Abstract
Context: Aggressive pituitary tumours causing Cushing’s Disease are very rare, difficult to treat, and usually resistant to conventional therapy. There is growing evidence for the use of temozolomide (TZM), an alkylating chemotherapeutic agent, as first line chemotherapy in tumours resistant to repeated neurosurgery, radiotherapy and adrenalectomy.
Objective: To present the response to TMZ in a rare case of an aggressive pituitary tumour in the course of Cushing’s Disease and to review the literature referring to similar cases.
Patient: In this report, we present the case of a 61 year old male patient who was diagnosed with Cushing’s Disease in the course of a pituitary invasive macroadenoma in 2011. The patient underwent 4 transphenoidal non-radical neurosurgeries (2012,2013) with rapid tumour progression, repeated non-radical bilateral adrenalectomy (2012, 2013) and stereotactic radiotherapy, and gamma knife surgery (2013, 2015). Histopathological examination revealed macroadenoma with high cell polymorphism and the presence of Crooke's cells, Ki- < 2%. Since 2015 the patient has been treated with 6 cycles of TMZ (320 mg per day for 5 consecutive days, 28-day cycle) with clinical and biochemical improvement and stabilized tumour size and no side effects. TMZ was continued for up to 9 cycles with a stable serum level of cortisol and ACTH being observed. However, clinical symptoms like headaches, visual field impairment, and finally hearing loss started to progress from the eighth cycle. After the ninth cycle of TMZ, there was a sudden increase in the size of the tumour, impairment of the cortisol and ACTH level, marked deterioration of the clinical status with the recurrence of severe headaches, narrowing of the visual field and hearing loss. At the beginning of 2016, a sudden clinical status and sight deterioration, strong headaches, drop of the right eyelid with widening of the pupil were observed. The patient died in February 2016.
Lessons: The case of our patient suggests that the response to the TMZ treatment monotherapy in aggressive pituitary tumour causing Cushing’s Disease could be partial and restricted to 7-8 cycles followed by rapid progression of the tumor mass. Therefore, further research should be carried out with regard to new methods to extend the responsiveness and duration of TMZ treatment and to investigate predictors of responsiveness. Wstęp: Agresywne guzy przysadki powodujące chorobę Cushinga występują bardzo rzadko, są trudne do leczenia i zazwyczaj oporne na terapię konwencjonalną. Istnieje coraz więcej dowodów na zastosowanie temozolomidu (TZM), alkilującego środka chemioterapeutycznego, jako chemioterapii pierwszej linii w tego rodzaju guzach.
Cel: Zaprezentowanie odpowiedzi na leczenie TMZ w przypadku agresywnego guza przysadki i choroby Cushinga oraz przegląd piśmiennictwa odnoszącego się do podobnych przypadków.
Opis przypadku: Mężczyzna w wieku 61 lat, ze zdiagnozowaną od 2011 roku chorobą Cushinga i inwazyjnym makrogruczolakiem. Przeprowadzono następujące zabiegi: cztery transsfenoidalne nieradykalne zabiegi neurochirurgiczne (2012, 2013 r.) z szybko postępującym guzem, powtarzaną nieradykalną obustronną adrenalektomię (2012, 2013 r.) oraz radioterapię stereotaktyczną i zabieg nożem gamma (2013, 2015 r.). Badanie histopatologiczne wykazało obecność makrogruczolaka z wysokim stopniem polimorfizmu komórkowego oraz obecność komórek Crooke’a, Ki- < 2%. Od 2015 roku pacjent przebył sześć cyklów leczenia TMZ z poprawą parametrów klinicznych i biochemicznych, z ustabilizowaną wielkością guza i brakiem efektów ubocznych. Leczenie TMZ kontynuowane było do dziewięciu cyklów, jednak objawy, takie jak bóle głowy, zaburzenia pola widzenia oraz utrata słuchu postępowały. Po zakończeniu dziewiątego cyklu leczenia TMZ nastąpił nagły wzrost wielkości guza razem z pogorszeniem stanu klinicznego. Pacjent zmarł w lutym 2016 roku.
Wnioski: Przykład opisywanego pacjenta może sugerować, że odpowiedź na monoterapię TMZ w przebiegu agresywnego guza przysadki powodującego chorobę Cushinga może być częściowa, z następującym później szybkim wzrostem masy guza. Wskazane jest prowadzenie dalszych badań, aby przeanalizować czynniki mające wpływ na odpowiedź oraz wydłużyć czas trwania leczenia TMZ

How much of the predisposition to Hashimoto's thyroiditis can be explained based on previously reported associations?
Purpose Our insight in the genetics of Hashimoto’s thyroiditis (HT) has become clearer through information provided by
genome-wide association studies and candidate gene studies, but remains still not fully understood. Our aim was to assess
how many different genetic risk variants contribute to the development of HT.
Methods 147 HT cases (10.2% men) and 147 controls (13.6% men) were qualified for the analysis. Intrinsic and environmental
factors were controlled for. Polymorphisms (SNP) were chosen based on the literature and included markers of the
genes PTPN22, CTLA4, TG, TPO among others, and of genomic regions pointed by GWAS studies. SNP were typed on a
microarray. Variants in the HLA-DRB1 gene were identified by Sanger sequencing.
Results Multivariate predisposition to HT was modeled. Based on the investigated group, a model of seven variables was
obtained. The variability explained by this model was assessed at only 5.4821% (p = 2 × 10-6), which indicates that many
dozens of factors are required simultaneously to explain HT predisposition.
Conclusions We analyzed genetic regions commonly and most significantly associated with autoimmune thyroid disorders
in the literature, on a carefully selected cohort. Our results indicated a lack of possibility to predict the risk of HT development,
even with a multivariate model. We therefore conclude that strong associations of single genetic regions with HT
should be interpreted with great caution. We believe that a change in the attitude towards genetic association analyses of
HT predisposition is necessary. Studies including multiple factors simultaneously are needed to unravel the intricacies of
genetic associations with HT

Menopauza dotyka wszystkie kobiety i ma wpływ na ich jakość życia. Oddziałuje na różne aspekty ich funkcjonowania, głównie zawodowego, społecznego, ale także rodzinnego i psychicznego. W tym okresie zmienia się sposób patrzenia na własną osobę, a także otaczający świat. Okres przekwitania przyczynia się do zmniejszenia samoakceptacji i zwiększenia depresyjności. Praca ma charakter poglądowy. Przedstawiono w niej definicję, podział oraz czynniki determinujące występowanie menopauzy. Zwrócono uwagę na zjawisko wczesnej menopauzy, która charakteryzuje się inną etiologią, czasem wystąpienia, ale podobnymi, choć niejednokrotnie bardziej niebezpiecznymi skutkami. W pracy poruszono temat występowania i konsekwencji wynikających z nieleczonej menopauzy, ponieważ niezdiagnozowana oraz nieprawidłowo „prowadzona” może być przyczyną powikłań oraz stanowić zagrożenie dla życia kobiety. Sposobami łagodzenia objawów menopauzy jest zastosowanie hormonalnej terapii zastępczej lub alternatywnych sposobów leczenia takich jak: kuracja ziołowa, joga, akupunktura, jak również prowadzenie prozdrowotnego stylu życia. Celem pracy było zaprezentowanie najnowszej wiedzy oraz zgromadzenie rzetelnych informacji na temat menopauzy jako nieuniknionego okresu w życiu kobiety, z uwzględnieniem wczesnej menopauzy oraz wpływu czynników determinujących jej wystąpienie. Zwrócono również szczególną uwagę na rolę położnej w opiece nad kobietą w okresie menopauzalnym.Menopause affects all women and it influences the quality of their lives. It has an effect not only on their professional, social or family life, but also on their mental well-being. The way women see themselves and surrounding them world changes a lot in this period. Menopause contributes to the decrease in self acceptance and it increases depression vulnerability.The thesis is an opinion-based work. The definition, division and factors determining menopause were herein presented. The attention was drawn to the phenomenon of early menopause which can be characterized with different etiology, period of occurence but with the similar and, very often, more dangerous effects.The thesis deals with the subject of its occurence and consequences resulting from not treating it, as the lack of its diagnosis or incorrect treatment may lead to complications and may be dangerous to a woman’s life. One of the ways to ease the symptoms of menopause is substitute hormonal therapy or alternative treatment such as herbal therapy, yoga, acupuncture but also a healthy life style.The aim of the thesis was to present the latest knowledge and to collect reliable information on the subject of menopause as an inevitable period in a woman’s life, with the inclusion of early menopause and the factors determining it. The attention was also drawn to the role of a mid-wife in taking care of a woman suffering from menopause
Samobójstwo w chorobach somatycznych
Celem niniejszej pracy jest analiza dostępnej literatury przedmiotu na temat występowania zamiarów
samobójczych w powiązaniu z obecnością schorzenia fizycznego.
Decyzja o samobójstwie wiąże się z konkretną diagnozą psychiatryczną. Występuje w depresji, ale także
w zaburzeniach lękowych, zaburzeniach osobowości typu „z pogranicza” czy schizofrenii znacznie częściej
niż w innych zaburzeniach psychicznych. Częstość zachowań samobójczych jest duża wśród młodzieży
dorastającej oraz osób starszych. Wykazano także wpływ współistnienia choroby somatycznej na
podejmowanie zachowań autodestrukcyjnych.
Zarówno lekarze pierwszego kontaktu, jak i klinicyści mają możliwość zapobiegania zachowaniom autodestrukcyjnym
poprzez monitorowanie zamiarów chorego, które mogą być spowodowane chorobami
somatycznymi, oraz obserwowanie pacjentaThe aim of this paper is the analysis of current literature on suicidal tendencies connected with physical
disability.
The decision of committing a suicide is connected with a specific psychiatric diagnosis. It is more often in
depression, anxiety disorders, borderline personality disorders and schizophrenia than in other mental
disorders. The suicide behaviour rate is higher among teenagers and the elderly. The influence of one’s
mental disease on auto destructive behaviours is a documented fact.
Both GP’s and clinicians have the opportunity to prevent the auto destructive behaviours by observing
their patients and monitoring their intentions, which might be caused by their somatic disorders
