    The spectrum of indications for rhinoplasty in Indian patients is very wide. An ill-defined nose with lack of projection and broad osteocartilagenous framework is the commonest problem. A large number of patients require narrowing of the framework by osteotomies, alar wedge resection, and augmentation by a suitable fill, preferably a cartilage graft.     Suction assisted lipectomy [SAL] as a replacement for conventional surgical excision of subcutaneous lipomata

    Introduction: Lipomata are avascular, painless, slow growing benign tumors of mature fat cells. Surgery is often sought for aesthetic reasons. Conventional excision of lipomata leads to visible scarring which in cases of large or multiple lipomata can be contrary to the purpose of the consult. We present the preliminary results of a novel suction assisted technique for lipoma excision developed at our institute. Materials and Methods: 18 patients with subcutaneous lipomata were included, who among them had a total of 51 lipomata. A suction cannula was inserted through a single sub- centimetric aesthetically placed incision under local anaesthesia to excise the lipomata. Postoperative complications and cosmetic outcomes were noted. Results: Complete excision of up to 17 lipomata was possible in all patients through a single sub- centimetric incision. Cosmetic results were excellent without any intra- or post- operative complications. Conclusion: Suction assisted lipectomy (SAL) is an aesthetic, simple and safe technique requiring no special training, with complete lipomata clearance with minimal complications. We strongly recommend that SAL with capsular extraction as an ideal replacement for conventional surgery in moderate [4-10 cm] to large lipomata [>10 cms] and in multiple lipomatosis

    Comparison of magnetic resonance imaging and histopathological response to chemoradiotherapy in locally advanced rectal cancer

    Magnetic resonance imaging (MRI) methods for chemoradiotherapy (CRT) response assessment of rectal cancer include posttreatment T staging (ymrT), tumor regression grading (mrTRG), volume reduction posttreatment, and modified RECIST measurement. We compared these methods in identifying good versus poor responders with the histopathological standards of T stage (ypT) and tumor regression grading (TRG). A total of 86 patients underwent CRT in a prospective phase II trial for MRI-defined locally advanced rectal cancer. Two readers independently assessed MRIs for ymrT, mrTRG, volume change, and RECIST. Parameters for each case were categorized as good or poor response and analyzed against ypT and TRG by univariate logistic regression. A total of 83 patients had evaluable imaging, and 78 had final pathology (five did not undergo surgery). Of these, 34 patients had good response (ypT0-3a) and 44 had poor response (> ypT3a). Also, 27 patients had favorable pathologic TRG (predominant fibrosis) and 51 had unfavorable TRG (predominant tumor). Good mrTRG and ymr 80 % showed an OR of 3.23 (95 % CI: 1.14-9.17), 4.25 (95 % CI: 0.92-15.45), respectively, for a good ypT score (P = 0.028), but there was no association for histopathological TRG. Favorable and unfavorable histopathology are predicted by both ymrT and mrTRG, and we recommend these parameters for post-treatment assessment of rectal cancers treated with CRT
