Badanie zależności nasilenia bólu pooperacyjnego od warunków wykonywania pomiarów

Ocena bólu w spoczynku jest ważna dla zapewnienia pacjentowi komfortu, natomiast w warunkach dynamicznych dla zredukowania ryzyka pooperacyjnych komplikacji, głównie krążeniowo-oddechowych. Celem pracy było badanie zależności pomiędzy nasileniem bólu pooperacyjnego ocenianego skalą VRS i NRS w spoczynku oraz w warunkach dynamicznych (wg schematu skali PHHPS) a skutecznością postępowania przeciwbólowego. Badanie prospektywne obserwacyjne przeprowadzono u 145 pacjentów w wieku 7–18 lat po zabiegach torakochirurgicznych.
Porównano średnie nasilenie bólu w spoczynku (n = 725 pomiarów) ze średnim nasileniem bólu z pomiarów uzyskanych w warunkach dynamicznych (n = 725 pomiarów). Przyjęto, że analgezja pooperacyjna jest satysfakcjonująca, jeżeli chory określił skalą VRS nasilenie bólu jako słabe (w spoczynku: NRS ≤ 2 punktów; podczas głębokiego oddychania i kaszlu: NRS ≤ 3 punktów) lub brak bólu (w spoczynku: NRS = 0; brak bólu przy kaszlu: NRS = 0). W warunkach dynamicznych odnotowano istotnie wyższe warto ści nasilenia bólu niż w spoczynku (VRS: 1 vs 0,4; NRS: 1,9 vs 0.8; p = 0,000 w teście Wilcoxona) oraz mniejszy odsetek pomiarów świadczących o skutecznej analgezji niż
przy pomiarze w spoczynku (74,1% vs 88,1%; p = 0,000 w teście McNemary). Warunki pomiaru nasilenia bólu mają istotny wpływ na ocenę skuteczności postępowania przeciwbólowego
u dzieci po operacjach torakochirurgicznych.It is postulated that postoperative pain should be evaluated both at rest and during deep breathing, and coughing. Assessment of pain at rest is important for making the patient comfortable while in dynamic conditions for reducing risks of cardiopulmonary complications after surgery. The purpose of this study was to recognize correlations between postoperative pain intensity monitored with the Verbal Rating Scale (VRS) and Numeric Rating Scale (NRS) at rest and in dynamic conditions (according to PHHPS) and the effectiveness of a pain management. A prospective observational study was performed
in 145 children aged 7–18 years old after thoracic surgery. The average pain intensity scores at rest (n = 725 measurements) were compared to average dynamic pain intensity scores (n = 725 measurements). It was assumed that postoperative pain relief was adequate if patient’s pain according to VRS was weak (at rest: NRS ≤ 2 scores; during deep breathing and coughing: NRS ≤ 3 scores) or no pain (at rest: NRS = 0; no pain during coughing: NRS = 0). Average pain intensity scores were signifi cantly higher in
dynamic conditions than at rest (VRS: 1 vs 0.4; NRS: 1.9 vs 0.8; p = 0,000; Wilcoxon test). The proportion of the measurements of effective analgesia was lower in dynamic conditions than at rest (74.1% vs 88.1%; p = 0,000; McNemar’s test). Natężenie bólu u chorych na młodzieńcze idiopatyczne zapalenie stawów a poziom aktywności i akceptacji choroby
Cel pracy: Rozpoznanie choroby przewlekłej, jaką jest młodzieńcze
idiopatyczne zapalenie stawów (MIZS), wpływa negatywnie
na aktywność młodego człowieka. Dodatkowo ból towarzyszący
chorobie może też w znaczny sposób powodować trudności z jej
zaakceptowaniem. Celem pracy była ocena stopnia natężenia bólu
oraz stopnia aktywności i akceptacji choroby przez młodzież w wieku
14-18 lat z MIZS.
Materiał i metody: Grupę badawczą stanowiła młodzież obojga płci
w wieku 14-18 lat z MIZS w okresie remisji. Badaniami objęto 50
osób. W badaniu zastosowano: skalę numeryczną (NRS 0-10), drabinę
Cantila, skalę aktywności własnego autorstwa (pogrupowane
pozycje kwestionariusza tworzą podskale: zmęczenia, aktywności
fizycznej, aktywności społecznej, aktywności intelektualnej) oraz
skalę akceptacji choroby (AIS). Analizę statystyczną przeprowadzono
za pomocą testów: Manna-Whitneya, Wilcoxona, χ2, Fishera, korelacji
rang Spearmana. Do obliczeń przyjęto istotność statystyczną
p < 0,05.
Wyniki: Większość badanych osób nie odczuwa obecnie bólu lub
odczuwa go w niewielkim stopniu. Młodzież wykazuje dużą aktywność
w sferze fizycznej, psychicznej i społecznej. Przeważająca
część pacjentów akceptuje swoją chorobę. Chorzy odczuwający
ból określany w skali NRS na 4-7 pkt gorzej adaptują się do życia
z chorobą w porównaniu z pacjentami bez dolegliwości bólowych
albo odczuwających ból o słabym nasileniu. Stwierdzono wysoką
korelację pomiędzy akceptacją choroby w chwili badania określoną
za pomocą drabiny Cantrila i skali AIS.
Wnioski: 1. Ból odczuwany przez chorych na MIZS ma niewątpliwie
negatywny wpływ na poziom ich aktywności oraz stopień akceptacji
choroby. 2. Wydaje się kluczowe uznanie bólu jako znaczącego
wykładnika akceptacji choroby.Objectives: Being diagnosed with a chronic disease, such as juvenile
idiopathic arthritis (JIA), may adversely affect the activity of
a young person. Additionally, the pain associated with the disease
may cause difficulty accepting it. The aim was to assess the intensity
of pain as well as the degree of activity and acceptance of the
disease in young people aged 14-18 years with JIA.
Material and methods: The study group consisted of youth of
both genders suffering from JIA aged 14-18 years during remission
of the disease. The study included 50 people and used the
following: numerical rating scale (NRS 0-10), Cantril ladder, activity
scale of own authorship (grouped items of the questionnaire form
subscales of: tiredness, physical activity, social and intellectual
activity) and Acceptance of Illness Scale (AIS). Statistical analysis
included the following tests: Mann-Whitney U test, Wilcoxon’s, χ2
test, Fisher’s, and Spearman rank correlation. The statistical significance
level was set as p value below 0.05.
Results: The majority of the respondents observed currently do not
feel pain or feel pain of low intensity. The young people are physically,
psychologically and socially active. The majority of patients
accept their illness. The patients who suffer from pain on the NRS
scale within 4-7 points do not adapt to living with the disease as do
patients without pain or experiencing pain of low intensity. There
was a strong correlation between acceptance of the disease at the
time of the study determined by the Cantril ladder and AIS.
Conclusions: 1. The pain suffered by patients with juvenile idiopathic
arthritis (JIA) has undoubtedly a negative impact on their
activity and acceptance of the disease. 2. It seems to be crucial to
recognize pain as a significant determinant of acceptance of the
disease
Demographic and clinical profile of adolescents suicide attempters admitted to an emergency department during the COVID-19 pandemic – a retrospective cohort study using hospital information system
BackgroundThe prevalence of suicidal attempts among adolescents during COVID-19 significantly increased compared with pre-pandemic estimates. The aim of the study was to explore the demographic and clinical profile of adolescent suicide attempters admitted to the emergency department during the COVID-19 pandemic.Material and methodThe retrospective analysis included, on the basis of electronic medical records in the CliniNet system, sociodemographic and clinical data of patients aged 10–18 years with a diagnosis of suicide attempt. Follow-up period: from March 20, 2020 to May 16, 2023.ResultsDuring the COVID-19 pandemic, there were 425 emergency department visits among adolescents aged 11–17 due to a suicide attempt, with the largest number in the 15–17 (69%) age range. The percentage of emergency department visits was higher among females (80%) and urban residents (75.3%). Self-poisoning was the most common cause of suicide attempts (52.4%), followed by self-harm (41.4%), hanging (3.2%) and jumping from a height (2.1%). The most common toxic substances in self-poisonings were antidepressants and antipsychotics, followed by paracetamol. About 70% of visits were associated with adolescent mental disorders, of which depressive disorder was the most common. One death per 425 visits was recorded (0.2%).ConclusionsAdolescents attempting suicide during COVID-19 were most likely female, aged 15–17, city dwellers, undergoing psychiatric treatment mainly for depressive disorders. The mental health consequences of the pandemic may be more long term, and further monitoring will be needed in the years to come
Paediatric epidural analgesia with “top-up” doses : evaluation of its effectiveness and side effects
A comparison of the efficacy of intravenous and epidural analgesia after thoracosurgery in children
Wstęp. Problem zwalczania bólu u dzieci w okresie pooperacyjnym jest nadal kwestią otwartą. Powszechnie stosowane metody jego
leczenia są źródłem wciąż nowych poszukiwań zarówno w standardowym stosowaniu opioidów, jak również w uznanych już sposobach
znieczulenia zewnątrzoponowego.
Cel pracy. Celem pracy było porównanie skuteczności analgezji dożylnej morfiną oraz analgezji zewnątrzoponowej bupiwakainą i bupiwakainą
z morfiną w pierwszej dobie po operacjach torakochirurgicznych u dzieci.
Materiał i metody. W badaniu wzięło udział 89 dzieci (podzielonych na 2 grupy) w wieku od 7 do 18 lat. W grupie I (n = 30) operowani
otrzymywali ciągły dożylny wlew morfiny. W grupie II (n = 59) ból leczono za pomocą cewnika zewnątrzoponowego w odcinku piersiowym.
W grupie analgezji zewnątrzoponowej wyodrębniono podgrupę II A (n = 30) - podawano 0,25-procentową bupiwakainę co
4 godziny i II B (n = 29) - podawano 0,25-procentową bupiwakainę z morfiną 2,5 mg/20ml co 6 godzin. Natężenie bólu oceniano pięciokrotnie
w skali Numerical Rating Scale i werbalnej.
Wyniki i wnioski. W badaniach wykazano, że dodanie morfiny do roztworu bupiwakainy zwiększa skuteczność i czas trwania analgezji
zewnątrzoponowej oraz zapewnia lepszy poziom analgezji od morfiny podawanej metodą ciągłego wlewu dożylnego. Uwzględnienie
zalet analgezji zewnątrzoponowej pozwala coraz szerzej stosować ją w torakoanestezji dziecięcej.Introduction. The treatment of postoperative pain is still an open problem. Commonly used methods for pain relief are subjects of new
research in the field of standard opioid use, as well as other well known methods of epidural anesthesia.
Aim of the study. The study was aimed at comparing the efficacy of intravenous morphine and epidural bupivacaine analgesia vs. bupivacaine
with morphine pain relief method applied during the first 24 hours after the thoracosurgery in children.
Material and methods. Eighty nine children, from 7 to 18 years old, were divided into two groups. Group I (n = 30) comprised patients
receiving morphine i.v. and group II (n = 59) included individuals receiving analgesia through a thoracic extradural catheter. Group II was
divided into two subgroups II A (n = 30) and II B (n = 29) consisting of patients receiving bupivacainum 0.25% every 4 hours and bupivacainum
0.25% with morphine 2.5mg/20ml every 6 hours, respectively. The intensity of pain was evaluated 5 times using verbal scale and the
Numerical Rating Scale.
Results and conclusions. Research showed that low dose morphine combined with bupivacaine gave good and long duration of thoracic
block and provided an improved analgesia than intravenous morphine. The benefits of epidural analgesia allow applying it frequently in
pediatric thoracic anaesthesia
Automatic Continuous Control of Cuff Pressure and Subglottic Secretion Suction Used Together to Prevent Pneumonia in Ventilated Patients—A Retrospective and Prospective Cohort Study
The ventilator bundle consists of multiple methods to reduce ventilator-associated pneumonia (VAP) rates in Intensive Care Units (ICU). The aim of the study was to evaluate how the continuous automatic pressure control in tapered cuffs of endotracheal/tracheostomy tubes applied along with continuous automatic subglottic secretion suction affect the incidence of VAP. In the prospective cohort (n = 198), the standard VAP bundle was modified by continuous automatic pressure control in taper-shaped cuff of endotracheal/tracheostomy tubes and subglottic secretion suction. VAP incidence, time to VAP onset, invasive mechanical ventilation days/free days, length of ICU stay, ICU mortality, and multidrug-resistant bacteria were assessed and compared to the retrospective cohort (n = 173) with the standard bundle (intermittent cuff pressure of standard cuff, lack of subglottic secretion suction). A smaller incidence of VAP (9.6% vs. 19.1%) and early onset VAP (1.5% vs. 8.1%) was found in the prospective compared to the retrospective cohort (p < 0.01). Patients in the prospective cohort were less likely to develop VAP (RR = 0.50; 95% CI: 0.29 to 0.85) and early-onset VAP (RR = 0.19; 95% CI: 0.05 to 0.64) and had longer time to onset VAP (median 9 vs. 5 days; p = 0.03). There was no significant difference (p > 0.05) between both cohorts in terms of invasive mechanical ventilation days/free days, length of ICU stay, ICU mortality and multidrug-resistant bacteria. Modification of the bundle for prevention of VAP can reduce early-onset VAP and total incidence of VAP and delay the time of VAP occurrence
Ocena stopnia nasilenia bólu u dzieci po zabiegach torakochirurgicznych
Introduction. Regular evaluation of pain intensity with age-specific measuring scales, which correspond to the perception skills of children,
comprises the basis for effective pain relief after surgical interventions.
Purpose. The purpose of this work was to evaluate the level of pain intensity in children during the first 24 hours after thoracic surgical
interventions and analyze the following research issues:
1. To what extent chosen pain-relief method, kind of surgical intervention, and patient’s gender affected pain intensity?
2. Which measuring scale for pain intensity (numerical or verbal) turned out to be more useful in postoperative pain evaluation?
3. What was the correlation between physilological parameters (blood pressure, pulse rate) and subjective pain experience in the studied
groups?
Material and methods. The study was performed in two groups of children aged from 7 to 18 years. Respondents from group I (n = 30)
received after surgery continuous intravenous infusion of morphine. Group II (n = 59) comprised patients with thoracic extradural catheter
and received 0.25% bupivacaine or 0,25% bupivacaine with morphine. Pain intensity was evaluated five times in both numerical and verbal
scales. Results and conclusions. The intensity of postoperative pain depends on the method of applied analgesia. No relation between pain
intensity and kind of thoracic surgical intervention or patient’s gender was found. Both scales measuring pain intensity (numerical and
verbal) turned out to be useful as far as postoperative pain evaluation was concerned. Numerical scale should be the first chioce for it allows
greated differentiation of pain intensity. Physilological parameters showed poor correlation with subjective pain assessment. Therefore, they
should be treated only as additional measuring tools.Wstęp. Regularna ocena stopnia nasilenia bólu przy użyciu skal dostosowanych do wieku i możliwości percepcyjnych dziecka stanowi
podstawę skutecznego uśmierzania bólu pooperacyjnego.

następujących problemów badawczych:

3. Jaka była korelacja między parametrami fizjologicznymi (ciśnieniem tętniczym krwi, tętnem) a subiektywnym odczuciem bólu w badanych
grupach?
Materiał i metody. Badania przeprowadzono w dwóch grupach dzieci w wieku od 7 do 18 lat. W grupie I (n = 30) operowani otrzymywali
ciągły dożylny wlew morfiny. W grupie II (n = 59) ból leczono przez cewnik zewnątrzoponowy w odcinku piersiowym za pomocą
0,25-procentowej bupiwakainy lub 0,25-procentowej bupiwakainy z morfiną. Natężenie bólu oceniano pięciokrotnie w skali numerycznej
i werbalnej.
Wyniki i wnioski. Natężenie bólu pooperacyjnego uzależnione jest od rodzaju zastosowanej analgezji, ale nie od rodzaju zabiegu torakochirurgicznego
ani płci badanych. Obie skale bólu (numeryczna i werbalna) są przydatne do oceny bólu pooperacyjnego, ale ze względu
na większą gradację bólu w skali numerycznej, w pierwszym rzędzie należy rozważyć użycie tej skali. Parametry fizjologiczne słabo korelują
z subiektywnym odczuciem bólu, dlatego powinny być jedynie elementem pomocniczym w jego ocenie
