    Current practice in proton therapy delivery in adult cancer patients across Europe

    BACKGROUND AND PURPOSE Major differences exist among proton therapy (PT) centres regarding PT delivery in adult cancer patient. To obtain insight into current practice in Europe, we performed a survey among European PT centres. MATERIALS AND METHODS We designed electronic questionnaires for eight tumour sites, focusing on four main topics: 1) indications and patient selection methods; 2) reimbursement; 3) on-going or planned studies, 4) annual number of patients treated with PT. RESULTS Of 22 centres, 19 (86%) responded. In total, 4233 adult patients are currently treated across Europe annually, of which 46% consists of patients with central nervous system tumours (CNS), 15% head and neck cancer (HNC), 15% prostate, 9% breast, 5% lung, 5% gastrointestinal, 4% lymphoma, 0.3% gynaecological cancers. CNS are treated in all participating centres (n = 19) using PT, HNC in 16 centres, lymphoma in 10 centres, gastrointestinal in 10 centres, breast in 7 centres, prostate in 6 centres, lung in 6 centres, and gynaecological cancers in 3 centres. Reimbursement is provided by national health care systems for the majority of commonly treated tumour sites. Approximately 74% of centres enrol patients for prospective data registration programs. Phase II-III trials are less frequent, due to reimbursement and funding problems. Reasons for not treating certain tumour types with PT are lack of evidence (30%), reimbursement issues (29%) and/or technical limitations (20%). CONCLUSION Across European PT centres, CNS tumours and HNC are the most frequently treated tumour types. Most centres use indication protocols. Lack of evidence for PT and reimbursement issues are the most reported reasons for not treating specific tumour types with PT

    Climate Driven Egg and Hatchling Mortality Threatens Survival of Eastern Pacific Leatherback Turtles

    Egg-burying reptiles need relatively stable temperature and humidity in the substrate surrounding their eggs for successful development and hatchling emergence. Here we show that egg and hatchling mortality of leatherback turtles (Dermochelys coriacea) in northwest Costa Rica were affected by climatic variability (precipitation and air temperature) driven by the El Niño Southern Oscillation (ENSO). Drier and warmer conditions associated with El Niño increased egg and hatchling mortality. The fourth assessment report of the Intergovernmental Panel on Climate Change (IPCC) projects a warming and drying in Central America and other regions of the World, under the SRES A2 development scenario. Using projections from an ensemble of global climate models contributed to the IPCC report, we project that egg and hatchling survival will rapidly decline in the region over the next 100 years by ∼50–60%, due to warming and drying in northwestern Costa Rica, threatening the survival of leatherback turtles. Warming and drying trends may also threaten the survival of sea turtles in other areas affected by similar climate changes

    Ten golden rules for optimal antibiotic use in hospital settings: the WARNING call to action

    Antibiotics are recognized widely for their benefits when used appropriately. However, they are often used inappropriately despite the importance of responsible use within good clinical practice. Effective antibiotic treatment is an essential component of universal healthcare, and it is a global responsibility to ensure appropriate use. Currently, pharmaceutical companies have little incentive to develop new antibiotics due to scientific, regulatory, and financial barriers, further emphasizing the importance of appropriate antibiotic use. To address this issue, the Global Alliance for Infections in Surgery established an international multidisciplinary task force of 295 experts from 115 countries with different backgrounds. The task force developed a position statement called WARNING (Worldwide Antimicrobial Resistance National/International Network Group) aimed at raising awareness of antimicrobial resistance and improving antibiotic prescribing practices worldwide. The statement outlined is 10 axioms, or “golden rules,” for the appropriate use of antibiotics that all healthcare workers should consistently adhere in clinical practice

    Why the clock ticks differently in Parkinson's disease: Insights from motor imagery and resting-state functional magnetic resonance imaging

    In Parkinson's disease (PD), an impaired perception of suprasecond time intervals has been reported. From a neurobiological perspective, dopamine is thought to be an important mediator of timing. Nevertheless, it is still unclear whether timing deficits in PD occur mainly in the motor context and are associated with corresponding striatocortical loops. This study attempted to fill this gap by investigating time reproduction in the context of a motor imagery task, and its neurobiological correlates in resting-state networks of basal ganglia substructures in PD.Nineteen PD patients and 10 healthy controls therefore underwent two time reproduction tasks. In a motor imagery task, subjects were asked to walk down a corridor for 10 s and reproduce the time spent walking during motor imagery afterwards. In an auditory task, the subjects had to reproduce an acoustically presented time interval of 10 s. Subsequently, resting-state functional magnetic resonance imaging was performed and voxel-wise regressions were conducted between striatal functional connectivity and performance in the individual task at group level and compared between groups.Patients significantly misjudged the time interval in the motor imagery task and an auditory task in comparison to controls. Seed-to-voxel functional connectivity analysis of basal ganglia substructures revealed a significant association between striatocortical connectivity and motor imagery performance. PD patients showed a different pattern of associated striatocortical connections as indicated by significantly different regression slopes for connections of the right putamen and left caudate nucleus.In accordance with previous findings, our data confirm an impaired time reproduction of suprasecond time intervals in PD patients. Our data imply that deficits in time reproduction tasks are not specific to motor context but reflect a general time reproduction deficit. According to our findings, impaired performance in context of motor imagery is accompanied by a different configuration of striatocortical resting-state networks responsible for timing
