    Carbon fiber masculinity: Disability and surfaces of homosociality

    In this paper I am concerned with instances in which carbon fiber extends performances of masculinity that are attached to particular kinds of hegemonic male bodies. In examining carbon fiber as a prosthetic form of masculinity, I advance three main arguments. Firstly, carbon fiber can be a site of the supersession of disability that is affected through masculinized technology. Disability can be ‘overcome’ through carbon fiber. Disability is often culturally coded as feminine (Pedersen, 2001; Meeuf, 2009; Garland-Thompson 1997). Building on this cultural construction of disability as feminine, in and as a technology of masculine homosociality (Sedgwick, 1985), carbon fiber reproduced disability as feminine when carbon fiber prosthetic lower legs allowed Oscar Pistorius to compete in the non-disabled Olympic games. Secondly, I argue that carbon fiber can be a homosocial surface; that is, carbon fiber becomes both a surface extension of the self and a third party mediator in homosocial relationships, a surface that facilitates intimacy between men in ways that devalue femininity in both male and female bodies. I examine surfaces as material extensions of subjectivity, and carbon fiber surfaces as vectors of the cultural economies of masculine competition to which I refer. Thirdly, the case of Oscar Pistorius is exemplary of the masculinization of carbon fire, and the associated binding of a psychic attitude of misogyny and power to a form of violent and competitive masculine subjectivity. In this article I explore the affects, economies and surfaces of what I call ‘carbon fiber masculinity’ and discusses Pistorius’ use of carbon fiber, homosociality and misogyny as forms of protest masculinity through which he unconsciously attempted to recuperate his gendered identity from emasculating discourses of disability

    Motivations for seeking experimental treatment in Japan

    In this article on innovative medical treatment for serious conditions in Japan we aim to revise two widespread notions: first, that people living with severe conditions are all waiting for a cure or are impatient to try out experimental treatment, in particular regenerative medicine. Showing that motivations for cure seeking are complex and linked to somatic identity, we argue that gaining a cure also means a new social normality, which for some people narrows the only normality that is meaningful to them; and, second, that people living with a serious (latent) condition necessarily define their lives as not normal in the light of normalization. People with a condition conceptualise normal life variously and multiply in the light of both individual and collective experiences. The two revisions are crucial to attempts at understanding what makes people seek experimental medicine. Comparing the narratives of people with four different conditions – spinal cord injury, Duchenne muscular dystrophy, Diabetes Mellitus type 1 and cardiovascular disease – it becomes clear that the difference between seeking treatment or not largely depends on somatic identities; rather than through notions of (ab)normality, it is more adequately understood in terms of the experience of somatic lacking and wholeness

    Comparative costs and activity from a sample of UK clinical trials units

    Background: The costs of medical research are a concern. Clinical Trials Units (CTUs) need to better understand variations in the costs of their activities. Methods: Representatives of ten CTUs and two grant-awarding bodies pooled their experiences in discussions over 1.5 years. Five of the CTUs provided estimates of, and written justification for, costs associated with CTU activities required to implement an identical protocol. The protocol described a 5.5-year, nonpharmacological randomized controlled trial (RCT) conducted at 20 centres. Direct and indirect costs, the number of full time equivalents (FTEs) and the FTEs attracting overheads were compared and qualitative methods (unstructured interviews and thematic analysis) were used to interpret the results. Four members of the group (funding-body representatives or award panel members) reviewed the justification statements for transparency and information content. Separately, 163 activities common to trials were assigned to roles used by nine CTUs; the consistency of role delineation was assessed by Cohen's κ. Results: Median full economic cost of CTU activities was £769,637 (range: £661,112 to £1,383,323). Indirect costs varied considerably, accounting for between 15% and 59% (median 35%) of the full economic cost of the grant. Excluding one CTU, which used external statisticians, the total number of FTEs ranged from 2.0 to 3.0; total FTEs attracting overheads ranged from 0.3 to 2.0. Variation in directly incurred staff costs depended on whether CTUs: supported particular roles from core funding rather than grants; opted not to cost certain activities into the grant; assigned clerical or data management tasks to research or administrative staff; employed extensive on-site monitoring strategies (also the main source of variation in non-staff costs). Funders preferred written justifications of costs that described both FTEs and indicative tasks for funded roles, with itemised non-staff costs. Consistency in role delineation was fair (κ = 0.21-0.40) for statisticians/data managers and poor for other roles (κ < 0.20). Conclusions: Some variation in costs is due to factors outside the control of CTUs such as access to core funding and levels of indirect costs levied by host institutions. Research is needed on strategies to control costs appropriately, especially the implementation of risk-based monitoring strategies

    Molecular Characterization of Spontaneous Mesenchymal Stem Cell Transformation

    Background. We previously reported the in vitro spontaneous transformation of human mesenchymal stem cells (MSC) generating a population with tumorigenic potential, that we termed transformed mesenchymal cells (TMC). Methodology/Principal Findings. Here we have characterized the molecular changes associated with TMC generation. Using microarrays techniques we identified a set of altered pathways and a greater number of downregulated than upregulated genes during MSC transformation, in part due to the expression of many untranslated RNAs in MSC. Microarray results were validated by qRT-PCR and protein detection. Conclusions/Significance. In our model, the transformation process takes place through two sequential steps; first MSC bypass senescence by upregulating c-myc and repressing p16 levels. The cells then bypass cell crisis with acquisition of telomerase activity, Ink4a/Arf locus deletion and Rb hyperphosphorylation. Other transformation-associated changes include modulation of mitochondrial metabolism, DNA damage-repair proteins and cell cycle regulators.     A practice change intervention to improve antenatal care addressing alcohol consumption by women during pregnancy: research protocol for a randomised stepped-wedge cluster trial

    Background: Despite clinical guideline recommendations, implementation of antenatal care addressing alcohol consumption by pregnant women is limited. Implementation strategies addressing barriers to such care may be effective in increasing care provision. The aim of this study is to examine the effectiveness, cost and cost-effectiveness of a multi-strategy practice change intervention in increasing antenatal care addressing the consumption of alcohol by pregnant women. Methods: The study will be a randomised, stepped-wedge controlled trial conducted in three sectors in a health district in New South Wales, Australia. Stepped implementation of a practice change intervention will be delivered to sectors in a random order to support the introduction of a model of care for addressing alcohol consumption by pregnant women. A staged process was undertaken to develop the implementation strategies, which comprise of: leadership support, local clinical practice guidelines, electronic prompts and reminders, opinion leaders, academic detailing (audit and feedback), educational meetings and educational materials, and performance monitoring. Repeated cross-sectional outcome data will be gathered weekly across all sectors for the study duration. The primary outcome measures are the proportion of antenatal appointments at 'booking in', 27-28 weeks gestation and 35-36 weeks gestation for which women report (1) being assessed for alcohol consumption, (2) being provided with brief advice related to alcohol consumption during pregnancy, (3) receiving relevant care for addressing alcohol consumption during pregnancy, and (4) being assessed for alcohol consumption and receiving relevant care. Data on resources expended during intervention development and implementation will be collected. The proportion of women who report consuming alcohol since knowing they were pregnant will be measured as a secondary outcome. Discussion: This will be the first randomised controlled trial to evaluate the effectiveness, cost and cost-effectiveness of implementation strategies in improving antenatal care that addresses alcohol consumption by pregnant women. If positive changes in clinical practice are found, this evidence will support health service adoption of implementation strategies to support improved antenatal care for this recognised risk to the health and wellbeing of the mother and child.Melanie Kingsland, Emma Doherty, Amy E.     The interactions of disability and impairment

    Theoretical work on disability is going through an expansive period, built on the growing recognition of disability studies as a discipline and out of the political and analytical push to bring disability into a prominent position within accounts of the intersecting social categories that shape people's lives. A current debate within critical disability studies is whether that study should include impairment and embodiment within its focus. This article argues it should and does so by drawing from symbolic interactionism and embodiment literatures in order to explore how differences in what bodies can do-defined as impairments-come to play a role in how people make sense of themselves through social interaction. We argue that these everyday interactions and the stories we tell within them and about them are important spaces and narratives through which impairment and disability are produced. Interactions and stories are significant both in how they are shaped by wider social norms, collective stories and institutional processes, and also how they at times can provide points of resistance and challenges to such norms, stories and institutions. Therefore, the significance of impairment and interaction is the role they play in both informing self-identity and also broader dynamics of power and inequality

    Smad phosphoisoform signals in acute and chronic liver injury: similarities and differences between epithelial and mesenchymal cells

    Hepatocellular carcinoma (HCC) usually arises from hepatic fibrosis caused by chronic inflammation. In chronic liver damage, hepatic stellate cells undergo progressive activation to myofibroblasts (MFB), which are important extracellular-matrix-producing mesenchymal cells. Concomitantly, perturbation of transforming growth factor (TGF)-β signaling by pro-inflammatory cytokines in the epithelial cells of the liver (hepatocytes) promotes both fibrogenesis and carcinogenesis (fibro-carcinogenesis). Insights into fibro-carcinogenic effects on chronically damaged hepatocytes have come from recent detailed analyses of the TGF-β signaling process. Smad proteins, which convey signals from TGF-β receptors to the nucleus, have intermediate linker regions between conserved Mad homology (MH) 1 and MH2 domains. TGF-β type I receptor and pro-inflammatory cytokine-activated kinases differentially phosphorylate Smad2 and Smad3 to create phosphoisoforms phosphorylated at the COOH-terminal, linker, or both (L/C) regions. After acute liver injury, TGF-β-mediated pSmad3C signaling terminates hepatocytic proliferation induced by the pro-inflammatory cytokine-mediated mitogenic pSmad3L pathway; TGF-β and pro-inflammatory cytokines synergistically enhance collagen synthesis by activated hepatic stellate cells via pSmad2L/C and pSmad3L/C pathways. During chronic liver disease progression, pre-neoplastic hepatocytes persistently affected by TGF-β together with pro-inflammatory cytokines come to exhibit the same carcinogenic (mitogenic) pSmad3L and fibrogenic pSmad2L/C signaling as do MFB, thereby accelerating liver fibrosis while increasing risk of HCC. This review of Smad phosphoisoform-mediated signals examines similarities and differences between epithelial and mesenchymal cells in acute and chronic liver injuries and considers Smad linker phosphorylation as a potential target for the chemoprevention of fibro-carcinogenesis

    "I Feel Confident Most of the Time": A Study of the Relationships Between Writing Transfer, Dispositions Toward Learning and Writing, and Perceptions of Classroom Contexts

    As recently as 2012, Elizabeth Wardle called for writing studies researchers to give "attention to the dispositions that students are embodying across fields and consider how the nature of those dispositions can either inhibit or enhance their ability to engage in the expansive learning and repurposing that I understand to constitute `transfer' of writing related knowledge" (p. 11). In responding to this call in the research, this dissertation project applies a dispositional lens to writing transfer as it occurs in specific contexts for learning. While theories concerning the connection between writing transfer and dispositions have been initially explored (Driscoll and Wells, 2012; Wardle, 2012), this connection has not yet been empirically and comprehensively examined. This project attempts such empirical and comprehensive examination of these connections through an analysis of data through a conceptual model which seeks to both capture and examine the relationships between dispositions, writing transfer, and contexts for learning. In order to begin to understand the relationships between transfer, dispositions, and learning contexts, the qualitative and empirical nature of this study presents findings that reveal the conceptual model in action from the actor's—or learner's— perspective. This dissertation uncovers the self-perceptions of seven individual learners in relation to writing transfer as they move across the high school to college transition and encounter particular contexts for writing. Findings from this project suggest that the discovery and evaluation of one's prior knowledge includes dispositional elements, including self-efficacy and self-regulation. Meanwhile, dispositions and prior knowledge are also elicited in specific ways within the contexts in which transfer is expected to occur. Interview data revealed how participants applied their dispositions as agents of their own learning as they moved into new post-secondary learning and writing contexts. Furthermore, it revealed how participants' dispositions transferred across the high school to college transition and the specific contexts within them, and how dispositions informed participants' transfer of prior knowledge and/or application of new knowledge. Specifically, the movement (or lack of movement) across the transfer "steps" of detect and elect for participants suggests that the process of transfer involves more than the single act of moving knowledge across contexts. In taking a broader view of transfer through a D-E-C-Enculturation perspective, this study reveals how transfer is highly bound up with dispositions, particularly self-efficacy, and how dispositions interact with a detection of prior knowledge and a decision-making election of whether or not to transfer and/or transform prior knowledge in the new context. Self-efficacy and other dispositions particularly impacted how such decision-making took place for subjects in this study. Finally, this project views dispositions through a conceptual model of transfer that looks not only at what prior knowledge students connect to new writing tasks and contexts, but also to how they detect and elect to pursue their connections to prior knowledge and how they are enculturated into academic communities. Thus, in pursuing research on the relationship between dispositions and transfer, I have attempted to locate where learning dispositions factor in the student's experiences and perspectives of transfer using Perkins and Salomon's (2012) detect-elect-connect framework for learning transfer, to which I have added the fourth step of enculturation. As extensive research on dispositions leading up to this project has argued, dispositions are always elicited in particular contexts. But, as this research also shows, dispositions can work broadly as they are carried into and out of contexts by individuals. Contexts also embody particular conditions and dispositions of field to which individual dispositions attune. This concept of individuals' attunement to communities or contexts for learning reveal the shaping and constructing nature of context and how individual dispositions may be enacted in learning or writing transfer. Ultimately, findings from this project suggest that further research on the connections between dispositions, contexts, and a D-E-C-Enculturation view of transfer can help classroom practitioners, learning theorists, and composition researchers more deeply consider the roles that dispositions play in learning and writing transfer
