    Learning about View of Life and Death, Especially from Children’s Death Caused by Natural Disasters in the City of Manado, Province of North Sulawesi,Indonesa

    This is a study conducted by our research group about the psychological background of local people in Manado,Indonesia. Interviews were made to a group of locals concerning their approach to death and consequent state of mind focused on their religious beliefs on Obon festival,conversation with spirits,reincarnation and the influence of holy men. We recognized the fantasy they believed that spirit comes back from afterworld in the ceremony. In addition,we understood the event that spirit returns from afterworld once a year and“offering” is common sense. However,these custom had difference depended on regions and their social rank. As a result,these local people were difficult to share the common sense about the event. Reporting to spirit and conversation to spirit were common events in whole regions in Indonesia,but the quality might be different. Especially,idea about reincarnation was modified among young and old generations.調査・事例等報

    There are many problem behavior of children in Japan such as bullying, juvenile delinquency, school violence, vandalism, drug etc. Many factors cause such problems, but we set up a hypothesis that the decline of self-esteem of children is very important factor. From this hypothesis and the examination of the precedence research regarding self-esteem, we set the following 3 points as the aim of this paper; 1) to review the history of Project Adventure (PA) program and to grasp of its definition and contents, 2) to grasp present conditions and problems of PA program in Miyagi Prefecture that firstly introduced it in Japan, 3) to search possibilities and limits of Miyagi PA program in the formation of the self-esteem of children. As a result of research, we conclude next three points: 1) PA is defined as the formalized process of experiential learning that makes a series of adventurous and challenge activities as the means for intending the change of the consciousness and behavior of students. PA program have been advanced the wide use and qualitative change in the process of the occurrence and diffusion in USA. It diffused partially after PAJ (Project Adventure Japan) is established in 1995 in Japan. 2) PA in Miyagi Pref. is being carried out as MAP (Miyagi Adventure Program) under the guidance of PAJ and the fundamental thought is expressed as the \u27cycle of experiential learning\u27. Miyagi Pref. is the only one prefecture which tackles with PA by the board of education as a whole, and promote the program in the cooperation of school education and social education. The program is developed on 4 contents; (1) training of leaders, (2) enlightenment activities, (3) upgrading of the institutions and equipments, and (4) introduction of PA to the school education activities. However, the training of the leader is restrictive, and training activities are relying on PAJ now. Also, it is very difficult to measure the educational effects of PA program to students. 3) The board of education try to introduce the PA program to all public schools, but some teachers and principals reject the PA program because they doubt its effect. The staffs of board of education think that such teachers and principals do not participate the training and do not understand significance of PA program. But we think there is other reason. In the process of introducing PA program to school education, it decreased elements of adventure that improve the self-esteem of children

    Effect of 5-Fluorouracil Treatment on SN-38 Absorption from Intestine in Rats

    5-Fluorouracil (5-FU)-based chemotherapies with irinotecan have been applied for the treatment of cancers, and a common dose-limiting toxicity is neutropenia and diarrhea. In this study, we investigated the effect of 5-FU treatment on expression levels of drug transporters for SN-38 transportation and SN-38 absorption from the intestine following 5-FU treatment. Expression levels of several drug transporters and nuclear receptors in rats after 5-FU treatment were evaluated. SN-38 absorption from the intestine was evaluated by SN-38 concentration levels in serum following SN-38 injection into the intestine of 5-FU treated rats. The levels of renal multidrug resistance protein 2 (Mrp2) on day 4 after treatment (400 mg/kg) showed significant upregulation, 359.2 ± 33.2% (mean ± S.E.) of control. Mrp2 levels in the intestine were downregulated to 26.2 ± 8.4% of control. 5-FU treatment (400 mg/kg) also significantly downregurated expression levels of P-glycoprotein (P-gp) and breast cancer resistance protein (Bcrp) to 41.2 ± 14.7%, 15.7 ± 4.3% of control, respectively. To evaluate SN-38 absorption from the intestine, SN-38 was loaded in to the intestine on day 4 after 5-FU treatment. Pretreatment with 5-FU significantly increased SN-38 concentration in the blood 30, 60 and 90 min after SN-38 administration. The area under the curve for SN-38 in the 5-FU group was significantly higher than in vehicle groups. 5-FU treatment decreased expression levels of P-glycoprotein and Bcrp in intestine. The present study suggests that combination chemotherapy of 5-FU with irinotecan (CPT-11) may elevate SN-38 absorption from intestine

    Cystoid Macular Edema during Treatment with Paclitaxel and Bevacizumab in a Patient with Metastatic Breast Cancer: A Case Report and Literature Review

    We present a case of a metastatic breast cancer patient with cystoid macular edema (CME) occurring during treatment with paclitaxel and bevacizumab. She had a history of neoadjuvant chemotherapy and partial mastectomy plus axillary lymph node dissection for stage IIB left-breast cancer. Twenty-four months later, she was diagnosed with multiple bone metastases and underwent chemotherapy with paclitaxel and bevacizumab. Thirty-three months after the initiation of the chemotherapy, she noticed bilateral blurred vision. The retinal thickening with macular edema was observed by optical coherence tomography, resulting in a diagnosis of CME. With cessation of paclitaxel and administrating ocular instillation of a nonsteroidal anti-inflammatory drug, her macular edema gradually reduced and disappeared in a month. While CME caused by chemotherapy is very rare, taxane may cause ocular adverse events such as CME. It is important to urge patients to consult an ophthalmologist promptly when they have visual complaints during taxane chemotherapy
