    The CAB model of pain-related activity avoidance: description and implications for research and practice

    Background and Purpose: Pain-related activity avoidance is a phenomenon that causes substantial annual patient morbidity. Therefore, it has been the subject of many recent studies related to physical therapist practice. The purposes of this review are: (1) to provide a rationale for considering cognition and affect in physical therapist practice, and (2) to propose the CAB Model for patient education in physical therapist management of pain-related activity avoidance. Method: Narrative review. Findings: \u27CAB\u27 is an acronym that emphasizes _Cognition and Affect in designing patient education programs that facilitate change in avoidant Behavior. Clinical Relevance: This review synthesizes literature that suggests pain-related cognitions and affect may be important targets for patient education by physical therapists, because they may serve as progenitors of pain-related activity avoidance. This narrative review provides a model for physical therapists to use in considering these features of clinical presentation and to guide future research

    Habitat Condition and Associated Macrofauna Reflect Differences Between Protected and Exposed Seagrass Landscapes

    Seagrass landscape configurations associated with different physical settings can affect habitat-structure and plant-animal relationships. We compared shoal grass (Halodule wrightii) habitat and macrofaunal variables between two fragmented seagrass landscapes at barrier-island locations subject to different disturbance regimes. Five seagrass habitat variables including above ground biomass (AGB), shoot number, per shoot biomass, epiphyte biomass and below ground biomass (BGB), differed significantly between the island landscapes. Per shoot biomass and epiphyte biomass also varied significantly over the seagrass growing season; and epiphyte biomass showed a strong landscape-time interaction. Abundances of microgastropods normalized to AGB differed significantly between landscapes. An inverse relationship between the abundance of microgastropods and epiphyte loading suggests a possible functional link. However, additional temporal mismatch between epiphyte loading and microgastropod abundance indicates that controls on epiphyte loading were complex. Seagrass habitat was more fragmented within the Cat Island (CI) landscape. Wind direction and strength imply that the CI landscape experienced more physical disturbance than the Horn Island (HI) landscape. This study highlights some potential links involving landscape configuration, habitat structure, and macrofaunal associations which can be further addressed using hypothesis-driven research

    Using Dynamic Contrast-Enhanced Magnetic Resonance Imaging Data to Constrain a Positron Emission Tomography Kinetic Model: Theory and Simulations

    We show how dynamic contrast-enhanced magnetic resonance imaging (DCE-MRI) data can constrain a compartmental model for analyzing dynamic positron emission tomography (PET) data. We first develop the theory that enables the use of DCE-MRI data to separate whole tissue time activity curves (TACs) available from dynamic PET data into individual TACs associated with the blood space, the extravascular-extracellular space (EES), and the extravascular-intracellular space (EIS). Then we simulate whole tissue TACs over a range of physiologically relevant kinetic parameter values and show that using appropriate DCE-MRI data can separate the PET TAC into the three components with accuracy that is noise dependent. The simulations show that accurate blood, EES, and EIS TACs can be obtained as evidenced by concordance correlation coefficients >0.9 between the true and estimated TACs. Additionally, provided that the estimated DCE-MRI parameters are within 10% of their true values, the errors in the PET kinetic parameters are within approximately 20% of their true values. The parameters returned by this approach may provide new information on the transport of a tracer in a variety of dynamic PET studies

    Spurious Shear in Weak Lensing with LSST

    The complete 10-year survey from the Large Synoptic Survey Telescope (LSST) will image \sim 20,000 square degrees of sky in six filter bands every few nights, bringing the final survey depth to r27.5r\sim27.5, with over 4 billion well measured galaxies. To take full advantage of this unprecedented statistical power, the systematic errors associated with weak lensing measurements need to be controlled to a level similar to the statistical errors. This work is the first attempt to quantitatively estimate the absolute level and statistical properties of the systematic errors on weak lensing shear measurements due to the most important physical effects in the LSST system via high fidelity ray-tracing simulations. We identify and isolate the different sources of algorithm-independent, \textit{additive} systematic errors on shear measurements for LSST and predict their impact on the final cosmic shear measurements using conventional weak lensing analysis techniques. We find that the main source of the errors comes from an inability to adequately characterise the atmospheric point spread function (PSF) due to its high frequency spatial variation on angular scales smaller than 10\sim10' in the single short exposures, which propagates into a spurious shear correlation function at the 10410^{-4}--10310^{-3} level on these scales. With the large multi-epoch dataset that will be acquired by LSST, the stochastic errors average out, bringing the final spurious shear correlation function to a level very close to the statistical errors. Our results imply that the cosmological constraints from LSST will not be severely limited by these algorithm-independent, additive systematic effects.Comment: 22 pages, 12 figures, accepted by MNRA

    Youth Football Injuries: A Prospective Cohort

    Background: There are approximately 2.8 million youth football players between the ages of 7 and 14 years in the United States. Rates of injury in this population are poorly described. Recent studies have reported injury rates between 2.3% and 30.4% per season and between 8.5 and 43 per 1000 exposures. Hypothesis: Youth flag football has a lower injury rate than youth tackle football. The concussion rates in flag football are lower than in tackle football. Study Design: Cohort study; Level of evidence, 3. Methods: Three large youth (grades 2-7) football leagues with a total of 3794 players were enrolled. Research personnel partnered with the leagues to provide electronic attendance and injury reporting systems. Researchers had access to deidentified player data and injury information. Injury rates for both the tackle and flag leagues were calculated and compared using Poisson regression with a log link. The probability an injury was severe and an injury resulted in a concussion were modeled using logistic regression. For these 2 responses, best subset model selection was performed, and the model with the minimum Akaike information criterion value was chosen as best. Kaplan-Meier curves were examined to compare time loss due to injury for various subgroups of the population. Finally, time loss was modeled using Cox proportional hazards regression models. Results: A total of 46,416 exposures and 128 injuries were reported. The mean age at injury was 10.64 years. The hazard ratio for tackle football (compared with flag football) was 0.45 (95% CI, 0.25-0.80; P = .0065). The rate of severe injuries per exposure for tackle football was 1.1 (95% CI, 0.33-3.4; P = .93) times that of the flag league. The rate for concussions in tackle football per exposure was 0.51 (95% CI, 0.16-1.7; P = .27) times that of the flag league. Conclusions: Injury is more likely to occur in youth flag football than in youth tackle football. Severe injuries and concussions were not significantly different between leagues. Concussion was more likely to occur during games than during practice. Players in the sixth or seventh grade were more likely to suffer a concussion than were younger players

    Effects of human immunodeficiency virus and metabolic complications on myocardial nutrient metabolism, blood flow, and oxygen consumption: a cross-sectional analysis

    <p>Abstract</p> <p>Background</p> <p>In the general population, peripheral metabolic complications (MC) increase the risk for left ventricular dysfunction. Human immunodeficiency virus infection (HIV) and combination anti-retroviral therapy (cART) are associated with MC, left ventricular dysfunction, and a higher incidence of cardiovascular events than the general population. We examined whether myocardial nutrient metabolism and left ventricular dysfunction are related to one another and worse in HIV infected men treated with cART vs. HIV-negative men with or without MC.</p> <p>Methods</p> <p>Prospective, cross-sectional study of myocardial glucose and fatty acid metabolism and left ventricular function in HIV+ and HIV-negative men with and without MC. Myocardial glucose utilization (GLUT), and fatty acid oxidation and utilization rates were quantified using <sup>11</sup>C-glucose and <sup>11</sup>C-palmitate and myocardial positron emission tomography (PET) imaging in four groups of men: 23 HIV+ men with MC+ (HIV+/MC+, 42 ± 6 yrs), 15 HIV+ men without MC (HIV+/MC-, 41 ± 6 yrs), 9 HIV-negative men with MC (HIV-/MC+, 33 ± 5 yrs), and 22 HIV-negative men without MC (HIV-/MC-, 25 ± 6 yrs). Left ventricular function parameters were quantified using echocardiography.</p> <p>Results</p> <p>Myocardial glucose utilization was similar among groups, however when normalized to fasting plasma insulin concentration (GLUT/INS) was lower (p < 0.01) in men with metabolic complications (HIV+: 9.2 ± 6.2 vs. HIV-: 10.4 ± 8.1 nmol/g/min/μU/mL) than men without metabolic complications (HIV+: 45.0 ± 33.3 vs. HIV-: 60.3 ± 53.0 nmol/g/min/μU/mL). Lower GLUT/INS was associated with lower myocardial relaxation velocity during early diastole (r = 0.39, p < 0.001).</p> <p>Conclusion</p> <p>Men with metabolic complications, irrespective of HIV infection, had lower basal myocardial glucose utilization rates per unit insulin that were related to left ventricular diastolic impairments, indicating that well-controlled HIV infection is not an independent risk factor for blunted myocardial glucose utilization per unit of insulin.</p> <p>Trial Registration</p> <p>NIH Clinical Trials <a href="http://clinicaltrials.gov/ct2/show/NCT00656851">NCT00656851</a></p
