    Uroflow studies in children: Correlation with age, weight and body surface area.

    Peak flow rate data (Q(max)) are important for diagnosis of lower urinary tract obstruction. However, the lack of uroflowmetry studies in children, makes their interpretation difficult. With this aim, we studied 167 boys classified in four groups according to their age (G1 : n = 48, 6-7 years; G2 : n = 43, 8-9 years; G3 : n = 37, 10-11 years; G4: n = 39, 12-14 years). We studied in all children, weight, height, body surface area, peak flow rate (Q(max) and the correspondant urinary volume (Vol). Means and standard deviations of Q(max) (ml/sec) were : 15 +/- 5 (G1), 15 +/- 5 (G2), 17 +/- 5 (G3) and 22 +/- 7 (G4) respectively. Corresponding urinary volumes (Vol) (mean standard deviation - in ml) were : G1 = 123 +/- 75; G2 = 122 +/- 79; G3 = 158 +/- 96 and G4 = 162 +/- 101. We found a significant correlation (p < 0,01) between Q(max) and Vol in groups G2, G3 and G4; and between Q(max) and height in groups G1 and G4.The authors demonstrated a positive correlation between maximum flow and voided volume, and an increase of Q(max) with age

    Genotyping of AR and PSA polymorphisms in a patient with Klinefelter syndrome, non-Hodgkin lymphoma, and adenocarcinoma of the prostate

    It has been hypothesized that the AR (androgen receptor) gene binds the two PSA (prostate-specific antigen) alleles with differing affinities and may differentially influence prostate cancer risk. In this article, we report a case of adenocarcinoma of the prostate in a 56-year-old man with Klinefelter syndrome (47,XXY) and non-Hodgkin lymphoma, as well as the AR and PSA genotype. AR and PSA gene polymorphisms were analyzed by polymerase chain reaction-based methods using DNA from peripheral white blood cells and the prostate cancer. We determined the methylation status of the AR gene on the X chromosome. The patient presents with the AG genotype for the ARE-I (androgen response element) region of the PSA gene. We detect the presence of two short AR alleles with 19 and 11CAG repeats each. Unmethylated alleles were demonstrated for both. The shorter allele was inactive in more than 60% of total DNA in both control blood and prostate cancer cells. The presence of short AR alleles and the G allele of the PSA gene may contribute to the development of prostate cancer in a 47,XXY patient. (C) 2004 Elsevier B.V. All rights reserved

    Urethral reconstruction after total penectomy for urethral cancer

    A 59-year-old white man developed a ventral ulcer with irregular limits in the middle portion of the penis. The result of the pathologic analysis was compatible with invasive squamous cell urethral carcinoma. A total penectomy was performed. In these cases, the usually recommended urinary diversion is perineal urethrostomy. However, due to the specifications of the case, perineal urethrostomy could not be performed. The literature did not offer any other alternative for patients with this same condition. Therefore, a urethral reconstruction using a groin skin flap had to be performed. Copyright (C) 2004 S.     Sexualidade e amamentação: concepções e abordagens de profissionais de enfermagem da atenção primária em saúde

    Este estudo objetiva conhecer as concepções sobre a sexualidade de profissionais de enfermagem que atuam na atenção primária em saúde e identificar como essa temática integra as práticas assistenciais desses profissionais a mulheres nutrizes. Trata-se de pesquisa qualitativa, exploratória e descritiva, cujos sujeitos foram oito profissionais de enfermagem atuantes em três unidades de atenção primária em saúde de município do Norte do Rio Grande do Sul. A coleta dos dados realizou-se via entrevistas semiestruturadas. Para seu tratamento foi utilizada a análise de conteúdo temática, da qual emergiram três temas: concepção sobre sexualidade; a sexualidade e a amamentação; sexualidade e práticas de enfermagem voltadas à mulher nutriz. O estudo permitiu constatar que os profissionais de enfermagem entendem o sentido amplo que a sexualidade representa e a relação que tem com a amamentação, porém não a abordam ao assistirem a mulher em processo de amamentação, sendo suas práticas sustentadas em abordagem biomédica
