    Fatores prognósticos em transplante pulmonar : a experiência da santa casa de porto alegre

    Introdução: O transplante de pulmão é considerado tratamento de escolha para alguns casos selecionados de doença pulmonar avançada. Diferentes fatores prognósticos têm sido relatados por centros transplantadores. O objetivo desse estudo é reportar a experiência recente da Santa Casa de Porto Alegre com essa modalidade terapêutica e destacar fatores associados a mortalidade. Métodos: Foi realizada uma análise retrospectiva de 130 pacientes submetidos a transplante pulmonar com doador cadavérico no período entre janeiro de 2004 e julho de 2009. Foram coletados dados do doador, do receptor, do trans e pós-operatório. Resultados: A idade média foi de 53,14 anos, variando entre 8 e 72. Oitenta pacientes (61,5%) eram do sexo masculino. As principais causas de doença pulmonar foram fibrose intersticial 53 (40,7%) e doença pulmonar obstrutiva crônica 52 (40%). A sobrevida atuarial em 1 ano foi de 67,7%. Variáveis correlacionadas com sobrevida foram: idade (p=0,004), distância percorrida no teste de caminhada dos seis minutos (p=0,007), doença arterial coronariana (p=0,001), necessidade de circulação extracorpórea (p=0,02), necessidade de transfusão no intra-operatório (p=0,016), pressão venosa central aumentada 12 e 24 horas pós-transplante (p=0,02 e p=0,001, respectivamente), aumento da pressão capilar pulmonar 24 horas póstransplante (p=0,01); tempo em ventilação mecânica (p 36 horas) e necessidade de reintubação são fatores prognósticos independentes em transplante pulmonar.Background: Lung transplantation has been established as current therapy for selected patients with end-stage lung disease. The objective of the present study is to report recent results from Santa Casa de Porto Alegre with lung transplantation and to search for prognostic factors. Methods: We performed a retrospective analysis of 130 patients who underwent lung transplantation at our institution from January 2004 to July 2009. Donor, recipient, intra and postoperative variables were collected. Results: The mean age was 53.14 years (from eight to 72) and 80 (61.5%) were male. The main causes of end-stage respiratory disease were pulmonary fibrosis 53 (40.7%) and chronic obstructive pulmonary disease 52 (40%). The actuarial 1-year survival was 67.7%. Variables correlated with survival were: age (p=0.004), distance in the six-minute walk test (p=0.007), coronary heart disease (p=0.001), cardiopulmonary bypass (p=0.02), intraoperative transfusion of red blood cells (p=0.016), increasing central venous pressure at 12 and 24 postoperative hours (p=0.02 and p=0.001, respectively), increasing pulmonary capillary wedge pressure at 24 hours (p=0.01); length of intubation (p 36 hours) and reintubation as independent prognostic factors in lung transplantation

    Inflammatory pseudotumor of the spleen concomitant with renal cell carcinoma: case report

    CONTEXT: Inflammatory pseudotumor is a rare benign lesion that can occur at a wide variety of primary sites. It is usually worrisome for the patient and the medical staff, since it cannot be clinically or radiologically distinguished from malignant entities. CASE REPORT: We report on a case of splenic inflammatory pseudotumor presenting with concomitant renal cell carcinoma. Despite the alarming macroscopic appearance of pseudotumors, their microscopic features usually confirm the inflammatory nature of such lesions. Evidence regarding the etiology of pseudotumors is still lacking, but hypotheses have been created

    Round pneumonia: a rare condition mimicking bronchogenic carcinoma. Case report and review of the literature

    CONTEXT: Round pneumonia is a condition usually described in children, with few reports addressing adult patients. It is an oval-shaped consolidation that, due to its radiological appearance, simulates bronchogenic carcinoma. Its evolution tends to be benign, although diagnostic dilemmas have sometimes required exploratory thoracotomy. Deaths caused by this condition have even been reported. To the best of our knowledge, there have been 31 previous cases of round pneumonia in adults reported in the English and Portuguese-language literature, of which only one was completely asymptomatic. CASE REPORT: The case of a 54-year-old female patient presenting a lung mass found on routine imaging evaluation is reported. Respiratory symptoms and signs were absent, but the patient had a significant history of smoking. Her physical examination gave normal results. On chest radiographs, a mass located in the middle third of the right lung was observed. Three weeks after the initial evaluation, the patient was admitted for a complete evaluation and for staging of a pulmonary malignancy, but repeated chest radiographs showed complete resolution
