Phenotype-based and Self-learning Inter-individual Sleep Apnea Screening with a Level IV Monitoring System
Purpose: We propose a phenotype-based artificial intelligence system that can
self-learn and is accurate for screening purposes, and test it on a Level IV
monitoring system. Methods: Based on the physiological knowledge, we
hypothesize that the phenotype information will allow us to find subjects from
a well-annotated database that share similar sleep apnea patterns. Therefore,
for a new-arriving subject, we can establish a prediction model from the
existing database that is adaptive to the subject. We test the proposed
algorithm on a database consisting of 62 subjects with the signals recorded
from a Level IV wearable device measuring the thoracic and abdominal movements
and the SpO2. Results: With the leave-one cross validation, the accuracy of the
proposed algorithm to screen subjects with an apnea-hypopnea index greater or
equal to 15 is 93.6%, the positive likelihood ratio is 6.8, and the negative
likelihood ratio is 0.03. Conclusion: The results confirm the hypothesis and
show that the proposed algorithm has great potential to screen patients with
SAS
Modeling Noise Coupling Between Package and PCB Power/Ground Planes with an Efficient 2-D FDTD/Lumped Element Method
An efficient numerical approach based on the 2-D finite-difference time-domain (FDTD) method is proposed to model the power/ground plane noise or simultaneously switching noise (SSN), including the interconnect effect between the package and the print circuit board (PCB). The space between the power and ground planes on the package and PCB are meshed with 2-D cells. The equivalent R-L-C circuits of the via and the solder balls connecting the package and PCB can be incorporated into a 2-D Yee cell based on a novel integral formulation in the time domain. An efficient recursive updating algorithm is proposed to fit the lumped networks into the Yee equations. A test sample of a ball grid array (BGA) package mounted on a PCB was fabricated. The power/ground noise coupling behavior was measured and compared with the simulation. The proposed method significantly reduces the computing time compared with other full-wave numerical approaches

Concurrent Use in Taiwan of Chinese Herbal Medicine Therapies among Hormone Users Aged 55 Years to 79 Years and Its Association with Breast Cancer Risk: A Population-Based Study
Background. The purpose of the present study was to analyze the concurrent use of Chinese herbal products (CHPs) among women aged 55 to 79 years who had also been prescribed hormonal therapies (HT) and its association with breast cancer risk. Methods. The use, frequency of service, and CHP prescribed among 17,583 HT users were evaluated from a random sample of 1 million beneficiaries from the National Health Insurance Research Database. A logistic regression method was used to identify the factors that were associated with the coprescription of a CHP and HT. Cox proportional hazards regressions were performed to calculate the hazard ratios (HRs) of breast cancer between the TCM nonusers and women who had undergone coadministration of HT and a CHP or CHPs. Results. More than one out of every five study subjects used a CHP concurrently with HT (CHTCHP patients). Shu-Jing-Huo-Xie-Tang was the most commonly used CHP coadministered with HT. In comparison to HT-alone users, the HRs for invasive breast cancer among CHTCHP patients were not significantly increased either in E-alone group or in mixed regimen group. Conclusions. The coadministration of hormone regimen and CHPs did not increase the risk of breast cancer

EBV-positive Hodgkin lymphoma is associated with suppression of p21cip1/waf1 and a worse prognosis
<p>Abstract</p> <p>Background</p> <p>About 30-50% of Hodgkin lymphomas (HLs) harbor the Epstein-Barr virus (EBV), but the impact of EBV infection on clinical outcomes has been unclear. EBV-encoded small RNAs (<it>EBER</it>s) are presented in all EBV-infected cells, but their functions are still less understood.</p> <p>Results</p> <p><it>EBER1 </it>was transfected into two HL cell lines, KMH2 and L428, and microarrays were used to screen for <it>EBER1</it>-induced changes. We found that <it>EBER1 </it>suppressed <it>p21</it><sup>cip1/waf1 </sup>transcription in HL cell lines. In addition, positive regulators of <it>p21</it><sup>cip1/waf1 </sup>transcription, such as p53, EGR1, and STAT1, were decreased. Suppression of <it>p21</it><sup>cip1/waf1 </sup>in the <it>EBER1</it><sup>+ </sup>HL cell lines was associated with increased resistance to histone deacetylase inhibitors or proteasome inhibitors, drugs known to cause apoptosis by increasing p21<sup>cip1/waf1 </sup>levels. On biopsy specimens, EBV<sup>+ </sup>HLs had weaker expression of both p21<sup>cip1/waf1 </sup>and active caspase 3. Clinically, suppression of p21<sup>cip1/waf1 </sup>in EBV<sup>+ </sup>HLs was associated with a worse 2-year disease-free survival rate (45% for EBV<sup>+ </sup>HLs <it>vs</it>. 77% for EBV<sup>- </sup>HLs, <it>p </it>= 0.002).</p> <p>Conclusion</p> <p>Although the underlying mechanisms are still relatively unclear, <it>EBER1 </it>inhibits <it>p21</it><sup>cip1/waf1 </sup>transcription and prevents apoptosis through down-regulation of p53, EGR1, and STAT1. The anti-apoptotic activity of <it>EBER1 </it>may be important in the rescue of Reed-Sternberg cells from drug-induced apoptosis and in the clinical behaviors of EBV<sup>+ </sup>HLs.</p
High Altitude Pulmonary Edema in a Patient with Previous Pneumonectomy
High altitude pulmonary edema (HAPE) is a life-threatening illness that can occur in individuals ascending to altitudes exceeding 2400 m. The risk factors are rapid ascent, physical exertion and a previous history of HAPE. This work presents a case study of a 74-year-old man who underwent left side pneumonectomy 40 years ago and subsequently experienced several instances of HAPE. The well-known risk factors for HAPE were excluded in this patient. We suspect that the post-pneumonectomy condition may be a risk factor for HAPE based on this case.
