_R=-23.2) and the point-like appearance in the optical images identify 1WGA J2223.7-0206 as a Narrow Line QSO. From the optical-UV-X-ray Spectral Energy Distribution we obtain a lower limit of the bolometric luminosity of 1WGA J2223.7-0206 (L_bol > 3 10^{45} erg/sec) implying, for accretion rates close to the Eddington limit, a black hole mass M_BH > 2.4 x 10^{7} M_{\odot}Comment: 6 pages, 5 figures, accepted by A&A, removed pag. 7 containing a duplication of Fig.

    Purification of the food-borne carcinogens 2-amino-3-methylimidazo [4,5-f]quinoline and 2-amino-3,8-dimethylimidazo[4,5-f]quinoxaline in heated meat products by immunoaffinity chromatography

    A rapid and simple scheme has been developed for the isolation and purification of two of the major mutagenic heterocyclic amines formed in heated beef products by affinity chromatography using monoclonal antibodies which recognize 2-amino-3-methylimidazo(4,5-f]quinoline (IQ). Two cell lines producing IgG antibodies were established following fusion of Sp2 or P3x.63 myeloma cells with spleen cells of immunized BALB/cby mice. The antigen was bovine gamma globulin haptenized with 2-(3-carboxypropylthio)-3-methylimidazo[4,5-f]quinoline. The antibodies were immobilized on CNBr-activated Sepharose 4B. IQ and MeIQx formed in heated beef products were partially purified by XAD-2 chromatography and then applied to the affinity columns. Purification by affinity chromatography was adequate for subsequent quantitative analysis by HPLC with UV detection. With this purification scheme as little as 1 g of beef extract or 15 g of fried beef could be assayed for IQ and MeIQx at the part per billion level. Both antibodies had similar affinity constants for IQ (9.3 × 106 and 6.7 × 106 M−1) and for MeIQx (7.1 × 105 and 2.7 × 105 M−1) and both were suitable for immunoaffinity purification of IQ from complex mixtures. MAb2 could be used as well to selectively remove MeIQx from meat products after partial purification by XAD-2. MAb1, despite having a 3-fold higher affinity than MAb2 for MeIQx, could not be used for affinity chromatography for this mutage
