    Actual and preferred contraceptive sources among young people: findings from the British National Survey of Sexual Attitudes and Lifestyles

    OBJECTIVE: To describe actual and preferred contraceptive sources among young people in Britain and whether discordance between these is associated with markers of sexual risk behaviour or poor sexual health. DESIGN: Cross-sectional probability sample survey. SETTING: British general population. PARTICIPANTS: 3869 men and women aged 16-24 years interviewed for the third National Survey of Sexual Attitudes and Lifestyles (Natsal-3) between 2010 and 2012. MAIN OUTCOME MEASURES: Reported source of contraceptive method(s) and preferred source if all were available and easily accessible. RESULTS: Of the 75% of young people (aged 16-24) who were heterosexually active (1619 women, 1233 men), >86% reported obtaining contraceptives in the past year. Most common sources were general practice (women, 63%) and retail (men, 60%): using multiple sources was common (women 40%, men 45%). Healthcare sources were preferred by 81% of women and 57% of men. Overall, 32% of women and 39% of men had not used their preferred source. This discordance was most common among men who preferred general practice (69%) and women who preferred retail (52%). Likelihood of discordance was higher among women who usually used a less effective contraceptive method or had an abortion. It was less likely among men who usually used a less effective method of contraception and men who were not in a steady relationship. CONCLUSIONS: Most young people in Britain obtained contraception in the past year but one-third had not used their preferred source. Healthcare sources were preferred. Discordance was associated with using less effective contraception and abortion among young women. Meeting young people's preference for obtaining contraception from healthcare sources could improve uptake of effective contraception to reduce unwanted pregnancies

    A mammalian sperm cytosolic phospholipase C activity generates inositol trisphosphate and causes Ca²⁺ release in sea urchin egg homogenates

    Injection of sperm extracts triggers Ca²⁺ oscillations in mammalian eggs similar to those seen at fertilisation. Here, we show that addition of sperm extracts to sea urchin egg homogenates causes Ca²⁺ release and inositol 1,4,5-trisphosphate (InsP₃) production. Furthermore depleting homogenates of phosphatidylinositol lipids using a phosphatidylinositol-specific phospholipase C blocked the sperm extract from causing InsP₃ production and a Ca²⁺ rise. A response could be recovered by the addition of phosphatidylinositol 4,5-bisphosphate to either sperm extracts or egg homogenates. These data indicate that sperm extracts contain an InsP₃-generating phospholipase C which may play a role in Ca²⁺ release at fertilisation

    Building an environmentally accountable medical curriculum through international collaboration

    Background: Global environmental change is associated with significant health threats. The medical profession can address this challenge through advocacy, health system adaptation and workforce preparedness. Stewardship of health systems with attention to their environmental impacts can contribute to mitigation of and adaptation to negative health impacts of environmental change. Medical schools have an integral role in training doctors who understand the interdependence of ecosystems and human health. Yet integrating environmental perspectives into busy medical curricula is not a simple task. Content: At the 2016 Association for Medical Education in Europe conference, medical educators, students and clinicians from six continents discussed these challenges in a participatory workshop. Here we reflect on emerging themes from the workshop and how to plan for curricular change. Firstly, we outline recent developments in environmental health and associated medical education. Secondly, we reflect on our process and outcomes during this innovative approach to international collaboration. Thirdly, we present learning objectives which cover core content for environmentally accountable medical curricula, developed through a reflective process of international collaboration integrating current literature and the workshop outcomes. Conclusions: International collaboration can bring together diverse perspectives and provide critical insights for the inclusion of environmental health into basic education for medical practitioners

    A philosophy to fit “the character of this historical period”? Responses to Jean-Paul Sartre in some British and American philosophy departments, c. 1945 - 1970.

    Anglophone philosophers are often associated with rejecting philosophy’s moral guidance function after 1945. This article builds on existing work on Jean-Paul Sartre’s reception in universities to show that, actually, many British and American philosophers embraced moral guidance roles by engaging with his work and that they promoted creativity and choice in society as a result. Sartre first came to philosophers’ attention in the context of post-war Francophilia, but interest in him quickly went beyond the fact that he was French and expanded to include the wider existentialist movement that he was a part of. Sartre had enduring popularity among English speaking philosophers because his philosophy resonated with the older British and American philosophies idealism and pragmatism that, like his, were inspired by Hegel. Sartre’s respondents also valued existentialism because, to them, it made certain Judeo-Christian principles relevant, thus protecting religion at a time when they believed it was threatened with decline, and by the advance of specialisation. Anglophone philosophers who were interested in Sartre spread their responses to him through teaching an expanding student population, but also reached the wider public through activism, journalism, broadcasting, and government advisory roles. In doing so, philosophers integrated existential ideas into several aspects of culture in post-war Britain and America
